Most post-vaccination discomfort can be managed at home. Rest, fluids, comfortable clothing, a cool damp cloth on a sore injection site and gentle arm movement are usually enough while a mild fever, soreness, tiredness or headache settles over the next few days. Check the person’s temperature if they feel hot, ask a clinician before giving medicine—especially to a child—and seek urgent medical care immediately for a suspected severe allergic reaction.

What counts as a common vaccine reaction?

Vaccines train the immune system, so short-lived inflammation is expected after some injections. The Centers for Disease Control and Prevention (CDC) says common effects are usually minor and go away within a few days. Depending on the vaccine, they can include a sore, red or swollen injection site; low-grade fever; tiredness; headache; muscle aches; chills; nausea; or a mild rash. These are possible vaccine-specific effects, not a checklist that every person will experience after every vaccine.

The NHS says redness, swelling and soreness where the injection was given commonly last two to three days. Feeling unwell or having a high temperature often lasts one or two days, although individual timing varies. The CDC’s current COVID-19 vaccine guidance also says effects generally resolve within a few days, but some people temporarily find normal activities harder.

A reaction that is mild and gradually improving is usually suitable for home care. A symptom that is getting worse, is worrying, or is not beginning to settle after a few days deserves advice from a healthcare professional.

What to do during the first few days

Let the person rest

Reduce strenuous activity while fever, aches or fatigue are present. Sleep and quiet activities are reasonable for a child or adult who otherwise seems alert and is breathing normally. There is no need to force bed rest once the person feels well enough to move around, but avoid exercise that makes dizziness, weakness or pain worse.

Offer frequent fluids

Encourage regular drinks rather than insisting on a large amount at once. Water, breast milk, formula and other usual fluids are appropriate according to age and the person’s normal diet. A child who is nauseated may manage small, frequent sips better than a full cup. Watch for an inability to keep fluids down, markedly reduced urination, unusual sleepiness or other signs that dehydration may be developing; those are reasons to contact a clinician.

Choose comfortable clothing

Dress the person in light, comfortable layers and keep the room at a comfortable temperature. Avoid bundling someone in heavy blankets to “sweat out” a fever. Shivering can occur with a temperature change, but adding excessive layers can make the person uncomfortable and can make temperature readings harder to interpret.

Ease a sore or swollen arm

Place a clean, cool, wet washcloth over the injection site for short periods. Do not put ice directly on skin. Use the arm gently during the day; normal, comfortable movement can help prevent stiffness. Stop or reduce movement if it sharply increases pain. Increasing redness, warmth, swelling or tenderness after the first 24 hours should be discussed with a healthcare professional, particularly when the area is spreading or the person is becoming more unwell.

How to check a child’s temperature

Use a thermometer, not touch alone

Skin can feel hot for reasons other than fever, and a hand cannot measure temperature reliably. Use a digital thermometer according to its instructions and record the reading, time and measurement method. This gives a clinician useful information if the illness progresses. The UK government’s under-five guidance for England defines fever as a temperature above 38°C.

Interpret the number with the child’s condition

A temperature reading is only one part of the assessment. Note whether the child is drinking, urinating, waking normally, breathing comfortably and responding as usual between periods of sleep. A child who looks seriously ill, is difficult to wake, has trouble breathing or cannot keep fluids down needs medical advice regardless of whether the number seems modest.

Infants and young children may have vaccine-specific instructions, and age thresholds differ between countries. Do not transfer an England under-five rule to another jurisdiction without checking local guidance.

Should you give pain or fever medicine?

Do not routinely premedicate

For COVID-19 vaccination, the CDC advises not taking pain relievers or other medicines before the appointment solely to prevent side effects. Premedicating can also hide symptoms that a clinician may need to assess, and it is not a universal recommendation for every vaccine or every person.

Ask about treatment after vaccination

After vaccination, ask a doctor, pharmacist or the vaccination provider whether an over-the-counter pain reliever is appropriate. The correct choice and amount depend on age, body size, pregnancy, allergies, liver or kidney disease, other medicines, the vaccine received and the local product formulation. Follow the package or clinician’s directions exactly, and never combine products that contain the same active ingredient.

For a child, the CDC specifically advises asking the child’s healthcare provider about a non-aspirin pain reliever and other comfort measures. Do not give aspirin to a child unless a clinician has prescribed it for a specific reason. Do not estimate a dose from an adult product, a sibling’s medicine or an old label.

England’s infant MenB exception

England has a tightly defined exception for babies receiving the routine meningococcal B (MenB) vaccine. UK government guidance says to give three doses of infant paracetamol after the two- and three-month vaccination visits: the first dose as soon as possible after vaccination, followed by the next doses four to six hours apart. The cited instructions specify 2.5 ml of a 120 mg/5 ml suspension.

This schedule is for England’s infant MenB programme. It must not be generalized to another vaccine, an older child, a different country, a different concentration or a different vaccination schedule. Check the current instructions supplied by the vaccination service and ask a healthcare professional if the product, dose or timing is unclear.

A simple home-monitoring plan

  1. Note the timing. Write down when the vaccine was given and when fever, soreness or other symptoms began. Different vaccines have different expected patterns.
  2. Check how the person looks and acts. Record alertness, breathing, fluid intake, urination and ability to move the affected limb comfortably.
  3. Measure temperature when useful. Use a thermometer for a child who feels hot or seems unwell, and record the result rather than relying on touch.
  4. Apply basic comfort care. Offer fluids, rest, comfortable layers, a cool damp cloth and gentle arm movement.
  5. Review rather than repeatedly treat the number. Reassess comfort and overall condition after rest and fluids. Medicine decisions should follow professional advice, not an automatic temperature threshold.
  6. Escalate if the course changes. Contact a healthcare professional when symptoms worsen, cause concern or fail to improve after a few days; seek emergency care for suspected severe allergy.

When to contact a healthcare professional

Call a doctor, pharmacist, vaccination provider or local health service when the reaction does not follow a reassuring course. For COVID-19 vaccination, CDC guidance specifically says to seek advice if redness or tenderness at the injection site gets worse after 24 hours, or if side effects are worrying or do not seem to be going away after a few days.

  • The fever, pain, rash or other symptom is worsening instead of gradually easing.
  • The injection-site redness, swelling or tenderness is expanding or becomes more painful after the first day.
  • The person is drinking much less, urinating much less, repeatedly vomiting or cannot keep fluids down.
  • The person is unusually drowsy, confused, difficult to wake, persistently inconsolable or not behaving normally.
  • Breathing is labored, the person has chest symptoms, or a new symptom is worrying you.
  • You are unsure which medicine, concentration or dose is safe, especially for an infant, pregnant person or someone with a chronic condition.

WHO’s general vaccine-safety advice is straightforward: “If you or your child has an unexpected reaction after vaccination, consult with your health-care worker.” Bring the vaccine name, date and time, symptom timeline, temperature readings and any medicine already given.

Recognize a severe allergic reaction

Serious allergic reactions are rare. The NHS says they usually happen within minutes of vaccination, and vaccination staff are trained to respond. A reaction can still become apparent after leaving the vaccination site.

Get immediate medical care if a severe allergic reaction is suspected. Warning signs can include trouble breathing, swelling of the lips, tongue or throat, widespread hives, fainting, collapse or rapidly worsening weakness. Do not wait at home to see whether these symptoms resolve. Use the emergency number and pathway for the country where you are; tell responders which vaccine was given and when.

A mild, localized rash or a sore arm is not automatically anaphylaxis, but rapidly spreading symptoms, breathing difficulty or collapse should be treated as an emergency. If symptoms are uncertain, err toward urgent assessment rather than delaying.

Why the same advice does not fit every vaccine

Expected reactions differ by vaccine, age and location. CDC vaccine-specific summaries describe fever and soreness after several vaccines, while other vaccines can have distinct timing or additional expected symptoms. A symptom listed for one vaccine should not be presented as inevitable after another.

COVID-19 aftercare instructions are specific to COVID-19 vaccination. The England paracetamol schedule is specific to infant MenB visits. Infant guidance can also be programme- and country-specific, so use the written instructions from the service that vaccinated the child rather than copying a schedule found for another region.

People with a history of serious allergy, immune-system problems, pregnancy, significant chronic disease or complex medication use should ask the vaccination provider what to expect before leaving. That conversation can clarify which symptoms are expected, which medicines are compatible and where to call if the course is not typical.

Common mistakes to avoid

Do not treat every post-vaccine symptom as an emergency

A sore arm, tiredness or a short-lived low-grade fever commonly improves within a few days. Monitor the person’s overall condition and the direction of change instead of reacting to every symptom with unnecessary treatment.

Do not dismiss a worsening reaction as “normal”

Expected does not mean every symptom is harmless indefinitely. Worsening injection-site pain after 24 hours, persistent or escalating fever, dehydration, altered behavior or any breathing problem warrants professional assessment.

Do not use a one-size-fits-all dose

Liquid medicines may come in different concentrations, and children’s doses are not interchangeable by volume. Read the exact label, use the supplied measuring device and ask a clinician or pharmacist when anything is unclear.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not confuse fever relief with prevention

Comfort medicine may reduce pain or fever after vaccination when a professional considers it appropriate; it is not a reason to take medicine before every appointment. Preventive dosing rules, when they exist, are tied to a specific programme such as England’s infant MenB guidance.

What to have ready before a vaccination appointment

  • The person’s current medicines, allergies and relevant medical conditions.
  • The name of the vaccine and the date of the appointment, if known.
  • A digital thermometer for checking a child who later feels hot.
  • Comfortable clothing that leaves the upper arm accessible without tight sleeves.
  • Contact details for the vaccination provider, primary-care service and local emergency service.

Ask before leaving how long the expected reactions should last, which symptoms require a call, whether a particular medicine is suitable and whether the person should follow any vaccine-specific instructions. Clear instructions at the appointment prevent guessing later.

Quick decision guide

Situation What to do
Mild soreness, redness, tiredness or low-grade fever that is improving Rest, offer fluids, use comfortable clothing, apply a cool damp cloth and move the arm gently.
Child feels hot or seems unwell Check with a thermometer, record the result and observe drinking, urination, breathing and alertness.
Unsure about a pain reliever or dose Ask a doctor, pharmacist or vaccination provider; do not guess or copy another person’s dose.
Injection-site redness or tenderness worsens after 24 hours, or symptoms remain worrying after a few days Contact a healthcare professional.
Difficulty breathing, swelling of the face or throat, widespread hives, fainting or collapse Seek immediate emergency medical care for suspected severe allergic reaction.

How long should you keep watching?

Pay closest attention during the first few days, when common reactions usually settle. Continue observing until the person is back to their normal pattern of drinking, activity and behavior. If improvement stops, symptoms return strongly or a new concerning symptom appears, contact a healthcare professional rather than assuming it is still an ordinary vaccine effect.

Keep a brief record if medical advice is needed: vaccine and time, symptom onset, temperature readings, fluid intake, medicines and the person’s response. That information helps the clinician distinguish a short-lived expected reaction from an unrelated illness or a less common vaccine-associated problem.

Adults, children and infants need different checks

Adults can usually describe pain, chills, headache or dizziness directly, while a baby may show discomfort by feeding less, crying more or sleeping differently. For either age group, look at the whole pattern: alertness, breathing, hydration and whether symptoms are easing. A normal-looking temperature does not override a person who appears seriously ill, and a mildly elevated reading does not by itself prove a dangerous reaction.

Infants deserve a lower threshold for asking advice because they cannot explain symptoms and because vaccine schedules vary. Keep the discharge information from the clinic available, including the vaccine name and any planned follow-up. Never substitute advice written for an older child or a different vaccine. If an infant is hard to wake, has breathing difficulty, is not feeding or has very few wet diapers, contact a clinician promptly.

Make comfort care practical

Set up a quiet place where the person can rest, with drinks within reach and a thermometer available if needed. Check the injection site in good light so you can tell whether redness is stable or expanding. A cool, damp cloth can be reapplied for comfort, but the skin should not become numb or damaged. Gentle movement means ordinary use of the arm, not forceful stretching or massage of a painful area.

Do not use a fever reading as a reason to wake a comfortably sleeping person repeatedly just to give medicine unless a clinician has instructed you to do so. Instead, check periodically that the person is breathing comfortably and can be roused normally. If you are concerned about any change, call for advice rather than trying additional home treatments.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When symptoms may be unrelated

People can develop a cold, stomach illness or another infection around the same time as vaccination. The timing alone does not prove that every symptom came from the vaccine. A symptom that persists beyond the usual few-day period, returns after clear improvement or comes with a new focal problem should be discussed with a healthcare professional. Give the clinician the complete timeline, including other sick contacts or symptoms, rather than assuming the vaccine is either definitely responsible or definitely unrelated.

The CDC’s “Possible Side Effects from Vaccines” page, dated July 31, 2024, emphasizes that effects depend on the particular vaccine. The NHS similarly describes common reactions as short-lived and notes that serious allergy is uncommon. These general patterns help with planning, but they do not replace the instructions given for the specific appointment.

How to prepare information for a call

Before calling, write down the vaccine and dose if known, the date and time it was given, when each symptom started, the highest measured temperature, what the injection site looks like, how much the person is drinking and urinating, and any medicine or home treatment already used. Mention allergies, pregnancy, chronic conditions and regular medicines. A clear timeline helps the doctor, pharmacist or vaccination service decide whether to reassure, review the person or direct you to emergency care.

If you leave a message, state the best callback number and describe any immediate warning sign first. For breathing trouble, facial or throat swelling, collapse or rapidly spreading hives, do not wait for a routine callback; use the local emergency pathway.

Frequently Asked Questions

How long does a mild fever after vaccination usually last?

Many common vaccine reactions improve within a few days. NHS guidance says feeling unwell or having a high temperature often lasts one or two days, while injection-site soreness, redness or swelling commonly lasts two to three days. Timing varies by vaccine and person. Contact a healthcare professional if symptoms worsen, are worrying or are not beginning to improve after a few days.

Should I give my child paracetamol or another pain reliever before vaccination?

Do not routinely give medicine before a COVID-19 vaccination just to prevent side effects. After vaccination, ask the child’s healthcare provider, pharmacist or vaccination service whether a non-aspirin pain reliever is suitable and which product and dose to use. Do not copy an adult or sibling’s dose.

Can I use a cool cloth on a sore injection site?

Yes. CDC guidance recommends a clean, cool, wet washcloth for pain or swelling. Use the arm gently during the day, but do not put ice directly on the skin or force movement that sharply increases pain.

What temperature is considered a fever in a young child?

UK government guidance for children under five in England defines fever as a temperature above 38°C. This rule is location- and age-specific, so check local guidance. Always consider the child’s behavior, breathing, hydration and alertness as well as the number.

When is a post-vaccine reaction an emergency?

Get immediate medical care for suspected severe allergy, including trouble breathing, swelling of the lips, tongue or throat, widespread hives, fainting, collapse or rapidly worsening weakness. Serious allergic reactions are rare and often occur within minutes, but do not wait at home if warning signs appear after leaving the clinic.

What is special about paracetamol after infant MenB vaccination?

In England’s infant MenB programme, government guidance specifies three doses after the two- and three-month visits: the first as soon as possible, then four to six hours apart, using the stated 2.5 ml dose of 120 mg/5 ml suspension. This does not apply automatically to other vaccines, ages, formulations or countries; follow current local instructions.

What details should I give a clinician if I call?

Have the vaccine name, date and time, symptom onset, temperature readings, fluid intake, urination, injection-site changes and any medicine already given. Also mention allergies, pregnancy, chronic conditions and regular medicines. This timeline helps distinguish a short-lived expected reaction from another illness or a problem needing assessment.

The Bottom Line

Mild fever, tiredness, headache and a sore arm usually settle within a few days. Use rest, fluids, comfortable clothing, a cool damp cloth and gentle movement. Ask a healthcare professional about medicines, follow vaccine- and location-specific instructions, and seek immediate emergency care for breathing difficulty, facial or throat swelling, widespread hives, fainting or collapse.
