Use a context-specific decision policy, not a universal confidence score. An AI agent should act when the task, authority, and limits are clear and the action is bounded with acceptably low consequences. It should ask when the unresolved issue is a user preference, intent, or authorization that it cannot safely infer. It should stop or defer when the action exceeds its granted scope or an unresolved safety, security, legal, or policy issue makes proceeding inappropriate.

For consequential, multistep work, have the agent show its intended plan for human review before execution and preserve a way to interrupt or redirect it while it runs. This article turns guidance from Anthropic, NIST, and OpenAI into an implementable policy while distinguishing their documented practices from editorial implementation heuristics.

The decision in one view

Every proposed agent action can be evaluated with five questions:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Is it authorized? The action must fit the agent’s permissions, purpose, and the user’s actual request.
  2. What could it change? Identify affected systems, people, data, money, reputation, and the severity of an error.
  3. What is uncertain? Separate a factual gap the agent can safely investigate from a preference, intent, or authorization question that belongs to a human.
  4. Can a person review or reverse it? A consequential action that is difficult to undo deserves a stronger checkpoint than a bounded, reviewable action. This reversibility test is an implementation heuristic, not a formula prescribed by NIST or Anthropic.
  5. Which control fits? Proceed within approved scope, ask one focused question, present a plan for approval, or stop and defer.

This sequence is a practical synthesis of the cited sources, not a published standard or empirically validated scoring model. A model’s self-reported confidence cannot grant authority that the deployment has not defined.

What “act,” “ask,” and “stop” should mean

Act: proceed inside a known boundary

Act when the objective and authorization are explicit, the operation is within configured permissions, and a mistake would be limited and recoverable. Examples include formatting a document in a specified style, collecting publicly available facts for a draft, running a read-only diagnostic, or applying an approved transformation to a named file set.

Act does not mean “do anything that appears useful.” The agent should use only the tools and data needed for the approved task, preserve an audit trail, and avoid silently expanding the objective. If a new action changes the risk, target, or authority, the original permission no longer automatically covers it.

Ask: resolve a human decision

Ask a focused question when the missing information is about what the user wants, which trade-off to choose, or whether the user authorizes a consequential step. A useful question presents the relevant options and consequence rather than asking the user to restate the whole task.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For example, “The report contains two conflicting revenue definitions. Should I use booked revenue or recognized revenue?” is an intent question. “The source file is missing the third-quarter column; may I look for it in the finance folder?” is an authorization question. The agent may be able to investigate a factual gap on its own, but it should not infer a preference merely because one option is statistically common.

Stop or defer: do not continue under a blocking risk

Stop when the action is outside the agent’s scope, the required authorization is absent, or an unresolved safety, security, privacy, legal, or policy issue makes execution inappropriate. Stopping should be explicit: state what blocked the run, preserve relevant evidence, and identify the human decision or remediation needed to resume.

Stopping is different from asking. If a user can safely choose between two authorized options, ask. If neither option is permitted or the agent cannot establish a safe operating condition, stop. Never turn a blocked action into a “best effort” execution that quietly bypasses the control.

Start with authority and scope

Capability and permission are separate. A model may be technically able to delete records, send a message, or change a production setting without being authorized to do so. Define authority outside the model’s prose: tool permissions, data boundaries, target resources, allowed operations, approval requirements, and explicit stop conditions.

Anthropic’s August 4, 2025 framework says humans should retain control over how their goals are pursued, particularly before high-stakes decisions. It gives canceling a subscription or downgrading a service tier as examples of actions that should require approval. Anthropic also describes Claude Code as read-only by default, with human approval before code or system modifications and persistent permissions available for routine, trusted tasks. Those are product-specific controls, not a universal industry standard.

Write an authority contract

For each agent, document at least:

  • Purpose: the business or personal outcome the agent is allowed to pursue.
  • Resources: named systems, folders, accounts, APIs, and data classifications it may access.
  • Operations: read, create, edit, send, purchase, publish, delete, or administer.
  • Targets: which records, recipients, environments, or customers are in scope.
  • Limits: spending, volume, time, rate, geography, and other boundaries.
  • Approval gates: actions that require a person before execution, plus who can approve them.
  • Stop conditions: policy violations, missing evidence, conflicting instructions, suspicious input, or tool failure.
  • Evidence and logs: what the agent must record so a reviewer can reconstruct its decision.

Keep permissions narrow and composable. A read-only research role should not inherit a write permission simply because the same agent might need it later. Temporary elevation should be explicit, time-limited, and logged.

Map context and consequence before choosing oversight

NIST’s AI Risk Management Framework (AI RMF) treats context as the starting point for deciding whether an AI use is appropriate. Its Appendix C describes human-AI configurations ranging from “fully autonomous to fully manual” and calls for clear human roles and responsibilities. The right oversight therefore depends on the deployment, not on the label “agent.”

Assess who and what can be affected

  • People: Could an error affect employment, health, education, housing, access to services, safety, or a person’s reputation?
  • Systems: Could it alter production infrastructure, financial records, identity settings, security controls, or regulated data?
  • Scope: Is the action confined to one reversible item, or can it propagate across accounts, customers, or environments?
  • Timing: Is there time for review, or would delay itself create material harm?
  • Evidence: Can the result be checked independently before it matters?
  • Recovery: Is there a tested rollback, and does it restore the original state rather than merely hide the error?

Do not treat reversibility as a reason to skip review when the interim effect is harmful. A mistaken public disclosure may be technically retractable but still irreversible for people who saw or copied it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use a consequence-based control table

Situation Typical control Why
Read-only, bounded, low-impact task with clear instructions Agent acts; log inputs, tools, and result The scope and consequence are limited and reviewable.
Task is authorized, but a user preference or trade-off is unresolved Ask one focused question; pause the affected branch The missing decision belongs to the user.
Several authorized steps form a consequential workflow Show the plan for approval, then allow monitored execution Human judgment is applied at the strategy level without approving every routine step.
Action changes code, data, infrastructure, money, or external communications Require an explicit approval gate appropriate to the impact The operation can create material or hard-to-reverse consequences.
Scope, identity, consent, policy, or safety cannot be established Stop and escalate; do not improvise Proceeding would bypass a blocking control.

The rows are implementation guidance, not a NIST classification. Adjust them to the people, systems, laws, and failure modes in your deployment.

Distinguish factual uncertainty from intent uncertainty

An agent should not pause for every unknown. If the gap is factual and the agent can investigate safely within its permissions, it may gather more information, compare sources, or report that the evidence remains incomplete. If the gap concerns preference, intent, consent, or authorization, it should ask.

Factual gaps the agent may investigate

  • A date or definition can be checked in an approved source.
  • A file path can be searched within the permitted workspace.
  • A software error can be reproduced in a sandbox or read-only environment.
  • Two records can be reconciled using a documented rule.

The agent should state what it checked, which sources were used, and what remains uncertain. It must not treat an unverified inference as a fact merely to avoid asking.

Intent and authorization gaps that require a question

  • Which of two business definitions should govern a report.
  • Whether a message should be sent now or saved as a draft.
  • Whether a proposed deletion includes archived or legally held records.
  • Whether a user accepts a cost, delay, quality reduction, or privacy trade-off.
  • Whether a person has consented to a use of their data.

Anthropic’s practical guidance describes the same distinction: an agent can research some information gaps itself, while preference or intent questions may require the user. Its warning is two-sided: an agent that “stops at every possible question” sacrifices autonomy, while one that “always pushes through” risks misunderstanding. The useful target is the smallest question that resolves the consequential ambiguity.

Choose between per-action approval and plan review

For a short, high-impact action, an approval prompt immediately before execution is often understandable. For a long workflow, approving every shell command, file edit, or API call can create prompt fatigue. Anthropic’s 2026 practical guidance contrasts action-by-action permissions with reviewing an intended plan up front for complex tasks.

Use per-action approval when

  • The action is consequential, unusual, or outside a routine pattern.
  • The reviewer needs to inspect exact parameters, recipients, records, or amounts.
  • The workflow is short enough that the prompt remains meaningful.
  • A mistake at this step would be costly even if the broader plan is sound.

Use plan approval when

  • The workflow contains many routine, low-impact steps.
  • The reviewer can understand the goal, sequence, tools, data, and stop points before execution.
  • The agent can stay within the approved plan and surface deviations.
  • A person can intervene during execution and cancel or redirect the run.

What a reviewable plan must contain

  1. Objective and success criteria in plain language.
  2. Resources, data sources, and permissions the agent will use.
  3. Ordered steps, including any external side effects.
  4. Assumptions and unresolved questions.
  5. Actions requiring separate approval.
  6. Expected outputs, logs, and validation checks.
  7. Rollback or containment steps if a check fails.

Plan approval is not blanket permission for unrelated actions. If the agent discovers a new target, changes the data category, or proposes a materially different operation, it should pause and request a revised approval.

Build a practical policy and control flow

The following pseudocode is a design aid, not a validated risk score or a standard runtime algorithm:

evaluate(proposed_action, context):
  if not within_authorized_scope(proposed_action, context):
      return STOP("scope or authorization is missing")

  impact = assess_people_systems_data_and_reversibility(proposed_action, context)

  if unresolved_safety_security_policy_or_consent_issue(proposed_action, context):
      return STOP("blocking risk requires escalation")

  if missing_preference_intent_or_user_authorization(proposed_action, context):
      return ASK(focused_question_with_options())

  if impact.is_consequential and workflow_has_multiple_steps(proposed_action):
      return REQUEST_PLAN_APPROVAL(plan_and_checkpoints())

  if impact.requires_specific_approval:
      return REQUEST_ACTION_APPROVAL(exact_parameters_and_effects())

  return ACT(within_limits_and_log_every_step())

Implement the controls in the tool layer where possible. An instruction telling a model not to delete records is weaker than an API that does not expose deletion, or exposes it only through an approval service. Use defense in depth: least-privilege credentials, environment separation, schema validation, recipient confirmation, rate limits, transaction previews, and immutable or access-controlled logs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Make questions answerable

A good prompt identifies the decision, the options, and the consequence of each. For example: “I found 184 records matching the cleanup rule. Option A archives them, which is reversible for 30 days; Option B permanently deletes them, which is not. Which should I do?” Avoid open-ended prompts such as “What now?” when the agent already knows the relevant choices.

Keep intervention possible during execution

Provide a visible pause or cancel control, bounded execution batches, checkpoints between stages, and a clear indication of what has already happened. The agent should not continue hidden background work after a user cancels unless the system explicitly documents and safely handles in-flight operations.

Set stop conditions and recovery paths

Stop boundaries should be written before deployment and tested with adversarial and ordinary cases. Useful triggers include:

  • Identity, consent, or authorization cannot be verified.
  • Instructions conflict across the user, system, tool, or policy layers.
  • Input appears malicious, poisoned, or unrelated to the authorized task.
  • A tool returns unexpected data, permissions, or side effects.
  • The proposed action exceeds limits for amount, volume, recipients, environment, or time.
  • A validation check fails or a required source is unavailable.
  • The agent would need to guess a material preference or legal interpretation.
  • The action affects a protected or high-impact domain without the required review.

On stopping, preserve the plan, relevant inputs, tool responses, approvals, and partial results. Tell the reviewer whether any side effect occurred, what state was confirmed, and what decision is needed. If rollback is safe and authorized, offer it as a separate, explicit operation rather than performing it automatically under a different assumption.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Measure whether the policy works

Do not evaluate an oversight design only by how often the agent asks. Track whether its questions and stops occur at the right moments.

Useful operational measures

  • Unauthorized-action prevention: attempts blocked before side effects.
  • Approval quality: whether reviewers had enough information to make an informed decision.
  • Question precision: questions that resolve a real intent or authorization gap rather than a checkable fact.
  • Intervention latency: time available to stop a run before the next consequential step.
  • Rollback success: whether tested recovery restores the intended state.
  • False pauses: interruptions that add friction without reducing meaningful risk.
  • Missed escalations: cases where the agent acted despite a condition that should have caused a question or stop.
  • Outcome impact: errors, complaints, privacy events, security events, financial loss, and other deployment-specific harms.

Anthropic reports that, on complex tasks, users interrupt Claude only slightly more frequently than on simple tasks while Claude’s own rate of checking in “roughly doubles.” The passage supplies no baseline counts, sample, or precise percentage, so it should be read as a qualitative product observation rather than an industry-wide benchmark. Your own logs are necessary to determine whether additional questions improve outcomes or merely create fatigue.

Review policies as lifecycle work

NIST AI RMF 1.0 (2023) organizes risk management into four functions: Govern, Map, Measure, and Manage. Governance is cross-cutting; Map establishes context and informs an initial appropriateness or go/no-go decision; Measure evaluates risks and controls; Manage prioritizes and responds to them. NIST describes the framework and its Playbook guidance as voluntary, not as a prescriptive agent runtime algorithm. The Playbook page was updated June 10, 2026, so teams should check the current edition when maintaining their policy.

Revisit permissions, stop conditions, prompts, logs, and reviewer roles after model changes, tool changes, incidents, new data uses, or changes in affected populations. A policy that was safe for a read-only prototype may be inadequate once the agent can write to production or contact customers.

Fix: Confidence can help prioritize verification, but it cannot establish authorization, consent, or acceptable impact. Bind execution to scope and controls, and require approval where the consequence demands it. The reviewed sources provide no validated universal numeric threshold.

Asking about every unknown

Failure: The agent interrupts for facts it could safely retrieve.

Fix: Permit bounded investigation, require source and uncertainty reporting, and reserve questions for intent, preference, authorization, and material trade-offs.

Letting a plan approval become unlimited access

Failure: The agent treats approval of one workflow as permission for new targets or side effects.

Fix: Bind approval to named resources, operations, limits, and success criteria. Require re-approval when those parameters change.

Relying on prose-only guardrails

Failure: A prompt says “never delete” while the connected tool still permits deletion.

Stopping without an operational handoff

Failure: The agent says it cannot continue but leaves the user unsure whether anything changed.

Fix: Report the blocker, confirmed side effects, preserved evidence, current state, and exact decision or remediation needed to resume.

A deployment checklist

  • Define the agent’s purpose, users, affected people, systems, and data.
  • List allowed tools and operations separately from merely technically available capabilities.
  • Set limits for targets, amount, volume, time, environment, and recipients.
  • Classify actions as act, ask, plan approval, action approval, or stop.
  • Write examples of factual gaps versus intent and authorization gaps.
  • Specify high-impact and irreversible actions that require a human gate.
  • Provide plan previews, pause and cancel controls, and deviation handling.
  • Log instructions, retrieved evidence, tool calls, approvals, outputs, and side effects.
  • Test ambiguous requests, conflicting instructions, malicious inputs, tool failures, and cancellation during execution.
  • Measure missed escalations, unnecessary prompts, intervention time, and recovery success.
  • Assign accountable human owners for policy, approvals, incident response, and periodic review.
  • Reassess the policy whenever the model, tools, data, users, or deployment context changes.

The bottom line

Give an agent room to act only where authority, scope, and consequence are understood. Ask when a human must choose the intent or authorize a meaningful trade-off. Stop when a boundary, safety condition, or required permission is missing. For complex work, review the plan before execution and retain intervention throughout the run. This context-first approach is more defensible than any single confidence percentage because it connects capability to permission, impact, human responsibility, and recoverability.
