You can estimate pupil diameter at home with a transparent millimeter ruler or pupil gauge. Have the person look at a distant target, compare both eyes in comfortable light and dimmer light, and record the approximate readings and symptoms. Do not shine a phone flashlight or any other bright beam directly into the eye. A home check is screening only: a persistent, new or symptomatic difference needs professional assessment.

What a home pupil measurement can—and cannot—tell you

The pupil is the dark opening in the center of the eye. It constricts in bright light and dilates in dim light, an automatic response that regulates how much light reaches the retina. A home measurement can document an approximate diameter and whether the two pupils appear to change together under the same conditions.

The American Optometric Association says pupil size can be estimated with a millimeter ruler. That is useful for documenting an observation, not for assigning a diagnosis. It cannot establish which pupil is abnormal, measure the speed or quality of the light response precisely, or identify a cause. A ruler reading cannot rule out eye injury, medication or eye-drop effects, glaucoma, an aneurysm, stroke or another neurologic problem. Even clinical instruments produce estimates affected by the device, examiner, environment and patient.

There is no single pupil size that is “normal” for every person or every situation. The American Academy of Ophthalmology (AAO) reports that about 20 percent of people have naturally different pupil sizes. Cleveland Clinic gives a separate estimate that about 15 percent of people experience anisocoria at some point. These figures use different wording and should not be combined into one prevalence rate.

What matters at home is a pattern: the approximate size of each pupil, whether the difference is present in both light and dark, whether the pupils look round and centered, and whether the appearance is new or accompanied by symptoms. A clinician must interpret that pattern with your history and examination.

Before you start: make the check safe and repeatable

  • Wash your hands and remove glasses, contact lenses or anything else that blocks a clear view of the eye. Do not put fingers, a ruler or another object on the eye.
  • Use a transparent millimeter ruler or pupil gauge. It should be held beside the eye, never touching the cornea, eyelid or eyelashes. A simple clear scale is inexpensive and accessible; its result is still only an estimate.
  • Sit securely. Having another person observe and read the scale is safer and usually more accurate than trying to measure your own eye. Support the subject’s head if that helps keep the gaze steady.
  • Choose a distant fixation target, such as a point across the room. Looking at something close can trigger accommodation (the near-focus response) and alter pupil size.
  • Use comfortable, even room lighting. Do not use a camera flash, phone flashlight or other bright beam aimed into the eye.
  • Have a way to record the side, approximate diameter, lighting condition, symptoms, medicines and eye drops. Note the date and time so a later observer can compare like with like.
  • If the eye is painful, injured, chemically exposed or suddenly looks different, stop the home check and seek professional advice rather than trying to obtain a better measurement.

Step-by-step: estimate pupil diameter with a millimeter ruler

  1. Set up the observer and target. Sit upright with your head supported if possible. Ask the person being measured to look at a distant object without shifting gaze between the ruler and the target. The observer should explain each movement before bringing the scale near the face.
  2. Position the ruler. Hold the transparent millimeter scale next to the eye, parallel to the iris. Keep it outside the line of sight and do not press on the eyelid. A second person should read the scale while the subject keeps looking into the distance. Hold the ruler as close to the same plane as the pupil as practical, because an angle or a ruler held farther forward can make the apparent diameter misleading.
  3. Measure in ordinary room light. Let the eyes settle in comfortable, even illumination. Estimate the widest part of each pupil in millimeters and record the right and left readings separately rather than only writing the difference. If the edge is not clear, write “approximately” and avoid implying precision that the view does not support.
  4. Compare shape and alignment. Note whether each pupil looks round, whether it is centered, and whether the two pupils appear similar in shape. Record any obvious drooping eyelid, eye deviation, visible injury or clouding that makes the edge hard to see. The ruler does not tell you whether an irregular shape comes from the iris, the pupil margin or an obstruction.
  5. Allow adaptation in dimmer light. Move to a safely lit, dimmer room and wait for the eyes to adapt before repeating the readings. Use enough light for the observer to see safely, but do not create darkness that makes walking or positioning hazardous. Record the condition as “dimmer room” rather than assuming it matches a clinical dark-room measurement.
  6. Observe the response without a bright beam. If you need to see whether both pupils change with a gradual room-light change, increase or decrease the ambient illumination slowly. Stop if there is discomfort, glare or anxiety. Home observation cannot substitute for the controlled light-and-fixation testing used in a clinical PERRLA examination.
  7. Document the context. Write down the date and time, room-light condition, eye side, approximate millimeters, symptoms, recent medicines or eye drops, and whether both pupils appeared to react together. Include anything that could have reached one eye, such as a topical product or chemical.
  8. Repeat under similar conditions. Take another set later rather than treating one reading or one photograph as definitive. Use the same observer, ruler, target and lighting when possible. Bring the record and any photographs to an eye-care professional if the difference persists, is new or is accompanied by symptoms.

How to interpret the readings

A small difference may be longstanding

Anisocoria means the pupils are not the same size. A difference that has been present for a long time, remains similar in different lighting and causes no symptoms can be a normal individual finding. The home record is useful for showing whether the appearance is stable, but it does not prove that the difference is harmless. A person who has never compared the pupils before may not know how long the difference has existed.

AAO’s 2026 patient guidance describes anisocoria as unequal pupil size and notes that many people have a naturally occurring difference. Cleveland Clinic’s 2024 estimate that about 15 percent of people experience anisocoria at some point is reported separately; neither figure supplies a personal threshold for when a difference is safe.

Light and dark comparisons matter

Compare the eyes in both brighter and dimmer conditions. If the difference is more obvious in bright light, the relatively larger pupil may not be constricting normally. If it is more obvious in darkness, the relatively smaller pupil may not be dilating normally. This pattern can help a clinician decide what to examine, but it is not a diagnosis you can make with a ruler.

Make the comparison fairly. A reading in ordinary room light is not directly equivalent to one taken beside a window, under a bright overhead lamp or with a camera flash. Note the environment each time, and do not repeatedly expose the eyes to intense light in an effort to force a response.

Look for change, not a magic number

The most useful home observation is often whether one pupil has suddenly changed compared with your previous photographs or notes, and whether both pupils continue to respond together. Do not compare a reading taken with a phone close to the face in one lighting setup with a ruler reading taken across a room in another; scale, angle and illumination can create an apparent difference.

A home reading also cannot determine whether the larger or smaller pupil is the abnormal one. The same visible difference can have different explanations, and a clinician may need to examine the iris, eyelids, eye movements, vision and neurologic status before deciding what it means.

Can a phone measure your pupils?

A smartphone photograph can document an apparent difference, but it does not provide a dependable millimeter measurement by itself. Camera distance, perspective, lens position, ambient light, automatic exposure, image processing and flash can all change the apparent size. Without a fixed scale and controlled geometry, a photograph is documentation rather than diagnosis.

If you take a photograph for a clinician, keep the setup consistent: use the same camera, distance and room lighting, avoid flash directed at the eyes, and include the date and symptoms. A helper should take the picture while the subject looks at the same distant target. Avoid cropping away the surrounding eye if the clinician needs to judge eyelid position or apparent alignment. Do not delay urgent care while trying to obtain a better image.

Some phone applications return a numeric pupil value, but no independently verified smartphone-app accuracy study is established for this use here. Treat an app result as an unvalidated estimate, not as a clinical measurement or a reason to dismiss symptoms.

How home methods compare with clinical assessment

Method Scale and repeatability Lighting and reactivity Main limitations
Transparent millimeter ruler or pupil gauge Accessible and inexpensive; gives an approximate diameter when held beside the eye Can be repeated in comfortable bright and dim conditions; cannot precisely quantify reactivity Reading is affected by viewing angle, ruler position, observer and room light; it cannot identify the abnormal pupil or cause
Smartphone photograph Convenient for documenting appearance; uncontrolled scale and perspective reduce measurement reliability Ambient light, exposure and flash can alter the image; a photograph does not test the response Camera distance, lens position and automatic processing can create apparent differences; app accuracy has not been independently established
Digital pupillometer or autorefractor in a clinic Provides a more quantified estimate under examination conditions Designed for controlled measurement and may assess response depending on the instrument AAO notes that estimates still depend on device, examiner, environment and patient factors
Comprehensive eye examination Uses professional instruments and repeatable clinical observations Can include controlled light and near-focus testing, slit-lamp examination and neurologic assessment Only a clinician can interpret the findings in the context of symptoms, history, medicines and injury

The practical choice at home is therefore documentation, not precision. A ruler is preferable to guessing from a photograph because it supplies a visible millimeter scale, but it remains crude. A phone is useful for showing what you saw and when you saw it. Neither method can replace an examination when the finding is new, persistent or symptomatic.

What clinicians check: PERRLA and more

A PERRLA examination checks whether the pupils are equal, round, reactive to light and responsive during near focus. The clinician uses a light and a fixation target while observing direct and consensual responses and the change associated with near focus. The acronym is commonly expanded as pupils, equal, round, reactive, light and accommodation.

Eye-care professionals also compare size in light and dark, shape, position, symmetry and reactivity. They may examine the eye with a slit lamp and assess eye movements, eyelids, vision and neurologic signs. These steps answer questions a home ruler cannot: whether the iris or pupil is structurally injured, whether an eye movement problem is present, and whether a systemic neurologic process could explain the finding.

Tell the clinician exactly when the difference was first noticed and whether it appears in both lighting conditions. Bring medication and eye-drop names, because an exposure affecting one eye can alter its pupil. Mention recreational substances, recent procedures, illness and any injury even if they seem unrelated.

When unequal pupils are an emergency

Seek emergency medical care for a sudden increase in the size of one pupil, especially when the change is new or rapidly worsening. AAO specifically advises urgent care for a sudden increase in one pupil’s size. Cleveland Clinic advises emergency evaluation when unequal pupils occur with sudden new pain or vision loss.

Call emergency services or go to an emergency department for anisocoria accompanied by any of these warning signs:

  • Severe or sudden headache
  • New double vision, blurred vision or reduced vision
  • Drooping eyelid or trouble moving the eye
  • Weakness, numbness, confusion or loss of coordination (ataxia)
  • Fever or neck stiffness
  • Recent head or eye trauma
  • Possible drug or chemical exposure

After trauma, exposure or newly used eye drops, seek prompt professional advice instead of repeatedly measuring at home. Bring the product name or exposure details, the time of onset and any photographs or measurements. A normal-looking repeat photograph does not cancel a concerning symptom.

Common measurement mistakes and how to avoid them

Shining a light directly into the eye

A phone flashlight or camera flash can cause discomfort and changes in pupil size, and a bright beam is an unnecessary safety risk. Use gradual room-light changes only. Clinical light testing is performed with appropriate technique and interpretation.

Measuring while looking at the ruler

Near focus can change pupil behavior. Keep the gaze on a distant target while the observer places and reads the scale from the side. If the subject keeps refocusing on the ruler, discard that reading and allow the eyes to settle before trying again.

Pressing the eyelid or touching the eye

Pressure can distort the eyelid and make the scale harder to read. Hold the ruler beside the eye without contact, and stop if the person feels pain or irritation. Never use the ruler to lift an eyelid or probe around the lashes.

Trusting one image

One photograph captures one lighting setup and one camera geometry. Repeat observations under comparable conditions and look for a consistent pattern over time. Keep the original file when possible so its date and time remain available.

Recording only the difference

Write both approximate diameters, the lighting condition and the time. A clinician needs the context, not just a statement such as “one millimeter different.” Note which side appeared larger and whether that changed between light and dark, while recognizing that the home observation cannot prove which side is abnormal.

Turning a screening check into a delay

The purpose of a home check is to communicate an observation. If there is sudden anisocoria, pain, vision loss or a neurologic symptom, arranging repeated measurements should never postpone urgent evaluation.

What to take to an eye-care professional

  • A dated list of right- and left-eye estimates in ordinary and dimmer light
  • Notes on whether both pupils appeared round and changed together
  • Photographs taken without direct flash, if available
  • The time the difference was first noticed and whether it is changing
  • All prescription medicines, over-the-counter medicines, recreational substances and eye drops used recently
  • Details of any head or eye injury, chemical contact or severe illness
  • Every symptom, including headache, pain, vision change, drooping eyelid, double vision, weakness, numbness, fever, neck stiffness or confusion

The clinician can then decide whether observation, a full eye examination, neurologic evaluation or emergency treatment is appropriate. If you are unsure whether a symptom is urgent, describe it when you call rather than relying on a home measurement to triage yourself.

Bottom line for a safe home check

Use a transparent millimeter ruler or pupil gauge beside the eye, keep the gaze distant, compare both eyes in comfortable bright and dim conditions, and record the context. Treat every result as an estimate. Do not shine a bright beam into the eye, and do not use a ruler or phone photograph to clear a new, persistent or symptomatic difference. Sudden anisocoria—particularly with pain, vision loss, headache, double vision, drooping eyelid, weakness, confusion or recent trauma—needs urgent medical evaluation.

Frequently Asked Questions

What size should my pupils be?

There is no single size that is normal in every person or lighting condition. Pupils constrict in bright light and dilate in dim light, so compare both eyes under the same conditions and have a clinician interpret a persistent or symptomatic difference.

Can I measure my pupils with my phone?

A phone photograph can document an apparent difference, but camera distance, perspective, exposure and lighting prevent it from being a dependable standalone millimeter measurement. Do not use the phone flashlight or flash directly into the eye.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is a small difference between pupils always dangerous?

No. AAO reports that about 20 percent of people have normally different pupil sizes, but a new or changing difference still needs medical assessment, especially with pain, vision changes or neurologic symptoms.

When should unequal pupils be treated as an emergency?

Seek emergency care for a sudden increase in one pupil’s size, or for unequal pupils with sudden pain, vision loss, severe headache, double vision, drooping eyelid, trouble moving the eye, weakness, numbness, confusion, fever, neck stiffness, ataxia or recent trauma.

Can I check whether my pupils react to light at home?

You may observe whether both pupils appear to change when room illumination changes gradually, but do not shine a flashlight or phone beam into the eye. Home observation cannot match the controlled light and near-focus testing of a clinical PERRLA examination.

Which pupil is abnormal when they are different sizes?

A ruler or photograph cannot determine whether the larger or smaller pupil is abnormal. Comparing the appearance in light and dark can provide useful information for a clinician, but identifying the cause requires professional examination.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should I keep measuring after an eye injury or chemical exposure?

No. After head or eye trauma, drug or chemical exposure, or newly used eye drops, seek prompt professional advice instead of repeatedly measuring. Bring the exposure details, timing and any measurements or photographs.

Is a pupil-gauge or millimeter ruler a precise medical test?

No. It is an inexpensive screening aid that gives an approximate diameter. Viewing angle, ruler position, observer, room light and patient factors affect the result, and it cannot measure reactivity or identify a cause.

The Bottom Line

A home pupil check can document an approximate, repeated difference with a transparent millimeter ruler, but it cannot diagnose anisocoria. Compare both eyes in comfortable light and dimmer light, avoid direct beams, and seek urgent care for sudden change, pain, vision loss or neurologic symptoms.
