Tell both your oncology team and the clinician who prescribes your antidepressant about any new, worsening, or troublesome symptom. Nausea, diarrhea, fatigue, sleepiness, and sexual changes can result from cancer, cancer treatment, an antidepressant, another medicine, or several causes at once. The safest response is a coordinated medication and symptom review—not diagnosing the cause yourself or changing a dose.
This guide explains what to record, which questions to ask, interaction issues to raise, and when to seek urgent help. It is general education, not individualized medical advice. Do not start, stop, switch, or add an antidepressant, antiemetic, supplement, or herb without instructions from your care team.
Start with a coordinated review, not a medication change
Contact the oncology office and the antidepressant prescriber when a symptom is new, getting worse, or interfering with eating, sleeping, daily activities, treatment, or relationships. Ask the clinicians to review the timing of symptoms against chemotherapy, radiation, surgery, immunotherapy, hormone therapy, antidepressant doses, pain medicines, antiemetics, sleep medicines, and other treatments.

The National Cancer Institute (NCI) advises using antidepressants under a doctor’s care and says, “Check with your doctor before you stop taking an antidepressant.” Abruptly stopping can cause problems; a prescriber may lower the dose gradually before a switch when a change is appropriate.
Prepare information your care team can use
- Make one complete medicine list. Include the antidepressant name, formulation, dose, dosing time, recent dose changes, cancer drugs, antiemetics, opioids, benzodiazepines, sleep aids, steroids, over-the-counter products, vitamins, teas, and herbs. Include medicines taken only as needed.
- Record the symptom pattern. Note when it began, whether it follows a dose or treatment session, how long it lasts, how severe it is, and what makes it better or worse. For nausea or vomiting, record the time of each episode and whether you can keep fluids down.
- Describe functional impact. Explain whether you missed meals, could not take a cancer medicine, fell asleep while driving or walking, stopped an activity, or experienced distress in a relationship. Impact helps clinicians judge urgency.
- Bring treatment dates. Mark chemotherapy, radiation, infusion, surgery, hormone-treatment, and scan dates alongside antidepressant changes. NCI defines acute treatment-related nausea as occurring within 24 hours and delayed nausea as occurring one to seven days afterward; that timing can help the team compare patterns.
- State your priorities. Tell the team which problem matters most—for example, controlling vomiting, staying alert, sleeping, maintaining appetite, preserving sexual function, or avoiding an interaction. Several acceptable approaches may exist, but the right choice depends on your cancer regimen and health history.
- Use the same information with both teams. Ask whether the oncology and prescribing teams can communicate directly or whether you should authorize information sharing. A shared list reduces the chance that one clinician misses a recent change made by another.
Keep a practical symptom record
A short log is often more useful than trying to remember a difficult week at an appointment. Use paper, a phone note, or a caregiver’s record. For each event, write the date and time, the medicine or treatment taken shortly beforehand, the symptom and its severity, what you were able to eat or drink, and what helped. Also note missed doses, vomiting after a dose, falls, near-falls, confusion, or unusually long sleep.
For diarrhea, record the number of episodes, urgency, blood, fever, and whether you can drink. For fatigue or sleepiness, note your sleep duration, naps, unintentional dozing, and whether you felt weak, dizzy, mentally slowed, or simply sleepy. For sexual changes, record when the change began, whether it is constant or situational, and any pain or dryness. These details help clinicians compare symptoms with infusion days, delayed treatment effects, and dose changes without asking you to diagnose the cause.
Caregivers can help by updating the medication list, noticing safety changes, and accompanying the patient to a visit. Ask the patient what information may be shared and preserve privacy around sexual or mental-health concerns.
Nausea, vomiting, and diarrhea
Why the cause is difficult to identify
Some antidepressants, including sertraline and duloxetine, can cause gastrointestinal symptoms such as nausea or diarrhea. Cancer and cancer treatment can independently cause nausea and vomiting, and constipation, infection, dehydration, pain, anxiety, or another medicine can add to the problem. The same symptom can therefore have more than one cause.

What to do
- Tell the oncology team and prescriber when the symptom started and how it relates to antidepressant doses and cancer-treatment days.
- Use the antiemetic plan prescribed for your cancer treatment exactly as directed. NCI states that a doctor can help find medicines and other ways of managing these effects that work best for the individual.
- Ask what to do if an antiemetic does not work, if vomiting prevents you from keeping medicines down, or if diarrhea continues. Do not add an over-the-counter antidiarrheal or change an antidepressant without checking first.
- Ask whether dehydration, abnormal blood tests, infection, or another treatment needs evaluation. The team can give individualized advice about fluids and food based on your treatment and medical conditions.
Do not assume that taking an antidepressant with food is always appropriate. Follow the label and the prescriber’s instructions, because food instructions differ among medicines and may matter when nausea is being evaluated.
When nausea needs prompt contact
Call the oncology service promptly for repeated vomiting, inability to keep fluids or essential medicines down, signs of dehydration, severe abdominal pain, blood in vomit or stool, fever, confusion, or rapidly worsening weakness. If there is an immediate emergency—such as collapse, severe breathing trouble, or inability to stay awake—use local emergency services.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsFatigue and sleepiness
Look for several contributors
Cancer-related fatigue may be compounded by an antidepressant, opioid, antiemetic, benzodiazepine, sleep medicine, antihistamine, anemia, infection, dehydration, poor sleep, pain, depression, or the cancer itself. NCI specifically notes that medicines can add to cancer-related fatigue. Record whether you feel physically weak, mentally slowed, or simply sleepy; those distinctions can point the review in different directions.
Medication review points
Tell the team when drowsiness occurs in relation to each medicine and whether you have nodded off unintentionally, fallen, or had trouble driving safely. NCI notes that mirtazapine’s sedation may be undesirable when combined with other sedating medicines such as opioids or benzodiazepines. That is a reason for a clinician-led review, not a reason to stop mirtazapine or another medicine on your own.
Until a clinician advises otherwise, avoid driving, operating machinery, or making safety-sensitive decisions if you are unusually sleepy or impaired. Ask whether dosing times, duplicate sedatives, sleep quality, anemia, thyroid problems, infection, or other reversible contributors should be checked. Do not compensate for sleepiness by adding caffeine, energy products, or a stimulant without asking the team.

Sexual changes: questions worth raising directly
Sexual concerns are common reasons people hesitate to report side effects, but they are legitimate treatment issues. Depression itself, cancer, surgery, radiation, hormone changes, pain, body-image changes, fatigue, and some antidepressants can affect desire, arousal, lubrication, erection, orgasm, ejaculation, or comfort. Do not assume the antidepressant is the only cause.
Recommended Free Tools
For women receiving cancer treatment
NCI cancer sexual-health information asks, “What sexual changes or problems are common among women receiving this type of treatment?” Depending on the cancer and treatment, concerns may include reduced interest, vaginal dryness, pain with penetration, difficulty becoming aroused or reaching orgasm, hot flashes, fertility concerns, or changes related to surgery and menopause. Ask the oncology team which effects are plausible for your treatment and which options are safe for you.
For men receiving cancer treatment
NCI also asks, “What sexual changes or problems are common among men receiving this type of treatment?” Men may experience reduced desire, erectile difficulty, problems with orgasm or ejaculation, infertility concerns, pain, or treatment-related hormone changes. Share the change, its timing, and whether it occurs with every sexual activity or only in certain situations.
How to start the conversation
- Use specific words such as “my desire dropped,” “erections are less reliable,” “sex is painful,” or “I cannot reach orgasm.”
- Ask whether cancer treatment, depression, the antidepressant, another medicine, or a combination could be involved.
- Ask which lubricants, moisturizers, pelvic-health services, fertility services, or prescription treatments are safe with your cancer regimen. Do not begin a hormone product or supplement without oncology approval.
- Include a partner or caregiver if you want support, while keeping private details you prefer to discuss alone.
Interactions, tamoxifen, and supplements
Tamoxifen and CYP2D6-inhibiting antidepressants
NCI health-professional guidance says potent CYP2D6 inhibitors such as paroxetine and duloxetine are not recommended with tamoxifen because they may reduce conversion of tamoxifen to its active metabolite, endoxifen. Another NCI page on anxiety and distress also flags paroxetine with tamoxifen. This does not mean every person taking tamoxifen should stop or switch an antidepressant. It means the oncology and prescribing teams should perform an individualized interaction review before starting, stopping, or changing therapy.
St. John’s wort and other herbs
NCI reports that people taking St. John’s wort during anticancer treatment had lower blood levels of those anticancer drugs. It also warns that St. John’s wort can interact with depression medicines in a way that raises serotonin to a dangerous level. Tell the team about every herbal product, tea, tincture, or supplement before using it; “natural” does not mean interaction-free.

What to bring to the review
- Photos of medicine labels or an up-to-date pharmacy list.
- Names and doses of supplements, including products used intermittently.
- Your cancer regimen, treatment dates, and the reason each medicine was prescribed.
- Any prior adverse reaction or allergy.
- The names of all pharmacies and prescribers if different clinicians manage different medicines.
How clinicians weigh antidepressant choices
There is no universally best antidepressant for someone receiving cancer treatment, and the available guidance does not establish a complete cancer-specific ranking of side-effect frequencies. Clinicians balance the dominant symptoms, cancer regimen, other medicines, medical conditions, previous response, and the patient’s goals. The NCI discusses symptom-based selection; the considerations below show why a choice must be individualized.
| Review area | Questions the team may consider | Why it matters during cancer treatment |
|---|---|---|
| Gastrointestinal effects | Are nausea, vomiting, diarrhea, constipation, appetite loss, or weight change already present? | An antidepressant’s gastrointestinal effects could add to treatment-related symptoms or complicate hydration and nutrition. |
| Alertness and sleep | Is insomnia the main problem, or is the person already sleepy from opioids, antiemetics, or other sedatives? | Sedation may be helpful in some circumstances and unsafe in others; the full sedative burden matters. |
| Weight and appetite | Is unintended weight loss occurring, or is weight gain a concern? | NCI notes that effects such as sedation or weight gain may be considered in particular clinical situations, but they are not goals to pursue without a clinician. |
| Sexual function | Are libido, arousal, erection, lubrication, orgasm, or fertility already major concerns? | Cancer treatment and depression medicines can both affect sexual health, so this priority should be explicit. |
| Heart and other conditions | Are there cardiovascular, neurologic, liver, kidney, or other comorbidities? | Medical conditions can make some options less suitable and may change monitoring needs. |
| Interactions | Does the regimen include tamoxifen, sedatives, opioids, anticoagulants, or supplements? | Drug metabolism and additive effects can alter safety or effectiveness. |
| Past response | What helped or caused problems previously? | Your history and response may be more useful than a general ranking of medicines. |
Avoid common self-treatment pitfalls
- Do not skip doses to test a theory. A missed dose can blur the symptom pattern and may cause problems from interruption.
- Do not share tablets or use another person’s taper. Dose, duration, formulation, and cancer medicines differ.
- Check duplicate ingredients. Cold remedies, sleep products, pain medicines, and supplements may add sedation or interact with prescribed therapy.
- Do not assume an over-the-counter product is harmless. Ask about antidiarrheals, anti-nausea products, hormones, vitamins, and herbs before use.
- Keep after-hours instructions visible. Know whether to call oncology, the prescribing office, a pharmacist, or emergency services for a particular warning sign.
Starting, stopping, or switching safely
Allow time for benefit
The NCI patient depression page says some antidepressants take three to six weeks to work and that treatment often starts at a low dose that is gradually increased. This is general patient guidance, not a guarantee and not a cancer-specific trial result. Do not increase a dose early because you do not yet feel better, and do not conclude that a medicine is ineffective without discussing the expected timeline with the prescriber.
If a change is needed
- Tell the prescriber why you want a change and list every symptom and interaction concern.
- Ask whether the symptom could be from cancer treatment or another medicine and whether an examination or test is needed.
- Ask for a written taper or cross-taper schedule if the clinician recommends stopping or switching.
- Confirm what to do if withdrawal-like symptoms, worsening depression, anxiety, or new side effects appear.
- Tell the oncology team about the planned change so cancer-treatment decisions and supportive medicines remain coordinated.
Never use someone else’s tapering schedule. The safest plan depends on the antidepressant, dose, duration, other medicines, cancer treatment, and your medical history.

Frequently Asked Questions
Could my nausea be from the antidepressant or from cancer treatment?
Either, both, or another condition may be involved. Record when nausea or vomiting occurs in relation to doses and treatment sessions, use the prescribed antiemetic plan, and ask the oncology and prescribing teams to review hydration, other medicines, and possible medical causes.
Should I stop my antidepressant before chemotherapy?
No. Do not stop, skip, or switch it without the prescribing clinician’s instructions. The National Cancer Institute advises checking with a doctor before stopping; a gradual dose reduction may be used when a change is appropriate.
Is sleepiness from mirtazapine dangerous during cancer treatment?
Mirtazapine’s sedation may be undesirable when combined with opioids, benzodiazepines, or other sedating medicines. Tell the team about unintentional dozing, falls, confusion, or driving impairment so they can review the full medication list. Do not change the medicine yourself.
How should I raise sexual side effects?
Use direct, specific language about reduced desire, pain, dryness, erection, ejaculation, arousal, or orgasm. Cancer, treatment, depression, and medicines can all contribute. Ask which lubricants, pelvic-health, fertility, or prescription options are safe with your regimen.
Can I take St. John’s wort or another natural product?
Ask the oncology and prescribing teams before using any herb or supplement. St. John’s wort has been associated with lower blood levels of some anticancer drugs and can interact with depression medicines in a way that raises serotonin dangerously.
What should I do if tamoxifen is part of my treatment?
Tell both clinicians before starting or changing an antidepressant. NCI guidance identifies potent CYP2D6 inhibitors such as paroxetine and duloxetine as not recommended with tamoxifen because they may reduce formation of active endoxifen. This requires an individualized review, not an unsupervised stop or switch.
How long might an antidepressant take to help?
NCI patient guidance says some antidepressants take three to six weeks to work and are often started at a low dose that is gradually increased. Your prescriber should set expectations and decide whether and when a change is needed.
The Bottom Line
Track symptoms and treatment timing, share one complete medicine and supplement list with both teams, and ask for an individualized interaction review. Do not change an antidepressant or add a product on your own; seek prompt or emergency help for severe symptoms, dangerous sedation, dehydration, or suicidal thoughts.
