Do not treat a chatbot answer as verified simply because it sounds confident or includes citations. Fact-check it by separating the response into individual claims, opening the original evidence, checking whether the evidence actually supports each claim, and confirming dates, context, and authority. For health, legal, financial, safety, and other consequential decisions, use primary authorities and qualified professionals instead of relying on an AI assistant alone.
ChatGPT, Claude, and Gemini can all produce useful first drafts as well as incorrect facts, invented quotations, misleading summaries, outdated information, and authoritative-looking references that do not support what they say. The practical method below works across all three systems.
Why an AI answer can be wrong while sounding right
Language models generate likely text from patterns in their training and, when available, retrieved material. They do not automatically establish that every sentence is true. A fluent paragraph can combine accurate background, an unsupported inference, and one fabricated detail without signaling where the boundary lies.

Anthropic says Claude can occasionally produce incorrect or misleading answers, including confusion about current events and convincing quotations that are not grounded in fact. Its guidance says users should not rely on Claude as a singular source of truth and should scrutinize high-stakes advice.
Google’s Gemini Apps guidance says Gemini can hallucinate and present inaccurate information as factual. It also warns that Gemini may misrepresent how it works, how it cites sources, or whether it has fresh information. Google advises users to double-check responses, particularly about people, and not to rely on Gemini Apps for medical, legal, financial, or other professional advice.
These warnings do not establish that one assistant is more accurate than the others. They are product guidance, not a matched independent test. Accuracy varies with the model version, prompt, subject, date, browsing or grounding settings, and the quality of available sources.

The seven-step fact-checking workflow
1. Turn the answer into separate claims
Start by marking every statement that could be checked. A single paragraph may contain several different propositions, and verifying one does not verify the others.
- Dates: “The rule changed in 2024.”
- People and organizations: “The agency issued this policy.”
- Numbers: “The treatment reduces risk by 30%.”
- Quotes: Text presented as someone’s exact words.
- Causal statements: “Event A caused outcome B.”
- Recommendations: “Use this method,” “sign this form,” or “take this action.”
- Definitions and classifications: What a term means or which category something belongs to.
Copy the answer into notes and give each checkable proposition a label, such as C1, C2, and C3. Keep interpretations separate from facts. “The law says X” is a factual claim; “therefore this is your best option” is advice that requires a different evaluation.
2. Identify the right kind of evidence
Decide what would count as authoritative before searching. The best source depends on the claim.

A blog or search snippet may help you locate evidence, but it should not automatically outrank the original record.

Click or copy the cited title, DOI, document number, or web address into a browser. Confirm that the page or document exists and is the item the answer describes. A plausible-looking citation can still be fabricated, mismatched, or attached to the wrong claim.

Check whether the cited source actually contains the relevant statement. A paper about correlation does not prove causation. A news report about a proposal does not prove that a rule took effect. A search result that contains a keyword is not evidence that the full page supports the chatbot’s conclusion.
If no source opens, the publication cannot be found, or the citation points to an unrelated document, label the claim unsupported. Do not repeat it as fact merely because several generated answers include the same reference.
4. Read beyond the quoted sentence
Read the complete paragraph, table, footnotes, definitions, and limitations around the relevant passage. Chatbots often compress a qualified statement into an unqualified one.
- “May” can become “does.”
- A result for a narrow study population can be presented as universal.
- An estimate can be presented as a measured count.
- A preliminary finding can be described as settled consensus.
- A source’s example can be mistaken for a general rule.
Compare the model’s wording with the source’s wording. Mark what is directly stated, what is a reasonable paraphrase, and what is the model’s inference. Only the first two can be attributed to the source, and a paraphrase must preserve its qualifications.
5. Check who published it and when
Record the publisher, publication date, last-update date, and the date on which you checked it. Current facts can change after a model’s knowledge cutoff or after a cited page was written.
For a current product feature, check the vendor’s documentation and release notes for your edition and region. For a legal or regulatory question, check the relevant jurisdiction’s current register. For statistics, find the data publisher’s latest release and verify whether the number is provisional, revised, seasonal, or limited to a particular population.
Do not infer freshness from a chatbot’s statement that it “searched the web.” Ask which sources and dates it used, then inspect those sources yourself.
6. Confirm important claims independently
Use a second credible source for claims that matter. Prefer independent sources that rely on primary evidence rather than copying the same summary. Agreement increases confidence only when the sources are genuinely independent and address the same scope.

Two pages repeating an unsourced statistic are not two independent confirmations. Conversely, an official record and a well-designed independent analysis may provide useful complementary checks: one establishes what happened, while the other explains significance or limitations.
7. Label the result and decide what to do
Give each claim a status:
- Verified: A suitable source directly supports the claim within the stated scope and date.
- Partly supported: The source supports a narrower or more qualified version.
- Unresolved: Evidence is incomplete, conflicting, inaccessible, or too old.
- Unsupported: The cited source does not exist or does not support the statement.
- Opinion or recommendation: The statement requires judgment rather than factual verification.
Rewrite your notes so verified facts, interpretations, estimates, and unresolved claims are visibly separate. If a claim cannot be supported, remove it or present it explicitly as uncertain.
How to verify citations, quotes, and numbers
Citations and references
Inspect the author, title, publisher, date, and document identifier. Search the exact title in the publisher’s own catalog or website. For academic work, confirm the journal, authors, year, and DOI in a trusted index or the journal itself. For government material, verify the issuing department and document number.

Save a copy or note the access date when the claim is important.
Quoted words
Search a distinctive phrase in quotation marks and locate a transcript, recording, official speech, court filing, or contemporaneous document. Check whether the quote is complete and whether the speaker, date, and setting match. If only secondary sources repeat it, call it a reported quotation rather than an established verbatim quote.
Numbers and calculations
Identify the unit, denominator, time period, geography, and population. Recalculate percentages from the source data when possible. A statement such as “cases fell 40%” is incomplete without knowing the baseline, interval, and whether the figure is adjusted.
Ask the assistant to show its calculation, then perform the arithmetic independently. A correct calculation applied to an incorrect input is still wrong.

Using search, browsing, and grounding without overtrusting them
Search-enabled answers can save time by locating documents and attaching citations. They do not remove the need to inspect the documents. Retrieval can select an outdated page, misunderstand a passage, omit an important source, or connect a citation to the wrong sentence.
When a system offers a “deep research” or search mode, ask it to list the sources, publication dates, and exact passages supporting each major claim. Then open those sources yourself. If the tool cannot show a source or passage, downgrade your confidence.
Google’s Gemini API documentation describes Search grounding as a way to connect a model to real-time web content and citations. It also says many AI models are experimental and may present factually inaccurate information, hallucinate, or produce problematic outputs. The documentation recommends rigorous manual evaluation. Do not describe API grounding as a universal feature of Gemini Apps or as a correctness guarantee.

Fact-checking high-stakes answers
The cost of an error should determine how much checking you do. For low-consequence brainstorming, a quick source check may be sufficient. For a decision that could affect health, rights, money, employment, safety, or someone’s reputation, use a stricter process.
- Write the exact decision and deadline.
- Separate factual premises from the assistant’s recommendation.
- Find the primary authority or professional guideline for each premise.
- Check the jurisdiction, eligibility rules, dates, exceptions, and definitions.
- Ask a suitably qualified professional to interpret the evidence for your circumstances.
- Keep records of the sources and version dates you relied on.
Do not use an AI answer as a diagnosis, legal opinion, investment instruction, safety clearance, or substitute for emergency services. Even a citation-backed answer can omit facts that a professional would need.

A better prompt can make an answer easier to audit, but it cannot make unsupported claims true. Try instructions such as:
- “List each factual claim separately and label any inference.”
- “For every claim, provide the source title, publisher, publication date, and the exact supporting passage.”
- “State what you do not know and whether the information may be outdated.”
- “Do not invent a citation. If you cannot verify a source, say so.”
- “Separate facts, estimates, opinions, and recommendations in different sections.”
- “Give the jurisdiction, date range, population, and units for every statistic.”
Use these outputs as a checklist for your own verification, not as proof that verification has already happened.
Common failure modes and how to recover
The citation exists but does not support the claim
Quote the relevant passage, identify the mismatch, and rewrite the claim to the narrower statement the source supports. If no narrower statement works, remove it.

The source is real but outdated
Find the latest edition, amendment, release, or data update from the same authoritative publisher. Preserve the older date in your notes if the historical version matters.
The answer mixes fact and advice
Split the sentence. Verify the factual portion against evidence, then evaluate the recommendation against your goals, constraints, and professional guidance.
Sources disagree
Check whether they cover different dates, regions, populations, definitions, or methods. If the disagreement remains, report it and explain which source has stronger authority for your question rather than choosing the more confident wording.
No source can be found
Mark the claim unsupported. Ask the assistant to withdraw it, but do not treat a revised answer as evidence unless it supplies a source you can inspect.
The answer concerns a person
Use especially careful verification. Confirm identity, role, dates, and the original statement through reliable records. Do not repeat allegations or sensitive personal information based only on generated text.

For work you may need to defend later, keep a small table:

This record prevents a common mistake: remembering that an answer had “a citation” while forgetting whether the citation actually matched the claim.

You can use them as research assistants, explainers, drafting tools, and search interfaces when you independently check the material that matters. You should not treat any of them as a singular source of truth. None of the official guidance reviewed here supplies a general consumer error rate or supports ranking one assistant as the most accurate.
The reliable habit is therefore tool-independent: decompose the answer, inspect original evidence, check context and dates, confirm important claims independently, label uncertainty, and escalate consequential decisions to authoritative sources and qualified professionals.
