Do not approve a primary-care AI tool because a vendor reports high accuracy or because a feature has a regulatory designation. Approve a defined use only after checking evidence for that use, fit with your patients and data, safety in the real workflow, clinician review, regulatory responsibilities, and a monitored plan for correcting or stopping the system.
Start with a use-specific decision, not a product score
“AI system” can mean a documentation assistant, a risk-prediction model, a triage service, an image or signal interpreter, or software that recommends a diagnostic or treatment action. Those functions create different hazards and require different evidence. A system that drafts a visit summary should not be evaluated with the same criteria as one that prioritizes patients for urgent follow-up.
Primary-care leaders should make the unit of approval a specific function in a specific setting. Define what the system does, who uses it, which patients are covered, what data it receives, what it produces, and what decision may follow. Then evaluate evidence and workflow performance against that statement. There is no official, universal pass/fail score for every primary-care AI application.

The U.S. Office of the National Coordinator for Health IT (ONC) describes clinical decision support (CDS) as a digital tool that presents timely, person-specific information at appropriate times to improve outcomes and quality. That definition highlights why context matters: timing, patient-specific inputs, presentation, and the resulting care process are part of the evaluation, not just the model’s internal metric.

Write a one-page context-of-use statement
Before asking whether a tool “works,” write down the intended use in language that a clinician, safety officer, vendor, and regulator would interpret the same way. The U.S. Food and Drug Administration (FDA) CDS policy navigator specifically emphasizes intended use and patient population as information needed for independent review. The World Health Organization (WHO) likewise treats a clear use case as a foundation for evaluating AI in health.
Record the seven essential fields
- Task: State the function precisely, such as drafting documentation, predicting readmission risk, routing messages, identifying possible medication interactions, or supporting triage.
- Intended users: Name the people who will view or act on the output: physicians, nurse practitioners, nurses, medical assistants, pharmacists, or administrative staff. Include their training and authorization.
- Patient population: Specify age ranges, pregnancy status if relevant, languages, conditions, insurance or access groups, and exclusions. Say whether the tool is for established patients, new patients, or both.
- Setting and point in workflow: Identify the clinic type, visit mode, care team, EHR context, and when the output appears. A result shown during a visit has different timing and escalation requirements from one generated overnight.
- Inputs: List required fields, data sources, units, coding systems, time windows, patient-entered information, free text, images, audio, and external data. State what happens when a field is absent, stale, contradictory, or out of range.
- Output and action: Describe the format, confidence or uncertainty display, and the clinical or administrative action that could follow. Distinguish a suggestion from an automatic order, message, referral, or alert.
- Boundaries and failure conditions: State what the system is not intended to do, when it must not be used, and who takes over when the output is missing, implausible, delayed, or disputed.
Example approval statement
“For adults seen in our primary-care clinics, the tool drafts a visit summary from the encounter transcript for clinician editing before it enters the record. It does not diagnose, recommend treatment, place orders, or generate patient instructions without clinician review. It is not used for emergency visits, pediatric encounters, or encounters conducted in languages unsupported by the validated speech model.”
This level of precision prevents a narrow validation study from being stretched into an unsupported claim about every feature or patient group.
Request evidence that matches the claim
Ask the vendor or system owner for an evidence packet tied to the context-of-use statement. WHO’s 2023 regulatory considerations identify analytical validation, clinical validation, and assessment of bias or discrimination as core considerations. The WHO framework for AI-based medical devices also treats evidence as a lifecycle activity from development through deployment and surveillance.
Evidence to request
- Algorithm description: A plain-language explanation of the model or algorithm, its intended output, major assumptions, and any rules or thresholds around it.
- Development data: Data sources, dates, sites, inclusion and exclusion criteria, labeling or reference standard, preprocessing, missing-data handling, and whether protected or sensitive attributes were used.
- Validation design: Separation of development and validation data, external sites, prospective or retrospective design, sample composition, follow-up period, and how the reference standard was established.
- Results: Metrics appropriate to the task, confidence intervals or uncertainty where available, calibration, subgroup results, and the rate and consequences of false positives and false negatives.
- Input requirements: Required fields, acceptable ranges, data freshness, supported devices or interfaces, language limitations, and behavior when inputs are incomplete or out of distribution.
- Clinical and operational limitations: Known failure modes, populations not studied, conditions that reduce performance, latency, downtime behavior, and examples of outputs that require escalation.
- Version history: Model, data, threshold, interface, and intended-use changes; release dates; validation performed for each material change; and the notification process for customers.
- Security and data handling: Where data are processed, retention and deletion rules, access controls, audit logs, and how a practice can retrieve its records for investigation.
Interrogate performance numbers
Never treat one aggregate accuracy figure as a complete evaluation. Ask what the metric means for the proposed decision and population. A prevalence-dependent positive predictive value can change when the clinic’s disease prevalence differs from the study population. A high sensitivity may come with a false-positive burden that overwhelms a small care team. A low average error rate can conceal unacceptable failures in a subgroup or in rare but dangerous conditions.
Ask these questions for every reported metric:
- What outcome, time horizon, and reference standard define a “correct” result?
- Which patients and sites contributed data, and what prevalence did they represent?
- Was the test set isolated from model development, tuning, and repeated evaluation?
- What are the false-positive and false-negative rates at the proposed operating threshold?
- How does the result change when data are missing, delayed, contradictory, or collected with another protocol?
- Were confidence intervals, calibration, and clinically meaningful decision thresholds reported?
- Which relevant patient groups were not represented or had too few cases for a reliable estimate?
Official guidance does not establish a single numerical threshold for all primary-care AI. The practice must decide what error consequences are acceptable for the defined use and document that rationale.

Check whether the evidence applies to your patients
Performance can change when a model moves between sites, populations, acquisition systems, clinical protocols, or documentation habits. FDA materials on postmarket monitoring warn that changed conditions and out-of-distribution inputs can produce unexpected outputs. A validation study at a tertiary hospital may not represent a community clinic, and a model trained on complete structured data may behave differently where histories are sparse.

- Age, sex, pregnancy status, race and ethnicity where relevant to the use, disability, and socioeconomic context.
- Languages, literacy, interpreter use, and communication channels.
- Prevalence and severity of the conditions the tool detects or predicts.
- Comorbidities, polypharmacy, and care complexity.
- New versus established patients, preventive visits versus acute visits, and telehealth versus in-person encounters.
- EHR version, coding practices, laboratory interfaces, device types, and data completeness.
Request subgroup evidence for groups that matter to your practice and identify groups for which evidence is absent. A reported subgroup analysis does not prove fairness for every relevant group or for your local site. Record what was studied, what was not, and what local testing can reasonably answer.
Probe missing and abnormal data
Use test cases with blank fields, stale medication lists, conflicting dates, unusual units, duplicate records, and values outside the development range. Confirm that the system flags insufficient information instead of silently filling gaps. Ask whether an out-of-distribution detector exists, what it displays, and what the clinician should do when it triggers. If the vendor cannot explain behavior for a required input, treat that uncertainty as a deployment risk rather than assuming the model will “fail safely.”
Run a controlled workflow evaluation before patient impact
ONC guidance says CDS should be clear, organized, and fitted to the provider’s workflow. ONC’s SAFER Guides include AI-enabled EHR systems and address configuration, validation, maintenance, implementation, testing, and monitoring. Evaluate the complete care process in a sandbox or other environment that cannot affect patient records, orders, or messages.
Use representative cases
Assemble de-identified or synthetic cases covering routine encounters, edge cases, common comorbidities, incomplete data, language variation, and the highest-consequence failure modes. Have representative users perform the actual steps they would use in production. Keep a record of the input, output, time, user interpretation, action taken, and any disagreement.
Check the workflow in order
- Input mapping: Verify that the system receives the intended patient, encounter, medication, laboratory, and demographic data. Test wrong-patient, duplicate, delayed, and unmapped fields.
- Availability: Measure whether required information is present at the moment the output appears. A theoretically accurate model is not useful when its inputs arrive after the decision.
- Presentation: Check readability, terminology, units, timestamps, confidence or uncertainty, and visibility of limitations. Ensure the output is distinguishable from a clinician-authored fact.
- Reviewability: Confirm that a clinician can inspect the relevant patient information, model basis or contributing factors where provided, and source text before accepting a recommendation. FDA’s CDS criteria use the principle that professionals should be able to “independently review the basis for such recommendations.”
- Action and escalation: Test acceptance, editing, rejection, referral, order, and emergency-escalation paths. Define who is notified and within what time.
- Disagreement: Give users plausible but wrong outputs and observe whether they recognize, correct, and document the discrepancy.
- Downtime: Disable the service, delay responses, and remove an upstream data feed. Verify that care continues with a documented fallback.
- Communication: Check how output status and clinician edits appear to other team members and, where applicable, in patient-facing materials.
Measure care-process effects
Choose local measures appropriate to the use: completion time, interruptions, alert volume, clinician overrides, documentation correction time, follow-up completion, duplicate work, communication failures, near misses, and safety events. ONC notes that CDS can affect efficiency, errors, outcomes, and cognitive or administrative burden, but its resources do not prescribe universal numerical targets. Set your own baseline and review period before testing so that “better” is defined in advance.

Design safety controls and keep a human accountable
Human review is not a checkbox. It must be feasible under actual time pressure, with enough information to detect a wrong answer and clear authority to override it. For each output, name the accountable clinician or team, the required review step, the documentation of acceptance or correction, and the escalation route for uncertainty.
Minimum safety controls
- Display the intended patient and encounter prominently.
- Show data timestamps and identify missing or stale inputs.
- Make uncertainty, confidence limits, and known exclusions visible where the system provides them.
- Prevent automatic orders, diagnoses, or patient messages unless the approved use explicitly requires them and includes appropriate controls.
- Provide a simple override or rejection action and capture the reason when clinically useful.
- Define urgent, unsafe, and unavailable states with a human escalation path.
- Maintain an audit trail of input version, output, user, edits, and downstream action.
For documentation assistance, verify that the clinician can compare the draft with the encounter and that unverified statements cannot be mistaken for observed facts. For triage or risk prediction, ensure that a low score never suppresses evaluation when symptoms or clinical judgment indicate danger.
Review regulatory status without treating it as a quality certificate
In the United States, FDA’s Clinical Decision Support policy navigator describes four statutory criteria relevant to certain non-device CDS functions, including information that enables a healthcare professional to independently review the basis for a recommendation. Whether a feature falls within that exclusion or is subject to FDA oversight depends on its exact function and intended use. A determination for one function does not automatically cover every feature in a product.
FDA’s digital-health guidance listing marks Clinical Decision Support Software as final, issued January 29, 2026. Use the current FDA materials to document the function-specific analysis, but do not present a regulatory status as proof that a tool is accurate, equitable, or suitable for your clinic. Regulatory review and clinical readiness answer different questions.
The FDA material summarized here is U.S.-specific and is not a legal determination. Practices in other countries must identify their national regulator and applicable rules. WHO’s 2023 regulatory considerations provide a broad, nonexclusive perspective rather than a binding jurisdiction-specific pathway.
Compare two systems with the same cases and criteria
If you are choosing between systems, freeze the context of use first. Give each option the same representative cases, data conditions, users, and workflow. Set required criteria before seeing scores, and document why a risk is acceptable or unacceptable. Do not name a winner from one composite score when the systems create different harms for different patient groups.

Plan monitoring before launch
Validation answers whether the system worked under a study design. Local evaluation answers whether it fits your people and workflow. Monitoring answers whether it remains safe as those conditions change. Establish all three separately.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsDefine a monitoring charter
- Baseline: Record pre-launch workflow, outcome, and safety measures.
- Frequency: Set review intervals based on clinical risk, volume, and how quickly the tool can change.
- Owner: Name a clinical owner, technical owner, safety or quality contact, and vendor contact.
- Signals: Track input distributions and missingness, output quality, ground-truth performance when it becomes available, subgroup patterns, overrides, complaints, adverse events, near misses, and workflow changes.
- Triggers: Define conditions that require investigation, temporary restriction, revalidation, or immediate pause. Use locally justified thresholds rather than implying a universal benchmark.
- Change control: Require notice of model, threshold, input mapping, interface, data-source, or intended-use changes. Decide what regression testing each type of change requires.
- Response: Maintain an incident-reporting route, investigation records, communication plan, and tested rollback or manual fallback.
FDA’s postmarket monitoring work explains that model utility can shift as systems, protocols, populations, and sites change. ONC SAFER resources likewise include ongoing maintenance and monitoring. Treat a vendor update as potentially material until you establish whether it changes model behavior, not merely as a harmless software patch.
Use explicit decision gates
A practical approval record can use four gates:
- Definition gate: The context-of-use statement, exclusions, accountable users, and fallback process are complete.
- Evidence gate: Validation methods, populations, limitations, and error consequences are sufficient for the clinical stakes; unresolved gaps are documented.
- Local readiness gate: Representative-case and workflow testing show that inputs map correctly, users can review and override outputs, and safety controls work under failure conditions.
- Lifecycle gate: Monitoring, incident response, change control, ownership, regulatory review, and pause or rollback procedures are funded and assigned.
Approval can be conditional: limit the tool to a studied population, require clinician sign-off, run a silent or draft-only phase, or prohibit use for specific encounters. A “not yet” decision is appropriate when the practice cannot detect harmful errors, cannot supply required data, or has no owner for monitoring.
Common evaluation mistakes and their fixes
“The demo looked accurate.”
Fix: Replace demonstrations with representative, adversarial, and incomplete cases scored against a defined reference standard.
“The regulator cleared it, so it is safe here.”
Fix: Analyze the exact function, intended use, population, and jurisdiction. Regulatory status does not substitute for local validation or workflow testing.
“The average metric is high.”
Fix: Examine calibration, prevalence, false-positive and false-negative consequences, subgroup evidence, and out-of-distribution behavior.

What a defensible approval file contains
Keep one version-controlled record containing the context-of-use statement; vendor evidence and unanswered questions; local population and equity assessment; representative test cases and results; workflow and downtime findings; safety controls; accountable roles; U.S. or other applicable regulatory analysis; monitoring measures and thresholds; change-control terms; incident and rollback procedures; and the final approval, restriction, or rejection rationale.
This file lets the practice explain not only why it adopted a tool, but also what it promised clinicians and patients, what evidence supported that decision, and what conditions would require reassessment. The appropriate endpoint is not a generic claim that an AI system is “accurate.” It is a documented determination that one defined function is sufficiently useful, reviewable, and controllable for one defined primary-care use, with a way to detect when that determination stops being true.
