Mild, period-like cramps for a few days and light spotting are common after an intrauterine device (IUD) is fitted. Bleeding can then follow a different pattern during the first 3–6 months: copper IUDs often cause more bleeding, while levonorgestrel (hormonal) IUDs commonly cause spotting that gradually decreases.

Severe or worsening pain, very heavy bleeding, fever, unusual or smelly discharge, possible pregnancy, or strings that cannot be felt or feel different need prompt medical assessment. There is no single number of days or amount of bleeding that is safe for everyone, so judge symptoms by their severity, direction and accompanying signs.

What cramps and spotting are usually expected after insertion?

The NHS says period-like pain can last for a few days after an IUD is fitted, and light bleeding or spotting may occur. These immediate symptoms reflect the fitting procedure and do not by themselves mean the IUD is out of position.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

After those first days, bleeding may follow the IUD’s longer-term adjustment pattern. The Centers for Disease Control and Prevention (CDC) describes spotting or light bleeding as common during the first 3–6 months with both copper and levonorgestrel IUDs. Population guidance cannot predict exactly what one person will experience, and it does not replace an examination when symptoms are severe, worsening or accompanied by other warning signs.

Do not use the mere passage of time as your only test of safety. Pain that is getting worse, does not settle, or is difficult to control deserves clinical advice even if the IUD was inserted recently.

Understand the two time periods

It helps to separate the immediate aftermath of fitting from the months in which your uterus adjusts to the device.

The first few days

Cramping that feels similar to a period and a small amount of bleeding or spotting can occur soon after insertion. The NHS describes this as a common short-term experience. The important feature is the overall trend: mild, stable or improving discomfort is more reassuring than pain that becomes stronger, suddenly changes or remains difficult to control.

You may notice bleeding on toilet paper, underwear or a liner, or a period-like flow. Guidance does not provide one universal number of days or one amount of blood that is safe for every person. Consider how the bleeding compares with your usual period, whether it is increasing and whether you feel unwell.

The first 3–6 months

Once the immediate fitting symptoms have passed, irregular bleeding can still be part of the adjustment period. CDC guidance from 2024 identifies spotting or light bleeding as common during the first 3–6 months with both copper and levonorgestrel IUDs. Copper IUDs can also cause heavier or prolonged bleeding during this period. With a levonorgestrel IUD, heavy or prolonged bleeding is uncommon, and spotting generally decreases with continued use.

These are patterns seen across groups of users, not a deadline for an individual. Your previous menstrual pattern, health history and the specific IUD can influence what you experience. A new change that is severe, worsening or accompanied by other symptoms should not be dismissed as routine adjustment.

How copper and hormonal IUD bleeding patterns differ

The two main IUD types share the same immediate aftercare concerns, but their bleeding patterns over the first months are different.

IUD type What may happen in the first 3–6 months What usually happens with continued use When to seek assessment
Copper IUD Spotting or light bleeding is common. Heavier or prolonged bleeding can also occur. Bleeding changes generally decrease with continued use, according to CDC guidance. The same urgent warning signs apply: severe or worsening pain, very heavy bleeding, fever, unusual or smelly discharge, possible pregnancy, or strings that are missing or changed.
Levonorgestrel IUD Spotting or light bleeding is expected and is common during the first 3–6 months. Spotting generally decreases with time. Heavy or prolonged bleeding is uncommon. Report the same warning signs promptly; a hormonal IUD does not eliminate the possibility of a complication.

The CDC’s time frame is a description of common use patterns, not a promise that bleeding will stop by a particular month. A person’s previous periods, medical history and the specific device can affect what is normal for them.

A practical plan for the first few days

1. Record what you are experiencing

Note when the cramps began, whether they are improving or worsening, and whether bleeding is only spotting or is substantially heavier than expected. Also note fever, discharge with an unusual or unpleasant smell, dizziness, or a possible pregnancy. This information helps a clinician decide how quickly you need to be seen.

A simple note can include the insertion date, the IUD type and brand if known, pain severity and location, bleeding pattern, temperature if you have taken it, discharge changes, and the time and effect of any medicine. You do not need to produce a perfect measurement; the direction of symptoms and associated warning signs are useful.

2. Use pain relief only if it is suitable for you

NHS patient guidance names paracetamol and ibuprofen as painkillers that can help period-like pain for a few days after fitting. The American College of Obstetricians and Gynecologists (ACOG) also notes that over-the-counter pain relief such as ibuprofen may help with discomfort around placement.

These are general options, not an individualized prescription. Follow the product label, check for allergies and medical conditions, consider other medicines you take, and ask a pharmacist or clinician if you are unsure. Pregnancy possibility, kidney or stomach problems, bleeding risks and other factors can change whether a medicine is appropriate. Do not use painkillers to postpone assessment when pain is severe, worsening, persistent or accompanied by warning signs.

Do not combine products or exceed the labeled amount because another medicine may contain the same ingredient. If you were given specific instructions by the clinician who inserted your IUD, follow those instructions unless another clinician tells you otherwise.

3. Keep the IUD in place

Do not pull the threads, attempt to reposition the device, or try to remove it yourself. If you are concerned that it has moved, the correct next step is a clinical check.

4. Watch the trend, not just one moment

Symptoms can fluctuate during a day. Reassess whether pain is settling or escalating, whether bleeding is becoming heavier, and whether fever, unusual discharge, pregnancy concerns or a string change has appeared. A symptom that is mild but steadily worsening deserves a call, just as a sudden severe symptom does.

What to do if the IUD strings are missing or feel different

The threads, sometimes called strings, extend through the cervix so a clinician can check the device and remove it later. If you cannot feel them when you normally can, or they suddenly feel shorter, longer or otherwise different, the IUD may not be in its expected position and contraceptive protection may be affected.

  1. Contact a sexual-health clinic, primary-care clinician or other qualified IUD provider promptly.
  2. Use another contraceptive method until a clinician has checked the IUD, following NHS advice.
  3. Do not attempt to find, pull or adjust the device yourself.

In the UK, NHS guidance lists a sexual-health clinic, NHS 111 or a GP as routes for urgent advice. Outside the UK, use your local urgent-care, primary-care or sexual-health service. If severe symptoms are rapidly deteriorating, use local emergency services.

A missing or changed thread does not prove that the IUD has come out or moved, but it means its position and your contraceptive protection should be checked. Tell the service whether you also have pain, bleeding, fever, discharge or a possibility of pregnancy.

Warning signs that need prompt medical assessment

Contact a clinician promptly if any of the following occur after insertion:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Lower-abdominal or pelvic pain that painkillers do not relieve.
  • Sudden pain, pain that is worsening, or pain that does not go away.
  • Very heavy vaginal bleeding. Official guidance does not set one pad-per-hour rule that is safe for every person, so seek advice based on how heavy the bleeding is for you, whether it is increasing, and whether you feel unwell.
  • A high temperature or fever.
  • Unusual or smelly vaginal discharge.
  • Possible pregnancy.
  • Threads that cannot be felt or feel different.

NHS side-effect guidance and CDC recommendations identify these symptoms as reasons for prompt assessment. A rapidly worsening condition, fainting, severe weakness or other emergency symptoms warrants your local emergency service rather than waiting for a routine appointment.

Do not wait for all warning signs to appear together. Severe pain alone, very heavy bleeding alone, or a possible pregnancy with pain or bleeding is enough to seek advice. If you are unsure how urgently to act, describe the symptoms to a clinician, pharmacist, urgent-care service or local health line.

Why assessment matters even though serious complications are uncommon

ACOG states that “Serious complications from IUDs are rare.” Its patient information also gives uterine-wall perforation during placement as occurring in about 1 out of every 1,000 placements. Those figures describe population-level risk; they are not a reason to dismiss a new or worsening symptom.

An examination can establish whether the IUD is in place and whether another cause needs treatment. The same warning signs apply to copper and levonorgestrel devices. An IUD’s general contraceptive effectiveness is over 99%, according to ACOG, but that effectiveness depends on the device being correctly positioned and does not make pain or bleeding automatically safe.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Prompt assessment is also important when pregnancy is possible. A clinician can decide what examination or testing is appropriate and advise on contraception while the IUD position is clarified. Do not remove the device yourself or wait for symptoms to become extreme.

How to decide whether to monitor or call

Monitoring at home can be reasonable when

  • The discomfort is mild and period-like.
  • The pain is stable or improving over the first few days.
  • Bleeding is light spotting without other concerning symptoms.
  • You have no fever, unusual discharge, possible pregnancy or change in the threads.

Continue observing the trend rather than relying on a fixed deadline. If the pattern changes for the worse, move from monitoring to clinical advice.

Call promptly rather than waiting when

  • Pain is severe, worsening, persistent or not helped by an appropriate over-the-counter option.
  • Bleeding is very heavy or increasing.
  • You develop fever, unusual or smelly discharge, or think you may be pregnant.
  • The threads are missing or feel different.

When calling, explain the IUD type if known, the insertion date, the direction and severity of pain, the amount and pattern of bleeding, any temperature reading, discharge changes, pregnancy possibility and what happened to the threads. Tell the clinician which medicines you have taken and whether they helped.

What the first 3–6 months can look like

Immediately after fitting

Cramping similar to a period and light spotting are common according to NHS fitting guidance. The cramps may be noticeable for a few days. Use only medicines that are appropriate for you and follow their labels.

During the adjustment period

CDC 2024 guidance describes spotting or light bleeding as common with both device types during the first 3–6 months. Copper IUD users may also experience heavier or prolonged bleeding in this period. With a levonorgestrel IUD, spotting generally decreases over time and heavy or prolonged bleeding is uncommon.

After the adjustment period

Many users notice that bleeding changes settle, but the guidance does not provide a universal date by which every person should be symptom-free. Persistent, severe or newly worsening symptoms should be assessed rather than explained away as “adjustment.”

If bleeding remains disruptive, contact the clinician who fitted the IUD or another qualified provider. They can review the device type, your medical history and the symptom pattern instead of assuming the IUD is the only possible cause.

Common mistakes to avoid

  • Assuming every symptom is normal because insertion was recent. Severe, worsening or persistent pain still needs assessment.
  • Using a fixed numerical threshold as a guarantee of safety. Official guidance does not establish one amount of bleeding or one duration of cramps that applies to everyone.
  • Stopping contraception when threads feel different. Use another method until the IUD has been checked.
  • Trying to pull or reposition the device. Only a qualified clinician should assess or remove it.
  • Taking medicine without checking suitability. Read the label and ask a pharmacist or clinician when health conditions, pregnancy possibility or other medicines could matter.
  • Waiting for multiple warning signs. One significant warning sign can justify a prompt call.

Questions to ask your clinician or pharmacist

  • Is my pain pattern appropriate for the device and insertion date?
  • Which over-the-counter pain reliever is safe with my medical history and current medicines?
  • Should I have the threads or IUD position checked?
  • What local service should I use outside office hours?
  • Could pregnancy be possible, and what assessment is needed?
  • How should I describe or monitor the bleeding pattern?
  • Which symptoms mean I should use emergency services rather than wait for an appointment?

Bring the device name or insertion paperwork if you have it. Different levonorgestrel products have different durations of approved contraceptive use, but the aftercare warning signs described here apply to the IUD currently in place.

Frequently asked questions

How long do cramps last after IUD insertion?

The NHS says period-like pain can last for a few days after fitting. There is no universal safe maximum duration. If pain is severe, worsening, persistent, not relieved by an appropriate medicine or accompanied by fever, unusual discharge, heavy bleeding, pregnancy concern or changed threads, seek prompt assessment.

Is spotting normal after getting an IUD?

Light spotting can occur immediately after fitting. CDC 2024 guidance also describes spotting or light bleeding as common during the first 3–6 months with both copper and levonorgestrel IUDs. Spotting that becomes heavy, is associated with significant pain or comes with other warning signs should be assessed.

Which IUD causes more bleeding?

Copper IUD users may have heavier or prolonged bleeding during the first 3–6 months, although these changes generally decrease with continued use. Levonorgestrel IUDs commonly cause spotting early on; spotting generally decreases over time, and heavy or prolonged bleeding is uncommon.

Can I take ibuprofen or paracetamol?

NHS guidance names paracetamol and ibuprofen as options that can help period-like pain for a few days, and ACOG mentions over-the-counter pain relief such as ibuprofen around placement. Check the product label and your personal contraindications with a pharmacist or clinician. Medicine should not delay care for severe or worsening symptoms.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What if I cannot feel my IUD strings?

Contact a qualified IUD provider promptly and use another contraceptive method until the device is checked, following NHS advice. Do not pull or try to reposition the device. In the UK, a sexual-health clinic, NHS 111 or a GP can advise; elsewhere, use the equivalent local service.

Does a high temperature after insertion need attention?

Yes. NHS side-effect guidance identifies a high temperature or fever after fitting as a reason for prompt clinical contact, especially when it occurs with pain or unusual or smelly discharge.

Can an IUD still work if I have spotting?

Spotting itself does not show whether an IUD is working. IUDs are over 99% effective in general, according to ACOG, but protection depends on correct positioning. If threads are missing or changed, use another method until a clinician checks the device.

When should bleeding be treated as an emergency?

Seek urgent advice for very heavy or increasing bleeding, particularly if you feel faint, severely weak or otherwise unwell. Because no single pad-per-hour rule fits every person, describe the amount and associated symptoms to a clinician; use local emergency services for severe or rapidly deteriorating symptoms.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What should I do if I cannot feel the strings?

Contact a qualified IUD provider promptly, use another contraceptive method until the device is checked, and do not pull or reposition the device yourself.

Can I take ibuprofen or paracetamol for cramps?

NHS guidance names both as general options for period-like pain for a few days, and ACOG mentions over-the-counter pain relief such as ibuprofen. Follow the label and check personal contraindications with a pharmacist or clinician.

When is bleeding after insertion an emergency?

Very heavy or increasing bleeding, especially with faintness, severe weakness or other serious symptoms, warrants urgent advice or local emergency services. No single pad-per-hour rule applies safely to everyone.

The Bottom Line

Mild, improving cramps and light spotting are common after IUD fitting. Copper IUDs can bring heavier bleeding early on, while levonorgestrel IUD spotting usually decreases. Seek prompt care for severe or worsening pain, very heavy bleeding, fever, unusual discharge, possible pregnancy or missing or changed strings, and use another contraceptive method until a changed or missing IUD is checked.
