Why does my RAG chatbot give wrong answers? Because retrieval-augmented generation (RAG) is a pipeline, not a guarantee of factual accuracy. A failure can start with missing or malformed source data, continue through retrieval and context assembly, or occur when the model makes claims its evidence does not support. Trace the question through those stages before changing settings.
How a RAG answer can go wrong
A RAG system indexes external information, retrieves passages related to a user’s question, and supplies selected context to a language model (LLM) to generate an answer. Each stage can fail independently. A chatbot may have access to a large document collection without the relevant passage ever being parsed, indexed, retrieved, or included in the context used to answer.
Start by separating three questions: Was the needed evidence available in the indexed source? Did the system retrieve and pass that evidence to the model? Did the model answer faithfully from what it received? LlamaIndex’s RAG overview describes the indexing, retrieval, and generation stages.
Trace one wrong answer through the pipeline
Capture a complete trace for a representative failed question. Keep the question, retrieved candidates and their order, scores if available, final context, prompt, and answer together. This reveals whether the break happened before generation or during it.
#1 Best Overall
- Verify the source. Find the expected fact in the original file and confirm that the file was added and processed successfully.
- Inspect parsed and indexed content. Check that the relevant wording, headings, table values, conditions, and surrounding definitions survived extraction.
- Check retrieved candidates. Determine whether a passage containing the expected evidence appears in the results. Note its position and score when available.
- Inspect final context. Compare the candidates with the exact material sent to the model. Ranking, context selection, or truncation may have removed a useful passage.
- Compare the answer with that context. If the evidence is present but the answer is unsupported or incorrect, investigate prompt behavior and generation settings.
- Repeat with a fixed question set. Run the same representative questions after each change so improvements and regressions are observable.
Fix missing or malformed source data
A retriever cannot return evidence that never made it into the index. Confirm that intended files were added and processed, then inspect the extracted text rather than assuming the source file’s layout was preserved. Tables, headings, scanned content, and conditions attached to a statement are worth checking when an answer is incomplete or misleading.
If you use OpenAI’s vector-store file API, its file reference documents file statuses and errors, including unsupported or invalid files. Treat this as a platform-specific diagnostic, not a universal indexing interface.
Adjust chunking and context assembly
Chunk boundaries affect what a retriever can match and what the model can understand. A very small chunk may contain a number but omit the definition or condition that makes it meaningful. A broad chunk may include the right fact alongside enough unrelated text to weaken relevance. Test boundaries against actual failed questions rather than adopting a chunk size by convention.

Rank #3
Also inspect the ordered passages that reach the model, not just a vector database’s initial candidate set. If the right passage is retrieved but not passed along, examine ranking, reranking, context selection, and truncation. Reducing context can make synthesis more focused, but it can also remove a definition or condition needed for a complete answer. LlamaIndex’s production guidance discusses structured retrieval and separating retrieval-oriented representations from the larger context used for synthesis. Its UpTrain integration documentation includes checks related to context reranking and conciseness.
Address retrieval misses: keywords, meaning, and constraints
When the expected passage is absent from retrieved candidates, check whether the failure involves an exact name, identifier, date, or other term that matters literally. Embeddings can retrieve semantically similar text yet miss a passage with the exact keyword. LlamaIndex discusses this limitation in its basic strategies guide.
Rank #4
Depending on the observed failure, test keyword-sensitive or hybrid search, query formulation, metadata filters, and structured retrieval. Exact-term matching and semantic similarity solve different problems; neither should be assumed to win without evaluation on your questions. If you use a hosted search service, check how its ranking controls work. OpenAI’s Evals API reference documents file-search ranking configuration, including hybrid-search weights and a score threshold. Those controls are specific to that platform.
When the evidence is present but the answer is still wrong
If the needed evidence appears in the final context and the model still gives an unsupported answer, the primary issue is no longer retrieval. Review the answer prompt and tell the model how to handle insufficient evidence: distinguish what the context states from inference, avoid adding unsupported details, and abstain or ask for clarification when the evidence does not answer the question.

Evaluate the response against the context actually supplied, not against what you intended the retriever to find. A fluent answer is not evidence that its claims follow from the passages. Prompt changes may help with a generation-side problem, but they cannot restore evidence that was missing or truncated upstream.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Evaluate retrieval and answer quality separately
Build a representative test set with user-style questions and expected relevant source documents. Where practical, add reference answers or explicit scoring guidelines. Keep retrieval evaluation separate from response evaluation so you can tell whether a change improved search, answer faithfulness, or both.
- Retrieval: compare returned document or chunk IDs with expected relevant IDs; metrics such as hit rate and mean reciprocal rank can show whether useful evidence appears and how highly it is ranked.
- Response: assess whether claims are supported by the supplied context, whether the answer addresses the question, and, when suitable references exist, whether it is correct against them.
LlamaIndex’s evaluation guide covers retrieval metrics and response evaluation, while its answer relevancy API describes assessing query-response relevance. These checks measure different properties: relevance alone does not establish factual correctness or faithfulness to context.

Account for data handling when choosing hosted services
If a hosted file-search or evaluation service is part of your pipeline, review its data retention and handling terms against your requirements before sending documents or questions. OpenAI’s data controls documentation describes platform controls and default retention behavior. Requirements vary by service and deployment, so verify the terms that apply to your own setup.
