Measure incident response as a chain of separately timed events, not one “response time” number. At minimum, record when the organization detects an incident, begins investigating an alert, contains the threat, restores normal service, and completes remediation. Define the start and stop event for every interval before calculating it, preserve the case-level timestamps, and report averages with medians and ranges. This approach shows where work actually takes time and prevents a fast-looking metric caused by delayed declaration or premature closure.
The terminology below follows the event boundaries in CISA’s FY 2025 CIO FISMA Metrics, with measurement and recovery practices informed by NIST guidance. CISA’s definitions are a federal reporting reference, not a universal private-sector performance target.
Why a single response-time number is misleading
An alert may arrive minutes after an attacker first gains access, while restoration may take hours and full remediation may continue for weeks. Calling all of those elapsed times “response time” hides the handoffs and decisions that matter. A single average can also improve for the wrong reason: analysts might postpone declaring an incident, close a ticket before recovery is complete, or omit cases with missing timestamps.

Use a timeline with named milestones. Each metric should be a documented pair of events, such as “incident detected” to “complete recovery,” rather than an informal label. Keep the original event records so that a reviewer can reconstruct how the duration was produced.

- Detection: when the organization discovers or detects an incident through automated or manual means.
- Identification or investigation: the interval associated with receiving and investigating an alert. CISA calls the average of this interval mean time to identify.
- Containment: when the spread or active impact is brought under control. Keep this as a local measure when the evidence is reliable and state the exact definition.
- Recovery: the time from incident start until complete recovery to normal operations.
- Resolution: the time from incident start through full remediation, recurrence-prevention work, and post-incident analysis.
These stages answer different management questions. Detection exposes monitoring gaps; investigation measures triage capacity; containment shows whether the threat was stopped; recovery shows service resilience; and resolution shows whether the underlying cause and recurrence risk were addressed.
Use explicit event boundaries for every metric
The first design decision is the clock. Write the event definition in plain language, identify the system that records it, and state what is excluded. The following model gives each interval a distinct purpose.
| Measure | Clock start | Clock stop | What it tells you |
|---|---|---|---|
| Mean time to detect (MTTD) | Chosen incident-start event, such as the earliest corroborated malicious activity | Organization discovers or detects the incident | How long the incident existed before discovery |
| Alert-to-investigation interval (often called mean time to identify) | Alert received by the organization | Investigation begins, using a defined action such as analyst acknowledgement or case creation | How quickly an alert enters human or automated investigation |
| Containment time | Detection or another documented local start event | Threat activity or spread is controlled according to the case definition | How long it takes to limit active impact |
| Mean time to recover (MTTR) | Incident start | Complete recovery to normal operations | How long the affected service or process takes to return to its defined normal state |
| Mean time to resolve (MTTRv or “time to resolve”) | Incident start | Full remediation, including recurrence prevention and post-incident analysis | Total time until the organization considers the incident fully addressed |
CISA defines mean time to detect as the average time for an organization to discover or detect an incident, and defines mean time to recover as the time from incident start until complete recovery to normal operations. Its mean-time-to-resolve definition extends through full remediation, prevention of recurrence, and post-incident analysis. The definitions and requested units (days, hours, or minutes) appear in the FY 2025 metrics form linked above.
The true beginning of an incident is often unknowable. If you use the first confirmed malicious event, the first affected asset, or the time an incident is formally declared, record that choice and its limitations. Do not present a declaration-based measure as if it captured the attacker’s dwell time.
Create a measurement dictionary before collecting data
A short data dictionary prevents two analysts from calculating different numbers under the same label. Store it with the metric definition and version it when the process changes.
| Field | Required decision |
|---|---|
| Metric name | Use one stable label, such as “containment duration” or “incident-start-to-recovery.” |
| Clock start and stop | Name the exact event, not merely “opened” or “closed.” State whether the timestamp is inclusive or exclusive at the boundary. |
| Unit and precision | Choose minutes, hours, or days. Keep the underlying timestamp precision even if the report rounds it. |
| Timezone | Store timestamps in UTC or include an explicit offset. Never mix local times without conversion. |
| Authoritative source | Identify the sensor, ticket field, case-management record, service monitor, or change record that supplies each event. |
| Inclusion rules | Specify incident types, severity levels, services, and reporting period. State whether duplicate alerts become one incident. |
| Paused intervals | Decide whether waiting for a vendor, customer, maintenance window, or business approval counts toward elapsed time. Report gross elapsed time separately from any net working-time measure. |
| Unknown or corrected timestamps | Mark estimates and late entries, retain the original value, and record who changed it and why. |
| Reopened cases | Define whether reopening extends the original resolution clock or creates a linked follow-up incident. |
| Quality status | Flag automated, human-entered, inferred, and missing events so readers can judge confidence. |
Do not silently substitute a convenient timestamp. For example, a ticket creation time may be later than the alert receipt, and a “resolved” status may precede restoration of every dependent service.

Build the incident timeline step by step
- Choose the population. State the reporting period, incident classes, severity scope, and whether the analysis covers all incidents or only those with complete records. Keep excluded cases in a separate list; exclusion is itself a data-quality result.
- Establish the incident-start event. Select the earliest defensible event available for your MTTD and recovery/resolution clocks. If the start is uncertain, store an uncertainty flag and describe the proxy used.
- Capture detection. Record the first automated or manual discovery that meets your detection definition. Preserve the source event ID and its original timestamp.
- Capture alert receipt and investigation start. If your operation reports an alert-to-investigation metric, record when the alert reached the organization and when investigation actually began. An acknowledgement alone may not equal substantive investigation, so define the action that qualifies.
- Capture containment. Use an observable condition, such as blocked command-and-control traffic, isolated affected hosts, or a verified end to unauthorized access. Pick the condition that matches your incident types and apply it consistently.
- Capture recovery. Define “normal operations” for the affected service or business process. A server being online is not sufficient if transactions, dependencies, data integrity, or user access remain impaired.
- Capture resolution. Record completion of eradication, control changes that prevent recurrence, required validation, and the post-incident analysis. If those activities continue after restoration, resolution must remain later than recovery.
- Reconcile the records. Compare security tooling, service monitoring, identity logs, change records, and case notes. Resolve timezone differences and document every adjustment rather than overwriting the evidence.
Use one incident identifier across these records. When several alerts refer to the same event, link them to a parent incident and retain the alert-level timestamps; otherwise, alert volume can distort investigation-time calculations.

One free scan finds every outdated or missing driver and matches the right update for your exact hardware.Free scan · exact hardware matchCalculate durations consistently
For each incident and metric, subtract the declared start timestamp from the declared stop timestamp:
elapsed duration = stop timestamp − start timestamp
Convert every result to the same unit before aggregation. CISA’s FY 2025 form accepts days, hours, or minutes, but a mixed-unit spreadsheet makes trends difficult to interpret. Keep full precision in the calculation and round only for display.
Use the mean without losing the distribution
The arithmetic mean is the sum of incident durations divided by the number of incidents. It preserves continuity with “mean time” labels, but one exceptionally long investigation can dominate it. Report the median—the midpoint after sorting—as well as a range or selected percentiles. Retain every case-level value so an executive can see whether a change reflects broad improvement or one outlier.
NIST’s SP 800-55v1 Measurement Guide for Information Security, dated December 2024, describes mean and median and recommends historical averages and acceptable ranges. It also warns that numbers alone may not show the whole story when the quality of the work behind them is poor.

Illustrative calculation
Suppose three incidents have incident-start-to-recovery durations of 4, 10, and 34 hours. The mean is 16 hours (48 divided by 3), the median is 10 hours, and the range is 4–34 hours. The mean communicates a single trend value; the median and range reveal that one case took far longer. This example is arithmetic only, not a performance benchmark.
Report a timeline, not just a score
A useful monthly or quarterly report contains both aggregate statistics and the evidence needed to interpret them.
- Number of incidents included, excluded, and missing one or more required timestamps.
- Mean, median, minimum, maximum, and selected percentiles for each interval.
- Case-level links or identifiers for unusually long and unusually short durations.
- Breakdowns by incident type, severity, affected service, detection path, and operating context.
- Counts of estimated, manually entered, corrected, and automatically generated timestamps.
- Outcome measures: containment success, service restoration, customer or mission impact, recurrence, and evidence completeness.
Put the definition and reporting period beside every chart. A graph labeled “MTTR” is ambiguous unless it says whether it means recovery to normal operations or full resolution.
Make comparisons fair
Do not compare unlike work. A malware outbreak affecting a single workstation, a cloud identity compromise, and a critical production outage have different dependencies and approval paths. Segment results when those differences change expected handling.
Useful comparison dimensions
- Incident type: phishing, ransomware, unauthorized access, data exposure, denial of service, or other locally defined categories.
- Severity and business criticality: include the service-level impact and required response commitment.
- Affected service or asset class: production systems, corporate endpoints, identity infrastructure, and third-party dependencies may have different recovery paths.
- Detection path: automated sensor, user report, partner notification, audit discovery, or other source.
- Operating context: business hours, overnight coverage, weekends, planned maintenance, or major release windows.
- Timestamp reliability: separate machine-generated events from human-entered or inferred milestones when their accuracy differs.
These dimensions are practical controls for interpretation, not universal segmentation requirements from NIST. Keep the unsegmented total for continuity, but never use it as the only view.
Prevent fast numbers from rewarding bad behavior
Premature incident declaration
If the clock starts only when an analyst declares an incident, delayed declaration makes detection and recovery appear shorter. Track the declaration time separately and disclose when it is used as a proxy for incident start.

Premature closure
Closing a case after the visible symptom disappears can understate recovery or resolution. Require evidence that normal operations are restored and that remediation and validation are complete for the resolution milestone.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Working time versus elapsed time
A net-hours metric can help capacity planning, but it should not replace gross elapsed time when customers or mission functions are waiting. If you subtract pauses, publish the pause policy and the gross duration alongside it.
Missing and edited timestamps
Do not impute a precise time without labeling it as estimated. Preserve an audit trail for edits, including the reason, editor, and source evidence. Report the proportion of records affected by missing or corrected events.
Reopened incidents and linked cases
A recurrence after closure can either extend the original resolution interval or be treated as a new incident with a linkage. Choose one policy, apply it consistently, and report recurrence separately so a short initial closure cannot conceal repeated failure.
Tool-driven optimism
Automation can produce reliable receipt and action timestamps, but it cannot decide whether “normal operations” or “full remediation” has been achieved. A platform that stores timestamps does not resolve ambiguous definitions.

Neither CISA’s federal reporting definitions nor NIST’s measurement guidance supplies a broadly applicable “good” response-time number. CISA’s framework is designed for agency reporting, and NIST emphasizes organization-relevant measures, historical analysis, and acceptable ranges. Do not import a vendor statistic or a single external number as a universal threshold.
When a target changes, version the definition as well as the number. A tighter target measured with a new start event is not directly comparable with the old series.
Pair speed with recovery and impact outcomes
Shorter elapsed time is valuable only when the work is effective. Pair each timing metric with outcomes that show whether the threat was controlled and the business recovered:
- Containment success and any spread after the containment milestone.
- Service availability, transaction backlog, data-integrity checks, and user impact during recovery.
- Scope and duration of business, customer, safety, or mission disruption.
- Recurrence within a defined follow-up period and whether the corrective control was implemented.
- Evidence completeness and confidence in each timestamp.
NIST’s SP 800-184, Guide for Cybersecurity Event Recovery, published in December 2016, emphasizes prioritizing recovery resources, testing realistic scenarios, maintaining continuity of mission functions, and learning from past events. Use those practices to test whether a timing improvement survives a realistic exercise rather than merely looking better in a dashboard.
Use current guidance without confusing its scope
NIST SP 800-61 Rev. 3 was published in April 2025 and supersedes Rev. 2 from 2012. It integrates incident-response recommendations into cybersecurity risk management and addresses detection, response, and recovery. The publication says that incorporating these recommendations can help organizations prepare for incident responses, reduce the number and impact of incidents, and improve the efficiency and effectiveness of detection, response, and recovery activities.
Use the CISA FY 2025 metric names when they make communication clearer, but keep your local data dictionary authoritative for operational decisions. If a regulator, contract, or sector rule imposes a specific definition, map it explicitly to your internal events instead of assuming that two similarly named metrics are equivalent.
A practical operating cadence
During each incident
- Assign one incident identifier and record the event source for every milestone.
- Capture timestamps as events occur, with timezone and confidence status.
- Mark pauses, handoffs, scope changes, and reopened work in the case record.
- Before recovery closure, verify the service-specific normal-operations criteria.
- Before final resolution, document remediation, recurrence prevention, and post-incident analysis.
After closure
- Reconcile the timeline within a defined review window while source logs are still available.
- Calculate all intervals from the same time standard and retain the raw values.
- Review outliers and missing data with the incident owner rather than deleting them.
- Compare results only with incidents that share the relevant context or show the segmentation explicitly.
- Feed lessons into detection rules, playbooks, staffing, recovery exercises, and service objectives.
This cadence turns response-time measurement into a feedback loop: precise events reveal bottlenecks, outcome checks prevent gaming, and lessons change the controls and plans that determine future performance.
