Strange requests for random .php files are usually automated reconnaissance, not proof that your server was breached. Treat a single 404 as unproven noise; treat bursts of guessed paths, unexpected 200 or 30x responses, clusters of 500 errors, suspicious POST data, or evidence of changed files and processes as an incident to investigate.

Preserve raw evidence before cleaning up, correlate NGINX access and error records with WAF, PHP-FPM, filesystem, and host telemetry, and then verify exactly where NGINX can execute PHP. The decisive question is not merely “what path was requested?” but “did the request reach executable code, and did anything change afterward?”

What strange PHP requests usually mean

Internet scanners routinely request filenames that are common in vulnerable content-management systems, plugins, control panels, and past malware campaigns. They may try /admin.php, random names such as /x.php, upload endpoints, double extensions, or paths copied from another site. A nonexistent path that returns a clean 404 and produces no PHP-FPM activity is normally a failed guess.

That conclusion changes when requests form a pattern or produce an unexpected result. OWASP recommends reviewing bursts of 40x responses because they can reveal directory and file scanning. A 500 response can mean a probe reached an application path that failed unexpectedly, although it can also be an ordinary application error. Status codes and request strings are triage signals, not proof of compromise.

Requests followed by new PHP files, outbound connections, or log gaps Possible successful execution or persistence Escalate to incident response and protect the host from further tampering

Preserve evidence before investigating deeply

Do not begin by deleting suspicious files, restarting services, or rotating logs. Those actions can destroy timestamps and process evidence. Record the server’s current time zone and clock status, and use a trusted time reference when comparing systems.

Copy the raw records

Collect the original NGINX access and error logs, WAF or security-module logs, PHP-FPM logs, system journal entries, authentication records, filesystem metadata, and relevant monitoring data. Include compressed and rotated files covering the period before and after the requests. Preserve the files in read-only or access-controlled storage outside the web server. OWASP’s guidance is explicit: “Log statistics or analysis should not be generated, nor stored, in the same server that produces the logs.”

sudo journalctl --since '2 hours ago' --no-pager > /var/forensics/nginx-php/journal.txt
sha256sum /var/forensics/nginx-php/* > /var/forensics/nginx-php/SHA256SUMS

The commands above create a temporary collection on the host so that files can be transferred to a separate evidence system. Move the collection promptly to storage that the web-server account cannot modify. Keep the originals untouched and document who handled each copy.

Check for tampering and gaps

Compare file modification times, ownership, rotation boundaries, and sequence numbers. A sudden truncation, missing interval, unexpected permissions change, or unexplained logging restart is itself evidence. CISA has documented intrusions in which PHP web shells were placed under plausible filenames and access, error, and authentication logs were deleted. A clean local log therefore does not prove that nothing happened.

Confirm what NGINX is actually logging

NGINX uses log_format to define each access-log record and access_log to select its destination. The documented default is logs/access.log in the combined format, but package layouts and virtual-host configurations commonly change that path. Inspect the active configuration rather than assuming a file name.

For investigations, each access record should provide enough context to correlate events:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • An unambiguous timestamp, preferably ISO 8601 with a time-zone offset.
  • Client address, forwarded-address handling, HTTP method, complete request line, and query string.
  • Status, response bytes, referrer, and user agent.
  • Total request time and upstream response time when available.
  • Upstream status, upstream address, and a request identifier.

A request ID is valuable only when the same ID is carried into WAF and application logs. If your deployment already supplies one, include it in every relevant stream; otherwise, correlate with precise timestamps, source address, and request details while recognizing that those fields are less reliable.

Separate access, error, and security evidence

Ordinary access logs show what NGINX received and the response it sent. Error logs explain routing, permission, rewrite, and upstream failures. A WAF or other security module has a different record: HTTP request and response details, policy decisions, violations, signatures, support or correlation IDs, and reasons for a PASSED or REJECTED outcome. F5 documents these security logs as separate from normal NGINX access logs. Do not infer a WAF decision from a 404 in the access log.

Evidence stream Questions it answers
NGINX access log Who requested what, when, with which method, and what status and timing were returned?
NGINX error log Did routing, permissions, rewrites, or an upstream connection fail?
WAF/security log Was the request inspected, blocked, passed, or matched to an attack signature?
PHP-FPM log Did PHP start a worker, emit a warning or fatal error, or exceed a limit?
Filesystem and integrity data Were scripts, configuration files, uploads, cron entries, or services created or changed?
Host telemetry Did a process spawn, make an outbound connection, authenticate, or alter logging?

Classify the request cluster

Start with a bounded time window around the first suspicious event. Extract the client address, method, URI, query string, status, user agent, request time, upstream status, and request ID. Group by source and by URI pattern rather than reading lines in isolation.

sudo grep -E '.php([? ]|$)|/.env|upload|cmd=|exec=' /var/log/nginx/access.log | tail -n 200
sudo awk '{print $1, $6, $7, $9}' /var/log/nginx/access.log | sort | uniq -c | sort -nr | head -n 50

These examples are starting points, not complete parsers; quoted request lines and custom log formats can change field positions. Export a copy to a dedicated analysis system if possible.

Look for high-value indicators

  • Encoded directory traversal, repeated URL decoding, or attempts to reach parent directories.
  • Double extensions such as a filename ending in both an image suffix and .php.
  • Requests for /.env, configuration backups, source archives, or common CMS and plugin paths.
  • POST requests to upload handlers, especially with executable content or unexpected MIME types.
  • Parameters resembling shell commands, interpreters, network utilities, or file operations.
  • Many requests using the same unusual user agent, regular spacing, or a rotating list of filenames.

None of these indicators establishes a breach on its own. A scanner can send a malicious-looking string that never reaches PHP, and legitimate applications can contain words such as upload or exec. Use them to prioritize correlation.

Interpret status codes with upstream evidence

404 responses

A 404 accompanied by no upstream request and no matching file generally means NGINX rejected a path guess. Confirm that conclusion in the error log and PHP-FPM log. If PHP-FPM records a worker handling the request, the configured routing may be forwarding nonexistent scripts, or a rewrite may be mapping the URI to existing code.

200 and 30x responses

An unexpected success or redirect from a guessed PHP name deserves priority. Determine whether the response came from a real script, a front controller, an authentication redirect, or a generic error page with a misleading status. Compare response length and headers with normal traffic, then inspect the application route and file metadata.

500 responses

A 500 is a symptom, not a verdict. Match it to PHP-FPM warnings, fatal errors, worker restarts, memory limits, and application logs. A failed exploit can produce the same status as a normal bug. Repeated 500s against one path, especially after a POST or unusual parameter, warrant code and filesystem review.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Verify whether PHP code could execute

The boundary between a web request and PHP execution is defined by the NGINX location rules and FastCGI parameters. NGINX documentation identifies SCRIPT_FILENAME as the script name passed to PHP. Review the active server block, not only an included example file.

sudo nginx -T 2>/dev/null | grep -n -A25 -B8 -E 'location .*php|fastcgi_pass|SCRIPT_FILENAME|try_files|fastcgi_split_path_info|root |alias '

Inspect the risky directives

  • root and alias: Confirm that URI-to-filesystem mapping cannot escape the intended document root and that an alias does not create an unexpected executable directory.
  • try_files: Ensure nonexistent scripts are rejected before FastCGI handoff. A common defensive pattern is to test the resolved file and return 404.
  • fastcgi_param SCRIPT_FILENAME: Verify that the value resolves to the intended file, with no user-controlled path segment being appended unsafely.
  • fastcgi_split_path_info and regex locations: Check how a path such as file.php/extra is split and whether the whole URI can be treated as a script.
  • Upload handling: Writable directories should not be executable. Do not rely on a file extension check alone.

Test a nonexistent script safely

From an authorized test client, request a clearly nonexistent PHP name and observe access, error, and PHP-FPM logs at the same time. The expected result is a 404 with no PHP execution. Repeat against upload directories and path-info variants if those features are enabled. Do not send exploit payloads to production; use a staging copy for security testing.

curl -i https://example.invalid/definitely-not-a-real-file-9f3c.php

The domain above is intentionally non-routable as an example. Replace it only in an authorized environment and keep the test name unique.

Check for an uploaded web shell or other persistence

OWASP warns: “If the server is configured to execute code, then it may be possible to obtain command execution on the server by uploading a file known as a web shell, which allows you to execute arbitrary code or operating system commands.” This is why an upload request followed by a request for a new PHP file is more serious than an isolated scan.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Review recently changed files

Search the document root, upload directories, temporary directories, framework cache locations, and deployment paths. Establish the correct baseline first; generated files and legitimate deployments can also be recent.

sudo find /var/www -type f -printf '%TY-%Tm-%Td %TH:%TM:%TS %u:%g %m %pn' | sort -r | head -n 200
sudo find /var/www -type f -name '*.php' -perm /111 -ls
sudo grep -R -n --include='*.php' -E 'base64_decode|evals*(|asserts*(|shell_execs*(|passthrus*(|systems*(' /var/www

Keyword searches produce false positives because frameworks and administrative tools may use these functions. Treat an unfamiliar file, obfuscated code, an implausible owner, or a file in a writable upload directory as a lead for manual review, not as an automatic deletion rule.

Inspect processes, connections, and scheduled tasks

ps auxwwf
sudo ss -plant
sudo systemctl list-timers --all
sudo crontab -l
sudo find /etc/cron* /var/spool/cron -type f -ls

Look for PHP-FPM children with unusual command lines, unexpected interpreters, persistent outbound connections, new services, modified timers, and jobs running as the web-server account. Capture output before terminating a process unless immediate containment is required.

Correlate authentication and credential use

Review SSH, control-panel, deployment, database, cloud, and application authentication events around the request times. A web-shell investigation is incomplete if an attacker could have reused an application secret or service-account credential. Record successful and failed logins, token creation, privilege changes, and access from unfamiliar addresses.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Contain and remediate after preservation

  1. Classify the scope. Identify affected virtual hosts, files, accounts, containers, and time range. Preserve the evidence set before altering the system.
  2. Reduce exposure. Restrict the suspicious endpoint, place the host behind an approved maintenance or isolation control, or remove it from rotation. Keep a controlled path for responders.
  3. Protect credentials. Rotate credentials that may have been present on the host or reachable by the application, including database passwords, API keys, deployment keys, and administrator sessions.
  4. Patch the cause. Update NGINX, PHP, frameworks, CMS components, and plugins. Correct insecure upload handling and routing rather than only blocking the observed filename.
  5. Remove unauthorized code methodically. Use a documented incident process, preserve copies for analysis, compare against a trusted release, and rebuild from known-good artifacts when integrity is uncertain.
  6. Restore trustworthy logging. Verify configuration, permissions, forwarding, rotation, and clock synchronization. Assume local logs may have been altered if an attacker had code execution.
  7. Monitor for recurrence. Keep detection rules and file-integrity monitoring active after recovery, and review the same indicators against historical traffic.

Harden NGINX, PHP, and log collection

Make the execution boundary explicit

Keep PHP locations narrow, reject nonexistent scripts before FastCGI, and avoid executing files in directories where the application or users can write. Review every regex location and rewrite that can reach PHP. Test normal routes, missing routes, path-info requests, uploads, and static files after each configuration change.

Reduce information disclosure without mistaking it for protection

The PHP manual says setting expose_php=off reduces information disclosed to attackers, while also warning that “In general, security by obscurity is one of the weakest forms of security.” Use it as a minor hardening measure, not as a substitute for patching, access control, safe uploads, or monitoring.

Forward logs off the host

Use NGINX’s supported syslog mechanisms or an approved logging agent to send access, error, and security records to a separate system. Restrict raw-log access, protect the transport, retain enough history for investigations, and use immutable or append-only storage where feasible. Ensure the receiving system preserves the request or support ID and original timestamp.

Choose controls by the failure you need to prevent

Control dimension Questions to ask Trade-off
Visibility Do you collect access, error, WAF, PHP-FPM, filesystem, and host telemetry? More streams improve detection but increase storage and administration
Correlation Can one request be followed across NGINX, WAF, PHP-FPM, and application logs? Request IDs help; retrofitting them may require configuration and code changes
Retention and tamper resistance Are records off-host, access-controlled, and protected from deletion? Longer immutable retention costs more and needs privacy controls
Detection depth Do rules cover signatures, anomalies, uploads, and file-integrity changes? Broader rules find more attacks but create false positives
Response speed Can you block, isolate, rebuild, and rotate credentials quickly? Fast automation can interrupt legitimate traffic if poorly scoped
Operational burden Who reviews alerts and validates suspected files and processes? Unstaffed alerts provide little protection regardless of tooling

A practical severity decision

Evidence level Typical findings Recommended handling
Low Isolated 404, no upstream activity, no file or process changes Document, retain the event, and improve visibility if fields are missing
Medium Scanning burst, suspicious POSTs, or unexplained 500s with no confirmed change Correlate all streams, inspect routing and uploads, and increase monitoring
High Unexpected executable file, PHP-FPM execution for a guessed path, command-like activity, or outbound connection Preserve evidence, isolate the host or endpoint, rotate exposed credentials, and follow incident response
Critical Confirmed web shell, privilege escalation, log deletion, lateral movement, or multiple affected hosts Use a formal incident process, rebuild from trusted images where necessary, and investigate the wider environment

What a defensible investigation conclusion says

A useful conclusion names the evidence and its limits. For example: “Between the recorded times, the source requested 37 nonexistent PHP paths. NGINX returned 404 for each, no request reached PHP-FPM, and filesystem and authentication review found no related changes. The activity is consistent with unsuccessful scanning; compromise was not evidenced in the available telemetry.”

If the evidence instead shows a request reaching a newly created script, state that plainly, preserve the file and surrounding logs, and treat the host as potentially compromised until containment and integrity checks are complete. Avoid declaring a breach solely from a strange filename or a single error status.
