Assume every piece of text an AI coding agent reads can be attacker-controlled. A README, issue, pull-request comment, build error, dependency, web page, or MCP tool response can contain an instruction aimed at the agent rather than at you. If the agent has shell, repository, network, or credential access, that text can lead to secret exposure, code execution, or unauthorized changes.

Detecting the problem requires more than searching for suspicious wording. Establish where the agent’s context came from, inspect the complete diff and tool-call history, examine anything that executes automatically, remove both the instruction and its effects, then rebuild and verify from a known-good state. The workflow below is designed for AI-assisted coding in local repositories and CI environments.

What a malicious instruction in AI-generated code looks like

A prompt injection is untrusted content that attempts to steer an AI system away from the user’s task or its safety rules. In a coding workflow, the payload may be visible prose, a code comment, an encoded string, or text returned by a tool. The agent can mistake that content for an authoritative instruction because it appears in the files or data it was asked to inspect.

Common locations

  • README files, contribution guides, changelogs, and issue templates.
  • Issue bodies, pull-request descriptions, review comments, and fork-PR metadata.
  • Compiler errors, test failures, generated logs, and pasted stack traces.
  • Dependencies, package metadata, install scripts, and documentation fetched from the web.
  • Dockerfiles, Makefiles, task-runner files, and deployment manifests.
  • MCP tool descriptions and tool responses.

OWASP’s guidance is direct: “Treat all repository content (issues, PRs, comments, READMEs) as untrusted input when processed by an AI coding agent.” The important distinction is between data that describes the project and an instruction that attempts to control the agent.

Warning signs in text

Look for content that addresses the agent directly, tells it to ignore earlier rules, asks for tokens or environment variables, requests a shell command, or introduces an objective unrelated to the assigned change. Examples include ignore previous instructions, print the contents of .env, or a request to upload a diagnostic archive to an unfamiliar host. Obfuscation, base64 text, zero-width characters, and instructions hidden in comments deserve the same scrutiny.

These indicators are leads, not proof. A legitimate security test may mention secrets or an adversarial string. Confirm the risk by examining what the agent did and what changed.

Why the agent’s privileges determine the damage

An instruction is dangerous when it can be converted into an action with the agent’s privileges. A suggestion-only assistant that cannot write files or call a network has a smaller blast radius than an autonomous agent that can run shell commands, read credentials, install packages, modify workflows, and push commits.

Workflow characteristic Lower-risk configuration Higher-risk configuration
Autonomy Produces a proposed diff for human approval Runs multi-step tasks and commits or merges changes
Input exposure Receives a narrow, reviewed file set Reads issues, PR comments, logs, web pages, and dependencies automatically
Tool and credential privilege Read-only workspace with short-lived, scoped tokens Shell access, production credentials, or broad repository tokens
Network egress Blocked by default or limited to an allowlist Arbitrary outbound HTTP, package downloads, or uploads
Executable configuration Workflow and build-file edits require approval Agent can change CI, package hooks, or deployment manifests without a gate
Verification Independent security and dependency checks Only the same agent’s tests are run

Passing tests does not prove that generated code is safe. Tests may not cover a new data-exfiltration path, a weakened authorization check, a malicious package hook, or a workflow change that executes only in CI.

Detection workflow: from provenance to independent verification

1. Establish provenance before editing

Write down every source the agent could have seen: named files, issue and pull-request text, comments, logs, web pages, dependency metadata, and tool descriptions or responses. Mark external and user-controlled material as untrusted. Preserve the original user request, the untrusted inputs, the generated diff, the tool-call log, and timestamps. This record lets you distinguish an intentional change from an injected one.

For a shared repository, identify the base commit and the actor that supplied each pull request. A fork or newly added dependency deserves particular attention because its content may be controlled by someone outside the maintainer group.

2. Search for instruction-like payloads

Search the relevant files and generated artifacts for language that targets an agent or requests a sensitive action. Review comments as well as executable code. Search terms such as ignore previous, system message, send secrets, curl, wget, base64, and do not tell the user can find leads, but do not treat a match as conclusive.

Inspect encoded or unusually long strings, hidden files, Unicode control characters, and comments adjacent to build commands. Then compare the content with the task: an instruction to alter authentication while fixing a user-interface defect is a strong contextual signal.

3. Review the complete diff, not just the requested file

Generate a diff against the known-good base and inspect every hunk, including new files and deleted tests. Look for:

  • Unrelated edits, especially in authentication, authorization, cryptography, logging, or privacy code.
  • Disabled tests, reduced validation, weakened error handling, or removed security policy.
  • New shell commands, subprocess calls, outbound HTTP requests, downloads, or telemetry.
  • Changes that read environment variables, SSH keys, cloud metadata, or repository tokens.
  • New obfuscation, dynamic evaluation, encoded payloads, or broad file-system traversal.

A useful review question is: “Would this line still be needed if the original task were completed without the suspicious text?” If the answer is no, isolate it for removal or escalation.

4. Inspect executable paths

Build and deployment configuration is security-critical because it executes automatically, often with trusted privileges. Review package.json lifecycle scripts such as preinstall, install, postinstall, prepare, and prepublish. Check GitHub or GitLab workflows, Dockerfiles, Makefiles, task runners, setup.py, pyproject.toml, and equivalent files for the project language.

Pay attention to newly introduced commands that pipe downloaded content to a shell, upload artifacts, alter permission settings, disable verification, or run on pull requests from forks. A harmless-looking documentation change can become dangerous if it modifies a script invoked during installation or deployment.

5. Check dependencies and CI actions

Verify package names, registries, maintainers, and versions. Use the lockfile and inspect both direct and transitive changes. A similarly named package can be substituted accidentally or deliberately. Review lifecycle hooks and install-time network access.

For CI actions, prefer immutable commit-SHA pins over mutable tags. Confirm that a newly added action is necessary, that its permissions are minimal, and that its inputs cannot be controlled by untrusted pull-request text. Review changes to permissions blocks, secret exposure, artifact uploads, and deployment triggers.

6. Audit what the agent actually did

Read the command and tool-call log in chronological order. Record commands run, files written, network destinations contacted, downloads performed, and credentials or environment variables accessed. Compare each action with the approved tool allowlist and the user’s task.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Unexpected behavior matters even when the final diff looks clean. An agent may have copied a secret into a temporary file, queried a private endpoint, or pushed a branch before reverting its edits. Check shell history where appropriate, repository audit logs, CI logs, proxy or firewall records, and cloud-provider access logs.

7. Run independent checks

Use security review and automated tools that were not authored solely by the same agent. Run static application security testing on the final diff, software-composition and dependency analysis, secret scanning, and policy checks for workflow and infrastructure changes. Have a separate reviewer inspect authentication, authorization, cryptography, secret handling, CI/CD, and deployment edits.

Independent verification should include the generated code and the configuration that runs it. A passing generated test suite is evidence that tests pass; it is not evidence that the agent followed the intended security boundary.

How to remove an injected instruction and its effects

1. Stop execution and isolate the workspace

Stop the agent, cancel active CI jobs, and prevent automatic deployment. Do not continue chatting with the same agent in the compromised context. Move the branch or workspace to an isolated environment and restrict network access while you investigate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

2. Preserve evidence

Save the original prompt, every untrusted input, the complete diff, tool-call and command logs, timestamps, and relevant CI or repository audit records. Preserve the suspicious file before editing it so investigators can determine whether the payload was delivered through a comment, dependency, tool response, or repository content.

3. Remove the payload and revert unauthorized changes

Delete the injected instruction from the source that carried it, but do not stop there. Revert every unrelated or unauthorized edit, restore disabled tests and security controls, and remove newly added hooks, actions, downloads, telemetry, and network calls that cannot be justified by the original task.

On a disposable branch, compare the work with the known-good commit and selectively restore files after review. A blanket reset can destroy useful evidence; a surgical edit can leave a hidden companion change. Have a second person confirm that the remaining diff maps to an approved requirement.

4. Rotate exposed credentials

If the agent could read or transmit a token, password, private key, cookie, cloud credential, or signing key, treat it as exposed. Revoke it and issue a replacement with the smallest practical scope. Review access logs for the period in which the agent ran, including repository, package-registry, cloud, and network logs. Do not assume that deleting a local file removes a credential from shell history, CI artifacts, caches, or remote logs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. Rebuild from a known-good commit

Start from a verified commit, reinstall dependencies from approved sources using the lockfile, and avoid reusing untrusted build caches or generated artifacts. Reapply only the intended change. Confirm checksums or signatures where your package and release process supports them.

6. Re-run independent checks and obtain approval

Repeat SAST, dependency, secret, and policy scans after cleanup. Review the final diff and agent-action log independently. Require explicit human approval for authentication, authorization, cryptography, secret handling, build scripts, workflows, deployment manifests, and any new command or network capability before merging.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

Prevention controls that reduce blast radius

Least privilege for every agent

Give the agent only the filesystem, repository, token, and network access required for the task. Prefer a disposable workspace, read-only checkout, short-lived credentials, and a separate identity that cannot deploy to production. Keep production secrets out of the environment entirely when they are not needed.

Narrow, labeled context

Supply only the files necessary to answer the request. Label issue text, comments, logs, fetched pages, dependency documentation, and MCP responses as data rather than instructions. Do not let an agent silently expand its context by crawling arbitrary URLs or reading unrelated repositories.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Allowlisted tools and egress

Allow only approved commands and MCP tools. Deny arbitrary shell execution and URL fetching where possible. Restrict outbound traffic to documented destinations, log every request, and make uploads require approval. A tool description is part of the attack surface: pin tool definitions and use snapshot-and-diff checks to detect changes.

Approval gates for executable and security-sensitive changes

Require a human gate for package hooks, CI workflows, Dockerfiles, deployment manifests, permission changes, and security-sensitive code. Configure CI to flag executable configuration changes and block merges on critical findings. Separate the person who approves a high-risk change from the process that generated it.

Provenance and dependency hygiene

Use lockfiles, verified registries, reviewable update pull requests, and immutable references for third-party CI actions. Inspect transitive dependency changes and install scripts. Keep generated artifacts traceable to a commit and build environment so an unexpected file can be investigated.

Measure the control, not just the output

OWASP’s AI Security Verification Standard (AISVS) identifies useful measures such as vulnerability density, mean time to detect, AI-attributable defect rate, prompt-injection detection rate, and fork-pull-request rejection rate. These are operational signals, not universal prevalence percentages. Track them over time to see whether review and containment controls are improving.

What’s actually slowing this PC down?

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A practical pre-merge checklist

  • All context sources are listed, and external or user-controlled text is marked untrusted.
  • The complete diff contains only approved files and requirements.
  • No tests, authentication checks, authorization rules, logging, or policy controls were weakened.
  • Shell commands, downloads, network calls, telemetry, and secret access are justified and reviewed.
  • Package hooks, workflows, Dockerfiles, task runners, and deployment manifests were inspected.
  • Dependencies and CI actions are verified, locked, and pinned where possible.
  • Agent commands, tool calls, destinations, file writes, and credential use match the allowlist.
  • Independent SAST, dependency, secret, and policy checks pass on the final diff.
  • Any potentially exposed credential has been revoked or rotated, with access logs reviewed.
  • Required human approvals are recorded before merge or deployment.

How to assess an AI coding workflow

Compare workflows on the controls that determine blast radius rather than on code-completion speed alone. Ask whether the tool can read untrusted repository content, which credentials it receives, whether it can modify executable configuration, where it can connect, and who approves the resulting diff.

Question Evidence to request
What can the agent read? Explicit file, issue, PR, log, dependency, web, and MCP context boundaries
What can it execute? Tool allowlist, shell policy, sandbox settings, and approval prompts
What can it access? Filesystem permissions, token scopes, secret locations, and network egress rules
What can it change automatically? Commit, push, workflow, package-hook, infrastructure, and deployment permissions
How is activity recorded? Command, tool-call, network, repository, and CI audit logs with retention
Who verifies the result? Independent reviewers and scanners separate from the generating agent

A completion that passes tests but can read secrets and push changes has a larger blast radius than a suggestion-only tool operating in a sandbox. Choose the workflow with the narrower context and stronger approval boundaries when the code or data is sensitive.

What is and is not known about prevalence

There is no universal prevalence statistic establishing how often malicious instructions appear in AI-generated code. Treat the absence of a published rate as a reason to use controls, not as evidence that the risk is rare. The observable facts in your own environment—unexpected instructions, agent actions, rejected fork pull requests, detected vulnerabilities, and time to containment—are more useful for prioritizing defenses.

Bottom line for a safe merge

Read repository and tool content as hostile input, then verify behavior rather than trusting intent. Stop and isolate a suspicious run, preserve evidence, inspect the full diff and executable configuration, rotate anything the agent could access, rebuild from a known-good commit, and rerun independent checks. Keep future agents inside least-privilege sandboxes with narrow context, allowlisted tools and network destinations, pinned dependencies and tool definitions, and human gates for security-sensitive changes.
