To find a therapist who is affirming of your identity, look beyond a profile label: decide what kind of support you want, gather candidates through several routes, and ask directly about their experience, training, and approach before you book. Then check qualifications, cost, access, and how comfortable you feel speaking with them. A directory listing can help you start a search; it does not establish that a therapist is the right fit.
This guide focuses mainly on U.S. provider searches, with LGBTQ+ and gender identity as prominent examples. The same process can help you screen for experience with other identities and intersecting needs. You do not necessarily need a specialist when the concern bringing you to therapy is not rooted in your identity.
Decide what you want from an affirming therapist
“Affirming” can mean different things in practice. Before searching, separate the support you want from the qualities you want in a provider. That makes it easier to recognize a useful match—and to avoid treating one label as proof of competence.
Write down your goals and priorities
Start with the reason you are considering therapy: for example, anxiety, grief, relationship stress, family conflict, or support around a transition. Then note how you want the therapist to understand or address your identity. You may want focused help with identity-related questions, a provider with experience serving people with your identity, or a therapist who will respect that part of your life without making it the explanation for every concern.

Decide whether you have a specific practical need as well. Some people are looking for a letter or documentation, while others prioritize a provider who shares an identity, uses a particular treatment approach, or can work with more than one intersecting concern. These are different screening criteria; being clear about which matter most helps you compare candidates.
It can help to sort your criteria into “must have,” “would be helpful,” and “not important to me.” For example, working with your age group, being able to address a particular concern, or meeting a documentation requirement may be essential. A shared identity, a particular format, or a certain kind of specialization may be a preference—or a requirement for you. There is no universal checklist: the point is to identify what would make care useful and respectful in your situation.
Choose the level of specialization you need
A therapist does not have to specialize in every salient part of your identity to offer respectful care. NAMI notes that specialization may not be necessary when the concern is not rooted in sexual orientation or gender identity. If you are seeking help specifically related to identity, or need a service that depends on relevant knowledge, ask for more focused experience rather than assuming general good intentions are enough.
For gender-affirming therapy, the American Psychiatric Association’s introductory guide describes the stance as affirming a patient’s gender identity rather than trying to “repair” it. It also describes practices such as making space for reflection, empathy, and processing; recognizing diversity; and connecting people with social supports or other services when useful. That guide is dated November 2017, so treat it as an introduction, not the sole or current standard for care. Ask providers about their present training and check current professional guidance relevant to your situation.
Build a shortlist from more than one source
Use several routes if you can. A single directory or referral may not show the full range of options, and each route has different limits. Availability, eligibility, fees, and telehealth rules can vary by location and change over time, so confirm current details with the provider or organization.
- Your insurer: Search the insurer’s provider directory or call the member help line. Ask about in-network clinicians who are accepting patients, whether the plan covers the type of care you need, and what your likely costs will be. A directory entry does not by itself confirm current availability or coverage.
- LGBTQ+ directories and organizations: Search relevant provider directories and contact local LGBTQ+ community or health centers and groups. Their referrals may help you find clinicians familiar with local needs, but still ask the provider about their own experience and approach.
- Health professionals: Ask a doctor or another clinician you trust for referrals. You can specify the identity-related experience or practical requirements you are looking for.
- Professional association tools: The American Psychological Association’s Psychologist Locator can search by city or ZIP code and practice area. It is one way to identify psychologists, not a certification that a particular person is competent in every identity-related concern.
A search such as “LGBTQ+ affirming therapist near me” can surface candidates, but search wording and profile labels are only starting points. For any listing, look for concrete details about the provider’s relevant work and then ask follow-up questions directly.
Keep the search practical
As you collect names, note where each referral came from and whether the person appears to meet basic requirements: license, age-group experience if relevant, location or telehealth eligibility, and the kind of service you want. Do not assume a provider is available, in-network, or qualified for a particular documentation request just because a directory shows a listing. Confirm those details before committing to an appointment.

A simple shortlist can prevent you from having to remember every detail after several calls. For each candidate, record the contact date, whether they are accepting clients, the next available appointment if offered, the session format, the expected cost and insurance status, and any answers to your key questions. You can keep notes brief and avoid writing down sensitive details you do not want stored. If a provider cannot meet a requirement, mark that clearly so you can focus on viable options.
Directories and referrals are ways to locate people, not endorsements or guarantees. The APA Psychologist Locator, an insurer directory, or an LGBTQ+ organization may each surface different candidates. You may need to check more than one route, especially if your needs include a particular identity-related specialty, language, accessibility feature, or age group.
Screen candidates before booking
If a provider offers a brief consultation or answers questions by phone or email, use that conversation to learn how they work. You can ask the same core questions of each candidate, which makes their answers easier to compare. You do not have to disclose more personal detail than you feel comfortable sharing in an initial contact.
Questions about identity-specific experience
- “My identity is ____. What experience do you have working with people with that identity?”
- “What training or continuing education have you had in this area?”
- “How would you make space for identity-related issues while also working on my other goals?”
- “How do you approach situations where identity is relevant to one concern but not necessarily to another?”
Listen for a clear, specific answer rather than relying only on a broad statement of support. You are trying to understand whether the therapist can talk about their experience, how they apply it, and how they will follow your priorities. You can also ask how they respond when they do not know something and how they seek appropriate consultation or learning.
A useful answer should leave room for your own account of what matters. You might ask how a therapist would handle unfamiliar terminology, a concern involving more than one identity, or a situation where you want to discuss an identity-related issue without making it the focus of every session. The goal is not to test a provider on every concept; it is to learn whether they can listen, be transparent about their experience, and respond to your stated goals.
Ask directly about conversion or reparative therapy if relevant
If avoiding conversion or reparative therapy matters to you, ask whether the provider offers it and what they mean by it. NAMI describes these practices as attempts to change sexual orientation or gender identity and recommends asking direct follow-up questions. If an answer is unclear, ask the provider to explain how they would respond if a client wanted support around identity without being steered toward changing it.
Ask about a letter or other documentation
If you are seeking a support letter, ask whether the therapist understands the requirements that apply to your particular insurer or legal process. Requirements can depend on the institution and situation, so do not assume one provider’s general experience guarantees that a specific document will meet them. Ask what the provider can offer, what information they would need, and whether they can explain any limits before you begin.

It is also reasonable to clarify timing, fees, and whether the therapist can provide the requested document at all. A therapist may need to understand your circumstances before answering, and the requirements can be set by an insurer or other institution rather than by the provider. Getting clarity before starting can help you avoid discovering later that the service does not match your need.

Compare professional fit and practical access
Identity-related competence is important, but it is not the only factor in choosing a therapist. The American Psychological Association emphasizes credentials and competence alongside rapport and personal comfort. Compare concrete qualifications and logistics as well as how it feels to talk with the person.
| What to compare | What to confirm | Why it matters |
|---|---|---|
| License and experience | Relevant license, experience with your concerns, and age-group expertise where needed | Training and experience should match the care you are seeking; an adult-focused practice is not automatically suited to a child or adolescent. |
| Identity-related experience | Work with people with your specific identity, relevant training, and approach to identity in therapy | A concrete discussion is more informative than a profile label alone. |
| Treatment approach and goals | How the therapist would work on your stated goals and whether the approach makes sense to you | You should be able to understand what therapy may involve and ask questions about it. |
| Cost and insurance | In-network status, expected out-of-pocket cost, billing details, and any relevant coverage limits | Costs and coverage depend on your plan and provider; verify them rather than relying on a search listing. |
| Availability and format | Whether the provider is accepting clients, appointment times, in-person or remote options, and location eligibility | Access depends on current availability and where you live. |
| Accessibility and language | Language, communication, disability access, and other accommodations you need | A provider’s practical setup should work for you, not merely appear suitable on paper. |
| Confidentiality and documentation | How confidentiality works in your situation and how records or requested documents are handled | Ask about circumstances and processes that matter to you before sharing sensitive information. |
| Comfort and rapport | Whether you feel listened to, respected, and able to speak honestly | Personal comfort is a meaningful part of fit, not a minor extra. |
Use the table as a comparison aid, not a scoring system that makes every factor equally important. Mark essential requirements separately from preferences. A candidate who cannot meet a non-negotiable need—such as working with your age group or providing an appropriate service—may not be a viable option even if other parts of the profile look promising.
Ask about the expected amount you will pay, not just whether a therapist “takes” your insurance. Coverage and out-of-pocket costs depend on your plan and the provider, so confirm them with the insurer and clinician. If you are comparing in-network and other options, clarify the billing arrangement and any coverage limits before scheduling. No single price or coverage rule applies to everyone.
For remote care, confirm that the provider can serve people in your location and that the format works for you. Ask about in-person availability if that matters, and consider appointment times, communication needs, language, and accessibility. Licensing, telehealth eligibility, insurance, and availability are location-specific and can change, so check directly rather than relying on old listings.
Confidentiality is another practical question, especially if you are considering sharing sensitive information or requesting documentation. Ask how confidentiality works in your circumstances, who may have access to records, and how any letter or other document is handled. You can ask general process questions before deciding what personal details to share.
Use an early session to assess the fit
A screening call can tell you something about a therapist, but the first appointment may reveal more about how the relationship feels. You are not expected to make a final judgment from one moment, but pay attention to whether the therapist can hear your goals and respond respectfully.

Notice what happens in the conversation
- Does the therapist listen to what you say you want help with, rather than assuming your identity explains every difficulty?
- Do they use the name, terms, and pronouns you request?
- Can they discuss identity-related issues without asking you to provide all the basic education?
- Do they make room for identity when it matters while also attending to the rest of your life and goals?
- Can you ask questions or disagree without feeling dismissed?
These are practical ways to assess whether the provider’s stated approach shows up in conversation. If something feels wrong, you can ask about it directly, decide whether it was a misunderstanding, or stop pursuing that candidate. You do not owe a therapist continued appointments simply because you have started.
Fit is not the same as finding a person who agrees with you about everything. It includes whether you can talk honestly, understand the therapist’s approach, and feel that your concerns are being taken seriously. If you are unsure after an early conversation, you can reflect on what felt helpful or uncomfortable, raise a question, or compare the experience with another candidate. You do not have to treat one meeting as a final verdict, but you can take your response seriously.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsAdditional considerations when choosing for a child or teen
For a child or adolescent, screen for experience with that age group, not only for experience with a particular identity. A clinician who works well with adults may not have the developmental expertise needed for younger clients. The American Psychological Association’s child and adolescent guidance, updated May 16, 2024, says that a gender-diverse identity alone is not necessarily a reason to seek psychotherapy; care should respond to the young person’s actual needs and be affirming.
Questions for parents or guardians to ask
- What experience do you have with child or adolescent development and the concerns we are bringing?
- What focused training do you have in gender identity development, if that is relevant to this young person?
- How do you work with the young person and family, and how do you decide what role each person has in care?
- When appropriate, how do you collaborate with other providers involved in the young person’s care?
- How familiar are you with current professional guidance, including the WPATH Standards of Care and relevant American Psychological Association guidance?
Ask how the provider will make space for the young person’s own experience, while explaining what family involvement and confidentiality look like in that setting. The young person’s response to an initial visit matters too. If the relationship is not working, the first clinician does not have to be the final choice.
Questions about family involvement should be specific to the young person and the provider’s setting. Ask how the clinician includes the child or teen in conversations, what the provider expects from a parent or guardian, and how privacy is handled. Do not assume that an adult therapist’s experience transfers automatically to a younger client; age-specific development and the way the clinician works with families are distinct screening points.
What to do if the first candidate is not a fit
It can take several calls to find someone who meets your needs, and the first therapist you contact may not be right for you. If a provider cannot answer a key question, lacks the required experience, or leaves you uncomfortable, return to your shortlist or use another search route. If the main obstacle is practical—such as cost, schedule, or location—say which constraint you need to solve and look for a provider who can meet it.

If one route has not turned up a workable option, try another: an insurer’s member help line, a community or health center, a trusted health professional, or a professional directory may yield different names. Revisit your priorities if needed, but do not downgrade a requirement that is essential to feeling respected or getting the service you need. It is acceptable to keep looking until the professional and practical fit is better.
Frequently Asked Questions
How can I tell whether a therapist supports trans people?
Ask about the therapist’s experience working with trans people, relevant training or continuing education, and how they make space for gender-related concerns alongside your other goals. A profile label can help you find candidates, but it does not establish competence. You can also notice whether the therapist listens, uses the terms and pronouns you request, and avoids treating your identity as something to change.
Do I need a therapist who specializes in my identity?
Not necessarily. NAMI notes that specialization may not be needed when the concern bringing you to therapy is not rooted in sexual orientation or gender identity. If you want focused help with identity-related concerns or need a service requiring specific knowledge, ask candidates about relevant experience and training.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Where should I look for an LGBTQ+ affirming therapist near me?
Try more than one route: your insurer’s provider search or member help line, LGBTQ+ directories and local community or health centers, referrals from a health professional, and professional association tools such as the APA Psychologist Locator. Search results and listings are starting points, not guarantees; confirm current availability, qualifications, coverage, and fit directly.
What should I ask if I want to avoid conversion or reparative therapy?
Ask directly whether the provider offers conversion or reparative therapy and what they mean by those terms. NAMI describes these practices as attempts to change sexual orientation or gender identity and advises direct follow-up questions. If the answer is unclear, ask how the therapist would support you without steering you toward changing your identity.
What should parents ask when looking for a therapist for a gender-diverse child or teen?
Ask about child or adolescent development expertise, focused training in gender identity development when relevant, how the clinician works with the young person and family, and collaboration with other providers when appropriate. You can also ask about familiarity with current guidance, including the WPATH Standards of Care and relevant APA guidance. A gender-diverse identity alone is not necessarily a reason for psychotherapy; care should address the young person’s actual needs and be affirming.
The Bottom Line
Start by naming what you want help with and which identity-related experience or practical requirements matter to you. Build a shortlist from more than one route, ask candidates directly about their experience and approach, and confirm credentials, cost, coverage, availability, and access. Then notice whether you feel listened to and respected. A directory label can point you toward a conversation, but it cannot make the decision for you—and it is okay to keep looking if the fit is not right.
