Evaluate the exact claim, not the headline. Identify who was studied, which drug and dose were tested, what the comparator received, how long follow-up lasted, what the prespecified primary endpoint measured, how large the between-group difference was, and what harms and uncertainties remained. Then verify whether that drug and indication are FDA-approved, still investigational, or compounded. A percentage of weight lost is only one result; it does not by itself show that people lived longer, functioned better, or avoided complications.
Start by narrowing the claim
A statement such as “the new drug cuts weight by 20%” leaves out the information needed to judge whether it applies to you. Before reading a result, write down the product’s generic name, brand name if relevant, formulation, dose, intended indication, and population. Note whether the claim concerns initial weight loss, maintenance after weight loss, a complication such as diabetes, or a cardiovascular outcome.
An approval is limited to the population and use described in the regulator’s decision. For example, the U.S. Food and Drug Administration’s March 2024 announcement about Wegovy concerned reducing cardiovascular death, heart attack and stroke risk in adults with cardiovascular disease and overweight or obesity, in addition to diet and physical-activity changes. That decision did not make every possible weight-loss or cardiovascular claim about the drug true for every adult.

- Population: age, sex, starting body-mass index, medical conditions, previous treatment and geographic setting.
- Intervention: exact molecule, dose, titration schedule, formulation and background lifestyle treatment.
- Comparator: placebo, another medicine, usual care or a no-treatment group.
- Outcome: body weight, BMI, a laboratory measure, a complication, a cardiovascular event, quality of life or survival.
- Time frame: treatment duration, follow-up after stopping and whether the result is a short-term response or evidence of durability.
- Status: approved for this indication, approved for another indication, under investigation or compounded.
If an article does not identify these elements, treat its headline number as incomplete rather than as a conclusion.

Check who was studied and what the design can answer
Randomization and a credible comparator
Random assignment helps balance known and unknown differences between groups. A placebo-controlled randomized trial can support a treatment comparison under its studied conditions, but it does not automatically predict what will happen when people miss doses, stop treatment, use different background care or take the drug for many years. Masking—keeping participants and investigators unaware of assignment when practical—can reduce expectations and measurement bias.
Record the trial phase, number randomized, number who completed treatment, reasons for discontinuation and the analysis population. A result based only on participants who completed every visit may look better than one that includes everyone originally assigned to treatment. Read how missing measurements were handled and whether the analysis followed the original random assignment (an intention-to-treat approach).
Why population and duration prevent careless comparisons
The FDA-described Wegovy cardiovascular-outcomes trial randomly assigned more than 17,600 participants with cardiovascular disease and overweight or obesity. Both groups received standard-of-care treatment and lifestyle counseling, and the trial measured cardiovascular events. The adolescent Saxenda example enrolled 251 participants and lasted 56 weeks. Those sample sizes and durations answer different questions; they cannot be lined up as if they were head-to-head weight-loss tests.
| Example | Population and design details | What was measured | How to interpret it |
|---|---|---|---|
| Wegovy cardiovascular-outcomes trial | More than 17,600 randomized adults with cardiovascular disease and overweight or obesity; standard care and lifestyle counseling in both groups | Major adverse cardiovascular events | Evidence for the studied cardiovascular-risk question, not a universal estimate of weight-loss efficacy |
| Saxenda adolescent study | 251 adolescents; 56 weeks | Primary: change in BMI standard-deviation score; body-weight change was secondary | Evidence limited to that age group, duration and endpoint hierarchy |
When a report combines studies, check whether the populations, doses, background treatment and follow-up are sufficiently similar for the combined estimate to make sense. A larger trial is not automatically a better answer to a different question.
Read the primary endpoint before quoting a result
Primary, secondary and exploratory outcomes
The primary endpoint is the main outcome specified before the trial begins. It drives the principal statistical test and is usually the result on which the study’s main conclusion rests. Secondary endpoints can add useful context, but each needs its own analysis. Exploratory findings are hypotheses for further study, not established benefits.
For adult weight-reduction efficacy, FDA’s January 2025 document recommends assessing mean percentage change from baseline body weight in the drug group versus control. That document is draft guidance: it contains nonbinding recommendations and is not for implementation. It is not a final rule or a guarantee that every approved study uses exactly that analysis.

Ask whether the reported number is:
- the average change from baseline in the treatment group;
- the average change in the control group;
- the difference between groups;
- the proportion reaching a prespecified threshold, such as at least 5% loss; or
- a result from a subgroup or later analysis.
The between-group difference is generally more informative than quoting only the treatment group’s change. A placebo group may lose weight through counseling, behavior changes or trial participation, and some participants may gain weight. A treatment-group percentage without its comparator can therefore exaggerate the apparent effect.
The Saxenda adolescent example
In FDA’s 2020 announcement about an adolescent Saxenda study, the primary endpoint was change in BMI standard-deviation score. After 56 weeks, the treatment group had an average reduction of 0.23 in BMI standard-deviation score, while the placebo group had no average reduction. Average body-weight change was a secondary endpoint and was not evaluated for statistical significance: the reported averages were a 2.65% loss with treatment and a 2.37% gain with placebo.
That qualification matters. You should not present the secondary weight figures as though they were the trial’s primary, statistically tested conclusion.
Put effects in absolute and comparative terms
For event outcomes, show event rates
Relative reductions can sound large when the underlying risk is small. Include the absolute event rate in each group, the absolute difference and the relative measure when the source provides them. In the Wegovy cardiovascular-outcomes trial, major adverse cardiovascular events occurred in 6.5% of Wegovy recipients and 8% of placebo recipients. The absolute difference was 1.5 percentage points in that trial’s population and follow-up context. These are event rates for the cardiovascular indication, not percentages of body-weight loss.
Also ask how many participants experienced the event, when events were counted, and whether the analysis was time-to-event. A hazard ratio is not the same thing as an absolute percentage-point reduction. If a report gives only a relative figure, say that the absolute baseline risk was not stated rather than inferring it.
For weight, describe the comparator and distribution
Mean weight change can conceal a wide range of responses. Look for confidence intervals, the spread of individual results, the percentage reaching clinically relevant thresholds and the number who discontinued. A mean does not tell a particular person exactly how much weight they will lose. Results may also differ when participants receive intensive counseling and frequent visits that are not available in routine care.

Do not rank percentages from unrelated trials
Comparing a 15% average loss in one study with a 12% loss in another is unreliable if the studies used different populations, treatment durations, dose schedules, adherence rules, definitions of baseline and comparators. A fair comparison requires the same outcome type and attention to design differences. If those conditions are absent, describe the results separately rather than declaring a winner.
Separate weight change from clinical benefit
Direct clinical outcomes
FDA defines clinical outcomes as measures of whether people feel better, function better or live longer. Examples include a heart attack, stroke, hospitalization, ability to perform daily activities or survival. A trial that measures only scale weight has not automatically demonstrated any of those outcomes.
Surrogate endpoints
A surrogate endpoint stands in for a direct clinical outcome because it is expected to predict it. Examples can include a laboratory value or a score used as an intermediate measure. FDA says extensive evidence is generally needed to validate a surrogate in the context where it is used, including evidence that it predicts or correlates with clinical benefit. A candidate or reasonably likely surrogate carries more uncertainty than a validated surrogate.
Accelerated approval can rely on a surrogate reasonably likely to predict benefit, with post-marketing studies intended to determine whether the predicted benefit is confirmed. Do not describe a surrogate improvement as proof of longer life or better function unless the trial measured and demonstrated that outcome.
Blood pressure, cholesterol, fasting glucose and A1C can provide valuable context, including for participants with type 2 diabetes, but a change in one of these measures is not by itself evidence that patients lived longer or avoided a cardiovascular event. FDA notes that 45% of new drugs approved during 2010–2012 relied on a surrogate endpoint; that historical figure is not a current approval-rate estimate.
Examine harms, tolerability and body composition
Use the same trial’s safety data
Record adverse events in each group, serious adverse events, treatment discontinuations and deaths. Distinguish events that occurred during treatment from those reported after treatment ended. A press release may highlight efficacy while placing important tolerability details in a table or prescribing document.

A trial’s size and duration do not settle every possible harm. FDA’s safety-communications program explains that wider or longer-term use can reveal adverse effects not found in preapproval trials, and that the agency continues reviewing trial and other evidence after approval. Consult the current prescribing information and safety communications for the specific drug rather than relying on an old article.
Ask what the scale cannot show
Weight loss can include fat, lean tissue and changes in body water. FDA’s 2025 draft guidance discusses measuring body composition in a representative sample, using baseline and follow-up DXA or a suitable alternative, to assess whether reduction is primarily fat rather than lean mass. That drug-development recommendation does not allow anyone to infer an individual’s body composition from a scale reading alone.
Look for how adverse events were defined, whether they were actively solicited or simply reported, and whether follow-up continued after discontinuation. A short study can establish short-term effects while leaving uncertainty about rare events, long-term exposure and weight regain after stopping.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Verify whether the product is approved, investigational or compounded
Approved for the exact use
“FDA-approved” should be followed by the indication and population. A drug approved for adults may not be approved for adolescents; a product approved for chronic weight management may not be approved for a particular cardiovascular claim. Check the current FDA label, dose and contraindications.
Under investigation
An investigational drug or unapproved indication is being studied, not established as effective or safe for routine use. Look for the registered trial, protocol, prespecified endpoints, analysis plan and publication. Match registration dates and endpoint changes to the published report. A favorable preliminary announcement is not equivalent to a completed peer-reviewed evidence base.

FDA says compounded drugs are not FDA-approved and are not reviewed by the agency for safety, effectiveness or quality before marketing. Its consumer alert identifies warning signs such as claims that a compounded product is the same as an FDA-approved drug, no screening or prescription by a licensed clinician, suspicious pricing or packaging and dosing confusion. Patients should obtain a prescription from a doctor and use a state-licensed pharmacy.

Do not treat a study of an approved product as proof for a compounded formulation. Differences in concentration, ingredients, sterility, labeling and preparation can change both benefits and risks.
A practical workflow for checking a news claim
- Copy the precise wording. Mark whether it claims weight loss, maintenance, a complication reduction or a cardiovascular benefit.
- Identify the product. Record generic name, formulation, dose, indication and regulatory status.
- Find the study. Note phase, randomization, masking, comparator, sample size, duration and analysis population.
- Locate the primary endpoint. Read the protocol or registry entry and determine whether the headline uses a primary, secondary or exploratory result.
- Calculate the comparison. Record treatment and control averages or event rates, the between-group difference, confidence interval and any stated p-value.
- Check missing data and discontinuation. Find out how many stopped treatment and whether the main analysis included everyone randomized.
- Separate direct outcomes from surrogates. State explicitly whether the trial measured how people felt, functioned or lived, or only an intermediate measure.
- Read the harms table. Include common and serious adverse events, discontinuations, deaths and the length of safety follow-up.
- Check current regulation. Confirm the exact approved indication, current label, post-approval communications and whether the product is compounded.
- State what remains unknown. Qualify generalizability, durability, rare harms, body composition and real-world adherence.
How to compare two drug claims fairly
Use the same questions for both products and keep results from separate trials separate. The following framework prevents a larger percentage from winning by default:
| Comparison axis | Question to answer | Why it matters |
|---|---|---|
| Population and indication | Were participants the same age, BMI range and medical-risk group? Is the claimed use approved? | Results apply first to the enrolled population and authorized indication. |
| Endpoint | Was the primary outcome percentage weight change, an event, a function measure or a surrogate? | Different endpoints do not represent the same benefit. |
| Comparator and effect | What did the control group receive, and what were the absolute and between-group results? | Group differences are more informative than treatment-only change. |
| Follow-up and durability | How long were participants treated and observed, and what happened after stopping? | Short-term loss does not establish long-term maintenance. |
| Tolerability | What adverse events and discontinuations occurred in each group? | A benefit must be weighed against treatment burden and harms. |
| Evidence source | Is the result from a full trial report, regulatory review, registry or promotional summary? | More complete sources reveal methods, missing data and limitations. |
| Uncertainty | What confidence intervals, subgroup limits, endpoint changes or unmeasured outcomes remain? | Uncertainty is part of the result, not an optional footnote. |
Only after aligning these axes should you make a comparative statement. If one study enrolled adolescents for 56 weeks and another enrolled adults with cardiovascular disease for an event-driven follow-up, a direct percentage ranking is not scientifically meaningful.

- Repeating the largest number: Replace a treatment-only percentage with treatment and control results and identify the endpoint.
- Calling a secondary result “the trial’s main finding”: Name the prespecified primary endpoint and label secondary outcomes.
- Turning a surrogate into a promise: Say that the measure is intermediate unless clinical benefit was directly demonstrated.
- Generalizing beyond the sample: State age, health status, duration and background care before applying the result.
- Ignoring dropouts: Report discontinuations and the analysis population; do not assume completers represent everyone assigned.
- Using an old safety summary: Check current prescribing information and post-approval communications.
- Equating compounded and approved products: Treat formulation, quality review and labeling as separate questions.
- Comparing unrelated trials: Explain differences in population, comparator, endpoint, dose and follow-up instead of ranking percentages.
What a reliable conclusion sounds like
A careful conclusion states the result and its boundary in the same breath: “In this randomized trial, among [the studied population], the drug changed [the prespecified endpoint] by [the treatment-versus-control difference] over [the follow-up period]. The study also reported [key harms or discontinuations]. It does not by itself establish [unmeasured clinical outcome, longer duration or another population].”
That format follows the principle that clinical trials are conducted to demonstrate that new medical products deliver a positive balance of benefit and risk. The balance can be judged only when the benefit, comparator, time frame, harms and remaining uncertainty are visible together.
