Hospital wastewater monitoring can help public-health teams detect signals of antimicrobial resistance in sewage, but it cannot diagnose an infection in an individual patient or prove that an outbreak is occurring. Its value is as one part of surveillance, interpreted alongside clinical data and local conditions.

What hospital wastewater surveillance can reveal

Wastewater and environmental surveillance (WES) uses population samples from sewage where sewer networks exist, or other human-impacted waters where they do not. The World Health Organization (WHO) describes WES as one component of multimodal public-health surveillance, rather than a substitute for clinical surveillance. WHO guidance published December 6, 2024 sets out a framework for prioritizing, implementing, and integrating these programs.

At a healthcare facility, sewage may contain signals associated with antimicrobial-resistant bacteria (ARB) and antimicrobial-resistance genes (ARGs). Those are related but distinct measures: finding a resistance gene does not by itself show which organism carries it or whether a patient has an infection. Neither measure is the same as detecting residual antimicrobial drugs.

A wastewater result also cannot be translated directly into a count of infected patients, a transmission route, an environmental exposure, or a health-risk estimate unless the study design supports that inference. The sample represents a mixed flow, and its meaning depends on what was sampled, how it was tested, and what else was happening in the facility and sewer.

How to decide whether a program is useful

WHO recommends prioritizing targets when the resulting data can inform significant public-health decisions in the local context, the methods are technically and operationally feasible, the work is ethically and legally acceptable, and WES can be integrated with clinical surveillance and other targets. A program should start with a specific decision it is meant to support, not simply with a test that can detect a target.

  • Define the local question: Decide what public-health action a signal could inform and which resistance targets matter locally.
  • Check feasibility: Assess whether the facility can collect representative samples consistently and whether the laboratory can use and interpret the chosen methods.
  • Address ethics and law: Establish appropriate governance for sampling, data handling, and communication before monitoring begins.
  • Integrate results: Interpret wastewater findings alongside clinical surveillance and other relevant information; do not treat a sewage signal as a patient-level diagnosis.

WHO’s wastewater topic page includes its 2025 antimicrobial-resistance summary and outlines these decision criteria: WHO wastewater topic page.

What a hospital study tested—and what it did not establish

A CDC-published study in the August 2026 supplement of Emerging Infectious Diseases monitored wastewater at one rehabilitation hospital for 16 months. It investigated five carbapenemase genes: blaKPC, blaVIM, blaOXA-48-like, blaNDM, and blaIMP. These details describe that study’s design, not a population-wide estimate or a universal monitoring standard. CDC Emerging Infectious Diseases study.

Study dimension Approaches examined What readers should take from it
Sampling method Composite, passive, and biofilm-based sampling The study compared methods; it does not establish a best choice for all facilities.
Sampling frequency Weekly, twice-weekly, or monthly collection These were study schedules, not a general recommendation for program frequency.
Analysis qPCR, metagenomics, and culturing Different techniques examine different kinds of evidence; results need interpretation in context.
Clinical comparison Wastewater measurements considered alongside clinical infections Few clinical infections occurred during the study, and sewer biofilms complicated correlations with clinical incidence.

The authors concluded that further work is needed to understand how wastewater measurements relate to clinical incidence. Detection of a target in sewage is therefore not proof that a patient is infected or that an outbreak is underway.

Why sewer and treatment context matters

Wastewater flows combine inputs from different sources. The U.S. Environmental Protection Agency (EPA) identifies both hospital and residential wastewater as contributors of antimicrobial resistance to sewer networks; hospitals are not the sole source. EPA is studying the relative contributions of these flows, resistant bacteria and genes in biosolids, and molecular and cultivation-based testing approaches. EPA antibiotic-resistance research.

  • YOUR PURCHASE MAKES A DIFFERENCE in people’s lives. Safe Home donates a portion of every test kit sale to clean water initiatives around the globe. See attached images with this listing, to learn more.

EPA also describes the movement of resistant bacteria and genes among people, animals, and the environment. The occurrence and fate of antimicrobial resistance in treated municipal wastewater and biosolids, and its significance for health and the environment, remain active research questions. EPA reported awarding $9 million in grants for work on these gaps on a page last updated July 13, 2026; that funding is not evidence of a particular health risk. EPA grants announcement.

These uncertainties mean a monitoring result should not be framed as evidence that treatment either eliminates antimicrobial-resistance risks or creates them. The interpretation depends on the target, sampling point, method, and supporting evidence.

How to interpret a signal responsibly

  • Describe what was detected—such as a resistance gene or resistant bacterium—rather than labeling it an infection count.
  • Keep the facility and sewer context attached to the result; a mixed wastewater sample is not a sample from one patient.
  • Compare findings with clinical surveillance and local conditions before drawing operational or public-health conclusions.
  • Separate a signal that warrants follow-up from evidence that establishes patient infection, transmission, or environmental harm.
  • Report limitations in the same context as the finding, especially where clinical cases are few or sewer biofilms may contribute to measurements.
