Measure sickness absence and presenteeism as two separate losses, then state exactly what your money estimate includes. For absence, multiply excess sickness-related hours or days by a documented compensation cost. For presenteeism, estimate health-related impairment during hours worked with a validated, job-appropriate instrument, convert the impaired time to a monetary value, and label the result as a modelled estimate rather than observed output. Add disability, replacement, turnover, or coworker effects only when they are in scope and are not already counted.

A credible result is therefore a range or transparent estimate with assumptions—not a single survey percentage presented as an exact bill. Team work, recovery of missed work, job demands, employee mix, survey design, and the wage or compensation basis can materially change the answer.

What you are measuring

“Absenteeism” and “presenteeism” describe different mechanisms. Sickness absence is time an employee does not work because of their own illness, injury, or another medical condition. Presenteeism is reduced capacity while the employee is present. An employer that counts only absence can miss substantial loss from people who attend work but cannot perform normally.

The boundary of the calculation determines the answer. A narrow employer-accounting view may include paid sick time and direct replacement costs. A productivity view estimates work not completed or completed more slowly. A broader economic view can include disability, turnover and replacement, delayed work, and effects on coworkers or teams. Keep those categories visible instead of combining them into an unexplained total.

The World Health Organization identifies sickness absence, presenteeism, and staff turnover among workplace consequences associated with mental-health conditions. Its 2022 guidance estimates that 15% of working-age adults have a mental disorder at any point in time; that is context, not a cost rate for your workforce.

Set the cost boundary before collecting data

Write a one-page measurement specification before opening payroll files or sending a survey. It should answer these questions:

  • Perspective: Are you estimating the employer’s direct outlay, lost productivity, or a wider economic burden?
  • Population: Which employees, locations, employment statuses, and job families are included?
  • Period: What start and end dates apply, and how will partial-year employees be handled?
  • Health definition: Does “sickness” mean the employee’s own illness only, or also caregiving, injury, mental-health symptoms, and work restrictions?
  • Cost categories: Will you include disability absence, overtime, temporary staff, turnover and replacement, or coworker spillovers?
  • Valuation: Will time be valued using base salary, total compensation, or another documented rate?

ACOEM’s expert panel recommended absenteeism, presenteeism, and employee turnover or replacement costs as core elements of workplace health-related productivity measurement. Treat them as separate fields so that a replacement cost does not get added again as a second estimate of the same lost hour.

A practical measurement workflow

1. Define the unit of analysis

Choose hours, days, episodes, or full-time-equivalent (FTE) time. Hours are useful when schedules vary; days are simpler when payroll records are day-based. Record the denominator—employees, FTEs, or scheduled work hours—so rates from different teams can be compared.

2. Gather consistent absence records

Use payroll, time-and-attendance, occupational-health, or case-management records where permitted. Separate the employee’s own sickness from caregiving, transport problems, weather, and other non-health causes when the question is specifically sickness cost. Retain the absence reason, duration, partial-day status, and whether the absence was paid or unpaid.

For population surveillance, NIOSH’s Current Population Survey method tracks full-time workers who worked fewer than 35 hours in the reference week because of their own illness, injury, or another medical issue. NIOSH states: “NIOSH monitors absences reported by full-time workers due specifically to their own illness, injury, or other medical issue.” This is a surveillance definition, not a complete employer-cost formula.

3. Measure presenteeism independently

Ask employees about health-related work limitations during a defined recall period, such as the previous two or four weeks. Capture the condition or symptom, hours worked while affected, and the degree or frequency of limitation. Do not infer presenteeism from attendance records: an employee can be present for every scheduled shift and still have substantial impairment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Convert both measures to money

Apply the selected compensation or output valuation only after the time measures are clean. Keep the formula, rate, period, and assumptions in the report so another analyst can reproduce the result.

5. Test alternative scenarios

Calculate at least a conservative, central, and high scenario. Vary whether missed work is recovered, whether coworkers absorb it, and whether the compensation basis is salary alone or total compensation. Report the resulting range rather than hiding uncertainty behind a single point estimate.

How to calculate the cost of sickness absence

Use an excess-hours or excess-days formula

A transparent starting equation is:

Estimated absence cost = excess sickness-related hours (or days) × compensation cost per hour (or day).

“Excess” must be defined. It can mean hours above a historical baseline, above a matched low-absence group, or above an internal target. State the comparison period and adjust for changes in headcount, seasonality, working time, and job mix.

Choose and document the compensation rate

Decide whether the rate includes base pay only, or wages plus employer taxes, benefits, and other compensation. Use the same basis for every included group, or explain why different rates are necessary. For salaried staff, derive an hourly value from the agreed annual work hours rather than assuming every employee works an eight-hour day.

A CDC employer-cost study used excess missed workdays multiplied by daily compensation for an eight-hour workday. Its historical U.S. input was $30.45 per hour in 2011, adjusted to $33.00 in 2015 dollars. Those are study inputs, not current wage rates; replace them with rates appropriate to your workforce and measurement year.

Account for what happens to the work

The missed hour can have different consequences:

  • Lost work: The task is delayed or never completed during the period.
  • Recovery: The employee makes up the work later, creating overtime or schedule pressure.
  • Transfer: A colleague performs the task, potentially creating coworker absence, fatigue, or delay.
  • Replacement: A temporary worker or contractor is hired at a different rate.

Model these paths separately. A compensation-based estimate of the absent employee’s time is not automatically equal to the value of delayed sales, missed service, or reduced team output.

Worked example

Suppose a 200-person department records 1,600 excess sickness-related hours in a quarter. If total compensation is valued at $42 per hour, the direct time estimate is 1,600 × $42 = $67,200. That figure does not yet include overtime, temporary labor, lost revenue, or coworker effects. If 400 hours are later recovered through overtime costing $58 per hour, report the $23,200 recovery cost as a separate component rather than replacing the original time estimate without explanation.

How to measure presenteeism at work

Define impairment, not simply attendance

Presenteeism questions should connect a health problem or symptom to work performance. Useful prompts ask how often the employee worked while affected, how many hours were affected, and whether specific activities—concentration, physical tasks, communication, decision-making, or safety-critical actions—were limited. A broad question such as “What percentage productive were you?” is difficult to interpret without a recall period and a defined work context.

Use a suitable instrument

The WHO Health and Work Performance Questionnaire (HPQ) is a brief self-report instrument covering health problems and workplace consequences, including sickness absence, presenteeism, and critical incidents. Its methodological literature also describes challenges in translating nonexperimental self-report data into illness costs or cost-effectiveness results.

Other instruments used in the literature include the Work Limitations Questionnaire (WLQ), Work Ability Index (WAI), and Work Productivity and Activity Impairment questionnaire (WPAI). No single tool is established as universally best. Select one using the following comparison:

Selection axis Questions to answer
Construct Does it measure absence, presenteeism, work limitations, safety incidents, or a combined set?
Evidence Is reliability and validity support available for your population, language, and job types?
Work-setting fit Do the items reflect physical, cognitive, customer-facing, remote, shift, or team-based work?
Decision value Will the results inform the intervention, staffing decision, or business case you actually need to make?
Practicality What are completion time, survey frequency, response rate, privacy, and data-governance requirements?
Valuation compatibility Can the output be linked to your hours, compensation basis, and team-recovery assumptions without double counting?

Design the survey to reduce bias

  • Use a fixed recall period and keep it the same at every measurement wave.
  • Explain that responses are for aggregated analysis, not individual performance ratings.
  • Collect job family, work pattern, and hours worked only to the level needed for analysis.
  • Track response rates and compare respondents with the eligible workforce where lawful and feasible.
  • Report missing responses and avoid silently treating a missing answer as zero impairment.
  • Repeat the survey at a consistent interval if you need trends, while avoiding unnecessary survey fatigue.

Monetizing presenteeism without overstating precision

A common model is:

Estimated presenteeism cost = affected hours × estimated impairment proportion × compensation cost per hour.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For example, 500 affected hours, an average reported impairment of 20%, and a $42 compensation rate produce 500 × 0.20 × $42 = $4,200. This is a modelled value, not a direct measurement of four thousand two hundred dollars of output. It assumes that the impairment percentage maps proportionally to paid time and that compensation is a reasonable proxy for output.

That assumption can fail in team production, variable-output jobs, safety-critical work, creative roles, and positions where one decision has disproportionate consequences. A person may complete routine tasks while creating rework, or a short safety lapse may matter more than several hours of slower work. Where operational metrics exist—cases completed, defects, service-level breaches, incidents, or rework—use them to test whether survey results move with observable outcomes. Do not claim that correlation proves causation.

Include wider costs only when the boundary permits them

Disability and restricted work

Long-term disability, partial restrictions, and return-to-work arrangements can be included when the analysis covers them. Separate time fully absent from time working with restrictions so the same hours are not counted as both absence and presenteeism.

Turnover and replacement

Turnover can follow health-related strain, but it is not automatically caused by a recorded sickness episode. If included, specify the attribution rule, vacancy duration, recruiting and training costs, and the period over which replacement costs are recognized. ACOEM lists turnover or replacement as a core productivity element, but the evidence and assumptions still need to be shown.

Coworker and team spillovers

When work is transferred, estimate the receiving employee’s additional time or delay separately. In tightly coupled teams, one absence may reduce the output of several people; in modular work, the task may be postponed with little immediate cost. Use scenario analysis if you cannot observe the spillover directly.

Control double counting and uncertainty

Before publishing a total, reconcile every component in a worksheet. Medical claims, statutory benefits, paid absence, lost productivity, overtime, replacement, and turnover may refer to overlapping events. Decide whether each item is a cash outlay, a transfer, or an estimated economic loss, and report the perspective.

Show the inputs that drive the result:

  • Number of eligible employees or FTEs and the observation period.
  • Definition and baseline used to calculate “excess” absence.
  • Hours or days absent, affected hours present, and treatment of partial days.
  • Compensation basis and currency, with the measurement year.
  • Survey instrument, recall period, response rate, and scoring method.
  • Assumed relationship between impairment and output.
  • Recovery, replacement, coworker, disability, and turnover scenarios.

Use sensitivity analysis to show which assumptions matter most. A range driven by two plausible recovery rates is more useful than false precision to the nearest dollar.

Historical figures: useful context, not current budgets

Older estimates illustrate why methods differ, but they should not be copied into a current employer budget. A 2004 synthesis reproduced in a NIOSH appendix estimated $392 per eligible employee per year for hypertension and $348 for depression and other mental illnesses, based on its prevalence and average-impairment assumptions. These condition-specific figures vary by method and are not universal benchmarks.

The CDC study’s national modelling used 139 million employed people in the United States in 2011. Its scenarios were built from historical inputs and examined whether work was recovered or spilled over to coworkers. Neither that population count nor its dollar assumptions represents a current workforce. Update every input and check comparability before using an older study as a benchmark.

How to report the result

A decision-ready report can use this order:

  1. Scope: population, period, perspective, and included categories.
  2. Absence result: sickness hours or days, baseline, compensation rate, and direct estimate.
  3. Presenteeism result: instrument, response statistics, affected time, impairment estimate, and valuation formula.
  4. Additional scenarios: recovery, replacement, disability, turnover, and coworker effects.
  5. Uncertainty: conservative, central, and high assumptions with sensitivity results.
  6. Interpretation: what the estimate can support, what it cannot establish, and which operational measure should be monitored next.

This format lets finance, human resources, occupational health, and operations see the same calculation without confusing an absence ledger with an estimate of lost output.

Common mistakes and fixes

  • Counting attendance as productivity: Add a health-related work-limitation measure for employees who worked while impaired.
  • Using a survey percentage as a bill: State the recall period, scoring, wage basis, and proportional-output assumption.
  • Mixing causes of absence: Separate own illness from caregiving and non-health reasons when sickness is the target.
  • Reusing old national rates: Replace historical wages, populations, and prevalence inputs with current, comparable data.
  • Ignoring team recovery: Model work lost, made up, transferred, and replaced as distinct scenarios.
  • Adding overlapping costs: Map each cost to an event and remove duplicate hours or replacement charges.
  • Choosing a tool by popularity alone: Check scientific evidence, setting fit, decision usefulness, and administration burden.

The strongest estimate is not the largest number. It is the one whose boundary, data, assumptions, and uncertainty are explicit enough for another person to reproduce and challenge.
