Keep each architecture decision record (ADR) focused on one significant choice, put the decision and rationale first, show the important trade-offs, and preserve history when the choice changes. An accepted ADR should not be silently rewritten: mark it superseded or deprecated, link a new ADR, and use explicit context changes as review triggers. Store records where their intended readers can version, search and maintain them.
What makes an ADR useful over time?
An ADR records an architecturally significant choice and the reasoning around it: the problem or constraint, the option selected, alternatives considered, and consequences that later engineers need to understand. It is not a meeting transcript, a complete design specification or a diary of every implementation detail. Its lasting value is that a future reader can answer four questions quickly:
- What did the team decide?
- Why did that decision make sense in the circumstances?
- What alternatives and trade-offs were considered?
- Is the decision still active, and what replaced it if it is not?
Martin Fowler describes ADRs as brief records of decisions and recommends putting the most important material at the beginning. The Architectural Decision Records organization similarly defines them around a significant design choice and its rationale. That narrow purpose is what keeps a collection readable: record decisions whose reasoning will matter to later work, not every design note.

Split unrelated choices into separate records. For example, choosing an event-bus technology and choosing a data-retention policy may be related in a project plan but have different alternatives, owners and review triggers. Separate ADRs let a later team supersede one decision without implying that every other choice changed.

Front-load the answer
Start with a meaningful title or identifier, status and a short decision statement. A reader scanning a directory should not have to read several paragraphs to find the outcome. Put supporting diagrams, benchmarks, meeting notes or detailed specifications in linked documents rather than expanding the ADR into a report.
How do I know when a decision deserves an ADR?
Create a record when a choice has a meaningful effect on the system’s architecture or on functional or non-functional requirements. Typical examples include a service boundary, persistence model, integration protocol, deployment topology, security approach or reliability trade-off that will constrain future work.
Do not use an ADR as a substitute for every design explanation. A local refactoring, a routine coding convention or a temporary implementation detail normally belongs in code comments, a design note or a pull request. The test is future relevance: would another team need the rationale to safely change or extend the system?

Capture the decision while context is available
Write the record while the assumptions, rejected options and constraints are still known. If discussion continues, use a proposed status and update the decision section when the team accepts or rejects it. A short, accurate record written near the decision point is more useful than a polished document reconstructed months later.
What should an ADR contain?
Choose a compact structure that the team can complete consistently. MADR provides full and minimal Markdown templates; Nygard and Y-Statement formats are other established alternatives. The exact template matters less than consistently recording the fields that explain the choice.

| Field | What to record | Why it helps maintenance |
|---|---|---|
| Title and identifier | A concise statement of the decision and a predictable number or name | Readers can recognize and link the record from indexes, commits and successor ADRs. |
| Status | For example, proposed, accepted, rejected, deprecated or superseded | Readers can tell whether the decision governs current work. |
| Context and problem | The need, constraints, assumptions and requirements that made a decision necessary | Later teams can test whether the original conditions still apply. |
| Decision | The option selected, stated directly | Prevents the rationale from obscuring the actual outcome. |
| Options and trade-offs | Serious alternatives with concise advantages and disadvantages | Explains why a plausible option was not chosen and prevents old debates from reopening without context. |
| Consequences | What becomes easier, harder, faster, riskier or more expensive | Sets expectations for implementation and future change. |
| Review trigger | A product, requirement, technology or assumption change that should prompt reconsideration | Creates a practical signal without inventing a calendar deadline. |
| Links and relationships | Supporting material, the ADR being superseded and, where useful, a link back from the successor | Preserves the decision timeline and lets readers follow evidence. |
| People and confidence | Decision makers, consulted or informed people, confirmation details or confidence when useful | Records accountability when those details help the team; omit them when they add ceremony without value. |
Write the context for a future reader
State the constraints that shaped the decision, such as latency needs, regulatory boundaries, team capabilities, compatibility requirements or an existing platform commitment. Avoid unexplained references to a conversation that only the original participants remember. If a discussion contains important evidence, summarize the conclusion and link the full material.
Show real alternatives, not a straw man
List the options that could reasonably have been selected. A brief table or bullet list is enough when it names the meaningful trade-offs. Explain why the chosen option fit the stated context, not why every other option was universally bad. This makes the ADR useful when the context changes: an alternative rejected for cost might become reasonable after a budget or scale change.

Status is a navigation aid, not a decorative label. Define a small vocabulary and apply it consistently. MADR examples include proposed, rejected, accepted, deprecated and superseded; Fowler commonly explains proposed, accepted and superseded. Your team may add states, but each must have an unambiguous meaning.
| Status | Meaning and maintenance action |
|---|---|
| Proposed | The decision is under discussion or awaiting approval. Do not present it as governing architecture. |
| Accepted | The team has adopted it and it describes the decision that governed the relevant work. |
| Rejected | The option or proposal was considered and not adopted. Keep the rationale so the same question is not revisited without new facts. |
| Deprecated | The decision is no longer recommended or applicable, but no single replacement necessarily governs every case. |
| Superseded | A later ADR replaces this decision. Add the successor’s identifier and update the new ADR with the predecessor link. |
Do not infer that an accepted ADR is permanently current. Its status says what happened to the record; the affected system and a later successor determine whether it still applies.
Should I edit an accepted ADR when the decision changes?
No. Leave the accepted ADR as a record of what was decided at that time. When a decision is materially modified or replaced, create a new ADR, mark the old one superseded or deprecated, and connect the records. Fowler’s guidance is explicit: “They should not be modified if the decision is changed, but linked to a superseding decision.”

What counts as a material change?
Create a successor when the selected technology, architectural boundary, governing constraint or important trade-off changes. A correction to a typo or a broken link does not create a new decision; make that editorial fix without altering the historical rationale. If the scope is narrowed, document the new scope in a successor when readers could otherwise misapply the old record.
How to link a decision chain
- Create the new ADR with its own sequential identifier and status, normally proposed until approved.
- In the new record, name the predecessor and explain what changed in context or requirements.
- After approval, set the predecessor’s status to superseded or deprecated and add a link to the successor.
- Update any index or rendered view so the relationship and current status are visible.
- Leave implementation history, commits and the original consequences attached to the original ADR.
This timeline prevents a future reader from believing that the old rationale supported a newer choice. It also shows which decision governed the system during each period.
How do I know whether an ADR is still current?
Review an ADR when work touches the architecture it describes or when an explicit trigger occurs. Fowler recommends noting context changes that should prompt reevaluation. The available guidance does not establish a universal calendar-based review interval, so do not assign a generic quarterly or annual expiry date as if it were a standard.
A practical current-status check
- Is the decision still active in the running system or current roadmap?
- Has another ADR superseded, deprecated or narrowed it?
- Do the predecessor and successor links agree, and does the index show the same status?
- Has a requirement, product goal, scale assumption, platform, regulation or risk tolerance changed?
- Does the implementation still match the consequences described?
When the answer to the last two questions is uncertain, open a decision task or write a proposed successor. Do not silently edit the old context to make it appear current.
Where should architecture decision records live?
Storage should match the decision’s scope and the people who need the rationale. For a choice tied to one codebase, a versioned Markdown file in that codebase is common because developers can review it with code, see history and update it through the normal pull-request workflow. A conventional directory is docs/decisions.

| Location approach | Best fit | Advantages | Risks to manage |
|---|---|---|---|
| Application or service repository | Decisions specific to one codebase | Version history, code-adjacent review, simple ownership and offline access | People outside the repository may not discover or contribute to the records. |
| Shared product or platform repository | Decisions spanning several related components | A common index and cross-component context | Ownership and scope can become unclear; links to implementation history may be farther away. |
| Published or shared documentation space | Ecosystem-wide decisions or teams including non-developers | Broader access and easier reading for people who do not work in source control | Ensure edits are versioned, permissions are clear and the authoritative copy is unambiguous. |
There is no universally best repository or documentation product. Compare scope, access, version history, discoverability, workflow fit and maintenance cost before choosing. A single code repository can be a poor fit when a decision spans an ecosystem or must be maintained by people without repository access.
Use predictable names and identifiers
MADR describes sequential filenames and a docs/decisions approach. A name such as 0007-use-event-stream.md is easy to sort and link. Large collections can be categorized, but category-local numbering can make identifiers ambiguous. Agree on whether identifiers are globally unique before the collection grows, and preserve the same naming rule for successors.
How can a team find the right ADR in a large repository?
Discovery becomes a maintenance problem when a directory contains dozens or hundreds of records. Keep a current index that lists the identifier, title, status, date and relationships. Use the decision’s words in its title rather than an internal project nickname, and make status and category searchable.
Index and rendered views
For a small collection, a hand-maintained index may be sufficient if pull-request checks keep it synchronized. For a larger collection, generate an index or rendered site from the Markdown files and provide repository-wide search. The Architectural Decision Records organization’s tooling directory, updated 23 September 2026, lists examples such as adr-log for index maintenance, Log4brains for rendering and command-line creation, and a Backstage ADR plugin for cross-repository search.
Recommended Free Tools
Those entries are examples, not a universal recommendation. Before adopting any tool, verify its current maintenance, compatibility with your repository and build system, security posture, access controls and ability to preserve links when records move. A tool that the team will not maintain is less useful than a simple, reliable index.

How should ADRs fit into everyday engineering work?
Use normal review and ownership
Submit an ADR through the same review path used for consequential code or design changes. Name an owner for the decision area, but do not make the owner the only person able to update status or link a successor. A record should survive team changes because its history is versioned and its vocabulary is documented.
Separate advice from the decision
Writing an ADR can expose conflicting assumptions and create alignment before implementation. Fowler’s discussion of conversational architecture describes gathering advice and recording what matters. Summarize the advice that affected the choice or link to the discussion; do not paste a chat or meeting transcript into the ADR. Keep the decision and consequences readable even when the underlying conversation was long.
Check the record when touching the affected architecture
Add “read the relevant ADR” to design and implementation checklists for changes in the decision’s scope. If the work confirms the decision, no rewrite is needed. If it exposes a changed assumption, open a successor or a proposed review rather than editing history.
A maintenance workflow you can apply
- Identify the architectural choice. Confirm that the decision affects architecture or a meaningful requirement and that its rationale will matter later.
- Create a concise draft. Add title, status, context, decision, serious alternatives, consequences, review trigger and relevant links.
- Make the outcome scannable. Put the decision near the top, use short paragraphs and link supporting material instead of copying it.
- Review assumptions and trade-offs. Ask whether the alternatives are genuine and whether the consequences include important costs, risks and operational work.
- Approve and version it. Record the accepted status through the team’s normal review process and store the file in the agreed location.
- Publish it to discovery surfaces. Update the index or generated view and verify that links resolve.
- Watch for triggers. Revisit the ADR when product context, requirements, technology, scale, regulation or another stated assumption changes.
- Preserve history. For a material change, write a successor, link both directions where useful and update the old status.
Common failure modes and fixes
| Failure | What the reader experiences | Fix |
|---|---|---|
| One ADR contains several unrelated decisions | A later change cannot be represented without rewriting unrelated history. | Split the choices and link them only where their dependency matters. |
| The decision is buried after background | Readers cannot tell what was chosen. | Put status and a direct decision statement at the beginning. |
| Only the winning option is documented | The same alternatives are reconsidered without understanding earlier trade-offs. | Record serious alternatives and concise pros and cons. |
| An accepted ADR is rewritten after a change | Historical rationale appears to support a decision it never addressed. | Preserve the old text, mark it superseded or deprecated and create a successor. |
| Status vocabulary is inconsistent | “Current,” “approved,” “active” and “accepted” are interpreted differently. | Define a small local vocabulary and apply it uniformly. |
| There is no discovery layer | A correct ADR exists but nobody can find it. | Use predictable names, an index, categories where useful and search or rendering at larger scale. |
| Review is assigned to an arbitrary calendar date | Teams perform ceremonial reviews even when nothing changed, or miss a change between dates. | Record concrete context triggers and check the ADR during work that touches its scope. |
| The ADR becomes a full design document | The important answer is lost in detail and the record is rarely updated. | Keep the ADR brief and link specifications, diagrams, measurements and discussions. |
How to choose a template and tooling approach
MADR’s current project guidance records version 4.0.0, released 17 September 2024, and offers minimal and full templates. The ADR Templates overview published by the Architectural Decision Records organization on 7 July 2026 also covers Nygard and Y-Statement formats. Choose one that captures your required fields without forcing information the team will not maintain.
Likewise, choose tooling according to collection size and workflow rather than popularity. A plain Markdown directory may be the most durable option for one repository. An index generator, renderer or cross-repository search plugin can help when navigation is the bottleneck. In every case, retain readable source files, ordinary version history and a clear authoritative location so the records remain usable if a tool is replaced.
A concise quality checklist
- Does the title describe one architectural decision?
- Can a reader find status and the decision statement immediately?
- Does the context explain the assumptions and constraints that mattered?
- Are serious alternatives and their trade-offs recorded?
- Are consequences concrete enough to guide implementation and future change?
- Is there a named trigger for reevaluation rather than an invented universal interval?
- Are predecessor, successor and supporting links accurate?
- Is the file in the location where its intended readers can access and version it?
- Will the index, rendered view or search surface show the current status?
- If the decision changes, will the team create a successor instead of rewriting history?
The governing principle
“The most important thing to bear in mind here is brevity,” Fowler writes. Brevity does not mean leaving out rationale; it means preserving the smallest complete explanation of the decision, its context, trade-offs and consequences. A useful ADR is short enough to read during a change, precise enough to prevent guesswork and stable enough to show how architecture evolved. Keep that balance, and maintenance becomes a normal part of architectural work rather than a documentation project that decays after launch.
