You cannot embed a live Microsoft Forms questionnaire directly inside the body of an Outlook 365 email using a Microsoft-supported method. Microsoft Forms provides an Embed option, but its embed code is intended for a webpage, Sway, or a document. For email, the supported option is to insert a link to the form.
You can still make the message look more prominent by adding an inline image or banner above the link. That image is only a visual call-to-action; it does not turn the email into an interactive Forms survey.
How to send a Microsoft Form from Outlook 365
The simplest supported workflow starts in Microsoft Forms and creates an email containing the respondent link.
The Tool Desk
Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →- Go to forms.office.com, or open Microsoft 365 and select Forms from the app list.
- Open the form you want to distribute.
- Select Collect responses.
- Select Email.
- When your designated email application opens, add the recipients.
- Edit the subject and message if necessary, then send the email.
Forms adds a short message and a link to the form in the email body. You can customize the surrounding text before sending it.
Choose the correct response audience first
Before copying or emailing the form, check who is allowed to submit responses. Under the form’s response-collection settings, Microsoft Forms supports these audience choices:

| Setting | Who can respond | When to use it |
|---|---|---|
| Anyone can respond | Anyone who receives the link | Public surveys, customer feedback, and external invitations |
| Only people in my organization can respond | Users signed in to your organization | Internal polls and employee questionnaires |
| Specific people in my organization can respond | Only the people or groups you specify | Restricted internal forms |
Be careful with forwarding. Anyone who receives the form link by email can forward that message to someone else. Depending on the audience setting, the forwarded recipient may be able to view and complete the form.
What the Forms sharing options actually do
After selecting Collect responses, the current sharing controls have distinct purposes:
Quick wins for a faster PC:
Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →| Option | Result | Works as a live form inside an Outlook message? |
|---|---|---|
| Copy link | Copies the respondent URL | No; it gives you a link to place in the email |
| QR code | Provides a downloadable PNG QR code | No; recipients scan it to open the form elsewhere |
| Embed | Copies embed code for a webpage, Sway, or document | No; Outlook does not render it as an email questionnaire |
| Creates an email containing a form link | No; this is the supported email workflow |
How to add a form link manually
If Forms does not open the Outlook client you want, use Copy link instead.
- Open the form and select Collect responses.
- Select Copy link.
- Open Outlook and create a new message.
- Write instructions such as “Complete the employee equipment survey.”
- Paste the Forms URL into the message body.
- Add recipients and send the message.
You can also select descriptive text and use Outlook’s link command to present the URL as a named call-to-action, such as Open the survey. The important point is that the recipient opens the hosted form in a browser rather than filling it out within the email body.

How to add a visual call-to-action image
An image can make a link-based invitation more noticeable. It does not embed the questionnaire, but it can provide a banner, button graphic, or QR code alongside the form link.
New Outlook for Windows
- Start a new message and place the cursor where the image should appear.
- Select Insert.
- Select Pictures.
- Choose the image and select Open.
Classic Outlook
- Place the cursor in the message body.
- Select Insert > Pictures.
- Browse to the image and select Insert.
Put the Forms link below or beside the image. Do not describe the image as the embedded form: recipients still need to follow the link to answer the questions.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsWhy pasting Forms embed code does not work in Outlook
The Embed option produces web embed code, typically intended to be placed in an HTML page, Sway, or supported document. Outlook’s message editor is not a supported destination for that Forms embed code. Pasting the code into an email will generally leave visible code, remove it, or fail to display a questionnaire.
This is different from inserting an image or hyperlink. An image is email content, and a hyperlink points to an external page. A live Microsoft Forms questionnaire is a hosted web application that Microsoft does not document as an Outlook email-body component.
Why Insert > Object is not a workaround
Some Outlook versions provide Insert > Object in the Text group. That feature is for file-based objects such as PDF files, Excel worksheets or charts, and PowerPoint presentations. Existing files can be selected through Create from File, with options such as Link to file and Display as icon.

Microsoft does not list Microsoft Forms as an Outlook object type. Therefore, inserting an object is not a method for creating an interactive Forms survey inside an email.
Recipient limits to check
Microsoft documents a limit of up to 500 individual recipients for a Forms invitation, including people included through groups and chats or channels. For Outlook, the documented limit is up to 500 individuals, or a group with more than 200 members. One invitation supports only one group.
For larger distributions, consider using a properly managed mailing list or another approved organizational communication method, then include the Forms URL in the message.
Which Outlook interface should you use?
The exact controls vary between Outlook versions. Microsoft’s mail-insertion instructions cover Outlook for Microsoft 365, Outlook 2024, Outlook 2021, Office 365 Small Business, Outlook.com, and New Outlook for Windows. If the New Outlook instructions do not match your screen, use the Classic Outlook instructions instead.

For add-ins, New Outlook and Outlook on the web use More apps > Add apps. While composing, use the Message tab or select Apps if the add-in is not visible. In Classic Outlook, the control may be called All Apps or Get Add-ins, depending on the version. This terminology concerns Outlook add-ins and does not provide a supported Forms-embedding feature.
Recommended email layout
A practical invitation can be structured like this:
Subject: Please complete the equipment survey
Hello,
Please complete this short survey by Friday. It should take about three minutes.
Open the Microsoft Form:
PASTE-FORMS-LINK-HERE
Thank you.
If desired, insert a small banner or QR code above the link using Outlook’s Insert > Pictures command. Keep the text link in the message as an accessible fallback.
FAQ
Can I paste a Microsoft Forms iframe into an Outlook email?
No. Microsoft documents the Forms Embed option for a webpage, Sway, or a document, not for the body of an Outlook email. Use the form link instead.
