You cannot guarantee that you will avoid Alzheimer’s disease, but you can lower several preventable or delayable risks across your lifetime. The most useful priorities are controlling blood pressure, LDL cholesterol, blood sugar and weight; staying physically active; not smoking; limiting alcohol; treating hearing and vision loss; addressing depression; remaining cognitively and socially engaged; preventing head injuries; and reducing air-pollution exposure when practical.

The World Health Organization (WHO) said in 2026 that up to 45% of dementia risk may be attributable to modifiable factors. That is a population estimate—not a promise that a particular person can prevent Alzheimer’s. Your plan should be adapted with a clinician, especially if you have hypertension, diabetes, high LDL cholesterol, depression, hearing or vision loss, or medicines that require monitoring.

What “lowering risk” really means

Alzheimer’s disease results from multiple interacting influences, including age, genetics, health conditions and environment. Some influences cannot be changed. Lifestyle measures target the part of risk that can be changed, and many of the same measures also reduce stroke, heart disease, falls, disability and other causes of cognitive decline.

Risk reduction is therefore a life-course project rather than a short challenge. A healthy choice made in midlife may affect later brain health, while changes made in later life can still improve cardiovascular fitness, mood, hearing, safety and independence. No supplement, app, wearable, single food or diet has been proven to independently prevent Alzheimer’s disease.

The 14 potentially modifiable risk factors

The 2024 Lancet Standing Commission identifies 14 factors that together represent important opportunities for prevention or delay at a population level.

Factor Practical response Important qualification
Less education Pursue sustained learning and skill development throughout life. Education is shaped heavily by early-life opportunity; later learning is beneficial but cannot retroactively replace formal schooling.
Hearing loss Obtain a hearing test, treat reversible problems and use properly fitted hearing aids when recommended. Hearing devices support communication and engagement; they are not a guaranteed Alzheimer’s prevention.
Hypertension Have blood pressure checked and follow an individualized treatment plan. Do not change medicines solely on home readings without clinical advice.
Physical inactivity Build regular aerobic and strengthening activity that fits your health and abilities. Consistency and safety matter more than an unsustainable program.
Diabetes Monitor and manage blood sugar with a clinician. Targets and medicines differ by person; avoid self-adjusting treatment.
Social isolation Schedule recurring contact and meaningful participation in groups, work, volunteering or family life. Online contact can help, but choose forms that provide genuine connection.
Excessive alcohol use Reduce intake and seek support if cutting down is difficult. People who drink heavily may need medical guidance to stop safely.
Air pollution Follow local air-quality alerts and improve indoor ventilation when feasible. Exposure cannot always be avoided; practical reduction is the goal.
Smoking Stop smoking and avoid secondhand smoke. Quitting also lowers stroke, diabetes and hypertension risks.
Obesity Use sustainable eating, activity and medical support to improve cardiometabolic health. Weight is only one measure of health; avoid unsafe crash diets.
Traumatic brain injury Use helmets and safer practices for cycling, contact sports, motorcycles and hazardous work. Seek urgent care for warning symptoms after a head impact.
Depression Seek assessment and effective treatment, including therapy, medication or both when appropriate. Persistent low mood, withdrawal or loss of interest deserves professional attention.
High LDL cholesterol Ask whether testing and treatment are appropriate for your cardiovascular risk. Medication decisions depend on the full clinical picture.
Vision loss Obtain regular eye care and correct treatable vision problems. Better vision can support mobility, social participation and cognitive activity.

Start with a personal risk-reduction appointment

Make a written list of your current conditions, medicines, family history, smoking or alcohol use, activity, sleep, mood, hearing and vision. Ask a primary-care professional which checks are due and which changes offer the largest benefit for you.

  1. Check vascular measures. Discuss blood pressure, LDL cholesterol, blood sugar and weight in the context of your age, medical history and other cardiovascular risks.
  2. Review barriers. Pain, breathlessness, balance problems, depression, hearing difficulty, unsafe housing or medication side effects can make healthy routines hard to sustain.
  3. Choose two or three first actions. A manageable plan—such as walking on scheduled days, arranging a hearing test and stopping smoking—usually works better than trying to change everything at once.
  4. Set a follow-up date. Bring home readings or a simple activity log. Use them to discuss progress, not to diagnose yourself.

Manage blood pressure, cholesterol, diabetes and weight

Vascular health is one of the most practical ways to support both brain and body. High blood pressure, diabetes, high LDL cholesterol and obesity are among the modifiable factors linked with dementia risk, and they also raise the risk of stroke and heart disease.

Blood pressure

Ask how often you should have your pressure measured and what target is appropriate. A validated home monitor can provide a series of readings for a clinician. Measure as instructed, record the date and time, and take the log to appointments. A monitor does not diagnose or prevent Alzheimer’s by itself, and a single unusual reading is not a reason to change medication without advice.

LDL cholesterol and blood sugar

Discuss whether you need an LDL test, diabetes screening or follow-up testing. Treatment may include food changes, activity, weight management and prescription medicines. The correct combination depends on your overall risk, kidney function, other conditions and possible side effects. Do not stop a statin, diabetes medicine or blood-pressure medicine because of an online claim.

Weight

Focus on cardiometabolic health rather than a particular appearance. A clinician or registered dietitian can help you choose an eating pattern and activity level that are realistic, especially if you have diabetes, take medicines affected by weight change, or have a history of disordered eating.

Move regularly and safely

WHO recommends physical activity as part of dementia-risk reduction. The best routine is one you can repeat without causing injury. Combine activities that raise your breathing rate with muscle-strengthening and balance work as your health allows.

Build a sustainable routine

  • Start below your maximum and increase gradually.
  • Break activity into shorter sessions if a long workout is unrealistic.
  • Choose walking, cycling, swimming, dancing, chair exercise or other options that fit your joints, balance and environment.
  • Add strengthening with body weight, resistance bands or weights when a professional says it is safe.
  • Use a tracker or pedometer only as an adherence aid; the device is not a treatment.

People with chest pain, unexplained fainting, severe breathlessness, major balance problems or significant chronic disease should ask a clinician how to begin. Stop and seek medical help for alarming symptoms during exercise.

Do not smoke and address alcohol use

Smoking

Stopping smoking is one of the clearest health actions available. It reduces exposure to vascular toxins and also lowers stroke, diabetes and hypertension risks. Ask about counseling, approved medicines and quit-support services. If you relapse, treat it as a signal to adjust the plan rather than a reason to abandon it.

Alcohol

Excessive alcohol use is a modifiable dementia risk factor and can worsen blood pressure, sleep, mood, falls and medication interactions. Set a reduction goal with professional support if needed. Anyone who drinks heavily every day or has had withdrawal symptoms should seek medical advice before stopping abruptly, because withdrawal can be dangerous.

Eat for cardiovascular and metabolic health

A Mediterranean-like pattern is a reasonable option to discuss: emphasize vegetables, fruit, beans, whole grains, nuts, fish or other nutritious proteins, and unsaturated fats while limiting highly processed foods and excess salt or added sugar. The aim is an overall pattern that supports blood pressure, cholesterol and blood sugar, not a list of “memory foods.”

NIA says evidence for any particular diet and Alzheimer’s prevention remains mixed. Avoid products that promise prevention through a supplement, powder or restrictive menu. If you have diabetes, kidney disease, food allergies, swallowing difficulty or a history of eating disorders, obtain individualized nutrition advice.

Treat hearing and vision loss

Hearing

Hearing loss can reduce conversation, learning and participation, increasing the risk of social isolation. Arrange a professional hearing assessment rather than relying on an untested amplification product. Remove earwax or treat other reversible causes when appropriate. If hearing aids are recommended, proper fitting, use and follow-up matter; hearing aids are a support for communication, not proof that dementia will be prevented.

Vision

Keep up with eye examinations and treat correctable problems. Updated glasses, cataract care or other appropriate treatment can make movement, reading, social activity and exercise safer. Seek prompt care for sudden vision loss, severe eye pain or new neurological symptoms.

Stay mentally and socially engaged

Learning and meaningful activity help maintain participation and address social-isolation risk. Choose activities that require attention and are enjoyable enough to continue: taking a class, learning an instrument or language, reading and discussing books, gardening, volunteering, making things, or helping with community projects.

Regular contact can be structured rather than spontaneous. Put a weekly call, walking appointment, club meeting or shared meal on the calendar. If transportation, hearing, vision, mobility or caregiving makes attendance difficult, ask a community organization or health professional about accessible options.

Commercial brain-training products may provide entertainment, but no consumer program is proven to prevent Alzheimer’s disease. Prefer varied, purposeful activities over paying for a promise of disease prevention.

Protect your head

Use a properly fitted helmet for activities where it is recommended, wear seat belts, follow workplace safety rules and reduce fall hazards at home. After a head impact, seek urgent medical advice for worsening headache, repeated vomiting, confusion, unusual drowsiness, weakness, seizure, speech trouble or loss of consciousness. A history of traumatic brain injury should be discussed with your clinician.

Recognize and treat depression

Depression can reduce activity, sleep quality, social contact and motivation. Symptoms may include persistent sadness, loss of interest, hopelessness, irritability, sleep or appetite changes, and difficulty concentrating. Arrange an assessment rather than assuming these changes are inevitable aging. Effective care may include psychotherapy, medication, treatment of contributing medical problems and practical social support. If you may harm yourself, contact emergency services or a crisis service immediately.

Reduce air-pollution exposure where feasible

Air pollution is a modifiable factor, although individual control varies by location and job. Check local air-quality advice, limit strenuous outdoor activity during severe pollution episodes, keep indoor spaces ventilated when outdoor air is cleaner, and use appropriate filtration if recommended for your setting. Avoid indoor smoke from tobacco and poorly vented combustion. These steps reduce exposure; they do not eliminate all pollution-related risk.

How the main options compare

Action Evidence and expected benefit Cost and access Supervision and safety
Blood-pressure management Repeatedly recommended for brain and cardiovascular health; NIA describes prevention evidence as encouraging but inconclusive. Checks may be available through primary care or validated home monitoring; medicines may cost extra depending on coverage. Clinician guidance is important for diagnosis, targets and medication changes.
Physical activity Recommended by WHO; supports cardiovascular fitness, mobility and mood, while Alzheimer’s-specific evidence is not a guarantee. Walking and home exercise can be low-cost; accessible facilities vary. Adapt intensity for chronic disease, pain and balance; seek advice when risk is high.
Smoking cessation Reduces several vascular risks in addition to possible dementia risk. Free counseling may exist; medicines and programs vary by region. Professional support improves safety and the chance of sustained quitting.
Alcohol reduction Addresses excessive-use risk and may improve blood pressure, sleep, falls and mood. Peer, behavioral and medical support options differ locally. Heavy daily drinkers should obtain advice before abrupt cessation.
Mediterranean-like eating pattern Reasonable for cardiometabolic health; evidence for one diet preventing Alzheimer’s is mixed. Costs depend on local food prices and cooking access. Individualize for diabetes, kidney disease, allergies and eating-disorder history.
Hearing care Recommended to preserve communication and engagement; prevention of Alzheimer’s is not established for every individual. Testing, fitting and devices may be expensive or subsidized depending on location. Use qualified hearing professionals and attend follow-up visits.
Cognitive and social engagement Supports participation and may address isolation; no consumer brain-training product is proven to prevent disease. Can be free through libraries, community groups, family and volunteering. Choose activities that are safe, meaningful and accessible.
Air-pollution reduction Addresses an identified modifiable exposure, with benefits influenced by local conditions. Following alerts is free; filtration and housing changes may cost more. Do not compromise ventilation or safety when using filtration equipment.

What if Alzheimer’s runs in your family?

Family history can increase risk, but it does not determine your outcome. Genes, shared environment and shared habits may all contribute. A family history is a reason to discuss your personal risks and screening needs with a clinician—not a reason to assume prevention is impossible.

Bring details about which relatives were affected and approximately when symptoms began. Ask whether blood-pressure, cholesterol, diabetes, hearing, vision, mood and lifestyle support should be prioritized. Genetic testing can have emotional, insurance and privacy implications; do not order a test solely because an advertisement suggests it will predict or prevent Alzheimer’s.

What the evidence can and cannot establish

The National Academies review summarized by NIA found “encouraging but inconclusive” evidence for blood-pressure management, increased physical activity and cognitive training. NIA also identifies diet, sleep, diabetes management and hearing devices as active research areas. This means the actions remain sensible for overall health, but no single one can be presented as a guaranteed Alzheimer’s-prevention treatment.

WHO’s 2026 estimate that up to 45% of dementia risk may be attributable to modifiable factors describes what could potentially be prevented or delayed across populations. It cannot be converted into a personal percentage. Even someone who follows every recommendation may develop Alzheimer’s, and someone with several risk factors may not.

A simple maintenance checklist

  • Keep scheduled primary-care visits and bring a current medicine list.
  • Know when your blood pressure, LDL cholesterol and blood sugar should be checked.
  • Keep activity, smoking and alcohol goals realistic and review them after setbacks.
  • Arrange hearing and vision assessments when communication, reading or balance changes.
  • Maintain recurring social and mentally stimulating activities.
  • Use helmets, seat belts and fall-prevention measures appropriate to your activities.
  • Follow local air-quality guidance and avoid indoor smoke.
  • Seek help promptly for depression, substance dependence or new neurological symptoms.
