Introduce an AI scribe as a controlled workflow change, not as an automatic time-saving feature. Start with a limited pilot, explain the tool to patients before or at the visit, make refusal genuinely easy, keep a non-scribe workflow ready, and require the clinician to check and correct every draft before signing it.
Protect appointment flow by moving routine education and preference recording into pre-visit messages or staff-supported intake. During the visit, the clinician should give a brief, conversational explanation, answer questions, and focus on the patient rather than on the software. Measure local documentation time, note quality, patient experience, appointment flow, and workload against a baseline before deciding whether to expand.
What an AI scribe does—and what it does not do
An ambient AI scribe listens to a clinical conversation and uses speech recognition and generative AI to produce a draft note. Depending on the product and configuration, audio, a transcript, the generated note, and technical metadata may be processed or retained. The draft is documentation assistance, not an independent clinical record and not a replacement for examination, listening, diagnosis, or judgment.

The clinician remains accountable for the final note. The AMA Journal of Ethics describes a patient-facing explanation that clinicians “review the notes and check them for any mistakes before finalizing and entering them into the EHR.” Treat that review as a required clinical control, not an optional quality check.

How do I introduce an AI scribe to my clinical team?
Choose a narrow, suitable pilot
Begin with a small set of clinicians, specialties, locations, and appointment types. Select encounters with a predictable workflow and enough documentation volume to reveal whether the tool helps. Exclude situations that your team cannot safely support at launch, such as visits requiring unusual privacy arrangements or workflows in which the tool cannot be paused reliably.
Name an accountable owner and include clinical leadership, operations, privacy and security, compliance, information technology, EHR specialists, and frontline staff. Patients and clinicians should have a route to report problems without waiting for a formal review cycle.
Map the real encounter from arrival to signed note
Write down each handoff rather than assuming the existing process will absorb the scribe. Your map should answer:
- When and where does the patient receive an explanation?
- Who records the patient’s preference and where is that preference stored?
- Who answers questions before the clinician enters?
- How does the clinician activate, pause, and stop capture?
- Where does the draft note appear, and how is it identified as AI-generated?
- Who edits, signs, and corrects the note?
- What happens if the patient declines, changes their mind, or the service is unavailable?
Run the map with actual staff during a normal clinic session. A theoretically short process can still create a queue if, for example, only one person knows how to record a refusal or stop a recording.
Set a non-scribe path before launch
A patient who declines must receive ordinary care without argument, delay, or a suggestion that they are being difficult. The fallback can be conventional typing, dictation, or handwritten notes, according to your existing policy. Train staff to switch to it immediately and tell clinicians exactly how to document a visit when the tool fails.
Train with realistic scenarios
Practice more than the ideal demonstration. Include a patient asking for a pause, a sensitive discussion, multiple speakers, a strong accent, background noise, an interruption, poor connectivity, and a patient who changes their mind. Clinicians should rehearse how to stop capture and how to continue documenting without the scribe.

What should we check about privacy, security, and compliance?
“HIPAA-compliant” is not a complete description of a deployment. Ask the vendor and your internal reviewers to document the actual data flow:
- What audio, transcripts, notes, identifiers, and metadata are collected?
- Where are data processed and stored, and in which jurisdictions?
- Which vendor personnel, subcontractors, or organizational users can access them?
- How long are each of the audio, transcript, and note artifacts retained?
- How can data be deleted, and what backups or derived data remain?
- Are data used for model training, analytics, or other secondary purposes?
- What incident-notification, audit-log, and access-control processes apply?
- How does the EHR integration authenticate, transfer, label, and preserve corrections?
- What support and recovery commitments apply when the service is unavailable?
Have privacy, security, compliance, and legal stakeholders review the answers for the deployment’s jurisdiction and use case. The legal analysis is principally US-oriented: the AMA Journal of Ethics notes that HIPAA protections for covered entities and business associates coexist with state or federal recording laws. Legal disclosure requirements for ambient documentation are not necessarily identical to ethical expectations, so do not copy a consent script from another state or country and assume it applies locally.
Do I need patient consent to use an AI scribe?
The answer depends on the jurisdiction, the organization’s policy, and the tool’s actual data practices. Obtain local legal advice rather than presenting a general script as legal advice. Regardless of the minimum legal rule, meaningful communication is an ethical and operational requirement. The American Medical Association states: “Patients deserve to know when AI is part of their care.”
Make consent understandable and voluntary
Tell the patient what the tool does, what information it handles, that the clinician reviews the draft, and how to ask questions or decline. Do not hide the explanation in a rushed checkbox. A refusal should not obstruct care under the organization’s stated policy, and patients should be able to change their preference.
Use the organization’s approved wording and adapt it to the product’s real behavior. A plain-language example is:
“We can use an AI documentation tool to listen to our conversation and prepare a draft note. I review and edit the note before it becomes part of your record. You can ask questions or decline, and we can continue without it. Is using it okay with you today?”

Explain what happens if the patient stops the session after capture has begun. If the system has already created audio or a transcript, state whether those data are deleted, retained, or incorporated into the record, using the actual policy rather than a promise the system cannot keep.
Move routine information earlier
Send a short explanation with pre-visit instructions, make translated and accessible versions available, and offer staff support for questions. Record the patient’s preference in the intake workflow so the clinician does not spend several minutes repeating standard information. Keep the clinician’s in-room explanation available for confirmation and questions, not as a substitute for informed communication.
Do not infer that one pilot’s consent rate is universal
A quality-improvement study at NYU Grossman School of Medicine, conducted from March through December 2024 with 103 patients and 18 clinicians, reported that 84 patients (81.6%) consented after basic information was provided, compared with 57 (55.3%) when more details about AI features, data storage, and corporate involvement were disclosed. This is a finding from one pilot, not a recommended level of disclosure and not a general consent rate. It shows why teams should explain the data practices honestly and test how their own patients respond.

It can if education, consent, device setup, and failure recovery are added to the clinician’s workload without redesigning the workflow. The safest approach is to separate routine tasks from the clinical conversation.
- Before arrival: Include the plain-language explanation and privacy contact in appointment messages, portal instructions, or registration materials.
- At intake: Let trained staff answer routine questions and record an opt-in, decline, or undecided status according to policy. Do not pressure a patient to choose quickly.
- At rooming: Confirm the preference and make sure the device or application is ready before the clinician begins the substantive discussion.
- At the start of the visit: Give a brief conversational explanation, invite questions, and ask for permission before recording starts.
- During care: Keep attention on the patient. Pause or stop when requested, during sensitive portions, or whenever the tool is mishearing the conversation.
- At checkout: Route the draft to the clinician’s review queue and preserve the ordinary path for a non-scribe note.
Do not promise a shorter visit or more appointments in advance. A tool can move work from after-hours documentation into in-visit review, and a small delay at setup can erase a later saving. Measure the complete workflow.
How do clinicians check AI-generated notes?
Review the draft against the actual encounter before signing. The clinician’s attestation must reflect personal judgment, not silently adopt generated wording.

A practical review sequence
- Open the draft in the designated review view and confirm that it belongs to the correct patient and encounter.
- Compare the chief concern, history, examination findings, assessment, and plan with what was actually discussed and observed.
- Check medication names, doses, frequencies, allergies, dates, referrals, follow-up intervals, and other details where a transcription error could cause harm.
- Look for omissions, unsupported statements, invented negatives, and incorrect speaker attribution.
- Remove wording that implies a test, examination, counseling conversation, or decision occurred when it did not.
- Correct the note in the EHR, verify that the final version is clearly labeled according to organizational policy, and sign only after the corrections are complete.
Never rely on the draft as a substitute for listening, examination, or clinical reasoning. If the service is interrupted, unavailable, or clearly inaccurate, return to ordinary documentation rather than trying to reconstruct a visit from an unreliable draft.

Record a baseline before deployment and use the same definitions afterward. Choose a small set that staff can collect consistently, then add safety or equity measures that matter for your setting.
| Measure | What to record | Why it matters |
|---|---|---|
| Documentation burden | Time spent documenting during and after clinic; time to note completion | Shows whether work is reduced or merely moved |
| Note quality | Correction rate, material errors, omissions, unsupported statements, and attribution problems | Detects clinical risk and review burden |
| Appointment flow | Setup time, visit duration, delays, cancellations, and recovery time after failures | Reveals disruption that a documentation metric can miss |
| Patient experience | Comfort, understanding, questions, refusals, and reasons for declining where appropriate | Tests whether communication is acceptable and equitable |
| Clinician and staff workload | Perceived cognitive load, added tasks, after-hours work, and support requests | Shows hidden operational costs |
| Safety and quality | Reported incidents, corrections after signing, delayed orders, and other local signals | Provides a go/no-go safeguard |
| Equity and performance | Results by specialty, language, accent, age group, and multi-speaker encounter where lawful and appropriate | Prevents an average result from concealing uneven performance |
Published evidence does not justify a guaranteed efficiency or burnout claim. A rapid review by Kanaparthy and colleagues identified 1,450 studies but included only six eligible real-world studies. The 2025 JMIR AI review found self-reported documentation time decreased in the included evidence, while standardized burnout measures and billing-based productivity were unchanged; the authors judged the evidence sparse. Use those findings as context, not as a forecast for your clinic.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How comfortable are patients with ambient documentation?
Comfort varies with the explanation, the setting, the subject of the visit, and the patient’s expectations about data. In the NYU quality-improvement study described above, 77 of 103 patients (74.8%) reported being comfortable or very comfortable with physician use of ambient documentation. The sample was a 2024 pilot, so it should inform questions for your own patient population rather than serve as a universal acceptance benchmark. The study is indexed at PubMed.
Ask patients whether they understood what was recorded, whether they felt able to decline, and whether the tool changed how freely they spoke. Provide a contact route for record questions or concerns, and make sure that route reaches someone who can investigate the underlying data and note.
What can go wrong, and how should the team respond?
The patient declines
Thank the patient, stop or do not start capture, and use the prepared non-scribe workflow. Do not debate the decision or imply that care will be worse. Record the preference only as permitted by policy and offer a way to revisit it at a later visit.

The patient changes their mind
Pause or stop using the tool immediately. Explain what happens to audio, transcripts, and drafts already created under the actual retention and deletion rules. Continue the visit and document conventionally if needed.
The draft contains a serious error
Do not sign it. Correct or replace the note, report the failure through the support and safety channel, and assess whether an order, prescription, referral, or patient communication was affected. Look for patterns rather than treating each error as an isolated user mistake.
Audio quality is poor or speakers overlap
Pause the tool, restate important details directly, and return to ordinary documentation if accuracy cannot be restored. Training should include multiple speakers, background noise, accents, and interruptions because a quiet demonstration does not represent every clinic.
Recommended Free Tools
The service or integration is unavailable
Switch to the documented fallback, avoid delaying the appointment, and capture the outage time and affected workflow. A recovery plan should specify who contacts support, how drafts are reconciled, and how clinicians verify that no encounter was left incomplete.
How should we decide whether to expand?
At the end of the pilot, review results with frontline clinicians, staff, governance owners, privacy and security, and patient representatives where available. Continue only if the tool meets locally defined thresholds for safety, privacy, note quality, appointment flow, and patient and clinician experience.
Expansion should be staged. Recheck data handling and EHR behavior when the vendor changes a model, retention setting, integration, or subcontractor. Update patient explanations and training when the tool’s capabilities change. Maintain an auditable record of who can access drafts, who corrected them, and how refusals and failures were handled.
The AMA Ed Hub’s June 16, 2026 module describes the governing principle as “Emphasizing that generative AI should augment, not replace, physician judgment.” That principle is operationalized by meaningful patient choice, a reliable fallback, transparent labeling, and clinician review before a draft becomes part of the medical record.
