Better OCR starts before recognition: capture a sharp, correctly oriented source, choose a parser that understands document structure, preserve the parser’s layout metadata, and send uncertain or high-consequence results to review. Plain text extraction cannot reliably retain tables, columns, checkboxes, figures, or reading order, so accuracy and layout must be evaluated separately on documents that represent your real workload.

What determines OCR accuracy

Optical character recognition (OCR) converts pixels into characters, words and, with a document-analysis model, structural elements. The quality of the pixels and the complexity of the page both affect the result. Microsoft’s OCR guidance states that “Document scan quality, resolution, contrast, light conditions, rotation, and text attributes such as size, color, and density can all affect the accuracy of OCR results.” A service can expose a confidence value for a word or field, but that value is not a universal accuracy percentage.

Accuracy also depends on what you ask the system to produce. A transcription task may need only characters in approximate order. A records workflow may need page boundaries, headings, table cells, selection marks, figures, and coordinates. Treat those as separate acceptance criteria: a page can have mostly correct words but unusable table structure, or clean tables with a few critical digits misread.

There is no single resolution or preprocessing setting that fixes every document. Microsoft recommends testing with a sample set representative of the content and real capture conditions, because performance varies by implementation.

1. Diagnose and improve the source before changing the model

Inspect representative pages at their original size before tuning an OCR engine. Include the worst pages, not only clean examples. Record the document type, language or script, page size, scan or camera method, and whether the PDF already contains digital text.

Look for defects that cause recognition errors

  • Blur or motion: Characters with merged strokes are difficult to recover algorithmically.
  • Skew and rotation: A tilted page can reduce recognition and disrupt line and column detection.
  • Glare, shadows and uneven lighting: Reflections can erase parts of characters; shadows can look like marks.
  • Low contrast or faint print: Light ink, carbon copies and faded thermal receipts may disappear during binarization.
  • Small, dense or colored text: Tiny type, tightly packed lines and text that blends into a colored background require particular scrutiny.
  • Cut-off edges: Cropping can remove the first or last characters, table borders, page numbers or checkbox labels.
  • Compression artifacts: Repeatedly saved or messaged images may contain block noise around letter edges.

Correct the capture where possible

  1. Rescan paper that is blurred, clipped or too faint instead of expecting OCR to reconstruct missing pixels.
  2. Place the page flat and align it with the camera or scanner so baselines are horizontal.
  3. Use even lighting without reflections or shadows; for camera capture, keep the lens parallel to the page.
  4. Capture the entire page, including margins, table borders and page numbers.
  5. Keep the clearest available source file and avoid unnecessary recompression.
  6. For a large collection, sample each scanner, camera setup and document batch; one clean page does not represent all inputs.

A dedicated document scanner can be sensible when paper capture is the bottleneck, but the evidence supports only the general principle that scan quality affects OCR. It does not establish a best scanner or guarantee improvement for every workflow. If evaluating hardware, match it to expected volume, page size, duplex needs and the condition of the originals, then measure the resulting error rate on your own sample.

2. Determine whether the PDF already has text

A PDF may contain selectable digital text, image-only scanned pages, or both. Rasterizing every page and running OCR can discard clean text that could have been extracted directly. Google Cloud’s Enterprise Document OCR documentation describes native PDF parsing that extracts embedded digital text while applying OCR to non-digital text. This mixed approach is useful for PDFs assembled from born-digital pages and scans.

A practical PDF check

  1. Open a representative page and try selecting and copying a sentence.
  2. Paste it into a plain-text editor and check whether characters, spaces and line order are sensible.
  3. Check several pages, because a single file can mix digital pages with scans.
  4. Route pages with reliable embedded text through text extraction and image-only pages through OCR, or use a parser that handles both natively.
  5. Compare the combined output with the original page sequence and retain page identifiers.

Embedded text is not automatically correct. It may be a bad hidden text layer from an earlier OCR pass, have incorrect reading order or omit visual elements. Validate it just as you would newly recognized text.

3. Choose a parser that matches the document

Use a basic OCR endpoint when the requirement is a character transcription from a simple page. Use a layout-aware document model when downstream users or systems need structure. Microsoft’s Azure Document Intelligence v4.0 layout model documents extraction of pages, paragraphs, lines, words, selection marks, tables, figures and sections, with Markdown output. The documented input coverage includes PDF, image formats and several Office formats. These are capabilities, not a promise that every complex design will be reconstructed perfectly.

When a layout model is necessary

  • Tables: You need row and column relationships rather than a stream of cell text.
  • Forms: You need labels, values and marked or unmarked selection boxes.
  • Multi-column pages: You need a reading sequence that follows the intended columns.
  • Reports and contracts: You need headings, paragraphs, sections, footnotes or page association.
  • Illustrated documents: You need figure locations or a way to review images alongside extracted text.

Google’s documentation describes configurable layout behavior and an optional legacy layout algorithm. It warns that enabling the legacy option might change text ordering. Test both configurations on your own multi-column pages before adopting one globally.

Verify language and script support

Check support for the exact language, script and model, not merely the service’s general language list. Google documents language hints, while Microsoft directs users to model-specific language-support documentation. Test mixed-language pages, diacritics, numerals and handwriting separately if they occur in your collection. Do not infer support for a particular script from support for a related language.

4. Preserve layout as structured data

Store the complete structured response whenever possible, not only a flattened text string. Preserve page numbers, text spans, bounding regions, lines, words and the relationships among paragraphs, tables, figures and selection marks. Those fields let you re-render, search by region, audit a value and troubleshoot reading-order errors later.

Use Markdown or text as a representation, not a visual clone

Layout services may emit Markdown or text in addition to structured JSON. Markdown can be convenient for search indexes, review queues and language-model pipelines because headings and table rows remain visible. Verify that generated Markdown retains the headings, rows and intended order needed by your consumer. It does not preserve every visual detail: exact typography, coordinates, page geometry, overlapping elements, complex columns and footnotes still require comparison with the original or retention of the structured response.

Microsoft’s Azure AI Search Document Layout skill can produce Markdown or text and include image-location metadata. Its documentation notes that DOCX and PDF inputs can yield different results because image handling differs. Keep the original file and page reference so a reviewer can inspect the source when a representation is ambiguous.

Recommended record for each extracted document

  • Original file identifier and immutable copy of the source.
  • Page number and, where available, bounding coordinates for each extracted element.
  • Recognized text plus the parser’s spans or offsets.
  • Tables with explicit row and column structure rather than tab-delimited text alone.
  • Selection-mark state and its associated label or region.
  • Word-level and field-level confidence values when exposed.
  • Parser name, model version, configuration and processing timestamp.
  • Review status and the corrections made by a human.

5. Use confidence to prioritize human review

Confidence is a review signal, not proof that a value is correct. Microsoft describes word confidence as a transcription-confidence score and recommends sampling your own documents to understand how values relate to extraction quality. A visually plausible error can still receive a high score, especially when a wrong digit forms a valid identifier.

Word confidence and field confidence answer different questions

Word confidence concerns the recognition of the characters. Field confidence also reflects confidence in locating the extracted value in the expected position. Microsoft recommends considering OCR confidence alongside field confidence rather than treating either one as a standalone verdict. For example, a date may contain high-confidence characters but be assigned to the wrong nearby label.

Set thresholds from consequences and measured samples

Microsoft gives 0.80 as an example cutoff for straight-through processing. That is an illustrative workflow value, not a universal default or a population accuracy statistic. Microsoft’s Content Understanding guidance similarly provides illustrative thresholds for critical, important and non-critical fields while saying thresholds should be determined experimentally for each use case.

  1. Classify fields by consequence: legal identifiers, payment totals and medical instructions generally deserve stricter review than a low-stakes search snippet.
  2. Collect a labeled sample that includes clean, degraded and unusual pages.
  3. For each field, compare confidence with actual correctness and record false accepts and unnecessary reviews.
  4. Choose a review band that meets your error tolerance and staffing capacity.
  5. Revalidate after changing the capture process, parser, language hint or document mix.

For critical values, review the image and the extracted value even when confidence exceeds your automatic threshold. Totals, account numbers, dates, dosage instructions and legal wording are poor candidates for blind acceptance.

6. Measure text accuracy and layout accuracy separately

Build a small validation set before deployment. It should cover the document types, layouts, languages, print quality and scan conditions that will actually occur. Compare recognition with the source page and track the errors that matter to the task.

Text checks

  • Character substitutions such as 0/O, 1/I and punctuation loss.
  • Missing or inserted words, spaces and line breaks.
  • Errors in decimal points, minus signs, dates, currency symbols and identifiers.
  • Language-specific diacritics, ligatures and script direction.

Structure checks

  • Reading order on single-column, multi-column and rotated pages.
  • Table row and column assignment, merged cells and header repetition.
  • Association of labels with form values and selection marks.
  • Heading and paragraph boundaries, footnotes and page breaks.
  • Figure location and whether captions remain associated with the figure.

Retest after every material change. A parser setting that improves one form can damage a two-column report. There is no independent, vendor-neutral benchmark in the available documentation that establishes a general accuracy winner, so feature descriptions should not be presented as head-to-head performance results.

OCR and layout options to compare

Decision area Questions to answer Why it matters
Input support Does the workflow accept the required PDFs, image types and Office files? Can it handle mixed digital and scanned pages? Unsupported or needlessly rasterized inputs create avoidable errors.
Structural output Are pages, paragraphs, lines, words, tables, figures, sections and selection marks available? Is Markdown or JSON provided? Flat text cannot reliably support tables, forms or layout-aware search.
Quality signals Are word, field or image-quality scores exposed, and do they correlate with correctness on your sample? Signals are useful for triage only after local validation.
Reading order How does the parser handle columns, rotated pages, sidebars and tables? Do optional layout modes change order? Incorrect sequence can make otherwise correct words unusable.
Language and script Is the exact language, script and hint mechanism supported by the selected model? General service support does not guarantee model-level support.
Operational limits What are the current file, page, latency and service constraints? Limits can affect batching and production design. Verify them in current documentation; the Azure AI Search layout skill, for example, documents a five-minute processing limitation for the called layout model.
Review workload How many pages or fields fall into the review band, and what is the cost of a missed error? The best threshold balances risk and human capacity, not a copied example.

Common failure modes and fixes

Text is correct but appears in the wrong order

Check whether the page has multiple columns, floating text boxes, sidebars or a legacy layout option enabled. Compare the output with the original page, test the parser’s alternate layout configuration and retain coordinates so a reviewer can reconstruct the intended sequence.

Tables become a paragraph of numbers

A plain OCR endpoint may have recognized every cell without identifying rows and columns. Switch to a layout-aware model, preserve table objects and inspect merged cells and repeated headers. Do not rebuild columns from whitespace unless the document set is controlled and your validation proves that approach reliable.

Scanned pages are blank or nearly blank

Confirm that the PDF page is image-only and that the selected workflow actually performs OCR. Inspect contrast, glare, rotation and clipping. If the page is faint or blurred, recapture it; software cannot recover characters that are absent from the image.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Confidence is high but critical values are wrong

Check the source image and field location, not only the score. Add representative variations to training or configuration data where the platform supports it, tighten review rules for high-consequence fields and measure false accepts separately from average confidence.

Markdown loses visual details

Keep the original PDF or image and the structured response with coordinates. Use Markdown for text-oriented consumers, but route complex figures, footnotes, overlapping elements and visually important tables to a page-image review path.

A production workflow you can repeat

  1. Define the acceptance criteria. Decide which text, fields and layout relationships must be correct and which errors require human approval.
  2. Assemble a representative sample. Include every major template, language, scan source and quality level.
  3. Diagnose inputs. Separate embedded digital text, image-only pages and mixed PDFs; correct capture defects where possible.
  4. Select the processing path. Use native PDF parsing for mixed content when appropriate and a layout model for tables, forms, columns and figures.
  5. Configure language and layout options. Test hints and optional algorithms on real pages, especially when reading order matters.
  6. Persist structured output. Save page association, spans, coordinates, tables, selection marks, figures, confidence and model metadata.
  7. Run automated checks. Flag missing pages, empty output, impossible dates, malformed totals and confidence values below your locally tested threshold.
  8. Review risky results. Send low-confidence spans and high-consequence fields to a person who can see the original page.
  9. Measure and iterate. Compare text and structure with the source, categorize errors and retest after each change.
  10. Monitor drift. Re-sample when a new scanner, template, language, model version or document source is introduced.

What “better OCR” should mean for your project

Improvement is not simply a higher average character score. A useful system produces text that is correct enough for its purpose, keeps the relationships that downstream processes require and exposes uncertainty before an error causes harm. Source quality, parser choice, structured retention and measured review thresholds work together. If any one is ignored, a clean-looking transcript can still lose the table, misorder the page or silently corrupt a critical number.

Documented capabilities from Microsoft and Google explain what their services can return and which settings exist; they do not establish that one provider is universally more accurate. Make the decision with a representative, labeled sample and report your own text-error, layout-error and review rates.
