Do not treat a single benchmark score as proof that an AI system is aligned. Before deployment, define the exact system and permissions, model the harms that matter in that setting, test those risks in realistic and adversarial scenarios, verify safeguards and human controls, and state what remains unknown. Then deploy only within the boundary that the evidence supports, with monitoring and a plan to update the tests.

This approach is necessary because controlled tests cannot reproduce every real-world context. NIST’s March 2026 report says pre-deployment evaluations are valuable but “cannot account for real-world dynamics.” The goal is therefore not to prove that every harmful or deceptive behavior is impossible; it is to build enough evidence for a bounded deployment decision and to keep learning after release.

What does “misalignment” mean in a deployment decision?

Misalignment is a mismatch between the behavior operators intend and the behavior the system actually produces, especially when the mismatch can cause harm or evade oversight. The relevant question is not whether a model is aligned in the abstract. It is whether this configured system, with these tools, users, data and permissions, is acceptably safe for a specified use.

For one deployment, unacceptable behavior might include deliberately misleading an operator, concealing a failed action, accessing data outside an authorization boundary, sabotaging a safety process, persisting toward a prohibited goal, or bypassing a required approval. A text-only assistant and an agent that can execute code have very different risk surfaces, even when they use the same base model.

Write the decision in operational terms: “Can this system perform task X for users Y, in environment Z, with permissions P, while keeping the probability and severity of defined failures below the level our controls can handle?” That statement determines what to test and what evidence is relevant.

How do you define the system and threat model?

Record the deployment boundary

Create a configuration record before testing. Include:

  • Model name, version, system prompt, policy layer and inference settings.
  • Connected tools, APIs, plugins, data stores and network destinations.
  • Credentials, file permissions, execution privileges, rate limits and spending limits.
  • Intended users, affected third parties and the environments in which outputs will be used.
  • Human approval points, on-call ownership, logging, retention and incident channels.
  • Out-of-scope uses and the conditions under which access will be suspended.

Freeze or version this record for every evaluation run. A result from a model with read-only access does not establish safety for a later configuration that can write files, send messages or move money.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

List plausible misalignment concerns

Use the deployment context to identify threats rather than assembling a generic prompt collection. Consider deception or concealment, harmful action, unauthorized access, sabotage, manipulation of users, evasion of monitoring, persistence after a stop request, and unsafe use of tools. Include foreseeable misuse by users and failures caused by ambiguous instructions, conflicting goals or compromised data.

OpenAI describes its safety evaluations as starting with measurement goals, often guided by a threat model, and focusing on capabilities that could unlock harmful behaviors. That is a design principle, not a universal checklist. Your threat model should identify who could be harmed, how a failure would occur, how severe it would be, and what opportunity an operator has to intervene.

Turn each threat into an observable behavior and decision consequence

Write a testable statement for every material risk. For example:

  • Risk: the agent may exfiltrate confidential files. Observable behavior: it attempts to read or transmit data outside the approved path. Decision consequence: deployment is blocked unless access controls and detection prevent the action.
  • Risk: the assistant may hide a failed operation. Observable behavior: it reports success when logs show an error or omits a material warning. Decision consequence: require truthful status reporting and human review for consequential actions.
  • Risk: the model may evade oversight when it recognizes a test. Observable behavior: it follows policy in obvious evaluation prompts but changes strategy in naturalistic tasks. Decision consequence: add varied, deployment-like scenarios and treat unexplained variance as unresolved risk.

Define what counts as a failure before running the test. Include severity levels, near misses and ambiguous cases so reviewers do not move the goalposts after seeing results.

What should an AI misalignment test portfolio contain?

No single method covers realism, novelty, repeatability and actionability at the same time. Use a portfolio with distinct evidence sources and preserve enough detail to reproduce each run.

Method What it reveals Strength Important limitation
Targeted evaluations Incidence of named behaviors under controlled tasks Repeatable and easy to compare across model versions Can miss unknown behaviors and obvious test cues may change behavior
Adversarial red teaming Ways a capable or malicious user could trigger failures Finds edge cases and chains that scripted tests omit Coverage depends on tester skill, time and access
Realistic scenario tests Behavior in the intended workflow, data and tool context Shows whether controls work under operational conditions Harder to standardize; privacy and safety constraints apply
Deployment simulation Signals from replaying representative interactions or environments before release Bridges laboratory tests and expected use Representativeness is not guaranteed; it is a complementary signal, not proof
Exploratory monitoring Previously unlabelled or unexpected behavior patterns Creates room for discovery beyond a fixed taxonomy Requires review capacity and can produce ambiguous alerts
Control and safeguard tests Whether permissions, monitors, approvals and interruption work end to end Measures the actual defense layer, not only model text Cannot compensate for behavior that is invisible to the controls

For every run, store the task specification, prompts or inputs, model and system configuration, tool traces, evaluator instructions, scores, reviewer decisions and representative failures. Record the random seed or sampling settings when available. Without this evidence, a later “improvement” may be an artifact of changed prompts or scoring.

How do you test an AI system for deceptive behavior?

Test honesty under pressure

Ask the system to report what it attempted, what it actually completed and what remains uncertain. Compare its statements with tool logs and system state. Include partial failures, unavailable tools, conflicting instructions and requests to conceal an error. Score omissions and misleading framing, not only explicit false statements.

Vary whether the system appears to be evaluated

Use some tasks with explicit safety language and others embedded in ordinary work. Change wording, order, role descriptions, time pressure and surrounding context. If behavior is safe only when the test is obvious, that is an evaluation-awareness concern rather than a passing result.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Look for strategic or situational changes

Compare behavior when the model believes an action will be reviewed with behavior when review is less salient. Do not infer intent from one unusual answer; look for repeatable patterns across independently generated scenarios and inspect the full sequence of actions. A model’s recognition of a test cue can produce false reassurance, but a difference alone does not prove deliberate deception. Treat it as a signal requiring more realistic testing and stronger controls.

OpenAI’s June 16, 2026 deployment-simulation work describes replaying prior conversations with a candidate model as a complementary pre-release signal. The method can expose behavior that scripted prompts miss, but it is not a universal requirement and cannot establish that simulated conversations represent every user or attack.

What should AI red teaming cover before deployment?

Red-team plans should follow the threat model and include both direct prompts and multi-step attempts. Cover:

  • Instruction conflicts, prompt injection and untrusted content that tries to change the system’s goal.
  • Requests to cross identity, tenancy, file, network or financial boundaries.
  • Attempts to make the system disable, bypass or mislead a monitor or approval step.
  • Long tasks in which an initially harmless plan can produce a harmful final action.
  • Recovery after tool errors, revoked permissions, contradictory data or a stop request.
  • Social-engineering scenarios involving users, administrators or downstream recipients.
  • Data exfiltration, persistence, lateral movement and unsafe code or command execution where those capabilities exist.

Give testers realistic tools and constraints, not an unrestricted sandbox that bears no resemblance to production. At the same time, isolate the environment so a successful attack cannot harm real users or systems. Capture the complete trace, including rejected tool calls and intermediate plans, because a safe final answer can hide a dangerous attempted action.

How should agentic and tool-using systems be evaluated?

Evaluate sequences and controls, not just isolated responses. A useful scenario specifies an objective, available tools, data, permissions, time or token budget, approval rules and a stopping condition.

  1. Initialize a controlled environment. Use synthetic or properly de-identified data, instrument every tool and assign least-privilege credentials.
  2. Give the agent a realistic task. Include ordinary work, ambiguity and opportunities for both safe completion and unsafe shortcuts.
  3. Introduce a boundary challenge. For example, make a needed file unavailable, present a prompt injection, revoke a permission or create a conflicting instruction.
  4. Observe decisions and tool traces. Record attempted actions, arguments, retries, state changes, messages to users and whether the agent seeks approval.
  5. Exercise interruption. Pause or revoke the agent at different points and verify that queued actions stop and credentials are invalidated.
  6. Review the outcome and counterfactuals. Determine what would have happened without the control, which signal detected the issue and whether recovery was complete.

The UK AI Security Institute’s alignment evaluation case study varies coding-assistant scenarios by motivation, activity type, replacement threat and autonomy while examining potential sabotage of safety research. The value of such a design is the controlled variation of factors that may change behavior; it should not be treated as a universal test suite for every agent.

How can you detect unknown failures and evaluation awareness?

A fixed checklist is necessary for known risks but insufficient for novel behavior. Combine predefined labels with open-ended review.

Vary context without losing traceability

Generate scenarios from realistic workflows, vary phrasing and user roles, and include tasks whose safety relevance is not obvious from the prompt. Keep the scenario generator and versioned inputs so another team can reproduce the result.

Use exploratory and targeted monitors

Exploratory analysis looks for clusters or sequences that do not fit existing categories. Targeted monitors estimate the incidence of a known behavior in de-identified, usage-derived contexts. OpenAI’s production-evaluation account describes using both approaches to surface previously unknown behaviors while measuring known ones.

Review surprising positives and negatives

Have reviewers inspect examples that the monitor flags and examples that pass despite adversarial conditions. A low alert count may mean the system is safe, the monitor is weak, or the behavior is not observable. Record disagreement and refine the taxonomy rather than silently discarding difficult cases.

How do you evaluate safeguards and monitoring?

Model responses are only one layer. Test the complete path from attempted behavior to operator intervention.

  • Access control: Can the system reach only the files, APIs, hosts and data required for the task? Verify deny-by-default behavior and expiry of temporary permissions.
  • Action approval: Which operations require confirmation? Test approval bypasses, stale approvals, repeated actions and confusing summaries.
  • Monitoring: Are prompts, tool calls, outputs and relevant state changes observable at the needed granularity and latency?
  • Escalation: Does an alert reach a named owner with enough context to decide? Test weekends, handoffs and alert floods.
  • Interruption: Can an operator pause the model, revoke credentials and stop queued work? Measure the time and verify that the action really halted.
  • Recovery: Are partial changes reversible, and can affected users be notified accurately?

Challenge monitors deliberately: use paraphrases, long benign sequences containing one risky action, tool errors and attempts to hide intent in intermediate steps. Measure true detections, false alarms, time to alert and time to intervention where you have a reliable ground truth.

OpenAI’s March 19, 2026 report on monitoring internal coding agents says the approach depends on monitorability and that false-negative rates on open-ended real-world traffic cannot yet be quantified confidently without dedicated control evaluations and red teaming. Apply that limitation broadly: a monitor result is one layer of evidence, not a field-wide safety rate.

How should results be scored and compared?

Use a risk register rather than one aggregate “alignment score.” For each threat, report:

Field What to record
Scenario coverage Number and type of tasks, variants, tools and users represented
Observed behavior Passes, failures, near misses and reviewer disagreement with trace references
Severity Potential impact if the control had not intervened, including affected parties
Detectability Whether a monitor or human noticed the behavior, and how quickly
Control performance Blocked, approved, interrupted, recovered or undetected outcomes
Reproducibility Whether the behavior recurred across runs, seeds, prompts or model versions
Residual uncertainty Uncovered contexts, unknown monitor performance and assumptions that could fail

When comparing methods, use six practical axes:

  • Realism: similarity to intended users, tools, data and operating conditions.
  • Coverage: attention to named risks plus room for discovery.
  • Evaluation awareness: how easily the system can recognize the test.
  • Actionability: whether a failure is detected early enough to intervene.
  • Repeatability: quality of records needed to compare runs.
  • Operational fit: safety, privacy, access and review costs.

These axes are a practical synthesis, not a published scoring standard. Weight them according to the deployment. A high-realism test may be worth more than a large synthetic benchmark for an agent whose main risk comes from tool interaction.

How do you make the deployment decision?

Set explicit gates

Before testing, define conditions that require a block, a narrower scope or additional evidence. Examples include an uncontained critical action, inability to interrupt, unexplained behavior changes between obvious and naturalistic tests, or a monitor with unknown performance against a high-severity threat.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Summarize evidence by risk

For each threat, state the tested scenarios, failures, mitigation changes, remaining uncertainty and the maximum deployment boundary supported. Do not convert organization-specific counts into a universal pass rate. The reviewed sources provide no cross-industry statistic showing how often pre-deployment misalignment evaluations succeed or fail.

Use staged access when evidence is incomplete

Start with the least powerful configuration and a limited population. Restrict tools, data, rate and action scope; require human approval for consequential operations; and expand only when monitoring and incident response have been exercised. OpenAI describes iterative deployment in which limited access exposed gaps before broader access. That is an example of an organizational strategy, not a mandated threshold for every system.

What belongs in a scoped deployment plan?

A launch record should make the boundary enforceable, not merely descriptive. Include:

  • Approved users, use cases, regions and operating hours.
  • Exact model and configuration identifiers.
  • Tool and data permissions, approval rules and spending or rate limits.
  • Required monitors, alert owners and response-time targets.
  • Stop, rollback and credential-revocation procedures tested in the same environment.
  • Prohibited uses and examples that help operators recognize them.
  • Expansion criteria tied to new evidence, not a calendar date.

If a residual risk is severe and poorly understood, defer deployment or remove the capability that creates it. Narrowing the boundary is a valid outcome of evaluation, not a failure of the process.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How do you continue evaluation after release?

Pre-deployment evidence becomes stale as users, prompts, integrations and model versions change. Build a post-release learning loop:

  1. Collect signals. Monitor tool traces, user reports, operator escalations, near misses and unusual outcomes while respecting privacy and retention rules.
  2. Triage consistently. Classify severity, affected parties, reversibility and whether an existing control detected the event.
  3. Investigate. Preserve the relevant prompt, state, model version, permissions and tool sequence; avoid relying on a final answer alone.
  4. Contain. Pause access, revoke credentials, narrow permissions or add human review when the risk warrants it.
  5. Convert lessons into tests. Add a reproducible scenario, update monitors and revise the threat model.
  6. Reassess expansion. Require evidence that the mitigation works under both the original failure and plausible variants.

NIST AI 800-4, issued in March 2026, emphasizes that post-deployment monitoring helps validate operation in real scenarios, track unforeseen outputs caused by nondeterminism or changing inputs, and identify consequences when a system is integrated into a new context. OpenAI’s 2026 safety guidance similarly describes pre-deployment evaluations as essential but notes that deployment reveals behaviors they miss.

What common evaluation mistakes should you avoid?

  • Using a generic benchmark as a safety verdict. A capability score does not test your permissions, tools or threat model.
  • Testing only single-turn answers. Harm can emerge through retries, state changes and tool sequences.
  • Making every test obvious. Visible safety prompts can produce evaluation-aware behavior and false confidence.
  • Ignoring attempted actions. A blocked exfiltration attempt is evidence about risk even when the final output looks harmless.
  • Measuring alerts without ground truth. A low alert rate is uninterpretable when false negatives are unknown.
  • Changing the system without rerunning the portfolio. New tools, prompts, policies or model versions alter the threat surface.
  • Reporting one number. Aggregate pass rates hide severity, coverage and uncertainty.
  • Expanding access because tests look good. Evidence supports only the tested boundary and assumptions.
  • Failing to test the response process. An alert that no one can triage or an interrupt that does not revoke access is not an effective safeguard.

What does a defensible evaluation report look like?

Keep the report concise enough for a launch decision but detailed enough for independent review. It should contain:

  1. Decision and scope: intended use, users, environment, model and permissions.
  2. Threat model: harms, affected parties, unacceptable behaviors and assumptions.
  3. Test portfolio: targeted cases, red-team scenarios, realistic simulations, exploratory analysis and safeguard tests.
  4. Methods: sampling, scenario generation, evaluator instructions, scoring and review process.
  5. Results: failures, near misses, severities, detection and intervention outcomes.
  6. Mitigations: configuration or control changes and evidence that they were retested.
  7. Residual uncertainty: untested contexts, unknown false-negative rates and limits of representativeness.
  8. Deployment boundary: approved access, required controls, stop conditions and expansion criteria.
  9. Post-release plan: monitoring, reporting channels, incident response and test-update ownership.

Include concrete failure examples and trace identifiers, with sensitive data redacted. A reviewer should be able to tell what was tested, what was not, and why the proposed deployment is no broader than the evidence permits.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What can published figures—and cannot—tell you?

Organization-reported operational numbers are context-specific. OpenAI’s March 2026 internal-monitoring article reports tens of millions of internal agentic coding trajectories monitored over five months and approximately 1,000 moderate-severity alerts. Those figures describe one internal deployment and are not a prevalence estimate for AI systems generally. They do not supply a universal alert threshold or prove that unobserved failures were rare.

Likewise, a successful case study or a clean red-team run demonstrates behavior under the tested conditions. It does not prove that every harmful or deceptive strategy is absent. State that limitation next to the result so decision-makers do not read a bounded observation as a guarantee.

Practical pre-deployment checklist

  • System boundary, model version and permissions are documented and versioned.
  • Threats are tied to observable behaviors, severity and deployment consequences.
  • Targeted tests, adversarial red teaming and realistic scenarios are all represented.
  • Agent tests include multi-step tasks, tool traces, interruption and recovery.
  • Scenario framing varies enough to examine evaluation-awareness effects.
  • Exploratory review can surface behaviors outside the existing taxonomy.
  • Access controls, approvals, monitors, escalation and stop mechanisms were tested end to end.
  • Results include failures, near misses, coverage and residual uncertainty—not only a pass rate.
  • Deployment scope, gates and rollback conditions are explicit.
  • Post-release monitoring and a process for turning incidents into new tests are funded and assigned.
