Improve retrieval accuracy by treating retrieval as a measurable pipeline, not as a single embedding-model choice. First make sure ingestion produces complete, clean, permission-aware chunks. Then establish separate lexical and vector baselines, combine them when your queries need both exact terms and semantic matching, increase the candidate pool, and add reranking to recover precision. Evaluate every change on a frozen set of representative questions, reporting Recall@k, precision@k, MRR, nDCG, latency, and cost by query class.

This order matters: a missing table row, broken heading, stale permission, or bad chunk boundary cannot be repaired by a more sophisticated generator. The sections below show where retrieval fails, which intervention addresses each failure, and how to prove that an apparent improvement is real.

What retrieval accuracy means in a RAG system

Retrieval accuracy is the quality of evidence selected before the language model writes an answer. A RAG application can produce fluent text while still failing if the relevant document never enters the candidate set, if the right chunk is buried below the cutoff, or if an irrelevant chunk is passed to generation. Separate those failure modes instead of judging the whole system by answer fluency.

Microsoft’s February 4, 2025 overview describes RAG as a set of techniques for improving knowledge retrieval and generated responses. Its practical distinction is useful: retrieval should protect recall (finding every plausible piece of evidence), while reranking should improve precision (putting the most useful evidence first). NVIDIA’s reference architecture likewise keeps retrieval, optional reranking, and generation as separately measurable stages.

Three questions to ask for every failed answer

  • Was the source ingested? Missing files, failed OCR, omitted tables, stale indexes, or permission mistakes create a recall failure before search starts.
  • Was the source retrieved? The relevant chunk may be absent because of poor chunking, vocabulary mismatch, an overly small candidate pool, or an overly restrictive filter.
  • Was the source used correctly? If the right chunk was retrieved but ranked below distractors or ignored by the generator, the problem is precision, context quality, or answer grounding rather than initial recall.

Fix the corpus before tuning search

Index quality sets an upper bound on retrieval quality. Build an ingestion check that records which documents were accepted, how much text was extracted, and which metadata fields were retained. Do not silently continue when a parser, OCR step, table extractor, or permission lookup fails.

Preserve document structure and provenance

Keep headings with the content they introduce instead of flattening every page into an undifferentiated text stream. Preserve table headers and row relationships, page or section identifiers, source document IDs, timestamps, language, document type, and access-control fields. These fields support both better ranking and later citation or audit.

Duplicate boilerplate can overwhelm distinctive content. Detect repeated headers, footers, navigation text, and legal disclaimers during cleaning, while retaining wording that changes the meaning of a section. Keep a stable source identifier on every chunk so an operator can trace a result back to the original page or section.

Check extraction and freshness explicitly

  • Compare the indexed document inventory with the authoritative repository.
  • Open samples containing scanned pages, tables, lists, footnotes, and multilingual text to verify extraction.
  • Confirm that updates and deletions propagate to the index and that timestamps reflect the current source.
  • Test tenant and permission boundaries with users who should and should not see each document.

These checks are more valuable than changing an embedding model when the correct evidence is absent or incorrectly represented.

Choose chunk boundaries that match the information need

Chunking divides a document into searchable units. Microsoft’s guidance groups common approaches into token or character limits, structure-aware splitting, and overlapping chunks. The appropriate size depends on document shape, query needs, and every model in the pipeline; there is no universal chunk length.

Start with meaningful structure

Prefer sections, subsections, list items, or complete table regions when those boundaries preserve meaning. Attach the nearest heading, document title, and location metadata to each chunk. A chunk that contains a paragraph without its heading may match less reliably because the terms defining its subject were discarded.

Use limits and overlap as controlled variables

Very small chunks can lose definitions, conditions, or references that a question depends on. Very large chunks can dilute the matching signal and consume context unnecessarily. Test token or character limits on a representative query set rather than selecting a value by habit. Overlap can protect sentences that cross a boundary, but excessive overlap creates near-duplicates, enlarges the index, and can crowd the candidate list with copies of the same passage.

Record the exact chunking configuration with each index version. When a result changes, you need to know whether the cause was a boundary, overlap, parser, embedding, or ranking change.

Handle tables and long documents deliberately

For tables, preserve column headings with each group of rows and retain units, dates, and qualifiers. A row without its header may be semantically unusable even if every value was extracted. For long documents, keep section and page metadata so retrieval can return a focused passage while the application can display surrounding context when needed.

Make query and chunk preprocessing identical

Apply the same cleaning operations to indexed chunks and incoming queries. Azure guidance explicitly recommends matching query preprocessing to chunk preprocessing and using the same embedding model for both sides of a vector search. If one side is lowercased, normalized, language-detected, or stripped of markup while the other is not, the representation space becomes inconsistent.

Define a shared preprocessing contract

  • Use the same normalization rules for case, whitespace, punctuation, and Unicode.
  • Apply the same language handling and tokenization assumptions.
  • Remove boilerplate consistently rather than only from documents or only from queries.
  • Version the preprocessing code and store that version with the index.
  • Regenerate embeddings when the preprocessing or embedding model changes; do not mix incompatible vectors without testing.

Keep the user’s original query for auditing even when a translated query is sent to search. This lets you compare intent fidelity and explain why a result appeared.

Establish lexical and vector baselines separately

Do not begin with a hybrid configuration whose gains cannot be attributed. Run a lexical baseline and a dense-vector baseline on the same frozen evaluation set, using the same filters, candidate count, and relevance labels.

Retrieval mode Strength Typical failure it exposes
Full-text or BM25 Exact identifiers, names, codes, and rare terms Misses paraphrases and conceptually similar wording
Dense vector search Conceptual similarity and paraphrased questions Can overlook an exact code or rare token that has weak semantic signal
Hybrid search Combines lexical and semantic evidence Requires fusion and weight tuning; extra query complexity can increase latency

These baselines show whether a failure is vocabulary-related or semantic. They also prevent a hybrid system from masking a regression in one underlying retriever.

Use hybrid search when queries need both exactness and meaning

Hybrid search runs sparse full-text and dense vector retrieval, then combines their ranked lists. Reciprocal Rank Fusion (RRF) is a common approach documented in Microsoft’s hybrid-search flow; weighted vector queries are another way to control the contribution of semantic search.

Tune fusion against labeled queries

Start with the two independent rankings and inspect disagreements. Queries containing product codes, error identifiers, names, or legal terms often benefit from lexical results. How-to questions, paraphrases, and concept descriptions often benefit from vectors. Adjust fusion weights or RRF settings only after measuring these query classes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A hybrid result is not automatically better. If lexical results dominate, paraphrase recall may remain low; if vectors dominate, an exact identifier can be displaced. Keep the original query and both component rankings in logs so a poor fused result can be diagnosed.

Protect recall with a larger candidate pool, then rerank

Reranking cannot recover a document that initial retrieval never returned. Increase the first-stage candidate count until Recall@k stops improving materially or latency becomes unacceptable, then apply a query-aware reranker or cross-encoder to that pool. Pass only the highest-quality evidence to generation.

Measure the two stages independently

  • First stage: measure whether every labeled relevant chunk appears anywhere in the candidate pool.
  • Reranking stage: measure whether relevant chunks move toward the top and whether distractors are removed.
  • Generation stage: score whether the final answer is faithful to and supported by the selected evidence.

Rerankers add computation and therefore latency. Compare the accuracy gain with the response-time and infrastructure cost on the same workload instead of assuming that a larger model is worth the delay.

Inspect context quality, not just rank numbers

Review the top results for duplicated boilerplate, broken boundaries, missing titles, contradictory versions, and incorrect source links. A high ranking score does not guarantee that the passage contains the complete condition needed to answer the question. Context inspection often points back to ingestion or chunking rather than ranking.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Translate difficult queries selectively

Query translation can improve recall when the user’s wording is vague or combines several intents, but every translation risks changing the question. Keep the original query and compare translated variants offline before enabling them broadly.

Four useful translation patterns

  • Augmentation: add missing context, such as an assumed product area or time frame, when that context is justified by the request.
  • Decomposition: split a multi-part question into separately searchable subquestions, then combine their evidence.
  • Rewriting: make an underspecified or conversational query explicit while preserving its intent.
  • HyDE: create a hypothetical answer and use its embedding to retrieve conceptually similar passages.

Reject a rewrite when it lowers intent fidelity, removes a constraint, or retrieves plausible but unrelated material. Evaluate translated and original queries side by side by query class; do not assume one strategy helps every question.

Apply metadata filters only when metadata is trustworthy

Filter by tenant, permissions, product, date, language, source, or document type when those fields are accurate and required by the question. Filtering narrows the candidate space and removes irrelevant evidence, which can improve both speed and precision.

The risk is silent recall loss. A wrong tenant ID, stale permission, incorrect language tag, or missing product value can suppress the only relevant chunk. Test filter logic with positive and negative permission cases and with documents whose metadata is incomplete. Log the filter values used for each query so an empty result can be distinguished from a genuinely empty corpus.

Tune approximate-nearest-neighbor indexes against your workload

Azure documents Hierarchical Navigable Small World (HNSW) and exhaustive K-nearest-neighbor (KNN) approaches, along with similarity metrics and HNSW parameters such as efConstruction, efSearch, and m. These settings trade index quality, compute, storage, recall, and query latency.

Understand the trade-offs

  • Increasing construction effort can improve graph quality but consumes more build compute and storage.
  • Increasing query-time exploration can improve recall while adding latency.
  • Exhaustive KNN can provide a reference point for recall because it does not rely on approximate graph traversal, but its cost may be unsuitable for production-scale workloads.
  • Similarity metrics must match the embedding model’s intended geometry; changing the metric is an index change that requires evaluation.

Tune these parameters after corpus, chunking, retrieval-mode, candidate-pool, and reranker decisions. Compare every setting on the same frozen query set and report both recall and latency.

Build an evaluation set that reflects real questions

Create a frozen, labeled set before optimizing. It should contain exact-term, paraphrase, multi-hop, long-document, table, multilingual, permission-filtered, and unanswerable questions. Label the relevant documents or chunks and include positive and negative examples.

Track retrieval metrics

Metric What it tells you
Recall@k Whether relevant items appear within the first k retrieved results
Precision@k How many of the first k results are relevant
MRR How high the first relevant result appears in the ranking
nDCG@k How well the ranking orders graded relevance within the first k results

Score answer faithfulness and citation support separately from retrieval. A system can have strong Recall@k yet produce an unsupported answer if the generator ignores evidence or combines passages incorrectly.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Report results by query class

Publish one aggregate score only as a summary. Break out exact identifiers, paraphrases, multi-hop questions, tables, multilingual queries, permission-filtered requests, and unanswerable cases. A change that raises the average while harming security-sensitive or unanswerable cases is not a safe improvement.

AWS recommends running a fixed evaluation set after every material change, tracking Recall@k and nDCG@k, and refreshing the set as real query patterns drift. Add newly observed failure modes without deleting the original cases, so historical comparisons remain valid.

Use a disciplined troubleshooting sequence

  1. Verify ingestion completeness. Check extraction, OCR, tables, metadata, permissions, and document freshness against the source repository.
  2. Inspect retrieved chunks. Look for boundary errors, missing headings, duplicated boilerplate, contradictory versions, and incorrect source identifiers.
  3. Run dense-only and lexical-only baselines. Establish which vocabulary or semantic cases each method misses.
  4. Add hybrid fusion. Tune weights or RRF settings using labeled queries and retain both component rankings for diagnosis.
  5. Increase candidate top-k. Confirm that relevant chunks enter the pool before spending latency on reranking.
  6. Add reranking. Measure precision and answer support together with the added response time.
  7. Add filters and query translation selectively. Enable them only where labeled tests show a gain without intent or permission regressions.
  8. Tune ANN parameters and embedding models last. Compare HNSW, exhaustive KNN, metrics, and model changes on the unchanged evaluation set.
  9. Monitor drift. Recheck performance after corpus, embedding, reranker, index, or query changes and when real query patterns shift.

Balance accuracy, latency, and cost

Choose configurations on four axes: retrieval recall, top-k precision and context quality, latency, and cost. A larger candidate pool or reranker can improve evidence quality while increasing response time. Hybrid search can improve exact-term coverage while adding fusion complexity. Richer chunks can preserve context but increase index size and embedding work. ANN settings can trade query speed against recall.

Record these measurements per query class and per index version. A single average latency or accuracy number hides the cases that matter most, such as permission-filtered requests or questions requiring a table row and its heading together.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Operational safeguards for production

Version every retrieval dependency

Store the corpus snapshot, parser and OCR versions, preprocessing rules, chunking limits and overlap, embedding model, sparse analyzer, fusion configuration, reranker, filters, and ANN parameters with each evaluation run. Without this record, a regression cannot be reproduced.

Keep evidence traceable

Return source identifiers, page or section locations, timestamps, and permission decisions with each retrieved chunk. This supports citation, incident review, and removal of stale or unauthorized content. It also makes it possible to distinguish a ranking problem from a source-data problem.

Watch for drift and security regressions

Monitor query classes, empty-result rates, low-confidence retrieval, changed document distributions, and permission-filtered failures. Re-run the frozen set after material changes, then add representative production failures to the evolving set. Never trade away tenant or permission boundaries for a small relevance gain.

A practical decision framework

  • If the correct document is absent from the index, repair ingestion, extraction, OCR, freshness, or permissions.
  • If the document is present but the passage is incomplete, change structure-aware chunking, heading retention, table handling, or overlap.
  • If lexical search finds codes but misses paraphrases, add dense retrieval; if vectors miss exact identifiers, retain lexical search.
  • If the relevant chunk appears only deep in the candidate list, raise first-stage top-k and test hybrid fusion before reranking.
  • If relevant chunks are present but distractors rank higher, add a reranker and measure its latency.
  • If filters remove valid evidence, correct metadata and authorization tests before loosening constraints.
  • If one query wording fails while a clearer variant succeeds, test rewriting, decomposition, augmentation, or HyDE with intent-fidelity checks.
  • If gains disappear after deployment, investigate corpus, query, embedding, reranker, index, and permission drift using the versioned evaluation record.

The reliable path to better RAG retrieval is therefore incremental and measurable: make evidence complete and traceable, align preprocessing, preserve structure, benchmark sparse and dense methods, fuse when appropriate, protect recall before reranking, and validate every change against representative labeled questions.
