A variant of uncertain significance (VUS) means that the evidence available today is not sufficient to tell whether a genetic change is associated with disease. It is an evidence category, not a diagnosis. A VUS does not confirm autism, explain a child’s traits, predict prognosis, or by itself justify treatment, extra surveillance, insurance choices, or reproductive decisions. The ACMG/AMP consensus is explicit: “A variant of uncertain significance should not be used in clinical decision making.”
Autism spectrum disorder (ASD) is diagnosed through developmental and behavioral evaluation. Genetic testing can sometimes identify an underlying cause or risks associated with a broader neurodevelopmental condition, but no single mutation is unique to autism. The appropriate response to a VUS is qualified interpretation, symptom-based care, and a plan for possible future review—not action based on the uncertain result alone.
What a VUS means on an autism genetic test
Laboratories classify genetic findings according to the strength of available evidence. The standard categories are pathogenic, likely pathogenic, variant of uncertain significance, likely benign, and benign. “Uncertain significance” means the laboratory cannot currently determine whether the change contributes to a disease. The classification describes the evidence for that variant; it does not, by itself, describe the person’s diagnosis or medical needs.
Evidence may be incomplete, contradictory, or too limited to support a more definite classification. A laboratory might not have enough information about how often the variant appears in the general population, whether it tracks with a condition in a family, what laboratory studies show, or whether the gene is firmly linked to a condition relevant to the person’s features. A VUS can later be reclassified when new population, family, functional, or clinical evidence becomes available, but reclassification is not guaranteed.

What the result does not establish
- It does not prove that a child has autism or any other condition.
- It does not prove that the variant caused developmental, learning, behavioral, or medical features.
- It does not predict the child’s future abilities, symptoms, or prognosis.
- It does not independently justify medication, supplements, a special diet, screening, surgery, or other treatment.
- It does not determine whether parents should have another child or establish a recurrence risk.
Does a VUS mean my child has autism?
No. Autism is a clinical diagnosis based on developmental history and assessment of social communication, restricted or repetitive behaviors, and related functioning. The American Academy of Pediatrics (AAP) explains that genetic tests may help identify an etiology or associated risks, but they are not diagnostic of ASD. Genetic findings can overlap among autism, intellectual disability, epilepsy, and other neurodevelopmental conditions.
There is also no single autism gene or mutation that accounts for most cases. The AAP reported that no individual pathogenic variant accounts for more than 1% of ASD cases. Therefore, even a clearly pathogenic result would need to be interpreted in the context of the person’s development and medical history; a VUS provides still less certainty.
How genetic testing fits into an autism evaluation
The AAP recommends offering a genetic evaluation and describes chromosomal microarray (CMA), fragile-X testing, and possible whole-exome sequencing (WES), depending on the history and prior results. The appropriate test depends on the reason for referral, the person’s features, sex, family history, and tests already completed. A VUS may be found during any of these types of testing, including a sequence test or a copy-number analysis.

Genetic testing surveys a large amount of variation. Not every change has been studied in enough people or in enough laboratory systems to determine its effect. A report can therefore contain a result that is real and accurately detected but whose medical significance is unknown.

CMA results illustrate the distinction between finding a change and knowing what it means. In its 2020 guidance, the AAP described an approximate CMA finding rate of 17%–42% in children with ASD when copy-number variants of uncertain significance are included. The range for definitively pathogenic copy-number variants in clinical ASD samples was 5.4%–14%, with a median of 9%. These figures are not a child’s probability of having autism; they describe findings reported in particular clinical testing settings and show why a detected copy-number change should not automatically be treated as causal.
For WES, the AAP cited a molecular-diagnosis yield of 26%–29% in large clinical studies in which neurodevelopmental disorders were the primary indication. A “molecular diagnosis” in those studies is not the same as an autism diagnosis, and yield varies with the people tested and the clinical question. The remaining results can include VUS findings or no reportable explanation.
Read the report without over-interpreting it
Ask the ordering clinician or a genetic counselor to walk through the report line by line. Keep the laboratory’s exact wording; small differences in a gene transcript or variant description can refer to different changes.
Details to identify
- Variant type: determine whether it is a sequence variant, a copy-number variant (CNV), or another finding.
- Gene and transcript: record the gene name and the transcript or reference sequence used by the laboratory.
- Exact nomenclature: copy the full variant description, including the laboratory’s notation.
- Classification: confirm that the report says VUS and note any conflicting classifications the laboratory lists.
- Test type and scope: identify whether testing was CMA, fragile-X testing, WES, a targeted panel, or another assay, and what the test could and could not detect.
- Gene–disease relationship: ask whether the gene is firmly established for a condition that matches the person’s features or whether that relationship remains uncertain.
- Evidence summary: ask which population, family, functional, or clinical observations led to the VUS classification.
A VUS may be reported in a gene that has a plausible biological connection but an incompletely established disease relationship. Plausibility is not proof. The clinical interpretation must combine the report with developmental findings, examination, family history, and other test results.

What to do after receiving a VUS
- Arrange a genetics discussion. Contact the clinician who ordered testing or request referral to a genetics professional or genetic counselor. Bring the complete report, prior evaluations, developmental records, and a three-generation family history if available.
- Separate the result from immediate care. Continue therapies, educational supports, developmental monitoring, and medical evaluations according to the person’s symptoms and established diagnoses. Do not start or stop treatment because of a VUS alone.
- Ask whether the test answered the original question. A counselor can explain whether the assay covered the suspected condition and whether additional evaluation, such as CMA, fragile-X testing, or WES, is appropriate based on the history and previous results.
- Make a written follow-up plan. Record who will contact the laboratory, what might trigger reanalysis, and how an amended report would reach the family.
- Revisit the result when appropriate. New evidence may support movement toward benign or pathogenic, or the result may remain uncertain. There is no universal reclassification timetable.
Can a VUS be treated?
There is no treatment for a VUS itself. Because its disease association is unresolved, using it as the basis for a therapy would violate the ACMG/AMP recommendation against using a VUS in clinical decision making. Care should target observed needs: developmental services, communication support, educational planning, sleep or feeding problems, seizures, anxiety, attention difficulties, and other clinically assessed concerns.
A VUS also should not be used alone to order disease-specific surveillance. If a person’s symptoms or family history independently warrant an evaluation, clinicians can pursue that evaluation without treating the VUS as proof of a diagnosis.
Will the VUS result change?
It can, but no one can promise that it will or predict when. Laboratories may receive new evidence from population databases, family segregation studies, functional experiments, or reports of other patients. That evidence can support a benign or pathogenic classification, or leave the finding unresolved.

- Does the laboratory routinely reanalyze VUS findings, or only when a clinician requests it?
- How often should the ordering clinician or genetics team review the report?
- Does the laboratory issue amended reports, and who receives them?
- Will the laboratory contact the patient directly, or must the family keep an updated relationship with the ordering clinic?
- What new clinical information should be sent to the laboratory?
Do not assume that automatic notification will occur. Laboratory policies differ, and contact information may become outdated. Keep a copy of the report and ask the ordering service how to request a future review.

Should the child’s parents or relatives get tested?
Sometimes testing parents or other relatives provides segregation evidence: information about whether the variant tracks with features in the family or arose newly in the child. In other cases, family testing adds no useful information, remains inconclusive, or creates confusion when relatives have subtle or unrelated traits. It can also have implications for relatives who did not expect to receive genetic information.
A genetics professional should decide whether testing is appropriate for this specific variant and family. The decision should consider the test’s purpose, the laboratory’s ability to interpret the result, possible benefits for diagnosis or recurrence-risk counseling, and the limitations of an uninformative result. Parental testing is not a way to convert every VUS into an answer.
Follow-up options compared
| Option | Purpose | Who usually initiates it | Potential benefit | Limitations | Cost and access |
|---|---|---|---|---|---|
| Genetic counseling or genetics consultation | Clinical explanation and coordination of care | Ordering clinician or genetics professional | Connects the report with development, examination, family history, and appropriate next steps | Availability varies; the VUS may remain unresolved | Coverage, referral rules, and local availability vary |
| Laboratory reanalysis | Evidence generation using newer data | Laboratory policy or a clinician request | May identify an amended classification or clarify newly available evidence | No universal timetable; reanalysis may still be inconclusive | Policy and whether a new test or request is needed vary by laboratory and health system |
| Parental or relative testing | Family segregation evidence | Genetics professional or ordering clinician | May help with diagnosis or recurrence-risk counseling in selected families | Can be uninformative or confusing and may reveal unexpected information | Coverage, consent, laboratory policy, and family availability vary |
| Symptom-based clinical evaluation | Diagnosis, treatment, and surveillance based on observed needs | Relevant medical or developmental clinician | Provides care without waiting for genetic certainty | Does not necessarily explain the genetic finding | Access depends on local services and insurance |
Questions to take to the appointment
- What is the exact variant nomenclature, gene, transcript, and test type?
- Is this a sequence variant or a CNV?
- Which evidence led the laboratory to call it a VUS?
- Does the report identify conflicting classifications?
- Is the gene established for a condition relevant to this person’s features?
- Would testing either parent or another relative add useful segregation evidence?
- How does this laboratory perform reanalysis?
- How often should the report be reviewed, and how are amended reports communicated?
- Which clinical evaluations are indicated by the person’s symptoms regardless of the VUS?
Common mistakes to avoid
Treating “uncertain” as “probably harmful”
Uncertainty is not a probability statement that the variant is dangerous. It means the available evidence does not support a confident disease association.
Using an online interpretation as a personal diagnosis
Public information can be useful background, but an individual report requires the laboratory’s methods, transcript, evidence, and the person’s clinical context. Use the ordering laboratory and qualified genetics professionals for interpretation.

Changing care while waiting for certainty
Do not delay developmental services or other indicated evaluations while waiting for reclassification. Conversely, do not add unproven supplements, restrictive diets, or medical surveillance because a VUS appears on a report.
Assuming a negative or uncertain result rules out a genetic contribution
A VUS is not a confirmed explanation, but it is also not proof that no genetic factor exists. Test scope, current knowledge, and the limits of the technology determine what a result can say.
How to explain the result to family members
A concise explanation is: “The test found a genetic change, but the laboratory does not yet know whether it affects health. It does not diagnose autism or tell us what treatment to use. We are following the child’s clinical needs and asking the genetics team how the result will be reviewed.” This wording avoids both false reassurance and unnecessary alarm.
Relatives should not be pressured into testing based solely on a VUS. If family testing is proposed, each person should receive a clear explanation of its purpose, possible outcomes, privacy considerations, and the possibility that it will not resolve the uncertainty.
Bottom line for families
A VUS in an autism genetic test is an unresolved laboratory finding, not an autism diagnosis and not a treatment target. Autism and related developmental conditions must be assessed clinically. Review the exact report with a genetic counselor or the ordering clinician, continue symptom-based care, and ask the laboratory about reanalysis and amended-report procedures. Parent or relative testing can be useful in selected cases, but only when a genetics professional believes it is likely to add interpretable evidence.
