Split by patient before you create slide-, scan-, specimen-, or visit-level records. Keep every linked datum from one person in one partition, fit every preprocessing and feature-selection step on development data only, freeze the pipeline, and evaluate the unchanged model on a representative holdout or a genuinely external cohort. If a test result changes the model, that dataset is no longer an unbiased final test.

A defensible evaluation answers two separate questions: how well does the model work within the population used for development (internal testing), and how well does the locked model work on new patients in a different setting or time period (external testing)? A multimodal classifier must also be compared with single-modality models on the same patients, under identical outcome definitions and evaluation rules.

What “without data leakage” means in cancer AI

Data leakage occurs when information from outside a model’s permitted development boundary influences training, tuning, preprocessing, feature selection, threshold selection, architecture choice, or model selection. The resulting score can look excellent because the system recognizes patients, duplicate specimens, acquisition sites, staining patterns, scanner signatures, or processing decisions that will not be available for a genuinely new patient.

The unit that must remain independent is the patient, not the file. One person may contribute whole-slide images, radiology studies, multiple specimens, longitudinal visits, molecular measurements, and clinical variables. Randomly assigning those records to different folds lets the model see part of the person during development and another part during evaluation. That estimates recognition of familiar individuals rather than generalization to new patients.

TRIPOD+AI states the separation requirement directly: “Evaluation data should be distinct from the data used to train the model, tune hyperparameters, or do model selection, such that there is no overlap in participants between the training and evaluation data.”

Define the prediction task before touching the data

Write a task specification that can be checked independently of the code. It should identify what is predicted, for whom, from which information, and at what time.

State the clinical question and prediction unit

  • Classify a diagnosis, estimate a prognosis, predict treatment response, or perform another explicitly named task.
  • Define whether one prediction is produced per patient, tumor, biopsy, slide, scan, visit, or time point. If the intended decision is patient-level, aggregate modality-level evidence without allowing records from the same patient to cross partitions.
  • For prognostic models, specify the index time, follow-up window, event definition, and censoring rules. A five-year risk estimate and a 30-day complication prediction are different tasks even when they use the same inputs.

Define the target population and intended use

Describe inclusion and exclusion criteria, recruitment dates, care setting, disease stage, and the population in which the model will be used. A cohort that excludes patients because a modality is missing may not represent deployment if missingness is common in practice. State whether the intended setting is one hospital, a network, or multiple institutions.

Use a reference standard, not an assumed “truth”

Explain how each label or outcome was established, who assigned it, and whether that process was independent of the model inputs. CLAIM 2024 uses the term reference standard because a clinical label may be an adjudicated approximation rather than absolute truth. For pathology, this could involve specified diagnostic criteria and expert review; for prognosis, it could be a documented event with a prespecified follow-up rule. Do not imply that one labeling process fits every cancer type.

Build patient-level partitions first

Create a stable patient identifier that links all material belonging to an individual: slides, regions, scans, specimens, visits, laboratory results, genomic profiles, and clinical records. Assign patients to development, tuning or internal-testing, and final-evaluation partitions before extracting patches, slices, tiles, embeddings, or modality-specific rows.

  1. Inventory linked records. Resolve aliases, transferred patients, repeat biopsies, rescans, and specimens split across laboratory systems. Document how duplicates and near-duplicates were identified.
  2. Assign the partition at patient level. Every record associated with a patient receives the same assignment. Do not assign a slide, image, or visit independently.
  3. Keep grouped folds intact. In grouped cross-validation, place every record from a patient inside one fold. TRIPOD+AI asks authors to explain how multiple samples from one person were handled and whether individuals stayed within a single fold.
  4. Freeze the assignment. Save the patient-to-partition map under version control. A later code change must not silently reshuffle people.
  5. Check for overlap. Compare patient identifiers, specimen identifiers, acquisition timestamps, hashes, and similarity matches across partitions. Investigate any match before fitting the model.

A patient-level split is necessary but not sufficient. If a scanner, staining batch, institution, or label workflow is almost perfectly associated with the outcome in one partition, the model can still learn a shortcut. Record those factors and test whether they explain performance.

Keep development, tuning, and final evaluation separate

All choices that can improve a score belong to model development. That includes architecture selection, hyperparameter search, feature selection, imputation strategy, normalization parameters, stain or scanner correction, augmentation policy, ensemble weighting, probability calibration, and operating-threshold selection.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use a development set for fitting and a tuning procedure—such as nested cross-validation or a dedicated tuning split—for those choices. Once the procedure is frozen, run it once on the final evaluation data. If you inspect that result and revise the method, the dataset has become development data; obtain a fresh final evaluation cohort for an unbiased estimate.

Fit transformations only on development data

  1. Estimate normalization statistics, scaling factors, imputation values, feature-selection rules, dimensionality reductions, and batch-correction parameters from development patients only.
  2. Apply the frozen transformations to tuning and evaluation patients without refitting them.
  3. Perform modality-specific processing—such as stain normalization, scanner harmonization, segmentation rules, or embedding extraction—under the same separation. If the processing algorithm learns from the full dataset, it can leak evaluation information even when the classifier itself is untouched.
  4. Record software versions, random seeds, preprocessing order, and fitted parameter files so the exact pipeline can be replayed.

Prevent indirect linkage leakage

  • Remove filenames, accession numbers, timestamps, or folder paths that encode patient or outcome information.
  • Check that repeated scans, alternate exports, mirrored images, and overlapping tiles are not distributed across partitions.
  • Inspect whether institution, scanner model, staining protocol, slide preparation, or label workflow is correlated with the target. Such variables may be legitimate predictors in a defined use case, but they are dangerous shortcuts when they merely identify the data source.
  • Document missingness for every modality. A model can exploit the fact that a test was ordered only for certain patients, so missingness itself needs a clinical interpretation.

Internal testing and external testing answer different questions

CLAIM 2024 recommends the term external testing for evaluation on data external to development, such as data from another site. It avoids the ambiguity of calling every held-out split “validation.”

Design Data origin What it estimates Permitted use Main limitation
Development training Patients used to fit model parameters Nothing unbiased about future performance Fit representations, coefficients, and preprocessing Optimism is expected because the model sees these outcomes
Tuning or internal testing Held-out patients from the development population Performance within the development setting after a specified resampling or holdout procedure Choose hyperparameters and compare candidates only until the procedure is frozen May not represent another hospital, protocol, or disease spectrum
Final internal evaluation Patient-held-out subset reserved before development decisions Unbiased estimate for that development population when untouched One-time locked evaluation Still shares the development setting and its acquisition patterns
External testing Distinct cohort not used in development, ideally another institution or time period Transportability to a new setting or population Evaluate a fixed model and report context differences Performance can change with prevalence, protocols, reference standards, or case mix

Internal testing may use a held-out split, grouped cross-validation, or bootstrapping. State whether every model-building step—including tuning—was repeated inside each resampling loop. A single random image split is not evidence of performance at a new hospital.

For external testing, describe differences in patient mix, disease prevalence, acquisition protocol, institutions, dates, and reference standards. A later time period from the same site can test temporal transportability; another institution tests additional sources of variation. Neither design is automatically superior—the choice should match the intended use.

Evaluate multimodal models on matched patients

Compare the fused model with each available modality-specific model on the same patient-level cohort, using the same outcome definition, partition map, preprocessing lock, and final evaluation process. This is a fair-comparison rule derived from general prediction-model guidance; no single fusion protocol is mandatory for every cancer.

Make the comparison interpretable

  • Report how many patients have pathology, imaging, clinical variables, and every combination of modalities.
  • Use a matched-patient analysis when comparing fusion with a modality-specific model. If the single-modality model includes patients lacking another modality, state that denominator and provide a sensitivity analysis on the intersection cohort.
  • Specify how missing modalities are handled: exclusion, an explicit missingness token, imputation, modality dropout during training, or a separate model. Evaluate the behavior that deployment will actually use.
  • Keep thresholds and calibration procedures identical in spirit and locked before comparing results. Do not select a favorable threshold separately for each model on the final test set.
  • Report whether the fused model’s apparent gain remains after accounting for differences in patient count, prevalence, and follow-up.

An example in the literature combines digital pathology and clinical characteristics for breast-cancer prognostication and evaluates the model in five external cohorts. Treat that as a study-specific design example, not as a performance expectation or a universal recipe for other cancers.

Report discrimination, calibration, and clinical utility

No single discrimination statistic captures whether a cancer classifier is safe or useful. Choose measures that match the outcome and intended decision, and include uncertainty.

Discrimination

  • For binary outcomes, report the c statistic or area under the receiver-operating-characteristic curve with a confidence interval or another stated uncertainty method. Include class counts and the denominator.
  • For time-to-event outcomes, use an appropriate time-to-event measure such as the concordance index, specify the prediction horizon, and explain censoring.
  • When prevalence is low or decisions depend on positive predictive value, add precision-recall information rather than relying on ROC AUC alone.

Calibration

Show whether predicted probabilities correspond to observed risk. A calibration plot, calibration-in-the-large estimate, and calibration slope can reveal systematic overprediction or underprediction that an acceptable AUC would miss. Describe any recalibration and ensure it was fitted using development data, not the final evaluation cohort.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Clinical utility and thresholds

If the classifier triggers biopsy, treatment, surveillance, or another action, report utility at clinically justified thresholds. Decision-curve analysis can show net benefit across a range of threshold probabilities. Explain how thresholds were chosen, lock them before final evaluation, and report sensitivity, specificity, predictive values, and likelihood ratios at those locked thresholds where relevant.

Examine transportability and subgroup performance

Performance can vary by hospital, country, scanner, protocol, disease stage, age, sex, ancestry, or other clinically meaningful clusters. TRIPOD+AI asks authors to explain how heterogeneity across clusters was handled and quantified.

  • Report site- or time-specific estimates when the sample size supports them, with uncertainty intervals.
  • Predefine clinically justified subgroups rather than searching dozens of slices until one looks favorable.
  • Show subgroup discrimination, calibration, and decision performance when those dimensions matter, and state when sample sizes make estimates unstable.
  • Explain whether a site effect reflects genuine biology, referral patterns, acquisition differences, or reference-standard variation.

There is no universal subgroup list for every cancer. The appropriate strata depend on the disease, intended use, available data, and ethical risk of unequal errors.

A practical end-to-end evaluation workflow

  1. Write the protocol. Define task, prediction unit, target population, index time, horizon, reference standard, modalities, and intended decision before modeling.
  2. Create the linkage table. Map each patient to all slides, scans, specimens, visits, clinical records, and modality-availability indicators.
  3. Reserve evaluation data. Assign patients to development, tuning or internal testing, and final evaluation. If external testing is planned, secure that cohort under a data-use boundary that prevents development feedback.
  4. Audit overlap. Search identifiers, duplicate files, near-duplicate images, repeated specimens, and temporal repeats across partitions.
  5. Implement grouped resampling. Use patient-grouped folds and document the grouping key, fold counts, and any exclusions.
  6. Fit preprocessing inside development. Learn normalization, imputation, feature selection, batch correction, and representation parameters without accessing evaluation patients.
  7. Tune and select the model. Use only development and tuning data. Record the search space, stopping rule, and selection metric.
  8. Freeze the pipeline. Lock architecture, weights or refitting procedure, preprocessing artifacts, modality-missingness behavior, calibration, and operating thresholds.
  9. Run internal and external tests. Apply the locked procedure once to each untouched cohort. Do not repair errors using final-test outcomes.
  10. Report completely. Provide cohort flow, denominators, acquisition context, reference standard, missingness, partitioning, model-building steps, uncertainty, calibration, utility, subgroup or site variation, and limitations.

Common failure modes and their fixes

Failure mode Why the result is misleading Correction
Randomly splitting image tiles or slide files Tiles from one patient appear in both development and evaluation, allowing memorization of morphology or artifacts Split by patient before tile extraction; keep all linked material together
Fitting normalization on the full dataset Evaluation distribution influences transformation parameters Fit the transformation on development data and apply it unchanged
Choosing a threshold after seeing test predictions The reported operating point is optimized for the test cohort Select the threshold in development or tuning data and evaluate it once
Using an external result to redesign the model The external cohort becomes part of development Label it development data and obtain a fresh external test set
Comparing fusion and single-modality models on different patients Case mix or prevalence, rather than modality fusion, explains the difference Use matched patients and disclose modality-availability denominators
Calling every holdout “external validation” Readers cannot tell whether the cohort was independent or merely held out internally Use “internal testing” for held-out development data and “external testing” for a distinct external cohort
Reporting only AUC Discrimination does not reveal probability accuracy or clinical consequences Add calibration, utility, uncertainty, and threshold-specific results
Ignoring acquisition shortcuts The model may identify site, scanner, staining, or workflow instead of cancer biology Audit acquisition variables, test across sites or periods, and explain their role in the intended use

What a reproducible report should contain

Use TRIPOD+AI for prediction-model reporting and CLAIM 2024 for the medical-imaging component. Provide item-level answers and mark items that do not apply rather than leaving the reader to guess.

  • Cohort flow, dates, sites, inclusion and exclusion criteria, and the number of patients at each stage.
  • Patient-level partitioning rule, grouped-fold design, overlap checks, and handling of repeated samples.
  • Modalities, acquisition devices and protocols, preprocessing, stain or scanner processing, and batch correction.
  • Reference-standard definition, label adjudication, outcome timing, censoring, and missing-data rules.
  • Every model-building step, including feature selection, imputation, normalization, architecture choice, tuning, calibration, and threshold selection.
  • Internal-testing procedure, whether the full model-building process was replayed in each resampling loop, and the locked final-evaluation procedure.
  • External-testing setting, differences from development, and reasons the cohort represents intended use.
  • Discrimination, calibration, clinical utility, confidence intervals or other uncertainty estimates, class counts, denominators, and subgroup or site variability.
  • For multimodal comparisons, patient counts for each modality pattern, missing-modality handling, and results on matched cohorts.

These details let a reader judge whether the reported score reflects generalization to new patients or recognition of characteristics already present during development. Disease-specific benchmark numbers cannot be transferred across cancer types, outcomes, cohorts, or study designs without evidence from the same setting.

The standard to use before claiming generalization

A multimodal cancer classifier is ready for a credible performance claim only after the patient-level partition is auditable, the entire preprocessing and selection pipeline is isolated from final evaluation, and the locked model has been tested on a cohort that matches the intended use. Internal testing describes behavior within the development population; external testing probes transportability. Reporting both, with calibration, utility, uncertainty, and modality-matched comparisons, is what turns a high score into an interpretable estimate.
