A recurrence-risk report is a decision aid, not a prediction of what must happen to you. Read it in three layers: the genomic assay score, the estimated chance of recurrence over a stated time period, and the estimated absolute benefit of an added treatment such as chemotherapy. Those figures are calculated for a defined cancer type and treatment plan, then adjusted alongside tumor size, grade, hormone receptors, HER2, lymph nodes, menopausal status, previous treatment, health conditions and personal priorities. Ask your oncologist to identify each number, state the time horizon and treatment assumptions, and translate the result into absolute risks rather than relying on the score alone.

What a recurrence-risk report is—and what it is not

After breast surgery, some people receive a gene-expression assay that examines activity in a set of tumor genes. The result is combined with pathology and clinical information to estimate prognosis and, for some tests and groups of patients, likely benefit from chemotherapy or extended endocrine therapy. The American Cancer Society lists Oncotype DX, MammaPrint, Prosigna, EndoPredict and Breast Cancer Index among commonly discussed assays.

These tests are prognostic when they provide information about the chance of recurrence, and predictive when evidence indicates that a result can help identify who is more likely to benefit from a particular treatment. A test can be prognostic, predictive, or both depending on the assay, cancer subtype and patient group studied. Even a validated result cannot establish with certainty whether an individual will recur or exactly how that person will respond to treatment.

“Recurrence” is not one outcome. A cancer can return in the breast or chest wall (local), nearby lymph nodes (regional), or another organ (distant or metastatic). A report may estimate one of these endpoints rather than all of them. Confirm which endpoint appears on your report before comparing it with another figure.

Find the three different numbers on the page

Patients often hear a single number and understandably treat it as a percentage. Reports usually contain several numbers that answer different questions.

Report element What it means Question to ask
Assay score A result on that test’s own scale. For Oncotype DX, the Breast Recurrence Score runs from 0 to 100. Is this the score, or is it a percentage risk?
Estimated recurrence risk A model-based percentage for a specified endpoint and follow-up period, usually under stated treatment assumptions. Is this distant, local, regional or overall recurrence, and is the time horizon five or nine years?
Estimated treatment benefit The modeled difference between outcomes with and without an added treatment, often expressed as an absolute percentage-point reduction. How much lower is my risk with chemotherapy, and what treatment is assumed?

A score is not automatically a probability. The National Breast Cancer Foundation uses a Recurrence Score of 18 as an example: 18 does not mean an 18% chance of recurrence. The separate risk field on the report is the percentage estimate. The same distinction applies to scores from other assays, whose scales and algorithms are not interchangeable.

How to read a typical Oncotype DX report

Oncotype DX is used mainly for selected early-stage, hormone-receptor-positive, HER2-negative invasive breast cancers. Your report may show the Recurrence Score, an estimated distant-recurrence risk and an estimate of chemotherapy benefit. Breastcancer.org’s patient explanation, dated June 11, 2026, emphasizes that the result must be interpreted with the rest of the tumor information.

1. Confirm the assay and version

Write down the exact test name, report date and version or platform shown. Ask whether the assay was validated for your cancer subtype, lymph-node category and menopausal group. A result generated for one clinical population should not be assumed to apply identically to another.

2. Locate the Recurrence Score

For Oncotype DX, the score is between 0 and 100. Current patient materials commonly group scores from 0–25 as a lower range and 26–100 as a higher range when chemotherapy benefit is being considered. Older materials may display three bands instead. These are population groupings, not personal guarantees or a substitute for the risk percentage printed elsewhere.

3. Locate the distant-recurrence estimate

NICE describes Oncotype DX estimates as commonly framed over five or nine years while a person receives endocrine therapy. Read the percentage together with its endpoint, follow-up period and assumed treatment. A five-year estimate and a nine-year estimate are different questions, even when they appear on the same page.

4. Locate the chemotherapy-benefit estimate

Determine whether the report gives an absolute reduction, a relative comparison or a category such as little expected benefit. Ask what chemotherapy regimen, endocrine therapy and patient subgroup were used to derive the figure. The estimate is not the chance that chemotherapy will prevent every recurrence; it is a model of the difference between specified treatment strategies.

Why the score cannot be read without pathology and clinical details

Genomic information adds one layer of evidence. The baseline risk can change substantially when it is combined with the following features:

Feature Why it matters in the discussion
Tumor size Larger tumors can carry different clinical risk even when two genomic scores are similar.
Grade Grade reflects how abnormal and rapidly dividing the cells appear under the microscope and contributes prognostic information.
Estrogen and progesterone receptors Hormone-receptor status determines whether endocrine therapy is part of the assumed treatment and affects which assays are appropriate.
HER2 status HER2-positive and HER2-negative cancers follow different treatment pathways; an assay validated for one group may not answer the same question for another.
Lymph-node involvement Node-negative and node-positive populations have different evidence bases. The number of involved nodes and whether disease is isolated or more extensive matter.
Menopausal status and age Evidence for chemotherapy benefit can differ between premenopausal and postmenopausal groups. Age also affects competing health risks and tolerance of treatment.
Previous treatment Surgery, radiation, endocrine therapy and any earlier systemic treatment alter the context in which a recurrence estimate is used.
Planned endocrine therapy The percentage may assume five years, another duration, or a particular endocrine strategy. A different plan changes the estimate.

Clinical risk can add information to a genomic score. A low score does not erase an aggressive tumor feature, and a higher score does not dictate one mandatory regimen. Ask your oncologist to show how each feature moved the estimate rather than presenting the assay as the entire answer.

How to ask whether chemotherapy is likely to help

The useful question is not “Is my score high enough?” It is “What are my absolute outcomes with each reasonable plan?” Request two side-by-side estimates:

  • Estimated distant-recurrence risk with the recommended endocrine therapy alone over the stated horizon.
  • Estimated distant-recurrence risk with endocrine therapy plus chemotherapy over that same horizon.
  • The absolute percentage-point difference between those estimates.
  • What evidence supports the estimate for your exact node status, menopausal group and tumor characteristics.

For example, a relative reduction can sound large while the absolute change is small if the starting risk is low. Conversely, a modest relative effect can represent an important absolute reduction when the baseline risk is higher. Have the clinician write both percentages and the time period on your visit summary.

Balance benefit against burdens that matter to you

Chemotherapy decisions include more than recurrence percentages. Discuss short-term effects such as fatigue, nausea, infection risk, hair loss and effects on work or caregiving, as well as longer-term risks that depend on the drugs proposed. If pregnancy is possible or future fertility matters, ask about fertility-preservation timing before treatment starts. Existing heart, nerve, kidney or other health problems may alter the balance. Your values—such as how you weigh a small potential benefit against treatment burden—are part of shared decision-making, not an afterthought.

What lymph nodes and menopausal status change

Do not transfer a threshold from a friend, website or older pamphlet to your case. Evidence is organized by node status and menopausal group, and the same score can lead to different discussions in those settings. Current materials often describe 0–25 and 26–100 ranges for chemotherapy conversations, but those ranges summarize study populations; they do not override age, menopause, pathology or preferences.

ASCO’s 2022 guideline update reports insufficient evidence to support routine Oncotype DX use for guiding adjuvant chemotherapy when four or more lymph nodes are positive. That does not mean treatment is never considered; it means the assay should not be treated as a validated stand-alone decision rule in that situation. Ask which evidence applies to the exact number of positive nodes in your pathology report.

Menopausal status also changes interpretation. A chemotherapy benefit observed in a premenopausal study group may reflect both direct cytotoxic effects and treatment-related ovarian suppression. Your oncologist should explain whether the evidence separates those mechanisms and whether endocrine alternatives are relevant for you.

Compare two reports or treatment options without mixing scales

If clinicians mention two assays, or an assay result alongside a clinical calculator, compare the assumptions rather than the raw numbers.

Comparison point What to verify
Intended population Node-negative or node-positive; premenopausal or postmenopausal; hormone-receptor and HER2 status; stage and other eligibility criteria.
Endpoint Distant recurrence, local recurrence, overall recurrence, late recurrence or another outcome.
Time horizon Five years, nine years or another follow-up period.
Treatment assumption Endocrine therapy alone, endocrine therapy plus chemotherapy, extended endocrine therapy or another regimen.
Evidence in your subgroup Whether prospective or guideline-supported evidence covers your node status and menopausal group.
Role of the result Whether it is prognostic, predictive of treatment benefit, or both.
Scale Whether the values are assay scores, percentages, risk categories or absolute percentage-point differences.

Never rank assays by saying one score of 20 is “higher” or “lower” than another assay’s score of 20. Different tests measure different gene sets, populations and endpoints.

  1. Identify the test. Which assay and version is this, and was it validated for my tumor subtype, node status and menopausal group?
  2. Separate the numbers. Which number is the assay score, which is the estimated distant-recurrence percentage, and which is the treatment-benefit estimate?
  3. Fix the time frame. Does the percentage cover five years, nine years or another period? Is it conditional on completing five years of endocrine therapy?
  4. Request absolute outcomes. What is my risk with endocrine therapy alone, what is it with chemotherapy added, and what is the absolute reduction?
  5. Review clinical modifiers. How do my age or menopausal status, tumor size, grade, receptor results, HER2 status and lymph nodes change the estimate?
  6. Discuss feasibility and values. Which side effects, existing illnesses, fertility goals, work or caregiving responsibilities and personal priorities should influence the choice?
  7. Check the pathology. Would a second pathology review clarify grade, receptor testing, HER2 or the number of involved nodes?
  8. Consider another opinion. Would a multidisciplinary breast team or second-opinion consultation change the interpretation or treatment options?
  9. Set a review point. When should the estimate be revisited if treatment, adherence, new pathology information or follow-up findings change?

Common interpretation mistakes and how to correct them

Mistake: treating the score as a percentage

Correction: Circle the report’s separate risk field and ask what endpoint and time period it represents. The score and the percentage are different outputs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Mistake: quoting a risk without its treatment assumption

Correction: Ask whether the estimate assumes endocrine therapy, chemotherapy, extended endocrine therapy or another plan. A percentage without that context is incomplete.

Mistake: assuming “low” means zero risk

Correction: A lower genomic result generally indicates a lower modeled risk within the studied population; it does not guarantee that recurrence cannot occur. Continue the treatment and follow-up plan unless your oncology team changes it.

Mistake: assuming “high” mandates one exact regimen

Correction: A higher result can increase the expected benefit of chemotherapy in an appropriate population, but regimen choice still depends on pathology, health, preferences and guideline-supported options.

Mistake: ignoring clinical risk

Correction: Review tumor size, grade, receptors, HER2, nodes, age, menopause and prior treatment alongside the assay. A discordant clinical and genomic picture deserves an explicit explanation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Mistake: comparing a friend’s number with yours

Correction: Confirm that the assay, endpoint, time horizon, treatment assumption and patient subgroup are the same. Usually they are not.

Mistake: using an outdated handout

Correction: Ask for the current report interpretation and guideline context. Older materials may show three score bands, while current patient materials commonly use 0–25 and 26–100 for chemotherapy discussions.

What the report cannot tell you by itself

The report cannot guarantee that cancer will or will not return, identify the exact date or location of a possible recurrence, or predict every side effect of treatment. It also cannot replace evaluation of a new symptom. NCI notes that management of recurrence depends on hormone-receptor status, previous treatment and whether the recurrence is local, regional or distant; those details are assessed clinically rather than inferred from a past score alone.

A genomic result is therefore one input into a shared decision between you and your oncology team. If the report conflicts with pathology, ask whether the tissue sample, receptor testing or clinical data should be reviewed. If the decision remains uncertain, a multidisciplinary consultation can help reconcile the evidence instead of forcing a single number to answer a question it was not designed to answer.

When to revisit the estimate

Risk estimates are conditional. They may change if the planned endocrine therapy changes, treatment is stopped or interrupted, new pathology information becomes available, or follow-up reveals a new clinical issue. Ask at the outset when the team will reassess the plan and which events should prompt an earlier call. Keep the original report, pathology summary, treatment dates and medication list together so that a later clinician can identify the assumptions behind the original estimate.

Why there is no universal recurrence percentage

No authoritative source provides one recurrence percentage that applies to every person with breast cancer. The meaningful estimate depends on the assay, tumor biology, lymph-node status, menopausal group, treatment plan and duration of follow-up. A trustworthy interpretation therefore sounds specific: it names the endpoint, states the time horizon, identifies the therapy assumed and shows the absolute difference between options. If any of those elements is missing, ask for clarification before using the number to make a treatment decision.
