Some soreness around a spinal cord stimulator (SCS) incision or implanted pulse generator can occur while the surgical sites heal, and recovery may take several weeks. Use the pain plan and activity limits given by your implanting team rather than treating a general guide as a personal diagnosis.

Call the implant team promptly for worsening or unusual pain, redness, swelling, drainage, fever, a new or painful stimulation pattern, or reduced therapy benefit. Do not rub, press, or manipulate the implanted components through the skin. This article focuses on SCS; if you have another nerve-stimulation device, follow that device’s manual and your own care team’s instructions.

What discomfort can mean after SCS surgery

An SCS system usually includes leads placed near the spinal cord and an implanted pulse generator, often positioned under the skin. There may be more than one healing area. Medtronic’s patient guidance says the implant site may hurt and that recovery can take several weeks while the leads settle. Boston Scientific also describes incision soreness after surgery.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Those statements describe common recovery guidance, not a guarantee that every pain pattern is harmless. Pain can also occur with an infection, movement of a lead, problems at the implant pocket, uncomfortable stimulation, or loss of therapy effect. The treating team has to examine you and, when needed, check the system to determine the cause.

Why relief may not start immediately

An SCS system may not be turned on until you have recovered from surgery. Therefore, the absence of pain relief in the first days or weeks does not by itself show that the system has failed. Ask your team when activation, programming, and follow-up are planned for your particular model and procedure.

Why a single “normal pain” rule does not work

Healing time, incision location, lead placement, device model, medical history, and the surgeon’s restrictions all affect recovery. A mild, gradually improving ache is different from pain that is becoming stronger, spreading, or accompanied by a wound change. Compare your symptoms with the instructions you were given at discharge and report anything that does not fit that plan.

First steps for safer comfort management

  1. Use the plan from the implanting team. Ask which medicines, if any, are appropriate for you, how to take them, and what to do if they do not control the pain. There is no universal medication recipe that applies to every SCS patient. Boston Scientific states: “Discuss the plan for surgical pain management with your doctor.”
  2. Protect the incisions and hardware. Follow the wound-care, dressing, bathing, and appointment instructions supplied for your procedure. Do not add a cleaning product, ointment, ice routine, heat routine, or medication without checking first; the reviewed manufacturer guidance does not establish one regimen for all patients.
  3. Limit movements your team restricted. Manufacturer instructions commonly warn against activities that stress the system, including bending, twisting, and lifting during early healing. The exact limits and duration differ by device and clinician, so use your written instructions rather than a generic timetable.
  4. Keep follow-up appointments. Programming, wound review, and assessment of the lead and pulse-generator sites are part of recovery. Tell the team whether pain is improving, stable, or worsening and whether stimulation feels different.

Activity precautions while the system heals

Early movement can place tension on healing tissue and the implanted system. Follow the restrictions supplied by your surgeon and the device manual. Boston Scientific describes specific post-surgical restrictions, while other manufacturers and clinicians may use different periods. The fact that one patient receives a particular restriction does not establish the same schedule for another model or operation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Movements to discuss before resuming

  • Repeated or deep bending at the waist
  • Twisting your trunk or reaching forcefully across your body
  • Lifting objects, pushing, pulling, or strenuous exercise
  • Any activity that stretches an incision or causes new pain at the lead or pulse-generator site

When an activity causes a new or sharp increase in pain, stop and ask the implant team whether it is appropriate to resume. Do not try to “test” the system by deliberately repeating a movement that hurts.

Do not press or massage the implant

Avoid rubbing, pressing, kneading, or repeatedly checking the device through the skin. Abbott warns that pressure can dislodge a lead, alter the implant’s position, or damage and erode the overlying skin. A tender pocket should be observed and reported, not massaged. Ask the team how to position yourself for sleep, work, and transport if the implant site is uncomfortable.

When to contact the implant team

Contact the doctor’s office immediately for redness, swelling, drainage at an incision, or fever, consistent with Boston Scientific’s patient guidance. Also make a prompt call for new, worsening, or unusual pain, especially when it occurs with a wound change or a change in stimulation.

Incision and skin warning signs

  • Redness that is spreading or becoming more pronounced
  • Increasing swelling around an incision or the pulse-generator pocket
  • Fluid or other drainage from the incision
  • Fever or feeling systemically unwell after the procedure
  • Skin that appears to be thinning, opening, or breaking down over the hardware

These signs do not prove an infection or another specific complication, and you should not wait for a diagnosis at home. Tell the team when the change began, how quickly it is progressing, and whether your temperature was measured.

Pain patterns that deserve a call

  • Pain that is worsening instead of gradually settling
  • Pain that is markedly different from the discomfort described at discharge
  • New pain after a movement, impact, or other event that may have stressed the system
  • Pain that persists despite the plan provided by your clinician

Manufacturers list infection, lead movement, implant-site pain, and loss of therapy effectiveness among possible problems. Those categories are reasons for clinical review, not a self-diagnosis checklist.

What to do if stimulation changes or becomes painful

Uncomfortable, jolting, or shocking stimulation is a recognized potential adverse event in manufacturer safety information. A new sensation, a change in the treated area, or a sudden reduction in benefit can have several explanations, including programming needs or a hardware issue. Contact the device team for instructions.

Do not make unsupervised setting changes

The reviewed guidance does not support a blanket instruction to change settings independently when stimulation becomes painful. Use the controller only as your implant team taught you, and ask before changing programs, amplitude, or other parameters. If stimulation is painful, note what you were doing, the setting or program displayed, where you felt it, and whether the sensation stopped when stimulation was turned off according to your prescribed procedure.

Reduced benefit is also information

Tell the team if pain relief fades, moves to a different body area, or no longer covers the area it previously helped. Bring your controller and device identification information to the appointment if requested. A reduction in benefit does not by itself prove that a lead has moved; examination and device assessment are needed.

A practical symptom-monitoring routine

A short record can help your clinician distinguish a stable recovery pattern from a change that needs attention. Keep it simple and follow any format your clinic provides.

  1. Record the location. Identify the incision, pulse-generator pocket, back, limb, or another area.
  2. Describe the sensation. Use words such as aching, burning, pulling, sharp, electric, jolting, or tender rather than relying only on a numerical score.
  3. Track the trend. Note whether it is improving, unchanged, or worsening over the previous day.
  4. Check related signs. Record redness, swelling, drainage, fever, skin changes, new weakness or other rapidly changing symptoms, and any stimulation change.
  5. List triggers and actions. Write down movements, activity, medication taken according to your prescription, and what happened afterward.

Share the record at follow-up or during a phone call. Do not delay a call for a serious or rapidly changing symptom merely to complete a diary.

Questions to ask before leaving surgery or a follow-up visit

  • Which areas should be sore, and what pattern would be unexpected?
  • What is my specific pain-management plan, including what to do if it is insufficient?
  • Which bending, twisting, lifting, driving, work, exercise, and sleeping restrictions apply to my device and procedure?
  • When should the system be activated, and what should I expect before activation?
  • Whom should I call during office hours and outside them?
  • What should I do if stimulation becomes painful, jolting, or ineffective?
  • How should I use the controller, charger, or other system equipment, and which changes require your approval?
  • What wound or skin changes require an immediate call?

Situation What may be happening Recommended response
Incision or pocket soreness that is stable or gradually improving Healing-related discomfort is possible; manufacturer pages describe soreness and recovery over several weeks. Follow the prescribed plan and activity limits, and mention it at follow-up.
Increasing pain, redness, swelling, drainage, or fever A wound complication, including infection, must be considered by a clinician. Call the doctor’s office immediately.
New, unusual, jolting, or shocking stimulation A programming or system issue, lead movement, or another cause is possible. Contact the device team for instructions; do not manipulate the implant.
Loss or reduction of therapy benefit Manufacturers identify loss of effectiveness as a possible problem, but the cause cannot be determined from symptoms alone. Arrange prompt review with the device team.
Severe or rapidly changing symptoms The situation may need assessment sooner than a routine appointment. Contact your treating team or seek appropriate urgent medical care rather than waiting.

Device-specific instructions matter

Advice can differ by manufacturer, model, region, surgical technique, and manual revision. Medtronic, Boston Scientific, and Abbott patient information all emphasize following the treating team’s instructions, but their examples and restrictions are not interchangeable. Keep the manual supplied with your system and take it to appointments when practical.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If you cannot identify your device model, check your implant card, controller, discharge paperwork, or clinic records. Do not assume that instructions for another SCS system apply to yours. The same caution applies if your device is not an SCS—for example, a different nerve-stimulation implant may have different programming, charging, activity, and wound guidance.

Planning daily activities while you recover

Ask for written guidance before resuming driving, work duties, exercise, lifting, or extended travel. The safe timing depends on the operation, your symptoms, the device, and the clinician’s assessment; a general calendar cannot replace those instructions. If sitting, standing, getting in and out of a vehicle, or sleeping puts pressure on an incision, tell the team rather than repeatedly adjusting the implant yourself.

Use the controller, charger, and any accessories exactly as demonstrated. Keep them available for follow-up if the team needs to review a program or device message. Do not infer that a setting, charging pattern, or accessory used with another model is safe for yours. If instructions are unclear, ask the clinic to demonstrate them again.

How a follow-up visit may evaluate pain

Your clinician may inspect the incisions and pocket, ask when the discomfort began, review activity or stimulation changes, and assess whether programming is providing the expected coverage. Depending on the findings, the team may arrange a programming visit, wound assessment, imaging, or another evaluation. These possibilities are reasons to contact the team, not predictions of what you will need.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Bring your implant card or model information, controller, medication list, discharge instructions, and symptom notes when requested. Tell the clinician about an impact, unusual movement, or sudden change even if the pain later eased. A temporary improvement does not establish that the system is unaffected.

What manufacturer safety lists can and cannot tell you

Medtronic, Boston Scientific, and Abbott describe potential issues such as infection, lead movement, implant-site pain, uncomfortable stimulation, and reduced therapy effect. These lists identify events that warrant attention; they do not say that every event will occur or provide a personal probability. No reliable population rate from these patient pages can predict how likely a particular cause is for you.

That is why a stable, improving incision ache can be monitored according to your discharge plan, while a new wound change, fever, worsening pain, or altered stimulation should prompt contact even when the symptom seems modest. The distinction is based on trend and associated signs, not on a single pain score.

What not to do

  • Do not press, rub, massage, or repeatedly probe the implant or lead path.
  • Do not begin strenuous lifting, twisting, bending, or exercise before your team clears it.
  • Do not choose a medication, dose, supplement, ice treatment, heat treatment, or wound product from a generic internet recommendation.
  • Do not assume that delayed activation means the system is ineffective.
  • Do not ignore a new stimulation pattern, reduced benefit, worsening pain, fever, redness, swelling, or drainage.
  • Do not attempt to diagnose a displaced lead yourself or adjust settings outside the instructions you were given.

When general information is not enough

This overview cannot determine whether your pain is routine healing, infection, lead movement, pocket trouble, or another condition. The manufacturer pages reviewed list possible complications but do not provide a population rate that can tell one individual how likely a problem is. A symptom that feels minor to one person may require review in another because of surgical timing, medical history, or device details.

For severe or rapidly changing symptoms, contact the treating team or seek appropriate urgent care. Bring your medication list, implant information, symptom timeline, and any controller messages if they are available. Those details can help the clinician decide whether you need a wound examination, programming visit, imaging, or another assessment.

Bottom line for day-to-day decisions

Expect that some incision or implant-site soreness may occur during a recovery period that can last several weeks, but judge the trend and accompanying signs rather than labeling all pain as normal. Protect the healing system, follow your own team’s restrictions and pain plan, avoid pressure over the hardware, and call promptly for wound changes, fever, worsening or unusual pain, changed stimulation, or reduced benefit. Your implanting clinicians—not a generic timetable—should decide what is safe for your device and recovery.

Frequently Asked Questions

Is pain around a spinal cord stimulator implant normal?

Incision or pulse-generator soreness can occur while the surgical sites heal, and Medtronic notes that recovery may take several weeks. Pain that worsens, changes character, or occurs with redness, swelling, drainage, fever, altered stimulation, or reduced benefit needs prompt clinical review.

How long should soreness last after SCS surgery?

There is no single timetable for every device or operation. Manufacturer guidance describes recovery over several weeks, but your surgeon’s instructions, the implant location, lead placement, and your medical history determine what is expected. Ask what trend and duration apply to you.

Should I use ice, heat, ointment, or an over-the-counter pain medicine?

Ask your implanting clinician first. The manufacturer pages do not establish one medication, dosing, icing, heating, or wound-product routine that applies to all patients and devices.

What should I do if stimulation feels shocking or painful?

Contact the device team for instructions. Note the program or setting displayed, where the sensation occurred, what you were doing, and any related pain or wound change. Do not change parameters or manipulate the implant unless your team has specifically taught you to do so.

What if my SCS suddenly provides less pain relief?

Reduced or moved coverage is information to report promptly. It does not prove that a lead has moved or that the system has failed. The team may need to examine you and assess programming or hardware.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When should I call immediately about the incision?

Boston Scientific advises calling the doctor’s office immediately for redness, swelling, drainage, or fever. Also call promptly for worsening or unusual pain, skin breakdown over the hardware, or rapidly changing symptoms.

Do these instructions apply to every nerve-stimulation implant?

No. This overview focuses on spinal cord stimulation. Other nerve-stimulation devices can have different programming, charging, activity, and wound instructions. Use your own device manual and care team’s advice.

The Bottom Line

Some soreness can accompany SCS healing, but worsening pain, wound changes, fever, painful stimulation, or reduced benefit should prompt a call to your implant team. Follow device-specific instructions and never press or manipulate the hardware.
