A human approval step is useful only when the reviewer can understand the proposed action, has authority to change or stop it, and knows the approval will apply to the action that actually runs. Design oversight around the task and its risks—not as a confirmation button added to every workflow.
When does a workflow need human oversight?
It depends on the task, its context, and the potential impacts—not simply on whether AI is involved. NIST’s AI Risk Management Framework (AI RMF) describes configurations ranging from fully autonomous to fully manual and notes that some systems may need human oversight while others may not. For example, its Appendix C says that some AI systems may not require oversight, while others may specifically require it. NIST AI RMF 1.0, Appendix C
Start by mapping what the workflow does, who could be affected, where the system is limited, and what could happen if it is wrong. Revisit that context as the system, its capabilities, and its impacts change. A checkpoint is most defensible when human judgment can materially reduce a meaningful risk or when a policy or applicable legal or sector requirement calls for it.

- Gate an action when its consequences warrant a person’s judgment, intervention, or accountability.
- Use a different control when a review would add delay without giving the reviewer useful information or a realistic ability to affect the outcome.
- Check applicable obligations separately: NIST’s framework is voluntary guidance, not a substitute for legal or sector-specific requirements.
What makes an approval meaningful?
A reviewer needs a defined responsibility, interpretable context, time and competence to assess the proposal, and authority to reject, revise, pause, or escalate it. A click or signed record alone cannot show that a person understood the proposal or exercised judgment.

Human review is not automatically a safeguard. NIST notes that AI opacity and human cognitive biases can shape judgment; depending on how people and systems interact, their combination can amplify bias or produce complementary strengths. How to design the approval workflow
- Map the task and its impacts. Identify the proposed action, affected people, operating context, system limitations, and plausible harms or benefits. Use that map to decide which actions need review and why.
- Assign distinct roles. Specify what the AI system proposes, what the operator monitors, what the reviewer evaluates, who owns the final decision, and who handles escalation. Document communication lines and train people for their assigned duties. - Show the decision in usable form. Give the reviewer the information needed to interpret the proposed action and its relevant context. Make uncertainty, important limitations, and the consequences of proceeding legible where they matter to the decision.
- Give the reviewer real authority. Make rejection, correction, pause, and escalation workable paths—not exceptions that require an informal workaround. Define who receives an escalation and what happens while it is pending.
- Bind approval to execution. The action shown for review must be the action authorized and released. Preserve the identity and parameters of the approved operation, and check that identity at execution rather than relying on the appearance of a confirmation screen.
- Monitor and revise. Review system performance and outcomes over time, provide feedback and appeal routes, and revisit whether the checkpoint still fits the task. NIST identifies ongoing monitoring, periodic review, and analysis of how often and why people override AI output as useful practices; it does not prescribe a universal override threshold. How to keep approval attached to the action that runs
Approval is an integrity boundary, not just a user-interface event. If workflow state changes after review, or if the reviewed representation differs from the operation used at execution, a person may approve one thing while another is carried out.

A September 2026 arXiv preprint by Adithyan Arun Kumar, Loopjacking: Hijacking Human-in-the-Loop Approval, reports tested cases in which workflow state or representation could separate the reviewed operation from the one executed. The paper describes mitigations including fully rendering the canonical operation for review and comparing it exactly at use time. Its purposive sample demonstrates possible failure modes; it does not estimate how common they are across products or the wider ecosystem. Read the preprint

- Preserve a stable identity for the reviewed operation.
- At the point of execution, verify that the operation still matches what was authorized; require renewed review if material details have changed.
- Keep a reconstructable record of what was presented, who authorized it, and what was executed.
How to tell whether oversight is working
Evaluate the workflow, not just whether its approval log contains a click. NIST’s AI RMF organizes risk management into four functions—Govern, Map, Measure, and Manage—and treats it as continuous across the system lifecycle. For a design review or software evaluation, examine these questions:
- Which actions are gated, and what risk or requirement justifies each gate?
- Can the reviewer see the actual operation and enough context to assess it?
- Does authorization remain bound to the operation that executes?
- Are reviewer competence, decision authority, and escalation responsibilities defined?
- Can an auditor reconstruct what was proposed, reviewed, approved, and carried out?
- Can people provide feedback or appeal, and does the organization periodically assess outcomes?
Track override frequency and rationale as signals to investigate, not as a universal pass/fail score. A low override rate alone does not prove good decisions; a high one may point to poor system output, a confusing interface, mismatched authority, or a workflow that needs adjustment. Interpret the numbers alongside case review and outcome monitoring.

Which NIST guidance applies, and what are its limits?
NIST released AI RMF 1.0 on January 26, 2023, describes it as voluntary, and says it is being revised.
