Reliable AI-agent monitoring needs two connected feedback loops: production tracing and repeatable evaluation. Trace every model generation, tool call, handoff, guardrail, and important application step so an engineer can reconstruct a run. Then turn representative traces and confirmed incidents into datasets and graders that run whenever prompts, models, tools, or routing change. Operational dashboards show that a service is unhealthy; trace review and task-specific evaluations explain whether the agent still did the right thing.
What AI-agent monitoring must explain
An agent can be available, fast, and inexpensive while still failing the user’s task. It may choose the wrong tool, omit a handoff, violate a policy, misread a tool result, or produce a plausible but incorrect answer. Conversely, a run can look unusual in a metric dashboard and still be valid. Monitoring therefore has two distinct questions:
- Is the service operating normally? Check request volume, completion and error status, latency, token use, cost, and resource behavior.
- Did the workflow produce the intended outcome? Check tool selection, handoffs, required fields, policy compliance, factual or business rules, and user or reviewer feedback.
Keep those signals separate in dashboards and incident discussions. A latency spike is not proof of a quality regression, and a successful HTTP response is not proof that the agent completed the task correctly.
Instrument a complete run before trying to improve it
Create one root trace for each user-visible task and attach child spans for every operation that can affect the result. The trace should let an engineer move from the final answer back to the first decision that diverged from the expected workflow. OpenAI describes this model in its Agents SDK documentation: “The Agents SDK includes built-in tracing, collecting a comprehensive record of events during an agent run: LLM generations, tool calls, handoffs, guardrails, and even custom events that occur.”

Use a stable trace hierarchy
A practical hierarchy is:
- Root trace: the user request or scheduled job, with a stable run ID.
- Agent-loop span: the planning, acting, and observing cycle, including iteration count and termination reason.
- Model-call spans: provider/model identifier, prompt or template version, sampling settings, response status, token counts, and finish reason.
- Tool spans: tool name and version, validated arguments, start and end times, result status, retries, timeout, and a redacted result summary.
- Handoff spans: source and destination agent, routing reason, transferred context, and whether the destination accepted the handoff.
- Guardrail spans: policy or validator name, input/output stage, decision, and remediation action.
- Application spans: retrieval, database access, queue operations, business-rule checks, and other integrations that influence the answer.
- Custom events: approvals, user corrections, cancellations, escalation, and domain-specific milestones.
Include workflow, release, prompt, model, tool, and routing versions on the root trace or its metadata. Those fields turn “it broke” into a comparison between two known configurations. Use the same run identifier in logs, metrics, support tickets, and evaluation records.
Capture enough context to diagnose, not everything by default
| Trace field | Why it matters | Implementation note |
|---|---|---|
| Start/end time and duration | Find slow model calls, tools, retries, and queue waits. | Record both wall-clock duration and, where useful, provider-reported duration. |
| Status and failure reason | Distinguish timeout, validation failure, policy block, provider error, and application exception. | Use controlled reason codes plus a human-readable detail. |
| Model and prompt versions | Connect a behavior change to a release or configuration change. | Store immutable version identifiers rather than only a mutable prompt name. |
| Tool arguments and result summary | Show whether the agent selected the right tool and interpreted its output. | Validate and redact arguments and results before export. |
| Token counts and cost fields | Expose workload shifts and unexpectedly expensive loops. | Keep input and output counts separate when the provider supplies them. |
| Feedback and outcome labels | Connect technical traces to task success. | Accept explicit ratings, reviewer labels, corrections, and downstream business outcomes. |
Do not make a final transcript the only artifact. The ordering and timing of events often reveal the cause: a tool was called twice, a handoff never occurred, a guardrail blocked an otherwise valid response, or a retry changed the context. At the same time, avoid indiscriminate payload capture. Define which inputs and outputs are needed for debugging, redact secrets and personal data, restrict access, and document retention and deletion rules.
Check SDK and deployment behavior
Tracing behavior is implementation-specific. In the OpenAI JavaScript Agents SDK, tracing is enabled by default in supported server runtimes, disabled by default in browsers and when NODE_ENV=test, and unavailable to organizations using OpenAI APIs under Zero Data Retention. In runtimes without the SDK’s automatic export loop, queued traces must be flushed before the process exits. Treat these as SDK-specific facts, not universal rules: verify the behavior of the version and runtime you deploy, and add a shutdown test that proves spans reach the backend.

Build an operational-health dashboard
Start with signals that tell you whether traffic is flowing and whether the workload has changed. Use distributions, not just averages, for latency and cost because a small number of pathological runs can be hidden by a mean.
| Signal | Useful breakdowns | What a change can indicate |
|---|---|---|
| Request volume | Endpoint, tenant, workflow, release, and time window | Traffic surge, missing traffic, a routing change, or an upstream outage |
| Completion and error status | Success, controlled refusal, timeout, provider error, tool error, application exception | Availability problems or a new failure mode |
| Latency | p50, p95, p99; total run and individual spans | Slow model responses, tool degradation, retries, or runaway loops |
| Token consumption | Input/output, model, workflow, and run iteration | Prompt growth, context bloat, repeated calls, or a model change |
| Cost | Provider, model, tenant, workflow, and release | Unexpected model selection, excessive retries, or higher usage |
| Feedback | User rating, correction, escalation, reviewer label | Quality deterioration that infrastructure metrics cannot see |
LangSmith lists token usage, latency percentiles, error rates, cost breakdowns, and feedback scores as examples of dashboard metrics. They are useful categories, not mandatory metrics or universal alert limits. Set alert conditions from your workflow’s baseline, risk, and service objectives. For example, a payment-authorizing agent deserves a stricter quality response than a low-risk drafting assistant, even if both have identical latency.
Evaluate behavior, not just availability
Write a task-specific success contract
Before choosing a grader, define what a successful run must accomplish. Express the contract as observable checks wherever possible:

- Was the requested intent classified correctly?
- Was the approved tool selected, with valid arguments?
- Did the agent stop when the task was complete instead of looping?
- Were required fields present and correctly formatted?
- Did the agent follow authorization, privacy, and safety rules?
- Did it hand off to the right specialist when the workflow required one?
- Did the final result satisfy the business rule, not merely produce fluent text?
Combine deterministic assertions with rubric-based review when a task has legitimate variation. Exact string matching is appropriate only when the output truly must be fixed; otherwise, it can classify a correct answer as a failure because of harmless wording differences.

Trace grading is a workflow-level debugging tool. Select normal runs, edge cases, refusals, tool failures, handoffs, and user complaints. Inspect the sequence and score the specific contract items rather than judging only the final text. This can expose wrong tool choice, missing handoffs, policy violations, and end-to-end changes that a final-answer score hides.
Read transcripts as well as scores. Anthropic’s evaluation guidance notes that transcript review can show whether the agent failed or whether a grader incorrectly rejected a valid result. When a score is surprising, inspect the trace, compare it with the rubric, and correct the grader or task definition before changing the agent.
Turn stable expectations into a dataset
After the team agrees on what “good” means, create a versioned evaluation dataset. Include the original input, relevant context, expected properties or reference answer, and labels for important edge cases. Keep cases that represent confirmed incidents and near misses; they are more valuable than a large collection of easy, repetitive examples.
Run the dataset after prompt, model, tool, retrieval, routing, guardrail, or orchestration changes. Store the configuration and per-case results so you can compare a candidate with the current baseline. OpenAI’s evaluation guidance distinguishes trace-based debugging from repeatable dataset-based evaluation: traces help discover the problem, while datasets measure whether a proposed change improves it.

Agents are often nondeterministic. Compare distributions of rubric scores, tool-selection accuracy, policy outcomes, latency, and cost across an adequate set of runs when the task warrants it. A single pass or a single “golden” transcript can miss intermittent failures. The appropriate number of cases and runs depends on risk, traffic, variance, and the cost of a bad decision; the cited documentation does not establish a universal sample size, pass-rate target, or alert threshold.

A practical monitoring and regression workflow
- Define the workflow boundary. Decide what counts as one user-visible task, where it starts and ends, and which downstream outcomes matter.
- Add the root trace and child spans. Instrument model calls, tools, handoffs, guardrails, retrieval, and application integrations. Propagate the run ID through asynchronous jobs and retries.
- Attach release metadata. Record prompt, model, tool, routing, guardrail, and application versions so every run can be grouped by configuration.
- Establish redaction and access controls. Decide which payload fields are retained, mask credentials and personal data, encrypt exported data, and restrict transcript access to people who need it.
- Baseline operations. Observe request volume, completion and error rates, latency percentiles, token use, cost, and feedback long enough to understand normal variation.
- Sample and review traces. Review ordinary runs and targeted cases such as retries, refusals, tool errors, handoffs, and escalations. Label the first point of divergence.
- Create graders and a dataset. Encode deterministic checks and rubric criteria. Add representative traces, confirmed incidents, and near misses as versioned cases.
- Run evaluations in change control. Compare a candidate configuration with the baseline before release, and record per-case differences rather than only an aggregate score.
- Alert on both layers. Page on service-health failures and route quality regressions to the workflow owner. Avoid treating every quality fluctuation as an infrastructure incident.
- Close the loop. For a confirmed failure, fix the model prompt, tool, routing, guardrail, integration, or grader; rerun the affected cases; and retain the case so a later change cannot silently reintroduce it.
Diagnose the first divergence in a failed trace
Start at the user-visible outcome, then walk backward through the trace. The first incorrect decision is usually more actionable than the last incorrect sentence.
The model response is wrong or incomplete
Check the prompt and model version, available context, truncation, sampling settings, and whether an earlier tool result was malformed. Compare the failing run with a successful run for the same case. If the model followed the supplied context but the context was wrong, fix retrieval or application logic instead of merely adding instructions.
The wrong tool was selected
Verify the tool descriptions, argument schema, authorization, and routing rules. Look for newly added tools with overlapping names or capabilities. A grader that checks only the final answer may miss this failure because a later step happened to recover.
A tool returned an error or misleading result
Separate timeout, transport failure, validation rejection, permission denial, and an application-level “not found” result. Check retries and idempotency: repeating a read may be harmless, while repeating a write may create damage. Record the raw failure category and a redacted result summary so the agent’s next decision can be explained.
A handoff or guardrail behaved unexpectedly
Inspect the source and destination agent versions, transferred context, handoff condition, and termination reason. For guardrails, determine whether the policy correctly blocked the content and whether the recovery path produced a useful response. A policy violation and an over-broad policy rejection are different defects.
The application integration changed the result
Check retrieval freshness, database state, feature flags, time zones, permissions, serialization, and external-service responses. Attach correlation IDs for downstream requests so an agent trace can be joined to the system that supplied the data.

The grader is the problem
Re-read the transcript against the rubric and compare it with labeled examples. If a valid result is repeatedly marked as a failure, correct the grader or expected label and document the decision. Do not “fix” the agent to satisfy a faulty evaluator.

Turn incidents into durable regression protection
Maintain a case taxonomy
Tag cases by failure stage and risk: model reasoning, tool choice, tool execution, retrieval, handoff, guardrail, formatting, authorization, latency, cost, or external dependency. Tag whether the case is a confirmed failure, acceptable variation, or grader error. This makes trend reports meaningful and helps an owner find the relevant slice of the dataset.
Use layered gates
- Fast deterministic checks: schema validity, required fields, forbidden actions, authorization, and tool-argument constraints.
- Trace and rubric checks: correct sequence, handoff behavior, policy adherence, and task completion.
- Sampled or online checks: production feedback, reviewer labels, and distribution changes after release.
A candidate that passes formatting but fails authorization should not ship. Conversely, a small change in wording should not block deployment when the task contract is unchanged. Make the gate reflect the consequence of the failure, and document exceptions rather than silently weakening the test.
Watch for regressions in cost and latency too
A quality improvement that doubles tool calls or context size may be unacceptable. Compare token use, cost, iteration count, and latency alongside quality scores for the same cases. Attribute changes to a specific span so an optimization does not hide a new failure in another part of the workflow.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Choosing a hosted, self-hosted, or hybrid approach
Choose a monitoring system by capability and constraints, not by a feature checklist alone. Verify current retention, residency, export, access-control, and pricing terms before sending production payloads.
| Option described by its current documentation | Relevant capabilities | Questions to verify |
|---|---|---|
| Braintrust | Offline evaluation, production observability, and experiment tracking. | How its current tracing coverage, data retention, access controls, and framework integrations fit your workflow. |
| LangSmith | Tracing, offline and online evaluations, dataset management, framework interoperability, OpenTelemetry support, and managed, BYOC, and self-hosted deployment options. | Which deployment and data-residency choices are available for your region and plan, and whether every agent event is captured. |
| Langfuse | Self-hosted, open-source observability and evaluation option. | Operational burden, upgrades, storage, authentication, and the feature parity you need. |
| Arize Phoenix/AX | Open-source tracing and evaluation with a SaaS option. | Current collector, evaluation, retention, and support behavior for your stack. |
| In-house telemetry | Maximum control over schema, storage, retention, and integration with existing logs and metrics. | Who will maintain trace viewers, graders, dataset tooling, access controls, and alert routing. |
Evaluate each candidate on five axes:
- Trace depth: Can it show the end-to-end workflow, tool calls, handoffs, guardrails, retries, and custom events?
- Evaluation lifecycle: Can reviewers inspect traces, create graders, version datasets, run comparisons, and support online evaluation?
- Stack fit: Does it support your agent framework, language, asynchronous jobs, and existing telemetry pipeline?
- Data control: Does its deployment model meet privacy, retention, residency, and deletion requirements?
- Operational workflow: Can owners query traces, correlate them with service metrics, receive feedback, and route incidents?
A hosted service can reduce the work of building trace search and evaluation runners, while self-hosting can provide tighter control over sensitive payloads and retention. A hybrid design is also possible: keep payloads in a controlled store while exporting redacted spans and aggregate metrics to a managed system.

Privacy, security, and reliability safeguards
- Minimize payloads: Store only fields needed to diagnose and evaluate the workflow; hash or tokenize identifiers when the original value is unnecessary.
- Redact before export: Remove credentials, payment data, authentication tokens, and sensitive personal information from prompts, tool arguments, and results.
- Separate access: Restrict raw transcripts more tightly than aggregate metrics, and log access to sensitive traces.
- Set retention by purpose: Use shorter retention for raw payloads and longer retention for redacted regression cases when policy permits.
- Protect trace integrity: Prevent agents or end users from rewriting status, labels, or evaluation results that operators rely on.
- Test exporter failure: Decide whether tracing is best-effort or part of the request’s reliability contract. Queue and retry exports without blocking user traffic unless the task requires an audit record.
- Flush on shutdown: Confirm that workers and serverless processes export queued spans before termination.
Common monitoring mistakes and fixes
| Mistake | Why it fails | Better practice |
|---|---|---|
| Monitoring only HTTP status and uptime | A valid response can contain an incorrect or unsafe task result. | Pair operational metrics with task-specific graders and trace review. |
| Recording only the final answer | The cause may be a tool, handoff, guardrail, or retry earlier in the run. | Capture an ordered trace of meaningful events. |
| Using one aggregate quality score | Averages hide high-risk failure classes and can reward easy cases. | Report per-case and per-category results, with severity labels. |
| Trusting a grader without validation | The evaluator can reject a valid answer or accept a harmful one. | Review transcripts, calibrate against labeled examples, and version grader changes. |
| Exact-matching every response | Legitimate variation appears to be a regression. | Grade required properties unless fixed text is genuinely required. |
| Alerting on unsupported universal thresholds | Risk and normal variance differ by workflow. | Set thresholds from a documented baseline and consequence analysis. |
| Capturing unrestricted production data | It increases privacy, security, and retention risk. | Redact, minimize, restrict, and test deletion. |
| Adding every incident but never removing stale cases | The suite becomes slow, redundant, and hard to interpret. | Deduplicate cases, preserve distinct failure modes, and review labels periodically. |
An incident runbook for a suspected regression
- Confirm the signal. Check whether the change appears in operational metrics, quality evaluations, user feedback, or more than one source.
- Freeze the comparison. Record the deployed workflow versions and compare them with the last known-good configuration.
- Find representative traces. Select a failing run, a successful run for the same or similar input, and any edge cases in the affected category.
- Locate the first divergence. Inspect model output, tool choice and result, handoff, guardrail, retrieval, and application spans in order.
- Validate the evaluator. Confirm that the grader’s expected behavior still matches the task contract.
- Apply the narrowest fix. Change the defective prompt, model, tool schema, routing rule, guardrail, integration, or grader rather than masking the symptom.
- Replay the affected dataset. Compare the fix with the baseline, including cost, latency, and important neighboring categories.
- Canary and observe. Release to a controlled portion of traffic when possible, then watch both operational and behavioral signals.
- Preserve the lesson. Add the confirmed failure or near miss to the regression set with a clear label and owner.
What “good” looks like after implementation
An engineer should be able to open one run and answer: which workflow and versions ran, what the model generated, which tools were called and with what validated arguments, what each tool returned, where handoffs and guardrails occurred, how long and how much the run cost, and why the final outcome was accepted or rejected. A release process should then answer a second question: did this configuration improve the representative dataset without introducing a new failure class, unacceptable cost, or latency problem?
That combination—complete traces for discovery and diagnosis, plus validated graders and repeatable datasets for change control—turns agent monitoring from a dashboard exercise into an engineering feedback loop.
Frequently Asked Questions
Should every production agent run be traced?
Trace coverage should match risk and diagnostic needs. For high-consequence workflows, full event traces are usually appropriate; for lower-risk workloads, sampled payloads with complete aggregate metrics may reduce cost and privacy exposure. Whatever sampling you use, retain enough representative traces to investigate failures.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsWhat is the first alert to create?
Start with alerts for clear service-health failures—such as error-rate or latency changes—then add workflow-specific quality alerts after you have a baseline and validated graders. The correct limits depend on task risk and normal variation; there is no universal threshold.
How do I know whether a model change caused a regression?
Compare the old and new configuration on the same versioned dataset, inspect per-case differences, and correlate failures with trace spans. Check prompt, model, tool, routing, guardrail, and application versions rather than attributing every change to the model alone.
Is a hosted observability platform required?
No. A hosted platform can provide trace search and evaluation features faster, while self-hosting or in-house telemetry can provide tighter data control. Select the deployment model that meets your privacy, retention, integration, and operational-maintenance requirements.
