Enforce least privilege for an autonomous AI agent outside the model. Give the agent its own short-lived identity, allow only the tools and data required for the current task, and place authorization in a policy decision point, API gateway, tool proxy, or execution service that the model cannot change. Let the model propose an action; let trusted software decide whether that exact action is permitted.
Automate narrowly scoped, low-risk and reversible work. Require an explicit, action-specific human approval before an agent sends an external message, executes code, changes permissions or production settings, deletes data, deploys, moves money, or performs another high-impact or irreversible operation. Bind approval to the actor, tool, target, normalized parameters, expiry and one execution attempt, then consume it atomically and record an audit trail.
Why autonomous agents need a separate security model
An autonomous agent is not merely a chat interface. It can make decisions, select tools, retrieve data, write files, call APIs and continue through several steps with limited supervision. NIST’s COSAiS project describes AI-agent systems as having “autonomous decision-making and taking action to operate with limited human supervision to achieve complex goals.” NIST’s NCCoE warns that the scale and range of actions taken by software and AI agents could increase exponentially as these systems become more capable.

An incorrect, ambiguous, hallucinated or prompt-injected instruction can become a real-world change when the agent has excessive functionality, excessive permissions or excessive autonomy. OWASP calls this combination excessive agency. A model’s system prompt is not a security boundary: a malicious document, tool response or user message can attempt to override it, and the model can still select an unsafe sequence of otherwise valid calls.

The safe design separates three questions:
- What is the agent allowed to propose? This is a model and workflow question.
- What is the agent allowed to execute? This is an authorization question enforced by trusted code.
- What must a person approve immediately before execution? This is a risk, governance and accountability question.
OWASP’s Agentic AI AAI9 guidance says agents should operate under the same change-management controls applied to human administrators, with additional automated guardrails for autonomous operation. Its Gen AI security guidance similarly recommends human approval for high-impact actions and authorization in downstream systems rather than relying on an LLM to decide whether an action is allowed.
The control architecture: proposal, policy, approval and execution
A robust implementation has distinct components. The model can plan and request a tool call, but it cannot grant itself a permission, approve its own request or directly reach a privileged system.
1. Agent and human identities
Create a unique identity for each agent, deployment or materially different workflow. Do not reuse a developer’s long-lived administrator token. The agent identity should be bound to the human or organization that initiated the task, while remaining distinguishable in logs and downstream access-control decisions.
Recommended Free Tools
Use short-lived credentials with a narrow audience, scope and lifetime. Issue them when a task starts, rotate them when risk changes and revoke them when the task ends, is cancelled or becomes suspicious. A credential should identify the agent, the delegated human principal, the environment and, where practical, the task or execution instance.
2. Tool proxy or policy decision point
Route every tool request through a policy decision point (PDP), API gateway, execution service or tool proxy. The proxy independently authenticates the caller, validates the requested operation and resource, classifies risk, checks approval state and either executes the call or denies it. The model never calls a privileged endpoint directly.
Complete mediation means every sensitive operation is checked at the point of use, including calls made several steps after the initial plan. A check performed only when a conversation starts is insufficient because the target, parameters, retrieved data or risk can change later.
3. Policy enforcement inputs
The enforcement component should evaluate at least:
- Agent identity and delegated human or service principal.
- Tool and operation name.
- Target resource, tenant, environment and data classification.
- Normalized parameters, including recipients, amounts, paths, queries and filters.
- Risk class, reversibility and expected side effects.
- Current approval identifier, approver identity and approval expiry.
- Credential, policy and task validity.
- Rate, concurrency, retry and transaction limits.
If identity resolution, policy lookup, parameter normalization, approval validation or audit logging fails, the request should fail closed. “Unavailable” must not be interpreted as “allowed.”
4. Execution service
A trusted execution service performs the operation after policy checks succeed. It should use an allowlisted function with a defined schema rather than a general shell or unrestricted URL fetcher. The service can apply additional validation, idempotency keys, transaction caps, sandboxing and network-egress restrictions before reaching the downstream system.

How to design least privilege for an AI agent
Start with a task-specific inventory
Write down the agent’s purpose and trust boundaries before granting access. Identify the human or service principal on whose behalf it acts, the resources it may access, the tools it may call, the environments it may touch and the data it may read or change.
For each proposed tool, record the smallest useful operation. “Ticket management” is too broad if the workflow needs only to read one queue and create a draft. “Cloud access” is too broad if the task needs only to inspect one service’s configuration in a nonproduction account.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minute- Resource names, tenants, repositories, buckets, accounts and environments.
- Allowed verbs such as read, preview, create, update, approve or delete.
- Permitted fields, records, paths, recipients and query predicates.
- Maximum amount, row count, file size, runtime, frequency and concurrency.
- Whether the operation may reach production, sensitive data or administrative controls.
Use deny-by-default permissions
Begin with no tool access and add one allowlisted capability at a time. Separate read from write, preview from commit and ordinary data from sensitive or administrative data. Remove unused plugins, connectors and scopes. Avoid wildcard API permissions, general-purpose shells, arbitrary code execution and broad network access when a narrow function can accomplish the task.
For example, a support agent might be allowed to retrieve the status of a named case and prepare a reply draft. It should not receive a token that can send any message to any recipient, export the customer database or change account permissions. A deployment assistant might generate a deployment plan and inspect build artifacts while lacking production credentials until an approved release step issues a short-lived token.
Constrain data as well as actions
Least privilege applies to information returned to the model. Restrict fields, rows, documents, tenants and time ranges. Mask secrets and unnecessary personal information before content enters the context window. A read-only token that exposes every customer record is still excessive privilege.
Bind delegated authority
Delegation should preserve who authorized the task. Downstream systems should be able to distinguish the agent, the initiating human and the organization. If a user can approve only their own projects, the policy service must enforce that relationship rather than trusting a project name supplied by the model.
Expire and review access
Set explicit end times for credentials and task grants. Reassess scopes after changes to tools, models, retrieval sources, memory, prompts, workflows or downstream systems. Remove permissions that are no longer used and retain evidence of revocations, denials, approvals, timeouts and residual risk.
Classify actions by impact and reversibility
Risk classification determines whether an agent can act unattended. A useful baseline is to consider both the potential impact and how easily the result can be undone. A reversible operation can still need approval if it exposes sensitive data or affects many users; an apparently small operation can be critical if it changes an identity or security boundary.
| Action class | Typical examples | Default mode | Required controls |
|---|---|---|---|
| Low risk and reversible | Search, read-only retrieval, bounded calculation | Autonomous | Narrow scope, output validation, rate limits and logging |
| Medium risk | Create or edit a draft, write a noncritical file, open a ticket | Preview plus approval according to policy | Exact target and parameter binding, audit trail and rollback |
| High risk | Send an external message, execute code, modify production configuration or change an access policy | Explicit human approval | Independent policy check, short-lived approval and step-up authentication where appropriate |
| Critical or irreversible | Delete data, transfer funds, grant privilege or deploy to production | Two-person or designated-approver workflow where organizational policy requires | Fail closed, idempotency or duplicate confirmation, immutable audit evidence and rollback or a circuit breaker |
This matrix is a starting point, not a universal compliance rule. Tune thresholds to resource classification, reversibility, financial or safety impact, blast radius and organizational policy. Unknown tools and unclassified operations should be denied until someone defines their risk and controls.

How to implement a human approval gate
Step 1: Stop before the side effect
Split each tool into a planning or preview operation and a commit operation whenever practical. The agent can calculate a change, render a message, produce a deployment plan or assemble a payment request without performing the side effect. The commit endpoint should require the policy and approval checks again, immediately before execution.

Present the approver with the information needed to understand the consequence:
- The agent identity and the human or organization on whose behalf it is acting.
- The exact tool and operation.
- The target resource, environment and recipient.
- Normalized parameters, including the final amount, path, query, message body or permission set.
- Expected side effects, affected records and estimated blast radius.
- Risk classification, reversibility and rollback instructions.
- Credential or approval expiry and the policy version used for the decision.
Do not display a vague “Continue?” prompt. An approval that does not identify the target and parameters cannot reliably authorize the operation the person thought they were reviewing.
Step 3: Bind approval to the exact request
Store an approval record containing the actor, tool name, target resource, normalized parameters, timestamp, expiry, policy version and a unique approval identifier. A cryptographic digest of the canonical request can make parameter substitution detectable.
If the agent changes the target, recipient, amount, path, query or any other material parameter, invalidate the approval and request a new one. Do not allow an approval for “send this draft” to authorize a later message that the agent rewrites.
Step 4: Authenticate the approver appropriately
Use step-up authentication for privilege changes, account recovery, payments, bulk deletion and production deployment. Require a designated approver or two people when organizational policy sets that threshold. The approver must be authorized for the resource and action; merely being available in a chat channel is not enough.
Step 5: Consume approval atomically
Immediately before execution, the policy service should verify that the approval is valid, unexpired, unused, bound to the current request and associated with the correct agent and delegated principal. It should atomically mark the approval as consumed while obtaining an execution lease. This prevents two concurrent retries from reusing one approval.
Use one execution instance per approval unless the policy explicitly allows a bounded batch. For an approved batch, include the maximum count, total amount, target set and time window in the approval itself.
Step 6: Execute and record the result
After the atomic check, the execution service performs the operation with the narrow credential and records success, failure, retries, downstream identifiers and any rollback. If the downstream system times out, do not blindly retry a non-idempotent operation. Query the operation status or require duplicate confirmation according to the risk policy.

Reference workflow for an agent tool call
- Receive request: accept the user’s task and create a unique task and execution identifier.
- Plan: let the model propose a tool and parameters, but treat the proposal as untrusted input.
- Normalize: canonicalize names, paths, recipients, amounts, filters and environment labels so equivalent requests compare identically.
- Authenticate: verify the agent credential and delegated human or service principal.
- Authorize: evaluate operation, target, data scope, risk, limits and current policy in the external policy service.
- Preview: for gated actions, display the exact normalized request and consequences to an authorized approver.
- Approve: create a short-lived approval bound to the request digest, actor, policy version and execution instance.
- Recheck: immediately before the side effect, repeat authorization and consume the approval atomically.
- Execute: call the allowlisted function with a short-lived credential, idempotency key and applicable limits.
- Audit: persist the proposal, decision, approval, execution result and errors; alert on anomalies.
A simplified approval object might contain the following fields. The exact schema depends on your authorization system, but the binding principles should remain:
{
"approval_id": "unique-value",
"agent_id": "agent-deployment-17",
"delegated_principal": "user-or-service-id",
"tool": "production.deploy",
"target": "service/example",
"normalized_parameters_digest": "canonical-request-hash",
"risk_class": "critical",
"policy_version": "2026-01",
"approved_at": "timestamp",
"expires_at": "timestamp",
"single_use": true
}
Never treat this object as proof by itself. The execution service must verify its signature or integrity, issuer, audience, expiry, one-time status and match with the current request.
Where authorization must live
Why prompts and model outputs are insufficient
Instructions such as “never delete data” or “ask for approval before sending” are useful behavioral guidance but cannot enforce access. Prompt injection can arrive through a web page, email, document, retrieval result or tool response. A model may also misclassify a request or omit a confirmation step. If the downstream API accepts the call, the control has already failed.
Recommended enforcement locations
- API gateway: centralizes identity, scopes, rate limits and network policy for service calls.
- Tool proxy: exposes only typed, allowlisted functions and validates parameters before forwarding them.
- Policy decision point: evaluates authorization and returns an allow, deny or approval-required decision.
- Execution service: holds privileged credentials, applies transaction safeguards and performs the side effect.
- Downstream system: independently enforces its own authorization and change-management rules.
Use defense in depth. The agent gateway should not be the only check if the underlying system can be reached another way. Downstream authorization must still reject a call made with an invalid, expired or over-scoped credential.

Limit blast radius when a control fails
Least privilege reduces what an agent can do; operational limits reduce how much it can do before detection or shutdown.
- Rate limits: cap calls per minute, hour and task.
- Transaction caps: limit monetary value, record count, file size and affected resources.
- Concurrency limits: prevent a loop from issuing many changes simultaneously.
- Retry limits: stop repeated attempts after bounded failures.
- Recursion and step limits: cap autonomous depth and total tool calls.
- Network egress controls: allow only required destinations and protocols.
- Sandboxing: isolate code execution and temporary files from production credentials and sensitive networks.
- Circuit breakers: halt an agent after anomaly thresholds, policy errors or a sudden increase in high-risk requests.
- Idempotency: use idempotency keys and safe retry semantics for operations that may be repeated.
- Duplicate confirmation: require a second confirmation when an operation cannot be made idempotent and failure could cause material harm.
Audit, monitoring and incident response
Record enough evidence to reconstruct what happened without relying on the model’s conversational memory. At minimum, capture:
- Agent identity, deployment or version and delegated human or service principal.
- Prompt or request identifier, task identifier and execution instance.
- Tool call, canonical parameters, target and data scope.
- Policy decision, risk classification, policy version and reason.
- Approval identifier, approver identity, authentication strength and expiry.
- Credential issuance and revocation events.
- Execution result, downstream transaction identifier, retries and errors.
- Rollback, cancellation, timeout and circuit-breaker events.
Protect audit records from alteration and restrict who can read sensitive content. Monitoring should alert on unusual privilege use, repeated approval-bypass attempts, abnormal tool frequency, access to new resources, repeated denials, sudden increases in high-risk actions and activity outside the task’s expected time window.
Prepare a response path before production. Operators should be able to revoke the agent’s credentials, disable a tool, suspend a workflow, invalidate outstanding approvals and stop downstream execution without waiting for a model response.
Testing before production and after every change
Test the complete path, not only the model’s refusal behavior. Preserve the tested agent version, tool policy, model and provider configuration, retrieval and memory settings, expected results and observed approval or denial behavior.

Abuse cases to exercise
- Prompt injection in a retrieved document, web page, email or tool response.
- Confused-deputy behavior in which the agent uses a user’s authority for another resource.
- Parameter substitution after approval, including changed recipient, amount, path or environment.
- Replay of an already consumed or expired approval.
- Concurrent requests attempting to consume one approval twice.
- Unauthorized tool calls, wildcard scopes and privilege escalation.
- Data exfiltration through a broad search, export or network destination.
- Fail-open behavior when the policy service, identity provider or audit store is unavailable.
- Timeouts and retries for non-idempotent operations.
- Excessive recursion, rate-limit evasion and tool-call loops.
Repeat these tests whenever tools, prompts, models, memory, retrieval sources, policies or downstream systems change. A model update can alter tool selection and parameter formatting even when the surrounding application code is unchanged.
Design-review checklist
- Is the agent a distinct principal with short-lived, revocable credentials?
- Is delegated authority tied to the initiating human or organization?
- Are tools, operations, data fields, resources and environments allowlisted?
- Are read and write, preview and commit, ordinary and administrative scopes separated?
- Does a policy component outside the model perform complete mediation?
- Are high-impact and irreversible actions explicitly classified and approval-gated?
- Does the preview show the exact target, normalized parameters, effects and rollback information?
- Is approval bound to one actor, tool, target, request digest, policy version and expiry?
- Is approval consumed atomically immediately before execution?
- Do changed parameters require a new approval?
- Does the system fail closed when classification, policy, approval or logging fails?
- Are rate, transaction, concurrency, retry and recursion limits enforced?
- Are privileged credentials held by the execution service rather than exposed to the model?
- Are downstream systems still enforcing their own authorization?
- Can operators revoke access and stop execution quickly?
- Are approvals, denials, errors, retries and rollbacks durably logged and monitored?
- Have prompt injection, replay, substitution, exfiltration and fail-open tests passed?
How to compare competing agent-authorization designs
When reviewing architectures, score each design on the dimensions below rather than choosing based on how convincing the agent’s conversational safeguards sound.
| Comparison axis | Questions to ask |
|---|---|
| Privilege granularity | Can permissions be limited to individual tools, operations, fields, resources, environments and time windows? |
| Separate identity | Can logs and downstream systems distinguish the agent, deployment and delegated human? |
| Delegation | Is the initiating principal preserved and checked against resource ownership and approval authority? |
| Read/write separation | Can the design expose preview and retrieval without exposing commit or administrative operations? |
| Policy location | Is authorization enforced in trusted infrastructure and again by downstream systems? |
| Approval binding | Does approval cover the exact canonical request rather than a conversational intention? |
| Expiry and replay protection | Are approvals short-lived, single-use or bounded, and atomically consumed? |
| Reversibility | Are rollback, idempotency, duplicate confirmation and circuit breakers available? |
| Failure behavior | Does an unavailable policy, identity or logging service deny the action? |
| Audit completeness | Can investigators reconstruct proposal, decision, approval and execution? |
| Monitoring | Are unusual privilege use, bypass attempts and volume changes detected? |
| Revocation and operations | Can access, tools and outstanding approvals be disabled quickly without redeploying the model? |
What “human in the loop” should and should not mean
A human approval gate is meaningful only when the person can understand the exact action, has authority to approve it and can stop or change the request. A notification sent after execution is monitoring, not approval. A single permanent “I trust this agent” consent is not approval for every future payment, deletion or privilege change.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Human review also should not be used to compensate for unlimited autonomy. If an agent can generate thousands of high-risk requests, a person clicking approve becomes a bottleneck and a weak control. Pair approval with narrow scopes, batch limits, clear previews, expiry, step-up authentication and automatic circuit breakers. For repetitive low-risk work, encode a documented policy that permits bounded automation and reserve human attention for exceptions.
Practical examples
External email
The agent may read an approved set of records and draft a message. The send operation should verify the recipient list, message body, attachments, data classification and delegated authority. Approval expires quickly and is invalidated if the recipient, content or attachment changes. The mail service should still enforce the agent’s sending scope.
Production deployment
The agent can inspect build results and propose a deployment plan. A designated approver reviews the exact artifact, service, environment, change window, migration steps and rollback method. The execution service obtains a short-lived production credential only after the approval is atomically consumed, and a circuit breaker stops the rollout if health checks fail.
Permission change
The agent may identify an access problem and prepare a proposed role change. The policy service checks that the requester and approver are authorized, that the target account and role are valid and that the change does not violate separation-of-duties rules. Step-up authentication and a second approver may be required. The grant should have an explicit duration and a corresponding revocation path.
Deletion or payment
Deletion and payment are critical because recovery may be impossible or costly. Require exact object or recipient lists, amount or quantity limits, duplicate protection, short-lived approval and stronger authentication. If the operation cannot be rolled back, require the designated or two-person workflow defined by organizational policy.
Bottom line for implementation
Build the agent as an untrusted planner with narrowly delegated capabilities, not as an administrator that happens to receive safety instructions. Give it a distinct identity, enforce deny-by-default scopes in trusted authorization infrastructure, and make every sensitive operation pass complete mediation at execution time. Let people approve exact, visible and bounded actions—not vague intentions—and make approvals short-lived, single-use, parameter-bound and auditable. With fail-closed behavior, blast-radius limits, revocation and continuous abuse testing, autonomous agents can handle routine work without turning model errors or prompt injection into uncontrolled system changes.
