The reliable fix is a grounded answer pipeline, not a better-sounding prompt. Retrieve current reporting and primary documents, preserve each source’s title, publisher, date and URL, rerank and trim the passages, test whether they contain enough evidence, generate only supported claims, cite those claims individually, and abstain or ask for clarification when a key fact is missing or contradictory. For consequential stories, an editor must verify the output against the original source.
What is going wrong: hallucination versus missing context
These failures look similar to a reader because both can produce a confident but misleading news answer. They require different repairs.
Hallucinations are unsupported or false claims
OpenAI defines hallucinations as incorrect or misleading outputs that can include fabricated quotes, studies, citations or references to non-existent sources, as well as overconfident answers to ambiguous questions. A fluent sentence is therefore not evidence. Every date, number, quote, identity and causal claim in a news answer needs a source check.
OpenAI’s 2025 explanation describes hallucinations as plausible but false statements and argues that many evaluation methods reward guessing instead of acknowledging uncertainty. Its SimpleQA results illustrate the danger of measuring answer rate without measuring restraint:

| o4-mini | 1% | 24% | 75% |
The figures are from OpenAI’s 2025 publication and describe that evaluation, not a guarantee for every news workflow. The contrast is useful: answering more often can make a system less trustworthy if it guesses when evidence is weak.
Missing context means the evidence cannot establish the answer
Google Research defines context as sufficient when it contains all information necessary for a definitive answer. Context is insufficient when it is incomplete, inconclusive, contradictory or missing a necessary fact. A retrieved article can be highly relevant to a topic yet fail to establish what the reader asked.
For example, a breaking-news story may confirm that an arrest occurred but not identify the charge, jurisdiction or time. A report may quote a company announcement without including the regulator’s filing that explains the legal status. A headline may say a policy was “approved” while the underlying document only says a proposal was released. These are context gaps, not invitations for the model to fill in details from memory.

Use the following sequence for every current-event answer. The order matters: generation comes only after evidence has been collected and tested.
1. Classify the question and its freshness requirement
First decide whether the reader wants a current event, a background explanation, or both. A question such as “What happened today?” needs a short freshness window and sources updated near the event. “How does this law work?” may need the enacted text and an authoritative explainer rather than the latest article alone.
- For breaking news, define the acceptable evidence age before searching, such as the previous few hours or the current day.
- For developing stories, record both the publication or update time and the time the event allegedly happened.
- For evergreen background, prefer the latest primary document while retaining earlier dates when they explain the timeline.
- Mark geography, jurisdiction, edition and time zone so a local report is not generalized to an entire country.
Store the user’s exact wording and any assumptions it contains. If the prompt says “the new rule,” the system should identify which rule, agency and jurisdiction are meant before it summarizes anything.
2. Retrieve broadly, then rerank and trim
Search several candidate passages rather than trusting the first result. Include the original reporting outlet, official statements, filings, court documents, regulator pages and other primary records appropriate to the story. OpenAI describes retrieval-augmented generation (RAG) as retrieving content to augment the prompt before generation.

Retrieval quality has two opposing failure modes. Too little context leaves a necessary fact out; too much irrelevant text drowns the signal and can itself cause hallucinations. OpenAI advises tuning search, reducing noise and including enough information in each result. In practice:
- Retrieve multiple candidates using the event, people, organization, location and date—not just a copied headline.
- Rerank passages for direct evidence of the requested claim, source authority, recency and geographic relevance.
- Remove duplicated wire copy, commentary that adds no evidence and passages about similarly named events.
- Keep the smallest set of passages that jointly covers who, what, when, where and why.
- Preserve the surrounding sentences needed to interpret a quote, number, denial or qualification.
A result should carry its title, publisher, author when available, publication and update times, event date, geography and exact URL. Those fields let the generator distinguish “reported on Tuesday” from “happened on Monday.”
3. Verify source identity and resolve dates
Before generation, confirm that every passage belongs to the stated publisher and concerns the intended event. Check whether a page is an original report, a repost, an opinion column, a press release or a later correction. Keep the original URL even when several outlets repeat the same wording.
Record at least two dates when they differ:
- Event date: when the action, incident or decision occurred.
- Publication or update date: when the source reported or changed its account.
When sources conflict, prefer the closest primary source—such as a court document, filing, regulator notice or direct statement—and describe the remaining disagreement. Do not silently merge incompatible accounts or choose the newest article merely because it is newest.
4. Apply a context-sufficiency gate
Ask a separate check before drafting: “Does the retrieved evidence contain every fact needed to answer this exact question?” The answer should be yes only when the passages establish the requested names, dates, quantities, status and relationships. Google’s work on sufficient context says a system should answer when the context is sufficient and say “I don’t know” when key information is lacking.
Use one of three outcomes:
- Sufficient: proceed, but restrict claims to what the passages entail.
- Partially sufficient: give a bounded answer, identify the missing fact and ask a focused clarification or explain what remains unconfirmed.
- Contradictory: report the conflict, identify which source says what and avoid a single definitive conclusion until it is resolved.
The gate should fail for a missing number, quote, identity, date, causal link or legal status. A model must not infer any of these from writing style, prior knowledge or a likely pattern.
5. Generate under evidence constraints
Give the model explicit instructions: use only the supplied passages; preserve uncertainty; do not add unsupported causal language; do not invent quotations; and attach a citation to each material claim. Require it to label allegations, estimates, preliminary findings and confirmed facts differently.

A useful internal representation is a claim ledger:
| Claim field | What to record |
|---|---|
| Claim text | The smallest factual statement that can be checked. |
| Evidence span | The exact passage that entails it. |
| Source identity | Publisher, title, author if available and URL. |
| Timing | Event date plus publication or update time. |
| Status | Confirmed, attributed, disputed, estimated or unknown. |
| Scope | Geography, jurisdiction, affected group and applicable edition. |
Generate from the ledger rather than from a loose collection of links. If no evidence span exists for a sentence, delete the sentence or mark the fact as unknown.
6. Cite at claim level
Place a citation immediately after the statement it supports, not once at the end of a paragraph containing several unrelated facts. A source that supports the date may not support the quoted motive or the claimed consequence. Split compound sentences when necessary so each part has an appropriate citation.
- Cite the original document for a quote, vote, filing, ruling or numerical result.
- Cite the reporting outlet for what it independently reported, while making clear that the statement is attributed.
- Do not cite a search-result snippet as if it were the underlying article.
- Keep the source title, publisher, date and URL in the system record even if the reader-facing design shows a shorter citation.
7. Verify every factual claim after generation
OpenAI’s GPT-5 evaluation uses a two-step process: extract relevant factual claims, then browser-fact-check each as true, false or unsure. Adapt that procedure for publication:
- Break the draft into atomic claims, including numbers, dates, names, quotes, comparisons and causal statements.
- Open the cited passage and mark whether it entails the claim, contradicts it or does not address it.
- Check that the quote is exact, that a number has the right unit and that a date refers to the event rather than the article.
- Compare disputed claims with the closest primary source.
- Rewrite, attribute or remove any claim rated false or unsure.
Do not treat a citation’s presence as proof of grounding. The cited text must actually support the wording used.

8. Abstain selectively and explain what is missing
Google’s publication on selective generation reports that models often answer incorrectly instead of abstaining when context is insufficient. Its method improved the fraction of correct answers among responses by 2–10% for Gemini, GPT and Gemma. The practical lesson is to make abstention a quality behavior, not a failure metric.
A useful abstention has three parts: state the limited fact that is established, name the missing or conflicting evidence, and give the next verification step. For example: “The agency announced a proposal on Tuesday, but the retrieved material does not establish whether it took effect. The final rule or an implementation notice is needed.” Do not write a broad refusal when a narrow, supported answer is possible.

9. Put an editor in the loop for consequential stories
The U.S. Department of Energy’s Generative AI Reference Guide states: “Have a human in the loop to verify the accuracy and validity of outputs.” Require that review for politics, public safety, health, finance, legal disputes, breaking events and any answer containing a quote or consequential allegation.
The editor should open the original source, verify the claim ledger, inspect unresolved conflicts and confirm that the freshness window still holds at publication time. A human review is not a substitute for retrieval and grounding; it is the final control for claims whose cost of error is high.
How to handle incomplete or conflicting news
When a key fact is absent
Do not reconstruct a missing fact from older coverage or from the model’s general knowledge. State exactly what the available evidence establishes and what it does not. If the reader’s wording is ambiguous, ask for the person, location, date or publication they mean. If clarification is not possible, answer the narrowest defensible interpretation and label it.
When reports disagree
Keep each account separate. Identify the source, timestamp and claim, then explain whether a primary record resolves the dispute. If not, use language such as “Source A reported X; Source B reported Y; the available documents do not settle the discrepancy.” Avoid averaging numbers or presenting a blended narrative that no source actually supports.
When a story is still developing
Use time-bounded wording: “As of [the recorded update time], the confirmed information is…” In production data, store the time; in reader-facing copy, include the date and time zone when freshness could change the answer. Schedule a recheck rather than implying that an early account is final.
A practical fact-checking procedure for an AI news summary
- Freeze the draft. Keep the exact model output and prompt so edits can be audited.
- Extract claims. Include every factual clause, not only the headline conclusion.
- Classify each claim. Mark it as event, identity, number, quote, chronology, attribution, cause, prediction or legal status.
- Match evidence. Link each claim to the smallest supporting passage and its source metadata.
- Check entailment. Ask whether a reasonable reader could derive the claim from that passage without extra assumptions.
- Check completeness. Confirm that who, what, when, where and why—when requested—are covered or explicitly marked unknown.
- Check freshness. Compare the source age with the question’s freshness window and the event date.
- Resolve conflicts. Prefer primary records and preserve unresolved disagreement.
- Rewrite cautiously. Attribute allegations, qualify estimates and remove unsupported causal language.
- Escalate. Send high-impact or uncertain items to an editor before publication.
Evaluation: measure grounding, not just answer accuracy
Maintain a fixed test set of real reader questions. Include time-sensitive examples, ambiguous prompts, false premises, incomplete context, conflicting reports and citation traps. Re-run it when retrieval, ranking, prompts or models change.

These references support measuring whether an answer should have been attempted separately from whether the attempted answer was factually grounded.

Choosing between two implementations
Whether you build a simple newsroom tool or a larger retrieval service, compare the systems on the same axes rather than on a single accuracy score.
| Comparison axis | What a stronger implementation does | Warning sign |
|---|---|---|
| Freshness window | Enforces an event-specific age limit and records time zones. | Uses the newest page without checking when the event occurred. |
| Primary-source retrieval | Finds filings, court records, regulator pages and direct statements alongside reporting. | Relies on summaries that cite no original document. |
| Recall and noise | Retrieves enough candidates, then removes duplicates and irrelevant passages. | Either misses the decisive source or floods the prompt with near-duplicates. |
| Reranking | Scores direct entailment, authority, recency and geography. | Ranks by keyword overlap alone. |
| Citation entailment | Checks every material claim against an exact evidence span. | Adds one general link to a paragraph of unrelated claims. |
| Answer completeness | Tests the requested who, what, when, where and why. | Produces a polished summary that omits the question’s decisive detail. |
| Abstention calibration | Declines when evidence is missing and answers when it is sufficient. | Either guesses routinely or refuses nearly everything. |
| Conflicting sources | Preserves disagreement and favors the closest primary record. | Blends incompatible reports into one unsupported account. |
| Latency and cost | Balances retrieval depth, verification and response time for the use case. | Cuts verification steps that protect high-impact stories. |
| Human correction workload | Tracks review time, corrections and escalations. | Claims success from model scores while editors repeatedly repair the same errors. |
A RAG system with a reranker can be evaluated on retrieval metrics and answer grounding separately. Google Research has linked its Vertex AI RAG Engine and LLM Re-Ranker work to improved retrieval metrics and RAG accuracy; any deployment should still validate those claims on its own news corpus and editorial test set.
Common failure modes and their fixes
“The answer has citations, so it must be reliable.”
Cause: citation presence is being scored instead of citation entailment.

Implementation checklist
- Define whether the prompt is current-event, background or mixed.
- Set a freshness window and record the event date, publication time and update time.
- Retrieve multiple sources, including the closest appropriate primary document.
- Store publisher, title, author when available, geography and exact URL.
- Rerank and trim passages so the prompt contains signal without duplication.
- Run a sufficient-context check before generation.
- Generate only claims supported by the supplied passages.
- Attach citations at claim level and preserve uncertainty.
- Extract and fact-check every material claim after generation.
- Abstain or ask for clarification when evidence is missing or contradictory.
- Require human verification for high-impact topics, quotes and consequential allegations.
- Track retrieval, grounding, completeness, abstention, freshness and editorial-cost metrics on a fixed test set.
Bottom line
News-answer reliability comes from controlling the evidence path: retrieve current and primary material, test whether it is sufficient, constrain generation to what it proves, cite each claim, verify the finished draft and make uncertainty visible. A system that occasionally says “I don’t know” for a clearly stated reason is safer—and more useful—than one that answers every question with unsupported confidence.
