A physical AI system is ready for a real-world deployment only when evidence from its intended operating context shows that it performs the required tasks, fails in a controlled way, can be supervised and recovered by trained people, and leaves residual risk within a documented tolerance. There is no universal readiness percentage or pass score. The accountable organization must define the use, hazards, operating limits, acceptance criteria, evidence, controls, and legal obligations for that specific system and jurisdiction.

Start by defining the deployment, not by scoring the model

“Physical AI” can mean an industrial robot, autonomous vehicle, drone, medical device, warehouse machine, consumer appliance, or another system that senses, decides, and affects the physical world. Readiness cannot be judged in the abstract. A system that is acceptable in a fenced test cell may be unsafe on a public road, in a hospital, or near untrained users.

Write a deployment definition before selecting tests. Record:

  • Intended task: the physical actions the system may perform, including speed, force, payload, route, duty cycle, and precision requirements.
  • Users and affected people: operators, maintainers, bystanders, customers, patients, road users, and anyone else who could be exposed to its decisions or motion.
  • Operating envelope: geography, indoor or outdoor conditions, lighting, weather, temperature, surfaces, connectivity, available maps, permitted hours, and known environmental exclusions.
  • System boundary: sensors, actuators, learned models, classical software, communications, cloud services, third-party data, power systems, safety controls, and human interfaces.
  • Foreseeable misuse: bypassed guards, overloaded equipment, stale maps, obstructed sensors, untrained users, adversarial inputs, maintenance shortcuts, and operation outside stated limits.
  • Dependencies: suppliers, update mechanisms, positioning services, network links, identity systems, external APIs, and spare parts.

The NIST AI Risk Management Framework (AI RMF) calls this context-setting the Map function. It is the foundation for an initial go/no-go decision and should involve domain experts, operators, users, and affected communities in proportion to the consequences of failure.

Convert hazards into a deployment-specific risk tolerance

List credible harms before choosing metrics. Consider injury, collision, unsafe motion, loss of control, property damage, privacy or security impacts, environmental harm, discrimination in access, and operational disruption. For each hazard, document the initiating conditions, likely severity, estimated likelihood, existing safeguards, and the remaining risk after safeguards.

State who may accept residual risk and what “within tolerance” means. A tolerance statement should identify prohibited outcomes, conditions requiring human approval, maximum exposure, and escalation authority. Do not substitute a generic model score for this decision. NIST provides a risk-management structure, not a universal threshold.

Use a living risk register

  • Assign an owner for every hazard and control.
  • Separate known risks, uncertain risks, and risks that cannot yet be evaluated.
  • Record assumptions such as sensor coverage, network availability, operator response, and maintenance quality.
  • Specify evidence needed to close each risk, not merely a planned test.
  • Set review triggers for incidents, near misses, software changes, new environments, supplier changes, and drift.

Define measurable acceptance criteria

Acceptance criteria should describe the actual task and operating envelope. Each criterion needs a test method, data set, metric, uncertainty treatment, benchmark or comparator where useful, owner, and pass condition. Include both normal operation and edge cases.

Readiness dimension What to measure Evidence that supports a decision
Task performance Success, completion time, precision, throughput, and quality for the intended task Representative trials with confidence intervals or other stated uncertainty, broken down by relevant conditions
Safety Hazard frequency, severity, control effectiveness, and exposure Hazard analysis, validation of safeguards, fault-injection results, and residual-risk assessment
Robustness Behavior under sensor degradation, weather, clutter, occlusion, delays, and unusual inputs Stress tests that reproduce credible deployment disturbances and document failure modes
Generalization Performance outside development data, locations, users, and conditions Independent or held-out tests matched to the target environment, with limitations clearly stated
Safe failure Detection, transition to a safe state, intervention, stop or rollback, and recovery time Observed response times and successful recovery in representative fault scenarios
Human oversight Operator comprehension, intervention success, workload, alert quality, and training effectiveness Human-in-the-loop trials using the actual interface, procedures, staffing, and time pressure
Operational integrity Logging, monitoring, update control, maintenance, calibration, security, and incident handling Runbooks, access controls, monitoring demonstrations, restore tests, and completed drills

Do not report a benchmark result without explaining how its data and conditions compare with deployment. A high laboratory score may have little evidentiary value if the real site has different surfaces, lighting, traffic, network latency, or human behavior.

Build an evidence plan that covers the lifecycle

NIST describes testing, evaluation, verification, and validation (TEVV) as lifecycle work. Plan evidence in layers so that a successful simulation does not conceal failures in the integrated machine.

1. Component and model tests

Test perception, prediction, planning, control, and non-AI software separately where meaningful. Check sensor calibration, timing, numerical limits, data quality, model versioning, and behavior after updates. Include negative tests: missing data, contradictory sensors, corrupted messages, stale maps, and out-of-distribution inputs.

3. Simulation and scenario testing

Use simulation to cover rare, dangerous, or expensive scenarios, but validate that the simulator represents the physical system closely enough for the claim being made. Track which simulated behaviors have been confirmed on hardware. Maintain a scenario catalog covering normal, boundary, degraded, and adversarial conditions.

4. Representative field trials

Run controlled trials in the proposed environment with the intended equipment, staffing, procedures, connectivity, and maintenance assumptions. Include different shifts, weather or lighting conditions, realistic workload, and interactions with people who are not part of the development team. Define stop criteria before starting and ensure that a trained safety lead can halt operations.

5. Independent review and challenge

Have people who did not build the system review the hazard analysis, test design, logs, unresolved anomalies, and proposed operating limits. Independence can be internal separation or an external assessor, depending on the risk and applicable rules. The reviewer should be able to reject evidence that is irrelevant, incomplete, or not reproducible.

Demonstrate safe failure, intervention, and recovery

Every important failure mode needs a designed response. “The AI will stop” is not a sufficient safety case unless detection, state transition, physical stopping behavior, human action, and recovery are specified and tested.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Define the safe state

For each hazard, state what the system does when confidence is low, a sensor is unavailable, a command is contradictory, communications fail, power is interrupted, or a safety monitor trips. The safe state might be a controlled stop, reduced speed, isolation of an actuator, return to a known location, or transfer to a qualified operator. The correct state depends on the machine and hazard.

Set response-time requirements from the hazard

Measure detection latency, decision latency, command latency, physical stopping or containment time, and time for a person to intervene. NIST identifies reliability, robustness, monitoring, and response time as safety-related measures, but it does not prescribe one numerical limit. Set the limit from the actual hazard, energy, speed, separation distance, and available protective equipment.

Test recovery, not just stopping

  • Confirm that the system remains stable after a stop or degraded-mode transition.
  • Require an explicit authorization before resuming hazardous motion where appropriate.
  • Verify that operators can identify the cause, isolate the hazard, and restore service without unsafe improvisation.
  • Record whether recovery loses task state, creates duplicate actions, or produces a new hazard.
  • Test repeated faults, not only a single clean fault.

Evaluate the whole system and its supply chain

Model risk across hardware, software, data, people, and physical processes. Third-party libraries, pretrained models, firmware, cloud services, maps, positioning, and remote support can all change the behavior or availability of the deployed system.

Maintain an inventory with versions, suppliers, licenses, support contacts, update authority, and end-of-life plans. Require change notification for components that affect safety or performance. Protect model files, configuration, logs, credentials, and update channels against unauthorized modification. Test degraded operation when a dependency is unreachable or returns invalid data.

Resilience includes recovery from cyber incidents, corrupted data, hardware replacement, lost connectivity, and unexpected restarts. Define backup, restore, rollback, and key-revocation procedures, then demonstrate them on the production-like configuration.

Test the human-AI configuration

Human oversight is a system property, not a checkbox. An operator may be nominally “in the loop” yet unable to understand a warning, intervene in time, or manage several systems at once.

Check operator understanding

  • Can operators state the operating envelope and known blind spots?
  • Do alerts explain what happened, what the system will do next, and what action is required?
  • Can operators distinguish a low-confidence advisory from a safety-critical stop?
  • Does the interface reveal stale data, sensor faults, degraded mode, and update status?

Check intervention quality

Use realistic staffing, fatigue, workload, lighting, protective equipment, and communications. Measure successful interventions, false interventions, missed alerts, time to action, and errors during handover. Train for abnormal situations, not only routine use. If supervision is remote, include network delay, video loss, limited field of view, and simultaneous alarms.

Make production deployment observable and reversible

Before launch, integrate the system into the real operating process and test it there. NIST’s AI RMF calls for monitoring during operation, incident and error reporting, response and redress, periodic testing, and criteria for pausing, modifying, or retiring a system.

Minimum operational controls

  • Structured logs: capture model and software versions, inputs or safe summaries, decisions, confidence or uncertainty indicators, commands, overrides, alarms, and environmental context while respecting privacy and retention rules.
  • Health monitoring: track sensor validity, compute load, timing, connectivity, calibration, actuator status, drift indicators, and safety-controller state.
  • Incident process: define severity levels, notification times, evidence preservation, investigation ownership, corrective action, and communication to affected people.
  • Change control: require review, regression testing, approval, staged rollout, and rollback for model, software, configuration, hardware, and data changes.
  • Maintenance and recalibration: set intervals and out-of-cycle triggers such as impact, sensor replacement, environmental change, or unexplained performance drift.
  • Pause and retirement rules: specify objective triggers, decision authority, safe shutdown, customer or worker notification, and conditions for re-entry.

Use staged exposure

  1. Shadow or advisory mode: the system observes or recommends while a qualified person controls the physical action. Compare predictions with outcomes and collect failures.
  2. Restricted production: limit geography, speed, hours, payload, users, or task types. Add heightened staffing and daily review of incidents and near misses.
  3. Controlled expansion: widen the envelope only after evidence shows that the new conditions were tested and residual risk remains acceptable.
  4. Routine operation: continue periodic tests, monitoring, training, and reassessment; approval is not permanent if the system or environment changes.

Use the correct legal and technical standard

Actual conformity requirements depend on the system type, intended use, jurisdiction, sector, and distribution model. Product-safety law, workplace rules, transport requirements, medical-device regulation, privacy law, cybersecurity obligations, and sector standards may all apply. A voluntary AI framework does not establish legal compliance or replace a required conformity assessment.

ISO 10218-1:2025, Robotics — Safety requirements — Part 1: Industrial robots, is a bounded example. ISO describes Part 1 as covering industrial robots as machines and Part 2 as addressing integration into complete systems. Application-specific hazards, such as welding, laser cutting, or machining, are addressed during application design. Part 1 lists exclusions including service robots accessible to the public, household consumer products, medical and healthcare robots, and robots for lifting or transporting people. Therefore, do not apply ISO 10218-1:2025 to every physical AI system; identify the standard and conformity route for the actual domain.

NIST AI RMF 1.0, NIST AI 100-1, published January 26, 2023, offers a broad voluntary structure across the AI lifecycle. NIST’s AI Risk Management Framework FAQs explain its voluntary status, and the NIST AI Resource Center states that the framework is being revised. Check the current edition and any applicable sector rules when making a deployment decision.

Comparison axis Questions to answer
Hazard and residual risk What harms can occur, how severe and likely are they, which controls work, and is the remaining risk within the named tolerance?
Deployment-condition performance Does the candidate meet task criteria across the stated envelope, including edge cases and degraded conditions?
Generalization and uncertainty How does performance change outside development conditions, what remains unknown, and how does test uncertainty affect the decision?
Safe failure and recovery Can the system detect faults, reach the defined safe state, be stopped or rolled back, and recover within the required time?
Human oversight and usability Can trained operators understand limits, intervene effectively, manage workload, and hand over control?
Operational readiness Are logging, monitoring, incident handling, maintenance, recalibration, supplier support, security, and reassessment in place?

Use written evidence for every comparison. If a value is unknown, label it as unknown and treat the uncertainty as a decision factor rather than silently assuming equivalence.

Run a documented go/no-go gate

The accountable decision maker should sign a gate record that connects evidence to the defined use and risk tolerance. A useful gate contains:

  1. Scope: system version, site, users, task, operating envelope, exclusions, and deployment dates.
  2. Evidence index: test reports, data descriptions, uncertainty analysis, field-trial logs, safety analyses, security assessments, human-factors results, and independent reviews.
  3. Open-risk list: unresolved hazards, assumptions, compensating controls, owners, due dates, and conditions under which operation must stop.
  4. Operating controls: staffing, training, access permissions, maintenance, monitoring, incident response, update process, and emergency procedures.
  5. Decision: approve, approve with restrictions, defer pending evidence, or reject.
  6. Reassessment plan: review frequency and triggers such as incidents, near misses, drift, environment changes, model updates, supplier changes, or new legal requirements.

Approval with restrictions is often more defensible than a binary claim of general readiness. State exactly where, when, by whom, and under what controls the system may operate.

Recognize common reasons to pause deployment

  • Tests use convenient development conditions rather than the target environment.
  • Performance averages hide a severe but infrequent failure mode.
  • There is no measured transition to a defined safe state.
  • Operators cannot intervene within the hazard-derived response time.
  • Logs are incomplete, clocks are unsynchronized, or versions cannot be reconstructed.
  • A third-party dependency can change without notice or cannot be rolled back.
  • Safety controls depend on the same model or software path they are meant to protect.
  • Residual risk has no named owner or acceptance authority.
  • Legal, regulatory, or conformity requirements are still unidentified.
  • The system is approved for a broader environment than the evidence covers.

What the evidence does—and does not—support

NIST states: “AI systems should be tested before their deployment and regularly while in operation.” This is a framework statement, not a guarantee that testing alone makes a system safe. Testing must be tied to hazards, operating conditions, human roles, and response controls.

The available official guidance does not provide a directly applicable statistic for physical-AI deployment readiness, an incident percentage, or a universal readiness score. Avoid presenting any generic number as an authoritative pass rate. Readiness is a documented, context-specific decision that must be revisited as the system, environment, people, or rules change.
