Measure an AI system over unstructured data as two connected funnels: first determine whether retrieval finds the right evidence, then determine whether generation turns that evidence into a complete, relevant, supported answer. Connect those quality results to a controlled business baseline, include every operating and failure cost, and keep monitoring after launch. There is no universal accuracy score or payback percentage; the right result depends on the task, corpus, users and risk of being wrong.

What you are actually measuring

A chatbot that answers from policies, contracts, tickets, manuals or reports can fail before the language model writes anything. The search layer may omit the governing clause, rank an obsolete version first or return pages that merely contain matching words. A response can also fail after retrieval: it may misread a passage, omit a required condition, combine conflicting documents or state an unsupported claim with confidence.

Keep these failure points separate in your scorecard. Retrieval metrics answer “Did the system find the evidence?” Generation metrics answer “Did the answer use that evidence correctly?” Business metrics answer “Did the resulting task become more valuable, faster or less costly than the previous process?” A blended score can conceal a critical failure, so publish the component results and the business baseline together.

NIST identifies accuracy, robustness, bias, interpretability, transparency, reliability, privacy, safety and security as distinct characteristics whose importance changes with context. Its AI Measurement and Evaluation program states: “The development and utility of trustworthy AI products and services depends heavily on reliable measurements and evaluations of underlying technologies and their use.” Treat that as a design requirement, not a slogan.

Start with a representative, versioned evaluation set

Your measurements are only as credible as the examples behind them. Build the set from real production-like questions and documents, while removing personal or confidential data that evaluators do not need. Freeze a corpus and index version for each test run so a score change can be traced to a model, prompt, parser, chunking rule or data change.

Include the hard parts of unstructured data

  • Long documents in which the answer is far from the matching heading.
  • Tables, footnotes, forms and scanned pages that require OCR.
  • Different metadata, naming conventions and access-control labels.
  • Duplicate files, superseded policies and genuinely conflicting versions.
  • Multilingual content and queries that use synonyms rather than document wording.
  • Edge cases, ambiguous questions, out-of-scope requests and questions for which the corpus contains no answer.

Label evidence and required answer content

For every query, label the relevant documents or chunks. If several passages are acceptable, record all of them and note whether one is authoritative. Write the answer elements that must appear, including qualifiers such as dates, thresholds, exceptions and responsible teams. NIST’s machine-generated-report framework calls these required elements “nuggets of information” and evaluates both completeness and accuracy, along with whether citations support the claims.

Record the query’s risk tier and intended user. A missing detail in an internal brainstorming answer is not equivalent to a missing exclusion in a safety procedure. Stratifying the set lets you report performance for high-risk cases instead of allowing a large volume of easy questions to hide them.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Protect the test set from accidental leakage

Keep a development set for prompt and retrieval tuning and a held-out set for the final comparison. Do not repeatedly tune against the same examples and then present that score as general accuracy. When documents change, retain the prior corpus version and rerun a stable “anchor” set so you can distinguish a system improvement from a content change.

Measure retrieval before judging the prose

Run the retriever alone against the labeled queries. Save the ranked documents or chunks, their scores and the index version. These measurements tell you whether the generator had a fair chance to answer.

Metric What it answers How to use it
Document or chunk recall Was at least one labeled relevant item returned? Report recall at the cutoff supplied to the model (for example, top-k) and separately for each risk tier.
Precision How much of the returned context is relevant? Use when relevance labels cover the returned items; low precision wastes context and can introduce distractions.
Rank-sensitive NDCG Are the most useful items near the top? Give higher credit when authoritative or highly relevant passages appear earlier. Microsoft Foundry’s document-retrieval evaluator reports NDCG among its search-quality measures.
Fidelity to known relevant material Does the retrieved set preserve the information needed for the task? Use the labeled evidence and required nuggets to identify partial coverage, not just a binary hit.
No-evidence failure count How often does retrieval return no required evidence? Track explicitly. The correct behavior for an unanswered question is a clear “not found,” not a plausible completion.

Inspect misses by cause: OCR failure, bad chunk boundary, stale index, synonym mismatch, access filtering, duplicate suppression or a ranking error. A higher top-k may raise recall while flooding the context with irrelevant passages; evaluate the trade-off rather than optimizing one number in isolation.

Score generation for groundedness, relevance and completeness

Evaluate each response against both the question and the exact context supplied to the model. Microsoft separates reference-based, reference-free, human or LLM-judged and RAG metrics; its catalog includes groundedness, relevance, retrieval and document-retrieval evaluators. Automated judges are useful for scale, but they need calibration against human decisions, especially for high-risk answers.

Faithfulness or groundedness

Check whether every material claim follows from the retrieved passages. A response can be fluent and still fail by inventing a number, extending a rule beyond its stated scope or blending two documents that do not apply together. Score unsupported claims separately from minor wording differences, because one unsupported safety instruction may outweigh several correct sentences.

Answer relevance

Determine whether the response addresses the user’s question directly, with the requested level of detail and no distracting material. A grounded answer that discusses a related policy instead of the requested one is still a task failure.

Context relevance

Assess whether the retrieved material is useful for answering the question rather than merely lexically similar or redundant. Low context relevance often explains hallucinations: the model is asked to reason from a context that does not contain the needed rule.

Completeness against required nuggets

Mark each required answer element as present, correct and appropriately qualified. This catches answers that cite the right document but omit an exception, effective date, approval step or limitation. Report the proportion of required nuggets covered and the rate of materially incorrect nuggets; do not let a long answer receive a high score for including only one correct fact.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Citation support

Validate that each citation points to the passage that supports the associated claim, not simply to a document with a similar title. Measure citation coverage and citation correctness separately. A citation that cannot be opened by the intended user is an operational failure even when the underlying passage is correct.

Use human review to calibrate automated scores

Create a review rubric before looking at results. Reviewers should label retrieval sufficiency, groundedness, relevance, completeness, citation support and the severity of any error. Include a “cannot answer from the corpus” outcome so reviewers do not reward confident guessing. For high-risk samples, require reviewers to inspect the source passage rather than judging the response alone.

Double-review a sample and resolve disagreements with an adjudicator. Compare automated evaluator decisions with the adjudicated labels, then adjust prompts, thresholds or escalation rules when the judge systematically misses subtle contradictions, unsupported inferences or citation problems. Keep a human-accepted accuracy measure in the dashboard; it is more interpretable than an uncalibrated judge score.

Measure the existing process first. For the same task mix, record volume, handling time, queue time, escalation rate, rework, error correction, user satisfaction and the business outcome that matters. Then run the AI process and compare like with like. A time saving is not automatically a benefit: monetize it only if the saved capacity produces additional throughput, avoids labor or reduces a measured delay.

Benefits that can be monetized when the baseline supports attribution

  • Time saved per completed task multiplied by the proportion that becomes usable capacity.
  • Higher throughput or faster service-level completion.
  • Revenue lift attributable to the assisted workflow.
  • Fewer escalations, avoided rework and lower correction effort.
  • Reduced loss when a documented control prevents a measurable error.

Separate direct savings from capacity released for other work. State the attribution window and assumptions, and report uncertainty rather than presenting an unqualified payback claim.

Costs that belong in the denominator

  • Model inference and embedding calls, including retries and evaluation runs.
  • Retrieval, vector or index hosting and other infrastructure.
  • Ingestion, parsing and OCR for new or changed documents.
  • Labeling, reviewer time and ongoing quality sampling.
  • Integration, access-control work, monitoring and observability.
  • Human review, overrides, incident response and correction of bad answers.
  • Change-management, training and support labor.

Also report cost per completed task, not only aggregate spend. A system that is inexpensive per query but requires frequent human correction may cost more per successful outcome than a slower, better-grounded alternative.

Run a controlled baseline-versus-AI pilot

  1. Define the task and success event. Specify what counts as completion, which errors are unacceptable and which business outcome will be monetized.
  2. Measure the baseline. Use the same task mix and capture time, quality, escalations, throughput and outcome data before AI assistance.
  3. Freeze the comparison corpus. Keep document and index versions fixed during the main comparison, while recording any unavoidable content change.
  4. Randomize where practical. Randomize users or tasks to the existing process and AI-assisted process. If randomization is impossible, use matched cohorts and document the differences.
  5. Apply the same quality rubric. Have reviewers assess both groups for correctness, completeness and severity, not just speed.
  6. Segment the result. Report document type, difficulty, user group and risk tier. Aggregate averages can hide a serious failure in one segment.
  7. Report uncertainty. Include sample sizes, confidence intervals or other uncertainty estimates appropriate to the experiment, and show how sensitive ROI is to disputed assumptions.

Databricks guidance for RAG over unstructured data recommends tracking response accuracy, latency, cost and stakeholder ratings while logging intermediate retrieval steps. Those intermediate records are essential when a poor answer could be caused by search, generation or a human workflow rather than by one undifferentiated “AI quality” score.

Instrument production so regressions are diagnosable

For every request, retain the minimum trace needed to reproduce and explain the result, subject to privacy and retention rules:

  • Query, user or cohort identifier and access context.
  • Corpus, parser and index versions.
  • Retrieved documents or chunks, ranks and scores.
  • Prompt template, model version and generation settings.
  • Response text and every citation shown to the user.
  • Latency by stage, token usage and infrastructure or model cost.
  • User feedback, accepted or rejected suggestions, overrides and corrections.

Redact sensitive content or store secure references where full text is not necessary. Define retention and access controls before enabling detailed traces; observability must not create a new privacy problem.

Set actionable alerts

Alert on retrieval misses, unsupported-claim rate, citation failures, latency, cost per completed task and drift in the business outcome. Thresholds should be tied to the risk tier and baseline, not copied from another deployment. Keep bug severity, response time and repair time in the incident record; NIST’s measurement playbook treats those reliability measures as part of evaluation.

Monitor the data as well as the model

Track ingestion failures, OCR quality, document freshness, duplicate growth, permission-filter errors and the proportion of queries outside the indexed corpus. A stable model can appear to regress when a source system changes its export format or when a new policy version is not indexed.

Common measurement mistakes and fixes

Using answer accuracy as the only metric

A correct-looking answer can come from the wrong source, and a retrieval miss can be hidden by a model’s general knowledge. Add retrieval labels, groundedness checks and citation validation.

Testing only clean, short documents

Production corpora contain scans, tables, duplicates, conflicting versions and multilingual material. Sample those conditions deliberately and report segment results.

Counting every time saving as realized ROI

Distinguish capacity released from cash or revenue actually realized. Include correction, review and incident labor before converting time into money.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Tuning and evaluating on the same questions

Maintain separate development and held-out sets, retain anchor cases and version every corpus, prompt and model change.

Ignoring “no answer” behavior

Label questions with no supporting evidence and reward a transparent abstention. Otherwise the system is incentivized to fill gaps with plausible but unsafe text.

Logging only the final response

Without retrieved chunks, versions, latency and costs, you cannot tell whether a regression came from ingestion, ranking, prompting or generation. Preserve the complete trace needed for diagnosis while enforcing privacy controls.

A practical reporting template

For each release or pilot, report:

  • Use case, users, risk tier and success event.
  • Corpus, index, model and prompt versions, with evaluation dates.
  • Test-set composition and the labeling method for relevant evidence and required nuggets.
  • Retrieval metrics and no-evidence failures by segment.
  • Groundedness, relevance, completeness, citation and human-accepted accuracy, including error severity.
  • Baseline and AI task volume, time, throughput, escalations, rework and business outcome.
  • All incremental costs, cost per completed task, benefit assumptions and incremental ROI.
  • Latency, reliability, privacy or safety findings, override burden and open remediation items.

Repeat the evaluation after material changes to documents, parsers, chunking, retrieval, prompts, models or access controls. For higher-risk deployments, combine model testing, red teaming and user testing; NIST’s ARIA planning manual identifies those three testing types as the components of an ARIA-style evaluation.

How to interpret the result

A credible conclusion is conditional: this system achieved specified retrieval and answer-quality results on a named corpus and task mix, at a measured cost and latency, with stated human-review and risk controls. It is not a universal claim that AI over unstructured data is accurate or profitable. If retrieval coverage is weak, fix ingestion, chunking, metadata or ranking before tuning prose. If evidence is found but answers are unsupported or incomplete, improve prompting, citation constraints, abstention and review. If quality is acceptable but ROI is negative, examine task selection, adoption, workflow integration and the full cost of supervision before expanding.
