Fixing inaccurate AI output is an operations problem, not a prompt trick. Treat each AI-assisted task as a system with measurable failure modes: identify what can go wrong, test realistic cases, add controls before the result is used, require an accountable review for consequential actions, and monitor what happens after release.

NIST calls confident false or contradictory generative-AI content confabulation (often called a hallucination or fabrication). Microsoft’s Azure OpenAI guidance similarly recommends scenario-specific harm assessment, test sets, iterative measurement, structured outputs, source checking, review and edit controls, feedback telemetry, incident response and rollback. Those are design options to evaluate in your workflow, not a universal checklist or guarantee.

What an inaccurate AI output is—and why confidence makes it dangerous

NIST’s Artificial Intelligence Risk Management Framework: Generative Artificial Intelligence Profile (NIST AI 600-1, 2024) defines confabulation as a generative-AI system generating and confidently presenting erroneous or false content in response to a prompt. The profile also discusses output that diverges from the input or contradicts an earlier statement. “Hallucination” is familiar language, but it can obscure the operational question: what did the system claim, what evidence should support it, and what happened because someone trusted it?

Not every nonfactual output is an error. A creative draft can intentionally invent a character or scenario. The risk appears when a workflow expects factual, complete, consistent or policy-conforming information and the output fails that expectation. A polished answer can therefore be more hazardous than an obviously broken one because a user may not slow down to verify it.

Risk is contextual. NIST highlights open-ended, long-form tasks and domains that require contextual or specialist expertise as particularly relevant settings for inaccurate or internally inconsistent output. Consequential uses deserve closer monitoring than low-stakes experimentation. A summary for a casual internal digest and a summary used in a regulated decision may use the same model, yet require very different approval, evidence and recovery controls.

Workflow example What an error could do Appropriate control posture
Casual internal digest Waste time or spread a minor misunderstanding Spot checks, clear labeling and an easy correction path may be proportionate
Customer communication Give a wrong commitment, price or policy explanation Source-grounded drafting, visible editing and approval before sending
Clinical or regulated decision support Influence a high-consequence decision or create compliance exposure Domain review, evidence verification, restricted automation, detailed audit trail and tested rollback

Step 1: Map the task and the cost of each error

Start with the workflow, not the model. Write down where AI enters, what information it receives, what it returns, and what people or systems do next. Microsoft recommends defining harms for the particular scenario and prioritizing them by likelihood and severity.

Document the workflow boundary

  1. Identify the AI component. Record the model or service version, prompt or instruction set, retrieval or reference sources, output format and connected tools. Include whether the system can send messages, update records, call an API or execute code.
  2. Describe inputs. Note data types, required fields, freshness, access permissions and common missing or ambiguous values. An answer cannot be checked reliably if the workflow does not preserve the records used to create it.
  3. Describe outputs and the next action. State whether the result is a draft, a recommendation, a classification, a number, a code change or an automatic transaction. Capture who sees it and what happens if it is wrong.
  4. List affected people and assets. Include customers, employees, patients, suppliers, regulated records, money, confidential information and system availability.

Turn vague “accuracy” into named failure modes

Break the problem into errors you can recognize. Examples include an unsupported factual claim, a wrong number or unit, a missing exception, a citation that does not support the sentence, a contradiction with the source record, an instruction that violates policy, an unsafe code change, an incorrect classification and an action taken without required approval. Distinguish errors that are reversible from those that create a durable or hard-to-reverse outcome.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Prioritize by likelihood and severity

Do not assume that the most frequent error is the most important. A rare error that sends a payment to the wrong account may outrank common harmless wording mistakes. Define the impact scale in business terms, then choose controls for the highest-risk combinations. Your scale and threshold are workflow decisions; the cited guidance does not establish a universal business-wide error rate or accuracy percentage.

Step 2: Build a representative evaluation set

Anecdotal examples are useful for discovery but insufficient for release decisions. Create a measurement set that resembles production and includes known failures. Microsoft recommends clear metrics, test sets and systematic manual and automated testing, followed by measurement after changes.

Collect realistic cases

  • Sample normal requests across teams, languages, document types and input lengths.
  • Add edge cases: missing fields, conflicting records, unusual units, ambiguous names, stale sources and requests outside the system’s scope.
  • Preserve incidents and user corrections as regression cases. Redact sensitive data while retaining the conditions that triggered the error.
  • Include adversarial or confusing instructions that a real user could submit, especially where the output can invoke a tool or alter a record.

Define the expected result with subject-matter experts

For each case, label the correct result or an acceptable range. Some tasks have one right answer; others allow several good summaries or classifications. Record required facts, prohibited claims, mandatory caveats and the source documents a reviewer should consult. A generated citation is not proof: verify that the underlying document actually supports the claim.

Choose measures that match the harm

Use separate measures for separate failure modes instead of collapsing everything into one score. Possible measures include factual support rate, critical-field accuracy, omission of required items, contradiction rate, citation-support rate, policy-violation rate, reviewer correction rate, unsafe-action rate and time to detect or correct an incident. For generated code, include test or build outcomes and human acceptance checks. Set thresholds from your own labeled data and risk tolerance; no source-backed universal threshold is available.

Test manually and automatically

Automated checks can catch formats, ranges, required fields, duplicate records and known policy terms. Human evaluators are needed for nuanced factual support, context, tone, ambiguity and domain judgment. Keep the test set stable enough to compare versions, while adding fresh production cases so the evaluation does not become a game of memorizing old examples.

Step 3: Add prevention controls before the output reaches a user or system

Controls should target the failure modes you measured. Microsoft describes several options, including supplying relevant source material, requiring citations, making instructions and output structure more specific, using predetermined responses for certain queries and limiting automatic execution. Test each intervention in the actual workflow; a control can create new failure modes or add friction without improving outcomes.

Ground answers in approved sources

Provide the model with the records it is allowed to use and identify source names or document identifiers in the output. Require a citation or quoted passage for factual claims when that is practical. Retrieval does not make an answer true: sources can be stale, incomplete or misread, and generated citations can themselves be confabulated. A reviewer must check the cited passage for support.

Make the instruction and output contract specific

State the task, audience, scope, date or jurisdiction, allowed sources, required fields and what to do when evidence is missing. Ask the system to return a structured object or form with explicit fields such as claim, source_id, confidence_reason and needs_review. Define a safe refusal or escalation response for requests outside the approved scope. Structured output improves checking; it does not guarantee factual correctness.

Use predetermined responses where variability is unnecessary

For fixed policy explanations, eligibility outcomes or safety notices, return an approved response selected by rules rather than asking the model to invent wording. Let the model fill only bounded fields, and validate those fields against the source system. This is especially useful when an incorrect answer would create a commitment or regulatory exposure.

Limit automatic execution

Separate drafting from acting. Require explicit approval before sending external communications, changing a customer record, issuing a refund, deploying code or calling a consequential tool. Apply tighter limits to actions that are difficult to reverse. If automation is allowed for low-risk steps, constrain parameters, log the request and provide a way to cancel or undo it.

Step 4: Design a review point people can actually use

Human review is an accountable control, not a magic guarantee. Microsoft advises designing the user experience to encourage people to review and edit generated output before accepting it, with reminders about possible inaccuracies. Review fails when people cannot see the evidence, are rushed, or are asked to approve a result they cannot meaningfully inspect.

Give reviewers the information needed to decide

  • Show the original request and the records supplied to the model.
  • Display citations next to the claims they are meant to support, with a direct path to the underlying document.
  • Make uncertainty, missing evidence and out-of-scope conditions visible instead of hiding them in a long paragraph.
  • Provide clear actions: edit, reject, request regeneration, escalate and accept.
  • Record who approved the result, when, what they changed and which source they checked.

Focus attention on measured weak spots

If local evaluation shows that numbers, dates, names or code are error-prone, highlight those fields for verification. Do not add warnings merely because a field looks important; test whether the warning improves detection and does not create alert fatigue. Ask reviewers to verify factual claims against source records and to test generated code before acceptance.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Match reviewer authority to the consequence

A reviewer must be able to stop the workflow, correct the output and obtain expert help. For specialized decisions, assign a subject-matter reviewer rather than relying on a general operator. If approval is ceremonial or the system proceeds automatically after a timeout, the “human in the loop” is not providing meaningful control.

Step 5: Monitor production, respond to incidents and recover

Pre-release tests cannot cover every production condition. Maintain telemetry and feedback that let you discover errors, understand their impact and assess whether mitigations work. Microsoft recommends feedback collection, relevant telemetry, incident response, rollback and phased release.

Capture the evidence needed for diagnosis

  • Workflow and model version, prompt or instruction version, retrieval-source versions and tool calls.
  • Input and output, subject to privacy and retention rules.
  • Reviewer decisions, edits, rejected outputs, user reports and downstream outcomes.
  • Latency, failures, refusal rates and validation results.
  • Links between an output and the business action it triggered.

Protect sensitive data, restrict log access and set retention periods that fit your legal and operational requirements. Telemetry that cannot be connected to an outcome will not tell you whether a change reduced harm.

Define an incident path

  1. Contain. Pause the affected automation, revoke a tool permission or route new cases to manual handling.
  2. Assess. Identify the affected time window, users, records and error type. Preserve the relevant logs and source documents.
  3. Correct. Notify affected stakeholders when appropriate, fix records or communications and address any immediate customer or safety impact.
  4. Investigate. Determine whether the cause was a model change, prompt change, stale source, retrieval failure, tool integration, data quality issue or reviewer breakdown.
  5. Recover and verify. Roll back to a known version or safer workflow, run the regression set, then release only after the control is shown to work.

Release in phases

Start with a limited audience, low-consequence actions or shadow mode in which the AI result is measured but does not drive the decision. Expand only when production evidence supports the change. Keep a rollback path for the model, prompt, retrieval index, output schema and connected tools; changing any one of these can alter error behavior.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 6: Reevaluate whenever the system changes

Repeat the evaluation after changing the model, prompt, retrieval sources, output format, connected tools or workflow context. Microsoft recommends experimenting across model versions and measuring harms and mitigations iteratively. A prompt that performed well against last quarter’s documents may fail when source structure, policy language or user behavior changes.

Use a change record that names the modification, affected workflows, expected benefit, new risks, test-set results, reviewer sign-off and deployment date. Compare both helpful outcomes and harmful outcomes. If a change improves average quality but increases a severe failure mode, do not ship it without an explicit risk decision.

How to choose between implementation approaches

The available guidance does not establish a winning vendor or product. Compare controls and platforms against the work they must perform in your environment.

Decision axis Questions to ask
Task fit Can it measure the output types and failure modes that matter for this workflow?
Evidence quality Can reviewers trace claims to supplied business sources and see whether those sources support the answer?
Review ergonomics Can people quickly flag, correct, reject or escalate an answer before downstream use?
Operational coverage Does it support test sets, metrics, feedback, telemetry, incident handling and rollback?
Risk and reversibility What level of human approval is appropriate before a result triggers a consequential or hard-to-reverse action?

The last axis is a risk-based design inference: NIST warns about consequential contexts, while Microsoft emphasizes incident handling and rollback. Treat it as a decision question for your organization, not as a feature comparison established by either source.

Common approaches that fail

“We added one perfect prompt”

Prompts can clarify scope and format, but they cannot remove uncertainty in source data, changing context, retrieval failures or model variation. Keep prompt improvements, then verify them with the evaluation set and production monitoring.

“The citation proves the answer”

A citation can point to an irrelevant passage, misrepresent what the document says or be invented. Require a reviewer or an automated support check to inspect the underlying text for each consequential claim.

“A confidence score is an approval”

A model’s confidence-like signal is not evidence that a statement is true. Use observable checks—source support, range validation, policy rules and human review—matched to the harm.

“A warning banner solves review”

Warnings help only if users notice them and have time, authority and evidence to act. Test warning placement and wording, and measure whether reviewers actually catch more critical errors.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“We can automate first and add controls later”

Automatic actions can propagate an error before anyone sees it. Begin with drafts, shadow mode or reversible actions, and increase autonomy only after the workflow demonstrates acceptable performance on its own data.

A practical operating checklist

  • Map the AI step, inputs, outputs, downstream actions and affected people.
  • Rank failure modes by likelihood, severity and reversibility.
  • Build a representative, labeled evaluation set with known incidents.
  • Choose task-specific metrics and thresholds from your own data.
  • Ground responses in approved sources and require checkable evidence where useful.
  • Use structured outputs, validation rules and predetermined responses for bounded cases.
  • Require meaningful review before consequential or hard-to-reverse actions.
  • Show evidence, uncertainty and edit or reject controls in the user interface.
  • Log versions, sources, outputs, decisions and downstream outcomes.
  • Run phased releases, incident response and tested rollback.
  • Reevaluate after model, prompt, source, format, tool or context changes.

How the NIST and Microsoft guidance fits together

NIST’s AI RMF and Playbook are voluntary resources; the Playbook says organizations may adopt suggestions to the extent they apply to their use case. NIST’s Generative AI Profile supplies the risk language and emphasizes why confident false content and consequential contexts deserve attention. Microsoft’s Azure OpenAI material is vendor-specific guidance, not a general legal or industry requirement. It provides practical examples for harm assessment, metrics, testing, review, telemetry, incident response and rollback.

NIST reports that more than 240 organizations contributed to development of the AI RMF. That figure describes framework-development participation, not evidence that the framework improves accuracy or that 240 businesses use it. The useful outcome for a workflow owner is not adopting a label; it is creating an auditable cycle in which errors are found, corrected, measured and prevented from recurring.
