Use your provider’s patient portal for information held by that provider, and keep a personal health record to summarize key details across doctors and hospitals. When you need to transfer records, use the provider’s electronic access and sharing options if available. These tools serve different purposes, and their features and privacy protections depend on who operates them.

Which tool should you use?

Tool What it can do What to check
Provider patient portal Show provider-held results, visit summaries, medications, allergies, appointments, and other details; some portals also support refill requests, forms, billing, and secure messages. Features vary by provider. Information from several providers may appear together only if those providers are connected to the same portal.
Electronic record access and sharing Help you access records held by doctors, hospitals, or health plans, check them, and share them with a clinician or trusted caregiver. Availability and completeness depend on what the organization makes accessible. You are responsible for protecting copies you download.
Personal health record Give you a concise summary of important health information gathered across care settings. You maintain it, so update it when information changes and distinguish your summary from official provider records.
Health or wellness app Depending on the app, track information or provide health education. Check who operates it, what it does with your information, and whether it is an organizer, wellness tool, or software with a medical function.

For test results, visit summaries, prescriptions, and routine questions, start with the relevant provider portal. For a cross-provider overview, maintain a personal health record. To move official records between care settings, look for electronic access and sharing options from the organizations that hold them.

How to find and use a provider portal

A patient portal is a website for personal health care. Depending on the provider, it may show test results, visit summaries, medication and allergy history, appointments, refill requests, forms, billing, and secure messages. You generally need to register with the provider. A portal may include information from more than one provider when they are connected to the same system, but do not assume it is a complete record of all your care. MedlinePlus explains patient portals and their typical uses.

  1. Find the portal for the organization that provided care. Use the provider’s website or ask its office how to register.
  2. Look for the information you need. Common sections include results, visit summaries, medications, allergies, appointments, and messages.
  3. Check details against your own records. If a medication, allergy, or other item appears incomplete or incorrect, contact the provider to ask how it can be corrected.
  4. Use messaging for routine matters. Portal messages are not for urgent or time-sensitive concerns; call the provider’s office for those.

How to get records from different doctors

Electronic access can help you see or share information held by doctors, hospitals, and health plans. What is available depends on what each organization makes accessible. The Office of the National Coordinator for Health Information Technology (ONC) recommends checking electronic records for accuracy and completeness and sharing relevant information with a clinician or trusted caregiver. Its Blue Button guidance describes ways to access and share health information.

What to put in a personal health record

A personal health record helps bring together important details when your information is spread across several clinicians and hospitals. MedlinePlus recommends keeping one, but does not prescribe a particular product or format. You can use paper or a digital document.

How to assess a health app’s privacy and role

Do not assume every independent health app is covered by HIPAA. Privacy and security requirements can depend on the operator, the context, and applicable federal or state laws. The HHS Office for Civil Rights directs developers and others to an interagency Mobile Health Apps Interactive Tool to help identify which laws may apply. Before entering health details, check who operates the app, what information it collects, how it is used or shared, and what account or device protections are available.

Also distinguish an information organizer or wellness app from software that performs a medical function. FDA oversight is function- and risk-based: some mobile software functions are not medical devices, some may receive enforcement discretion, and others are subject to oversight. FDA points to higher-risk examples such as software that diagnoses conditions or controls the function of a traditional medical device. A health record or wellness app is not automatically FDA-approved or regulated simply because it concerns health. See the FDA’s digital health policy resources.

When an education service is not a personal record

MedlinePlus Connect is a free National Library of Medicine service for health organizations and IT providers. It can return relevant MedlinePlus education links in response to diagnosis, medication, lab, or procedure codes sent by a portal or electronic health record. It is an integration service, not a consumer tool for storing or managing your personal record. Details are available from MedlinePlus Connect.
