To fix an outdated or incorrect Copilot Notebook answer, inspect the citation first, then correct the notebook’s References list. Remove superseded files or webpages, add the latest authoritative source that your account can access, and rerun a narrowly worded prompt that names the intended file and asks for its publication date and exact supporting section. Open the new citation and compare it with the answer before relying on it. There is no documented universal “refresh all answers” command: reference editing and rerunning are the supported correction workflow, while a refresh action documented for OneNote’s Overview Page applies only to that page.
Why Copilot Notebook cites an old or wrong source
A Notebook answer is grounded in the sites, folders, and files added to that notebook and available under your permissions. Copilot does not automatically search every document in your organization merely because the document exists. If the current policy, report, or specification is not in the notebook’s references, it is not a dependable grounding source for the answer.
An earlier file or an old webpage can remain in the reference set beside the current version. Microsoft explicitly warns that Copilot can give outdated answers when it is pointed to an old website or an earlier version of a file. Similar filenames, copied documents, archived pages, and duplicate references make it easy for the model to select the wrong item. A citation proves which source Copilot used for that response; it does not prove that the source is current or that the generated conclusion is correct.

The correction procedure
1. Open the citation before changing anything
Select the citation attached to the suspect sentence. Read the source title, publication date or version, and the exact passage Copilot used. Check whether that passage actually supports the claim, rather than assuming that a plausible-looking citation is evidence.
Microsoft’s guidance recommends reviewing citations and confirming critical details. Save the source’s URL or file location while you investigate; you will use it to distinguish the authoritative version from look-alikes. If the citation opens an archived page, an obsolete file, or a passage that says something different from the answer, you have identified a grounding problem or a generation error.
2. Inspect the notebook’s References list
Open the notebook’s References area and inventory every site, folder, and file included there. Confirm that the source you want Copilot to use is actually listed. A current document stored elsewhere in SharePoint, OneDrive, or your organization’s website is not automatically consulted.

3. Remove superseded and duplicate material
Delete or remove an earlier file, obsolete webpage, or duplicate copy when it can compete with the current source. Do not leave an archived policy beside the active policy unless the distinction is intentional and you will name it explicitly in your prompt. Keep a historical document only when the question genuinely concerns that historical period; otherwise it increases ambiguity.
Use the source’s own version number, effective date, or “last updated” field rather than a filename alone. For webpages, replace an old URL with the current canonical page. For files, add the authoritative version from the system of record and remove downloaded or emailed copies that may have stopped changing.

4. Add the latest authoritative source
Add the current file, folder, or public webpage to References. Verify that you can open it with the same account and that the content is readable. In a work or school account, permissions still apply: being able to see a link in a message does not guarantee that Copilot can use the underlying file.
If the current information is behind a sign-in wall, paywall, or unsupported linked subpage, Copilot may not be able to ground on it through the documented web-reference path. Use an accessible, approved copy or add the supported OneDrive or SharePoint location when your organization permits it. Do not paste confidential material into a public webpage merely to make it accessible.
5. Make the diagnostic prompt unambiguous
Rerun a small question that identifies the intended source and requests evidence. A useful pattern is:
Using only the file “<exact file name and version>,” answer <question>. Confirm the publication or effective date, and show the exact section supporting each key claim. If the file does not answer the question, say so.
If you have two similarly named documents, include their full names, dates, or locations and state which one is authoritative. Ask for one issue at a time instead of combining several topics; a narrow prompt makes a wrong reference easier to spot.
Microsoft specifically suggests asking Copilot to confirm the publish date and identify the exact section it used. This is a diagnostic request, not a guarantee that the generated answer will be correct.

6. Inspect the new citation and compare text
After Copilot responds, open every citation that supports a material claim. Compare the cited wording with the sentence in the answer, including qualifiers, dates, exceptions, and units. If the answer says a rule applies to all users but the source limits it to a region or plan, correct the interpretation yourself before sharing it.
When the source contains no answer, treat a confident general response as ungrounded. Microsoft notes that Copilot may provide a more general answer when it cannot find relevant accessible sources. Ask it to identify the missing evidence instead of filling the gap from general knowledge.
7. Submit feedback if the corrected grounding still fails
If the current reference is accessible, the old item is removed, and a narrow prompt still produces an incorrect result, use Settings and more > Feedback in the Copilot experience. Include the prompt, the incorrect citation, the source version you expected, and any relevant account or surface details that your organization allows you to share. Feedback does not replace verification, but it gives Microsoft a reproducible example.

Do not assume that two people asking the same question have identical grounding behavior. Microsoft documents different source access and reference limits for Copilot Chat and Microsoft Copilot. The following comparison describes the documented distinctions; exact controls can change as features roll out.

The reference counts are product limits, not accuracy scores. A notebook with fewer sources can still produce a wrong answer if the one source is stale or ambiguous. Conversely, adding more documents can make source selection harder and can push a needed reference outside the set considered for grounding.
How to audit a notebook with many references
Group sources by authority
Separate current policies, specifications, procedures, and background material. If a single question should be answered by one policy, keep that policy prominent and remove unrelated versions. A concise, authoritative reference set is easier to verify than a complete dump of every file on a topic.

Record dates and status
For each important item, note its publication date, effective date, version, owner, and whether it is current, superseded, or historical. Copilot can read these labels, but you still need to check that the labels are accurate and visible in the source. A filename such as “final” is not a reliable version marker.

Rename or organize files so that dates and version numbers are unambiguous where your system permits it. In your prompt, use the exact filename and version. If two documents must remain together for historical comparison, state the time period you want and ask Copilot to cite the relevant version.
Reduce the set when the answer matters
For a high-consequence answer, temporarily remove unrelated references or create a focused notebook containing the current sources. This is especially useful when the notebook approaches the applicable grounding cap. Microsoft’s documentation explains the cap behavior in its Copilot Notebook answers and insights guidance.

Copilot cites an old file even though the new file is present
Ask: Use only “<new file name, version, date>.” What is the answer to <question>? State the file’s publication date and quote the exact section. Do not use “<old file name>.” Then check that the old file has actually been removed from References. Naming an excluded file in the prompt does not override a reference list that still contains it, so the list must be corrected first.
Copilot gives a general answer instead of using the source
Ask it to list the accessible references relevant to the question and explain which one supports each claim. If it cannot identify a supporting passage, inspect the source type and permissions. A public web link may still fail if the content is an unsupported linked subpage, while a OneDrive or SharePoint file may fail for an account that lacks permission.
Two versions disagree
Ask Copilot to produce a side-by-side difference list with each statement tied to a version and date. Decide which version governs the reader’s situation outside Copilot, then rerun the final question using only that source. Do not let the model silently merge conflicting rules.

The citation opens, but the answer overstates it
Rerun the prompt requesting the exact quotation, surrounding section heading, date, scope, and exceptions. Compare those details manually. A citation can be genuine while the generated wording drops a condition or applies a limited rule too broadly.
Is there a refresh button?
Microsoft documents a refresh action for the OneNote Overview Page after references are added or removed. That instruction is specific to the Overview Page. It should not be described as a universal command that re-indexes every Notebook answer or guarantees a correction.
For ordinary Notebook answers, the reliable sequence is to edit References, remove the stale item, add the current source, and rerun the prompt. If a response still cites the old item, verify that you are in the same notebook and account, reopen the References list, and check whether the old URL or file remains through a folder-level reference.

Microsoft’s Copilot Notebook getting-started guidance and Notebook frequently asked questions describe feature availability and account-dependent behavior. If a control shown in those instructions is absent, check the product surface, license, and rollout status before assuming your prompt is wrong.
Verification rules for important answers
- Check the source, not only the citation label. Read the cited passage and confirm that it supports the claim.
- Check recency. Compare publication, effective, and revision dates with the date relevant to your decision.
- Check scope. Verify geography, audience, product edition, plan, and time period.
- Check omissions. Look for exceptions, definitions, footnotes, and conditions that the generated sentence may leave out.
- Check access. Confirm that the source is available to the account and supported by the current Copilot surface.
- Check conflicts. If another authoritative reference disagrees, stop and resolve the conflict rather than averaging the statements.
Grounding improves relevance, but it does not make answers infallible. Microsoft advises reviewing sources and confirming critical details. Treat a citation as a lead for verification, not as independent proof.
A repeatable checklist
- Open the citation attached to the questionable claim.
- Read the cited title, date or version, and supporting passage.
- Open the notebook’s References list.
- Confirm the intended current source is included and accessible.
- Remove obsolete, duplicate, or irrelevant references.
- Add the latest authoritative file, folder, or supported public webpage.
- Check whether the notebook is near the applicable reference limit.
- Run a narrow prompt naming the exact source and requesting its date and section.
- Open the new citation and compare its text with every important claim.
- Verify scope, exceptions, and permissions independently before acting.
- Use Settings and more > Feedback if the corrected setup still produces a wrong citation or answer.
This process answers the practical questions “Why is Copilot citing an old file?” and “How do I make Copilot use the latest source?” by correcting the inputs Copilot is allowed to use, making the requested source explicit, and requiring evidence that can be checked.
