PHP mail can show the “wrong” sender because an email has two sender identities. The visible From: header is what most people see, while the SMTP envelope sender (the MAIL FROM reverse-path) controls bounce handling and may appear as Return-Path:. Set a mailbox on your own domain in From:, put a visitor’s address in a validated Reply-To:, and set the envelope sender separately when your Unix mail transport needs it. Also remember that mail() returning true only means the configured transport accepted the message; it does not prove delivery.
What “wrong address” means in PHP mail
Start by deciding which address is wrong. A received message can contain several relevant fields:

SMTP carries the envelope reverse-path in the MAIL FROM command. Under RFC 5321 (2008), the receiving system can construct Return-Path: from that envelope information. It is therefore normal for From: and Return-Path: to differ.
PHP’s mail() call must produce a From: header, either in the headers argument or through configuration. On Unix-like systems, PHP commonly hands the message to a local sendmail-compatible program; on Windows, PHP can use its configured direct-SMTP settings.

Inspect the raw message before changing code
Do not rely only on the address displayed in a mail app. Open the message’s “show original,” “view source,” or equivalent raw-message view and record the complete From:, Reply-To:, Sender:, and Return-Path: lines. Also note any authentication or relay-added headers because they can reveal which system rewrote the message.
- If
From:is unexpected, inspect the PHP call, shared mail helper, framework defaults, and any server configuration that adds headers. - If only
Return-Path:is unexpected, investigate the envelope sender, the local MTA, or the relay. Changing the visibleFrom:header alone will not correct it. - If the message has the expected headers but never appears in the mailbox, treat that as a delivery problem, not a sender-header problem. Check transport logs, SMTP responses, spam or quarantine folders, and relay policy.
Keep a copy of the raw message and the application log for one test submission. Comparing a message generated before and after each change is faster than guessing from the mail client’s abbreviated display.
Set a controlled From address and a separate Reply-To
Use a mailbox or domain that the site operator controls for From:. Put the visitor’s address in Reply-To: so staff can respond without pretending to send mail from an unauthorized domain. A typical plain-text message looks like this:

?>The fixed address belongs to the site, not to the person filling in the form. Many relays reject or rewrite a message whose From: domain is not authorized for the account or server. A visitor’s address is also untrusted input, so validate it before placing it in Reply-To: and reject carriage-return or line-feed characters. Never concatenate unvalidated form fields into arbitrary headers.
The fourth argument can be a string containing multiple header lines or an array of header names and values, as shown above. Whichever form you use, make the intended From: value explicit rather than relying on a host-wide default. Keep the subject and body separate from header construction.

Correct the envelope sender on a Unix-like server
When PHP invokes a local sendmail-compatible transport, the fifth mail() argument can pass command-line options. Use the -f option with a server-controlled address to set the envelope sender:

?>This changes the address supplied in the transport’s envelope, which is the value that can become Return-Path:. It does not replace the visible From: header, so set both deliberately when you need both identities to be consistent.
PHP states that the additional-parameters value is escaped internally, but it still recommends sanitizing it. Do not let a form field determine the -f value. Use a constant or a server-side allowlist such as no-reply@example.com.
The local sendmail implementation may apply its own policy. The web-server account might need to be listed in /etc/mail/trusted-users; otherwise sendmail can add an X-Warning header or otherwise flag the submission. Check the selected MTA’s logs and configuration when the fifth argument appears to be ignored.
Check the platform-specific PHP configuration
Windows direct SMTP
Windows PHP installations that send directly through SMTP use the SMTP, smtp_port, and sendmail_from directives in php.ini. Confirm that the host and port point to the intended SMTP service and that sendmail_from is a mailbox authorized by that service. A typical configuration has the following shape:

sendmail_from = no-reply@example.comThe exact host, port, encryption, and authentication requirements come from your SMTP service. The mail() interface’s Windows transport does not by itself provide the diagnostics or authenticated workflow of a full SMTP library. After editing php.ini, restart the relevant web server or PHP worker pool and confirm the loaded configuration through the runtime environment; editing a different php.ini has no effect.
Unix-like systems
Unix-like installations commonly delegate to the executable named by sendmail_path. Inspect that directive, identify which MTA or wrapper it invokes, and then check that system’s sender-rewriting, relay, and logging rules. Changing a PHP header cannot override an MTA policy that rewrites or rejects the envelope.
After changing sendmail_path, MTA settings, or process-level PHP configuration, restart the service or worker process that actually runs the application. A long-lived PHP worker can continue using old configuration until it is recycled.
Understand why mail() returns true but no email arrives
A true return value means the configured delivery system accepted the message for processing. It is not a confirmation that the recipient server accepted it, that the mailbox stored it, or that the recipient will see it. A false result indicates that PHP could not hand the message to its configured transport, but a true result ends PHP’s synchronous responsibility at that handoff.

- Check the local MTA or relay log for the message, the envelope sender, the recipient, and the SMTP response.
- Look for a queued, deferred, rejected, or bounced status rather than assuming immediate delivery.
- Check the recipient’s spam, quarantine, and filtering folders.
- Verify that the relay authorizes the
From:domain and that the envelope address is permitted. - Compare the raw message headers with the application’s intended values.
If the relay reports acceptance but the destination later rejects the message, the rejection belongs to the relay-to-recipient stage. If there is no MTA or relay record at all, investigate the PHP configuration, the sendmail path, permissions, and the web-server account.

Prevent header injection in forms
Header injection occurs when untrusted input inserts a newline and creates additional headers or recipients. Treat every form value as hostile until validated. For an email field:
- Validate the address with an email validator appropriate to your application.
- Reject any value containing carriage return (
r) or line feed (n) characters, even if another validator accepts it. - Allow the value only in
Reply-To:after validation; never use it asFrom:,Sender:, the envelope address, or a sendmail option. - Keep recipient addresses, subjects, and headers under separate validation rules. Do not join user input into a block of raw header text.
- Log validation failures without logging sensitive message content unnecessarily.
A fixed sender also limits spoofing: your application sends from a domain it controls, while the visitor’s address remains a reply destination. This arrangement is safer and more compatible with relay authorization than copying the visitor’s address into From:.
When to move beyond mail()
For occasional, low-volume messages on a correctly configured server, mail() can be adequate. PHP’s manual notes that it is inefficient for larger volumes and points to mail packages; an SMTP-capable library or managed transactional relay is usually easier to authenticate, monitor, and troubleshoot at production scale.

Whichever option you choose, authorize the sending domain, use a stable envelope address for bounces, retain enough logs to trace a message, and keep visitor input out of the visible sender unless your relay explicitly permits that design.
Production verification checklist
- Send a test message to a mailbox where you can view the raw source.
- Confirm that
From:is the intended site-controlled mailbox. - Confirm that
Reply-To:is the validated visitor address, when a reply is needed. - Confirm that
Return-Path:matches the intended envelope policy, recognizing that the receiving system writes that header. - Record whether the test used Unix sendmail or Windows direct SMTP; the applicable settings differ.
- Check MTA or relay logs for acceptance, queueing, rejection, and final SMTP responses.
- Test a deliberately invalid visitor address and a value containing newline characters; both must be rejected before header construction.
- Restart the actual PHP worker or web server after configuration changes.
- For production traffic, document the authorized sender domain, envelope address, relay credentials, log location, and bounce-handling procedure.
Frequently asked questions
Can I make Return-Path: equal From: just by changing the header?
No. Return-Path: reflects the SMTP envelope reverse-path, not merely the visible header. Set the envelope sender through the Unix transport’s -f option or the corresponding SMTP/library setting, then verify what the receiving system records.

Why should the visitor address be in Reply-To:?
It lets staff reply to the visitor while the message still comes from a domain and mailbox controlled by the site. This avoids unauthorized From: values that a relay may rewrite or reject.
Does a successful mail() call guarantee delivery?
No. It only reports acceptance by the configured delivery system. Use transport logs, SMTP responses, bounce information, and the recipient’s filtering folders to determine what happened next.

What should I check if Unix mail adds an X-Warning header?
Review the local sendmail trust policy. The web-server account may need to be listed in /etc/mail/trusted-users; also inspect the MTA logs for the exact warning and sender policy applied.
