To evaluate an AI model for secure use with untrusted documents, test it inside the application that will process those documents—not just in a standalone chat. Put malicious instructions through the real upload, extraction, retrieval, filtering, and context-assembly path, then check whether the system discloses information, misleads users, or takes unauthorized actions. The results apply only to the model version and application configuration you tested.
Why untrusted documents can change an AI system’s behavior
A document can contain ordinary information and instructions aimed at the model. If an application places that content in the model’s context, the model may treat an instruction inside the document as a direction to follow. This is indirect prompt injection: the attacker’s instruction arrives through external content rather than as a direct user prompt.
That content might come from a user-uploaded file, a retrieved passage, a web page, an email, a tool response, or a summary generated earlier in a workflow. Storing a document or receiving it from a third-party service does not make its contents trustworthy. OWASP’s LLM01:2025 guidance describes prompt injection as a vulnerability in how models process prompts and how input can influence other parts of a system, potentially leading to guideline violations, harmful output, unauthorized access, or decisions being influenced.
The consequences depend on the application’s agency and business context. A system that only summarizes public material has a different exposure from an agent that can search private records, send messages, update business data, or approve transactions. Evaluation should therefore measure potential harm in the intended workflow, not simply whether a model produces a suspicious answer.

A general safety score or a direct-prompt test cannot establish that a document-processing system is secure. The application’s prompts, extraction logic, retrieval settings, permissions, tools, and safeguards all affect what happens. A useful test recreates those conditions and observes both the model’s response and the system’s side effects.
Define the deployment and the harms before testing
Start by mapping how outside content reaches the model. Include every route that exists in the planned deployment, not just the most obvious file-upload control. A malicious instruction may enter at one stage and influence a later stage, such as when a retrieved passage is summarized before an agent receives it.
- Content routes: uploaded documents, extracted text, retrieval chunks, web pages, emails, tool outputs, and earlier model-generated summaries.
- Assets at risk: confidential files, personal information, credentials, internal instructions, business records, and data the current user is not authorized to see.
- Possible effects: disclosure in an answer, misleading recommendations, unauthorized access, or a tool action that changes or shares information.
- Existing controls: input filters, retrieval restrictions, tool permissions, parameter checks, approval steps, alerts, and logging.
For each route, record what the application is supposed to do and what it must not do. Be specific about the user’s authorization and the agent’s allowed actions. For example, distinguish “summarize documents the user can access” from “search every document in the company.” That distinction determines whether a result is a security failure.

OWASP notes that indirect-injection likelihood depends in part on an attacker’s ability to manipulate external data before ingestion; the attacker may not be the person currently using the application. Your threat model should account for that separation. A test that assumes the attacker controls the current user prompt misses the case where an unsuspecting user asks a harmless question about a poisoned file.
Build an attack set that reflects the workflow
Create a collection of test documents and related inputs that represent realistic attacks against the assets and actions you identified. Do not rely on one dramatic example. Different formats and phrasings can behave differently, and a system that handles one attack string may still fail on a variant.
Include distinct attack patterns
- Instruction override: text in a document tells the model to ignore prior directions or change its task.
- Role confusion: content poses as a system message, administrator, policy, or trusted instruction.
- Hidden or low-visibility text: instructions are placed in content that a person may not notice but that extraction or OCR can pass to the model.
- Malicious links or references: content tries to direct the model or a tool toward an unsafe destination or action.
- Data extraction: instructions ask the model to reveal protected context, private records, or information unrelated to the user’s request.
- Formatting and encoding variants: the same intent appears with altered spacing, wording, layout, or encoding.
Adapt each pattern to the actual product. If the application processes scanned forms, include text that enters through OCR. If it retrieves short passages, test instructions that remain meaningful when a document is split into chunks. If an agent consumes tool output, include adversarial text in that output route as well as in uploaded files.

Use benign controls
Add ordinary documents that contain imperative language but are not attacks, as well as documents with no relevant instructions. These controls help reveal whether a filter or model is merely rejecting unusual text rather than distinguishing malicious instructions from legitimate document content. They also show whether a mitigation breaks normal tasks such as following a recipe, summarizing a policy, or extracting a requested procedure.

Keep a record of each test item’s intended attack, source route, relevant asset, and expected safe behavior. A test case should make clear what counts as a failure: for instance, revealing a protected value, following a document’s instruction to send data, or giving the user a false account of what a file says.
Run tests through the production-equivalent path
Submit the attack set using the same mechanisms and configuration that the deployed product uses. That means the same document upload or retrieval path, file extraction, filters, detection, context assembly, prompts, model version, tools, and permissions. A direct chat prompt containing the attack is not a valid substitute for an indirect-injection test because it bypasses the route under evaluation.
- Prepare the test environment. Use a controlled environment with representative data and the production-equivalent configuration. Avoid exposing real sensitive information unnecessarily; use suitable test data for protected assets.
- Ingest the test documents normally. Upload files, make them available to retrieval, or introduce content through the relevant integration. Do not paste the malicious text into a different interface merely to simplify testing.
- Trigger ordinary user tasks. Ask the questions a legitimate user would ask, such as requesting a summary, locating a fact, or drafting a response based on the document.
- Exercise each external-content route. Include retrieval results, extracted text, tool responses, and generated summaries wherever they can enter model context.
- Preserve evidence. Record the document version, retrieved passages, relevant system and user prompts, model version, tool calls, configuration, visible output, and application state needed to explain the result.
This setup tests the combined system, which is what users and attackers encounter. It also helps identify whether a failure came from document extraction, retrieval, a filter, the model’s behavior, a tool permission, or an application action.
Measure outcomes and side effects, not detector alerts alone
A filter or alert firing is useful evidence about a control, but it does not by itself show that the system prevented harm. Conversely, a test may produce a harmful result even if no detector reports an issue. Inspect what the model said and what the application did.
- Instruction-following failure: Did the model follow an instruction embedded in external content instead of treating it as untrusted material?
- Disclosure: Did the answer or a tool call expose sensitive information the user was not authorized to receive?
- Misleading output: Did the system present fabricated, distorted, or attacker-directed information as a trustworthy summary or recommendation?
- Unauthorized action: Did the system propose or execute a tool action that was not permitted by the user’s request or the application’s policy?
- Control response: Was the event blocked, escalated, logged, or surfaced to an operator in a way that reduced its impact?
Inspect tool-call arguments and resulting application state, not just the final text shown to the user. An agent might quietly retrieve a protected record, create a draft, or alter data even if its visible response looks harmless. Judge the severity in the context of the application: the same behavior can have very different consequences depending on the data exposed and the action enabled.

For consistent review, define failure criteria before running tests. A team can use its own severity categories—for example, distinguishing a suspicious but contained response from confirmed data disclosure or a completed unauthorized action—but should document the definitions rather than imply they are a universal standard. Record mitigations and alerts alongside the outcome so that a blocked attempt is not confused with an unobserved success.
Repeat runs and compare candidates fairly
Model behavior can vary between runs. Repeat test cases and include variations in wording, formatting, or encoding. One successful refusal does not demonstrate reliable resistance; one failure is important evidence but may not characterize the full behavior of the system.
When comparing models or system configurations, keep the conditions constant: use the same attack set, model versions as identified, prompts, tools, permissions, application settings, and repeated-run procedure. If a candidate uses a different safeguard or prompt, treat it as a different system configuration and document the change. Otherwise, a difference in outcome may be attributed to the model when it actually came from another control.
Recommended Free Tools
Useful comparison dimensions include attack success and harm severity; consistency across repeated and varied inputs; performance when attacks arrive through documents, retrieval, or tool outputs; and the effectiveness of permission, validation, approval, and detection controls. For retrieval systems, include the degree to which ingestion integrity and provenance are covered.
Report the test scope with the results. State what was tested, the relevant model and configuration, which content paths were exercised, how many and what kinds of cases were included, how runs were repeated, and what outcomes counted as failures. A result from a limited attack set is evidence about that tested setup, not a universal safety ranking of models.
Test the surrounding security controls
Model behavior is only one part of the security boundary. OWASP’s guidance and the OWASP LLM Verification Standard v2.0 emphasize controls around untrusted content and tool access. Evaluate whether an attack that influences the model can actually reach sensitive data or cause a consequential action.

Limit privileges and tool access
Give the model and its tools only the permissions required for the task. Separate access to read information from authority to send, modify, delete, approve, or publish it. Restrict agent tools to the narrowest useful set, and check access at the application or service boundary rather than relying on the model to remember a rule in its prompt.
Validate tool parameters
Check tool arguments before execution. Validate destinations, recipients, record identifiers, operation types, and other security-relevant values against application policy. A well-formed tool call is not necessarily an authorized one; validation should establish that the requested operation is allowed in context.
Require approval for high-risk actions
Use human approval where an action could expose sensitive information or create significant consequences. The approval step should show enough information for a reviewer to assess what will happen, including the target and relevant content, rather than presenting an opaque “confirm” button. Test that an injected instruction cannot bypass or silently pre-approve this control.
Keep trust boundaries explicit
Separate untrusted document content from system instructions and application policy. Make the provenance of external material clear to the model and to the application where possible, while recognizing that labeling alone is not a complete defense. Treat stored data, third-party API responses, tool output, and earlier model completions as potentially untrusted if they can contain attacker-controlled text.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. For RAG, verify integrity and provenance across the pipeline
Retrieval-augmented generation adds stages where a document can be introduced, altered, split, indexed, retrieved, and assembled into context. Evaluate those stages as part of the same workflow, rather than treating retrieval as a neutral lookup step. OWASP’s RAG Security Cheat Sheet addresses controls across this pipeline, including document poisoning.
Quick wins for a faster PC:
Repair Windows errors before they cause bigger problemsFix Now →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Clear out junk files and repair common Windows errorsFree Scan →- Maintain document manifests and verify digests against an approved baseline.
- Reject documents when integrity checks fail and generate an alert for investigation.
- Record who supplied each document, when and from where it arrived, and what approval or review applied.
- Check that retrieval and access controls do not return material the current user should not see.
- Inspect the passages actually retrieved and supplied to the model when investigating a test result.
A digest match establishes consistency with the baseline used for comparison; it does not establish that the document is safe. A malicious instruction can be present in an approved document from the start, so integrity checks do not replace prompt-injection testing, provenance review, or access controls.

How to interpret published attack-success figures
OWASP’s prompt-injection prevention cheat sheet reports attack-success results of 89% for GPT-4o and 78% for Claude 3.5 Sonnet, attributing them to Hughes et al. (2024) and describing up to 10,000 augmented prompts per request. OWASP explicitly cautions that these are results for the models and configurations tested, not universal predictions.
Those figures are not a current ranking of commercial models and do not establish how every document-processing workflow will perform. They concern particular models and configurations, not a shared test of all the prompts, tools, permissions, filters, and document pipelines in your application. Use them as a reason to test the system you plan to deploy, not as a substitute for that evaluation.
NIST’s January 2025 blog discusses agent hijacking through malicious instructions in ingested data and describes evaluation of defenses as ongoing work. NIST’s guidelines page, updated September 30, 2026, lists automated benchmark-evaluation practices as an initial public draft. That context is relevant to the evolving field, but it should not be presented as a complete or final benchmark standard for document security.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteWhat a useful evaluation report should contain
A report should let another team understand what was tested and reproduce the comparison without implying more certainty than the evidence supports. Include:
- The application purpose, intended users, document sources, and actions available to the AI system.
- The assets and harms considered, plus the criteria used to classify outcomes and severity.
- The model version, prompts, tools, permissions, filters, retrieval settings, and other relevant configuration.
- The attack and benign-control cases, their content routes, and the variations and repeat-run method used.
- Observed answers, relevant tool calls and application-state changes, detected or blocked events, and any failures.
- Known limitations of the test set and configuration, along with changes made in response to findings.
Keep comparisons tied to their test conditions. Model behavior and product configurations change, so a passing result is not a permanent certification. Re-run relevant cases when the model, prompts, tools, document pipeline, permissions, or protective controls change.
