When a new math topic feels impossibly difficult, the obstacle is often a specific prerequisite concept or procedure that is not yet secure—not an inability to learn the whole subject. Find the exact upcoming idea, identify what it depends on, collect a few pieces of reasoning-rich evidence, and teach only the gap that blocks access. Then check the prerequisite again while the learner returns to grade-level work.
This approach replaces broad “catch-up” tests with a short cycle: map prerequisites, elicit and interpret evidence, provide just-in-time support, and adjust from the next check.
What a math learning gap is—and why it can make a new topic hard
A learning gap is a prerequisite idea, representation, language skill, or procedure that the learner needs for the next topic but cannot yet use reliably. The gap may be narrow. A student beginning addition of fractions with unlike denominators may understand fractions generally but still be unable to generate equivalent fractions or compare fraction magnitude. A student starting linear equations may know multiplication facts but not preserve equality when performing the same operation on both sides.
New work often exposes a gap because topics are connected. If the prerequisite is fragile, the learner may spend working memory reconstructing an earlier idea while trying to understand the new one. The visible symptom—an incorrect answer, slow work, or avoidance—does not by itself identify the cause.

Do not treat a gap as a label for the learner. It is a description of evidence at a particular point in a particular learning sequence. The New Zealand Ministry of Education’s teacher guidance advises reading teaching sequences horizontally to find prior learning that is foundational to the current year, then building diagnostic and formative assessment around those prerequisites before a new unit. Its practical instruction is: “Build your diagnostic/formative assessment around these prerequisite concepts and procedures before you start teaching your new unit.”
There is no universal test or cross-grade mastery cutoff that works for every curriculum. Use the standards, textbook sequence, or course progression the learner is actually about to enter.
The five-part cycle for locating a gap
- Start with the next topic. Name the exact concept or procedure and map the knowledge it requires.
- Ask a small number of targeted questions. Include a task, a representation, and an explanation—not only a final answer.
- Read the work for patterns. Compare correct, partly correct, and incorrect responses and ask clarifying questions.
- Respond to the smallest blocking gap. Pre-teach, run a short focused group, or scaffold the prerequisite in the moment.
- Check again and adjust. Give a parallel task or explanation opportunity, then continue gathering evidence during the new learning.
Iowa Department of Education guidance describes formative assessment as a cycle of setting goals, eliciting evidence, interpreting it, identifying a gap, giving feedback, and adjusting instruction. The cycle matters more than any single quiz: evidence should change what happens next.

Step 1: Map the prerequisites for the exact topic
Name the upcoming learning precisely
“Fractions” or “algebra” is too broad for diagnosis. Write the next teachable outcome in observable terms, such as “add fractions with unlike denominators and explain why the common denominator works” or “solve a one-step equation and justify each equality-preserving operation.” The narrower the outcome, the easier it is to identify what must already be in place.
Work backward one or two steps
List the concepts, procedures, representations, and language the learner must coordinate. For fraction addition, the list might include unit fractions, equivalent fractions, common multiples, and the meaning of the numerator and denominator. For area of a triangle, it might include multiplication, measuring length, the rectangle area formula, and understanding that a triangle can be related to a rectangle by decomposition.
Use the immediately preceding step in your local learning progression as a starting point. New Zealand guidance recommends looking across teaching sequences to identify prior-year learning that is foundational to current-year work. A curriculum from another country may order ideas differently, so adapt the map rather than importing it unchanged.
Separate concepts from procedures
A learner may perform a rule without understanding it, or explain an idea without executing the written procedure fluently. Record both. For adding unlike fractions, “multiply denominators” is a procedure; knowing that the two fractions must be renamed in equal-sized parts is conceptual understanding. A diagnostic that checks only answers can miss this difference.

Choose the minimum prerequisite set
Do not test every earlier skill. Select the few prerequisites that directly unlock the next lesson. New Jersey’s accelerated-learning guidance recommends identifying prerequisites that bolster current grade-level content instead of automatically reteaching an entire prior course or grade. This keeps intervention connected to the learner’s destination.
Step 2: Elicit evidence with short, targeted tasks
Use a handful of prompts that can be completed quickly but reveal thinking. The task should resemble the upcoming mathematics without turning into a full unit test. Ask the learner to solve, represent, and explain.
| Evidence source | What it can reveal | Useful follow-up |
|---|---|---|
| One carefully chosen problem | Whether the prerequisite procedure is available in a familiar format | “Show the step you decided to do first.” |
| A visual model or number line | Whether the learner can connect quantities, symbols, and magnitude | “Place your answer and explain its position.” |
| A worked example with an error | Whether the learner can notice and explain a misconception | “Which line would you change, and why?” |
| An oral explanation | Vocabulary, relationships, and reasoning that written work may hide | “How do you know this method will always work?” |
| A transfer item with different numbers | Whether the idea is general or tied to memorized steps | “Would your method still work here?” |
Include an opportunity to explain or represent thinking. A correct answer reached by a fragile method is different evidence from a wrong answer accompanied by sound conceptual reasoning. Record the strategy, not just the score.
Keep prompts neutral
Ask, “What were you thinking at this step?” rather than “Why did you make that mistake?” Neutral language encourages the learner to reveal a method instead of hiding it. If a response is ambiguous, ask for another example or a representation before inferring a cause.

When choosing between a worksheet, interview, exit ticket, or digital item, judge each option on four questions:
- Which prerequisite does it reveal?
- Does it show reasoning as well as the final answer?
- How closely does it match the upcoming topic and local curriculum?
- Will the result point to a clear next instructional action?
A longer assessment is not automatically more diagnostic. A short, well-aligned prompt can provide better information than many unrelated questions.
Step 3: Analyze the work for patterns, not isolated mistakes
South Africa’s Department of Basic Education defines error analysis in its Grade 6 Mathematics Diagnostic Assessment as examining response errors for possible explanations. Repeated responses can indicate relative skill proficiency or a misconception, but one unexplained wrong answer is not a definitive diagnosis.

Sort the evidence
Look at correct, partly correct, and incorrect responses together. Mark where the reasoning changes: choosing an operation, setting up a representation, carrying out a calculation, or interpreting the result. Note whether the same pattern appears with different numbers or formats.
Look for high-value patterns
- Consistent procedural slip: The learner understands the model but repeatedly misaligns digits or drops a negative sign. This calls for a focused procedure lesson and guided practice.
- Misapplied rule: The learner adds numerators and denominators when adding fractions. Ask what the denominator represents and use equal-sized visual parts.
- Representation mismatch: A number line shows the right quantity, but the symbolic answer does not match it. Connect the model to notation before adding more computation.
- Language barrier: The mathematics is available in symbols, but words such as “difference,” “per,” or “at least” are misunderstood. Clarify mathematical language and ask the learner to restate the condition.
- Strategy selection problem: The learner can calculate but chooses an operation unrelated to the situation. Use a word problem and require a sentence explaining why the operation fits.
- Fluency constraint: A sound strategy becomes inaccurate or extremely slow under routine calculation. Separate conceptual teaching from brief, supported fluency practice.
Ask for a second look
Have the learner solve a parallel item, draw a model, or explain a peer’s worked example. If the pattern disappears, the original response may have reflected attention, notation, or context rather than a stable misconception. If it persists, you have stronger evidence for a targeted response.
Step 4: Choose a response that is small, explicit, and connected
Pre-teach a prerequisite before the lesson
Use a short session when the prerequisite is absent but the learner can engage with the upcoming topic after a refresh. State the goal, model one example, solve one together, and give a closely related independent check. Then return to the grade-level task rather than extending the review indefinitely.
Use a focused small group
Group learners who show the same prerequisite need, not everyone who received a low score. Use several brief lessons with cumulative review and frequent opportunities to explain. Regroup when new evidence shows that needs have diverged.
Scaffold in the moment
If the gap is narrow, embed a prompt, visual, partially completed example, or reference representation in the grade-level lesson. Fade the scaffold as the learner demonstrates independent use. The goal is access to current mathematics, not permanent simplification.

Pick the symptom - the matching free tool is one click away.
Make the next step explicit
Translate evidence into an action: “Tomorrow, model equivalent fractions with a strip and number line before adding,” or “During equation practice, require an equality statement after each inverse operation.” Avoid a vague plan to “review fractions.”
Step 5: Check again and adjust while new learning continues
After the focused response, use a new but comparable task. Ask for the same kind of reasoning in a different context so you can see whether the prerequisite transfers. A learner who succeeds only on the practiced numbers may need more varied examples or a clearer connection to the new topic.

Continue to collect evidence during instruction: listen to partner explanations, inspect one worked line, use an exit prompt, or ask for a quick representation. Iowa’s formative-assessment cycle treats this ongoing interpretation and adjustment as part of teaching, not as a separate testing event.
The 2009 What Works Clearinghouse guide on Response to Intervention for elementary and middle schools recommends screening students at risk and monitoring progress. Its evidence ratings are not uniform: screening is labeled moderate evidence, while progress monitoring is labeled minimal evidence. Use monitoring as a practical feedback routine, not as proof of a universal measurement formula.
Evidence-informed supports for elementary intervention
The What Works Clearinghouse practice guide Assisting Students Struggling with Mathematics: Intervention in the Elementary Grades, released in March 2021, is specifically about intervention in grades K–6. It presents six recommendations:
- Provide systematic instruction. Sequence examples deliberately, model thinking, guide practice, and revisit ideas cumulatively.
- Use clear mathematical language. Define terms, connect everyday and mathematical meanings, and expect learners to explain relationships.
- Use concrete and semi-concrete representations. Move from objects to drawings, diagrams, and symbols while making the connections explicit.
- Use number lines. Number lines can show magnitude, equivalence, operations, and the relative size of answers.
- Teach word-problem solving deliberately. Represent the situation, identify known and unknown quantities, choose an operation, and justify the result.
- Use timed activities to build fluency. Keep fluency practice brief, accurate, and supported by already understood concepts.
These are intervention recommendations, not a claim that every learner needs every support or that a recommendation applies identically to secondary, adult, or every mathematical topic. Match the support to the diagnosed gap and preserve conceptual reasoning while building efficiency.

Map the access points
Suppose the next lesson asks students to add 1/3 + 1/4. Relevant prerequisites include understanding the denominator as the size of equal parts, finding or recognizing a common multiple, generating equivalent fractions, and interpreting the sum’s magnitude.
Elicit evidence
Ask the learner to place 1/3 and 1/4 on a number line, rename each with a denominator of 12, and explain why the renamed fractions have the same values. Then ask for the sum using a visual or symbols.
Free tools Windows power users keep installed
One-click scans. No signup required.
Interpret patterns
If the learner can rename fractions but places them incorrectly, the gap may be magnitude or number-line understanding. If the learner says 1/3 = 1/4 because both have numerator 1, the issue is the meaning of denominator. If the model is accurate but the final addition is not, focus on connecting the model to the symbolic procedure.

Respond and recheck
Use fraction strips or a number line for the specific conceptual need, solve a parallel pair such as 1/5 + 1/2, and ask the learner to explain the common unit. Once that prerequisite is secure, return to the grade-level set rather than reteaching every earlier fraction operation.
Worked example: solving a one-step equation
Map the access points
For an equation such as x + 7 = 12, identify understanding of equality as a relationship, inverse operations, integer or whole-number computation as appropriate, and the habit of checking a solution by substitution.
Elicit evidence
Ask the learner to balance a model, solve the equation, and explain why subtracting 7 from one side requires subtracting 7 from the other. Include an equation with the unknown on the other side to see whether the method is general.
Interpret patterns
A learner who writes x = 12 − 7 but cannot explain preserving equality may have a procedural shortcut without the underlying concept. A learner who balances a model correctly but makes an arithmetic error needs computation support, not a reteach of equality.
Respond and recheck
Use a balance representation, narrate the same operation on both sides, and ask for a substitution check on a new equation. Fade the model when the learner can justify the symbolic steps.
How families can use the same method at home
Families do not need to recreate a school test. Ask the learner what the next topic is, then choose one short task that uses its likely prerequisite. Invite a drawing, explanation, or estimation before correcting the answer. Listen for the strategy and ask, “What made you choose that operation?”
If a pattern appears, practice only that idea for a few minutes with varied examples. Use household objects, paper folds, a number line, or a written model when they clarify the concept. Share the observed strategy with the teacher so school and home are responding to the same evidence. Avoid turning every homework session into a full-grade review.

- Giving a broad prior-grade test: Replace it with a prerequisite check tied to the next unit.
- Using the score as the diagnosis: Inspect steps, representations, and explanations.
- Assuming one wrong answer proves a misconception: Ask a clarifying question or collect a parallel example.
- Reteaching everything that came before: Select the smallest gap that blocks current access.
- Separating intervention from grade-level work for too long: Pre-teach or scaffold, then reconnect the learner to the current lesson.
- Keeping the same support after evidence changes: Recheck and fade, intensify, or replace the scaffold.
- Applying elementary guidance unchanged to older learners: Note that the 2021 intervention guide targets grades K–6 and adapt methods to the learner’s course and age.
A practical record for each learner
A one-page record can keep the cycle actionable:
- Upcoming outcome: the exact concept or procedure.
- Prerequisite: the specific idea, representation, language, or procedure to verify.
- Evidence: the task, learner work, explanation, and recurring pattern.
- Instructional response: pre-teach, small group, scaffold, or fluency practice, with the representation and language to use.
- Recheck: the parallel task and the date or lesson when it will be used.
- Decision: fade support, continue, intensify, or investigate a different prerequisite.
This record describes what the learner can currently do and what instruction should happen next. It avoids turning a temporary gap into a permanent label.
