A smartphone camera can estimate pupil diameter, but only as a carefully controlled observation. Record video with the phone fixed at a constant distance, keep lighting and gaze consistent, calibrate against a known scale when it is safe to do so, and summarize many frames rather than trusting one photograph. The result can document a trend for an eye-care professional; it cannot diagnose an eye or neurological disorder.

What a smartphone measurement can tell you

The visible pupil is the dark opening in the iris. A phone can capture its apparent edge and, with a scale reference or a validated geometric calibration, convert pixels into an approximate millimetre value. That estimate is useful for documenting what you observed under a particular light level and for comparing the two eyes under matching conditions.

It is not equivalent to a clinical pupillometer. Pupil diameter changes continuously, and the camera, lighting, gaze, eyelid position and image processing all affect the boundary that you see. A casual selfie, a single flash photograph or an app that simply draws a circle over one frame can therefore be misleading.

Pupil diameter is not pupillary distance

Pupil diameter is the width of one pupil. Pupillary distance (PD) is the distance between the centres of the two pupils, a measurement used to position eyeglass lenses. A millimetre ruler, mirror or phone app may provide a home estimate of PD, and professional measurement is more accurate, according to Cleveland Clinic, but PD does not tell you how wide either pupil is.

Why video is preferable to one still image

Normal pupils make small, spontaneous changes called pupillary hippus. American Academy of Ophthalmology EyeWiki describes fluctuations of approximately ±0.5 mm and explains that a single snapshot can catch the pupil near its largest or smallest moment. Current pupillographic instruments record and analyse a sequence of frames for this reason. A phone video lets you discard blinks and poor frames and report a median or typical range instead of an accidental extreme.

Prepare a controlled, safe setup

Choose the phone and camera

Use a phone that can record several seconds of video without refocusing constantly. Note the model and whether you used the front or rear camera. Front-facing cameras may include infrared illumination or computational processing that differs between models; do not assume that two phones produce interchangeable measurements. Disable beauty filters, portrait effects and automatic enhancement if the camera provides those controls.

Control the room light

Work in a dim room with steady, indirect illumination. Let your eyes adapt for a short, consistent period before recording, and do not change the light between the left- and right-eye measurements. A bright screen, a flashlight or a camera flash can constrict the pupil during capture, so a flash photograph measures a light response as much as a resting diameter. Avoid sunlight from a window, flickering lamps and a light source positioned so it creates a reflection over the pupil.

Keep distance, angle and gaze fixed

Brace or mount the phone so the camera is perpendicular to the face and remains at a fixed distance. Keep your head still and look at a distant, neutral target rather than at the phone screen. Looking close changes accommodation and can alter pupil size; looking to one side changes the apparent shape through perspective. EyeWiki’s pupillography guidance emphasizes consistent distance and angle for comparable readings.

Use a non-contact scale only when safe

An absolute millimetre value requires a known scale. A flat printed calibration target or a millimetre ruler can be held beside the face, in approximately the same plane as the eye, without touching or obstructing the eye. Do not place a ruler, card or any other object close to the cornea. A reference held nearer or farther than the eye is affected by perspective, so its apparent size cannot be used directly without geometric correction. If you cannot position a reference safely and flat, report relative values and the imaging conditions instead of inventing millimetres.

Step-by-step smartphone workflow

1. Record the conditions before you start

Write down the date and time, phone model, camera used, approximate camera-to-face distance, room-light description and whether you used contact lenses, medication or eye drops. These details matter when you repeat the test or show the result to a clinician. Do not stop or change prescribed medication merely to obtain a different pupil measurement.

2. Set the room and allow adaptation

Use the same dim, steady room for both eyes. Sit comfortably and allow a short, consistent adaptation period. Keep displays, lamps and other bright sources out of the camera’s field of view. If you need to turn on a light to operate the phone, do so before adaptation and return to the same setting before recording.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

3. Mount the phone

Use a stand, tripod or solid support. Place the camera at eye level and perpendicular to the face, then mark the position so the distance does not drift. A helper can operate the recording button from behind the phone. Do not hold the phone at arm’s length for one eye and move closer for the other; that changes the scale and angle.

4. Hold a distant neutral target

Choose a point well beyond the phone and keep both eyes relaxed. Ask a helper to check that your head is not turning and that your eyelids are not covering the pupil. Keep the same target for every clip. If you must measure each eye separately, cover the other eye loosely without pressing on it; pressure or bright light entering the uncovered eye can change the response.

5. Record several seconds of video

Capture both eyes in the same clip when the framing allows it; otherwise make back-to-back clips without changing the setup. Record several seconds rather than one still. Avoid speaking, blinking repeatedly or shifting focus during the segment. Do not use a flash. If the phone repeatedly hunts for focus, pause and reposition it rather than accepting a blurred frame.

6. Add a reference when you need millimetres

If a safe reference is available, keep it flat and beside the face in the eye’s plane for the entire clip. A printed target with a known spacing is preferable to a flexible ruler that can curve. Do not tape or attach anything to the eyelid or place a reference over the eye. If the reference moves, discard that segment; a changing scale invalidates the conversion.

7. Select and measure frames

Pause the video and reject frames with blinks, glare, motion blur, eyelid shadows or a turned gaze. In the remaining frames, estimate the dark pupil opening from one edge to the opposite edge. Measure multiple frames spread across the clip, not a run of nearly identical frames. Repeat the process for the other eye using the same criteria.

8. Summarize rather than cherry-pick

Report the median or a typical range for each eye, along with the number of usable frames. Keep the individual readings if possible. A single unusually large or small value is more likely to reflect hippus, a blink, focus error or a lighting change than a stable anatomical difference.

9. Repeat on another day for asymmetry

When the question is whether one pupil is consistently larger, repeat the complete setup on another day under matched conditions. Compare left and right eyes within each session and then compare the pattern across sessions. A one-time difference can result from gaze, exposure, eyelid shadow or normal oscillation.

Converting pixels to an approximate millimetre value

Use a same-plane reference

If the pupil measures 120 pixels across and a reference segment in the same plane measures 600 pixels, and that segment is known to be 5 mm, the simple scale estimate is:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

pupil diameter (mm) = pupil pixels ÷ reference pixels × reference length (mm)

In that example, the estimate would be 1 mm. This arithmetic is only as valid as the geometry: the reference and eye must be at effectively the same distance from the camera, the face must not be angled, and lens distortion must be small or corrected. A ruler held several centimetres in front of the eye can produce a precise-looking but wrong number.

Measure an edge consistently

Define in advance what counts as the pupil edge: the transition from the darkest opening to the iris, not the outer iris boundary or a corneal reflection. Use the same edge rule for both eyes and every frame. Reflections, eyelashes and eyelid shadows may hide the true boundary; mark those frames unusable instead of guessing.

Report uncertainty honestly

Include the median, the observed range, the number of frames, the lighting description and whether a physical reference was used. For example, a statement such as “the left eye was typically 3.2–3.6 mm and the right 3.0–3.5 mm in a dim room, measured from 20 usable frames with a ruler reference” is more informative than a single value with extra decimal places. The figure is an observation under those conditions, not a universal resting size.

Phone workflow, apps and dedicated instruments

These options are not interchangeable. Compare how each establishes scale, handles repeated frames and light responses, validates its output and protects your data.

Option Calibration and scale Repeatability Light-response data Validation evidence Privacy, cost and availability
Controlled phone video Requires a same-plane known reference or a documented geometric calibration; a casual ruler placement is vulnerable to perspective. You can sample many frames and repeat on different days, but results depend on the phone, setup and manual edge selection. Usually records appearance only; it does not automatically quantify a standardized light reflex. Suitable for documentation, not established as a clinical replacement for a pupillometer. Uses existing hardware; local storage and sharing are under your control, but model-specific camera processing varies.
Consumer smartphone app May provide its own calibration, but no single consumer app is established as universally validated for pupil diameter by the available evidence. Depends on whether it analyses video, controls geometry and rejects poor frames; read the method rather than assuming accuracy from an app label. Some apps claim reflex measurements, but those claims require independent validation against a clinical instrument. A 2025 study found significant disagreement between one smartphone prototype, penlight testing and a commercial pupillometer. Price, data collection, regulatory status and availability vary; check the developer’s privacy policy and validation details.
Dedicated handheld pupillometer Designed for optical measurement with a device-specific calibration procedure. Purpose-built alignment and automated frame analysis generally reduce user-dependent variation, subject to correct operation and calibration. Specialized instruments can measure diameter and pupil responses to light. EyeWiki describes specialized and commercial pupillometers, but specifications and regulatory status differ by model. Purchase price, maintenance, availability and regulatory clearance are model-specific; verify them before purchase.

Do not substitute unrelated phone tools

WHOeyes is an official World Health Organization app for near and distance visual acuity, not pupil-diameter measurement. The WHO states that it does not replace regular checks by an eye-care professional. A pupillary-distance app measures the spacing between pupils, not their diameters. Smartphone fundus-photography attachments are designed for retinal imaging and require controlled alignment and lighting; they are a different task from measuring the pupil opening.

What published smartphone research actually shows

A controlled research prototype

The paper “At-Home Pupillometry using Smartphone Facial Identification Cameras” describes a research system that combines an RGB selfie camera with a front-facing near-infrared facial-identification camera to track absolute pupil dilation. UC San Diego’s DigiHealth Lab reports sub-millimetre accuracy for its prototype under controlled conditions. That result belongs to the specified hardware, software and setup; it does not establish that every consumer phone can make a clinical measurement.

A warning from a 2025 prototype study

The 2025 study “Using smartphone pupillometer application to measure pupil size and light reflex: An unsuccessful prototype and analysis of the causes of failure” found statistically significant disagreement between the prototype, penlight testing and a commercial pupillometer. The finding is a direct reason to treat app output as an estimate and not as an interchangeable reading from clinical equipment.

Why accuracy varies from phone to phone

  • Exposure and focus: Automatic exposure can brighten or darken the iris, while autofocus blur softens the pupil edge.
  • Lighting: A flash, bright screen or changing room light can constrict the pupil during the measurement.
  • Geometry: Distance, camera angle and lens distortion alter apparent size and shape.
  • Gaze and accommodation: Looking at the phone or to one side changes the pupil and makes a circular opening appear elliptical.
  • Eyelids and reflections: Shadows, eyelashes and corneal glare can hide part of the boundary.
  • Hardware processing: Front cameras may use infrared illumination, noise reduction or computational sharpening that differs by model.

Troubleshoot an unreliable result

The pupil looks different between left and right clips

First check whether the phone moved, the room light changed, the camera refocused or your gaze shifted. Re-record both eyes in one clip if possible. Use the same exposure and focus settings, reject glare-obscured frames and compare medians rather than single images.

The edge is too blurry to measure

Increase camera stability, clean the lens, improve focus without adding a bright light and move the phone only if you can re-establish the same distance. Do not sharpen a blurred image aggressively; enhancement can create an artificial boundary. If the eye is partly covered by an eyelid, ask the subject to relax and repeat rather than pulling the lid open.

The reference gives implausible numbers

Confirm that the reference has a known printed length, lies in the eye’s plane and stays still. A curved ruler, a reference held in front of the face or a camera pointed obliquely can produce a systematic scale error. If those conditions cannot be met, omit the millimetre conversion and retain the video and relative comparison.

An app gives a precise number but no method

Precision displayed to two decimal places is not proof of accuracy. Look for a description of calibration, frame averaging, camera requirements, validation against a clinical device, data retention and regulatory status. If those details are absent, use the app only as a visual record and do not use its number to make a diagnosis or treatment decision.

How to interpret unequal pupils safely

Confirm the observation

Repeat under matched lighting and gaze, preferably on another day. Record whether the difference appears in dim and bright conditions, whether both pupils react to light, and whether the difference is stable across frames. Do not press on either eye or shine a bright light directly into it to force a response.

Arrange an eye-care assessment

Arrange professional evaluation for a persistent or newly noticed difference in pupil size, an abnormal light response or a discrepancy that remains after repeated matched measurements. Bring the original clips, your frame summary and the conditions you recorded. A clinician can examine the eyes, review medications and assess causes that a phone cannot distinguish, including ocular or neurological disorders.

Seek urgent care for warning symptoms

Get urgent medical attention when a new pupil-size change occurs with severe headache, eye pain, a drooping eyelid, double vision, confusion, weakness or a recent head or eye injury. Do not delay evaluation while trying to improve a phone measurement; the recording is supporting information, not a triage test.

Protect your privacy when recording your eyes

Eye videos are biometric images. Prefer processing that stays on the phone, disable automatic cloud backup when appropriate and review an app’s permissions before granting camera, microphone, location or contact access. If you send a clip to a clinician, use the practice’s approved channel and remove unrelated faces or identifying details. Keep the original file with its date and conditions so that later edits do not obscure what was actually recorded.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A practical reporting checklist

  • Phone model, camera and any infrared or enhancement mode used.
  • Camera-to-face distance, angle and whether the phone was mounted.
  • Room-light description and adaptation time.
  • Target distance and whether both eyes were recorded together.
  • Presence or absence of a safe same-plane calibration reference.
  • Number of usable frames, median or typical range for each eye and rejected-frame reasons.
  • Medication, eye-drop, contact-lens and recent-injury information relevant to interpretation.
  • Copies of the original clips, not only screenshots or app-exported numbers.

A careful smartphone record can make a conversation with an eye-care professional clearer. It should remain a repeatable observation with stated limits, never a stand-in for examination or a diagnosis.
