Catch a failing security control by checking three things: whether it is implemented correctly, whether it operates as intended, and whether it achieves its security objective. A policy or configured tool is not proof on its own. Assess real operation, document the evidence and limits, correct risk-prioritized deficiencies, then retest.
What does it mean for a security control to work?
NIST describes a security control assessment as determining the extent to which controls are implemented correctly, operating as intended, and producing the desired outcome. Those are separate questions: a safeguard can be installed but misconfigured, configured correctly but inconsistently used, or operating as designed without adequately reducing the risk it was meant to address. NIST’s definition is in its glossary entry for security control assessment.
That distinction changes what counts as evidence. A written policy can show what an organization expects; it cannot, by itself, establish what happens in practice. A configuration can show how a system is set up; it does not necessarily establish that the control operates reliably or produces the intended result.
How to assess a control in practice
Use a documented plan and procedures suited to the control, system, and risk. NIST SP 800-53A Rev. 5 is its final guide for conducting control assessments, and its procedures can be tailored to an organization’s risk-management needs. It is a framework to adapt, not a universal test or a guarantee that every control can be assessed with the same evidence.

- State the objective and scope. Describe what the control is supposed to prevent, detect, or enable. Identify the system, relevant conditions, and risk tolerance in scope. A specific objective makes it possible to assess results rather than just the presence of a policy or product.
- Choose evidence and a method. Decide what would demonstrate implementation and operation for this particular control. Depending on the control, evidence may include configurations, records, procedures, and observed results. Select procedures that fit the control and its environment; NIST SP 800-53A Rev. 5 provides customizable assessment procedures.
- Test all three effectiveness dimensions. Check whether the control is implemented correctly, operates as intended under relevant conditions, and achieves the stated outcome. Record the scope, evidence, method, limitations, and result so another assessor can understand how the conclusion was reached.
- Investigate gaps and exceptions. Compare actual operation and results with the objective. Missing evidence, inconsistent operation, or an unmet outcome calls for investigation. Do not accept a checklist pass when the evidence conflicts with it.
- Prioritize and correct deficiencies. Document the weakness, assess its risk, assign corrective work, and communicate the security implications to responsible decision-makers. Assessment findings should lead to action, not merely a record of noncompliance.
- Retest and monitor. After remediation, assess the control again to check whether its operation or outcome changed. Continue monitoring selected controls and relevant system or environmental changes between formal assessments.
NIST’s SP 800-53A Rev. 5 explains assessment procedures. Its January 25, 2022 announcement makes the point plainly: “Control assessments are not about checklists, simple pass/fail results, or generating paperwork to pass inspections or audits.”
What evidence can reveal that a control is failing?
The right evidence depends on the control and the conditions in which it is supposed to work. The aim is to establish more than a statement of intent: look for evidence of implementation, operation, and outcome. A document review may help establish requirements or procedures, but should not stand in for evidence of actual operation.

- Implementation evidence: relevant system configurations or other records that show whether the safeguard was put in place correctly.
- Operational evidence: records, procedures, or observed results that show whether it is being carried out as intended.
- Outcome evidence: results that help determine whether the safeguard achieves its stated security objective.
If evidence is missing or contradictory, treat that as a question to resolve, not as proof that the control works. Record what was examined and any limitations so the conclusion does not overstate what the assessment established.

When more than one method could assess a control, compare them against the decision you need to make. These are practical comparison factors, not a NIST scoring scale.

- Coverage: Which controls, environments, and operating conditions will the method include?
- Evidence quality: Can it show actual operation and outcomes, rather than only policy or configuration?
- Repeatability: Could another assessor repeat the method and understand how the conclusion was reached?
- Independence: Is a self-assessment appropriate, is independent review needed, or should both be used?
- Cadence and change response: How often will evidence be refreshed, and which changes should prompt reassessment?
- Risk fit: Does the assessment scope reflect the organization’s risk tolerance and the potential impact of control failure?
How do you catch control degradation between assessments?
Formal assessments are snapshots. NIST SP 800-137 describes information security continuous monitoring as a way to maintain visibility into assets, threats, vulnerabilities, and the effectiveness of deployed controls. That visibility can support timely responses when controls appear inadequate, rather than waiting for the next scheduled assessment.
Use ongoing monitoring for selected controls and relevant changes in the system or environment, then reassess when new evidence suggests that a control may no longer work as expected. SP 800-137 is a foundational publication from 2011; check its current status and any applicable organization- or sector-specific requirements before treating it as a compliance authority. What should the assessment record contain?
A useful record lets decision-makers understand what was assessed and why the conclusion is credible. Keep the scope, objective, method, evidence, limitations, and result together. For each deficiency, document its risk, corrective action, responsible party, and retest result. This makes it possible to distinguish a control that was never verified from one that was assessed and found effective.
