Find the first boundary where the expected parent-child relationship disappears, then determine whether the cause is context propagation, missing instrumentation, or telemetry export. A trace remains continuous only when the sending component injects its context, the receiving component extracts it before creating downstream spans, and every important operation is instrumented and exported to the same backend.
What a missing or fragmented trace actually means
A trace is a connected set of spans describing one request or agent run. Each span represents work such as prompt handling, model invocation, tool execution, queue processing, database access, or a remote agent step. The parent-child links between spans are what let a viewer reconstruct the order and causality of that work.
When a trace is missing or fragmented, three different problems can look similar:

A free scan shows the junk files, broken settings and background clutter dragging Windows down - then fixes them in one click.Free scan · Windows 10 & 11- Context propagation failure: the downstream process never receives the upstream trace context, or it receives it but does not use it when starting its span. The downstream span may therefore have a new trace ID or no expected parent.
- Instrumentation gap: context arrived correctly, but the operation was never instrumented. No span can appear for an uninstrumented operation.
- Export or visibility failure: the span was created locally but was sampled out, sent to a different collector or project, rejected, or hidden by backend settings.
Start by locating the exact break rather than searching the entire system for “missing traces.” Record one affected request, the last visible span, the first operation that should have followed it, the services or processes involved, and the time window. Search both traces and logs with the trace ID and span ID. This narrows the investigation to one producer-consumer boundary.

Use the trace model to locate the break
Trace ID, span ID, and parent span ID
The trace ID identifies the overall execution. A span ID identifies one unit of work, while a parent span ID identifies the span that caused that work to begin. A healthy multi-step agent run usually has one trace ID across its service, worker, tool, and model boundaries, with parent-child relationships that match the execution flow.
Capture these values at each hop:
- The upstream span ID that ends immediately before the hand-off.
- The downstream trace ID, span ID, and parent span ID.
- The service name, process or worker identity, and timestamp for each span.
- The destination used by the exporter or collector.
If the downstream trace ID differs, inspect propagation first. If the IDs match but a span is absent, inspect instrumentation and sampling. If local logs show a span ID but the tracing backend does not, inspect export and backend routing.
Why logs can prove a span existed
OpenTelemetry describes context propagation as the mechanism that correlates traces, metrics, and logs across execution units. Correlated logs can show that a worker received a request and created a span even when the central trace view is incomplete. A log containing the same trace ID and span ID as an upstream request is evidence that the worker had usable context; it does not by itself prove that the span was exported successfully.
OpenTelemetry summarizes the purpose of propagation this way: “With context propagation, signals (traces, metrics, and logs) can be correlated with each other, regardless of where they are generated.”
Inspect every producer and consumer boundary
1. Verify injection at the sending side
At an HTTP, messaging, queue, or tool-adapter boundary, the producer must inject the current context into the carrier. The carrier might be HTTP headers, message properties, or framework metadata. Capture the outgoing carrier immediately before the request is sent, not only in an application log created earlier.
For HTTP propagation with OpenTelemetry’s default propagator, inspect the W3C Trace Context traceparent header. It carries the upstream trace and parent information in a standardized form. Confirm that the value is present on the actual network request and still present after any proxy, gateway, retry wrapper, queue bridge, or tool adapter.
2. Verify extraction before downstream spans start
The receiver must extract the context before it creates the server, worker, or tool span. Extraction that happens after the span starts cannot retroactively make that span a child of the caller. Log the extracted trace ID and parent span ID at the first line of the receiving handler or worker entry point.

Then compare the extracted values with the producer’s outgoing values. A mismatch indicates one of the following:
- The carrier was altered or dropped in transit.
- The receiver is reading a different header or metadata field.
- A framework created a fresh context before extraction.
- Multiple propagation formats are configured inconsistently.
3. Confirm the extracted context is used
Extraction only places a context in scope. The downstream tracer must use that context as the parent when starting the span. Some frameworks automatically bind extracted context to the request handler; custom middleware, thread pools, and tool wrappers may not. Compare the parent span ID recorded on the first downstream span with the span ID that was active at the sending boundary.
4. Check that intermediaries preserve the carrier
Proxies and API gateways can remove headers they do not recognize. Messaging bridges can copy application fields while dropping tracing metadata. Retries can create a new request path whose headers differ from the first attempt. For each intermediary, capture the carrier entering and leaving it. A missing traceparent after one hop identifies the responsible boundary much faster than inspecting the final backend trace.
Separate propagation from instrumentation
When context is correct but a span is absent
A correctly propagated context cannot create a span for an operation that was never instrumented. Check whether the model client, tool library, queue consumer, database driver, or custom function has instrumentation enabled for the deployed language and version. Agent frameworks often instrument orchestration steps while leaving user-defined tools or network clients manual.
Look for evidence in local telemetry rather than assuming the backend discarded the span. If the downstream handler logs the expected trace ID but no child span ID is ever created, the operation is probably not instrumented or span creation is disabled on that code path.
When a span exists but its content is incomplete
A span can be present while important attributes or events are missing. Content recording may be disabled for privacy reasons. GenAI semantic-convention attributes may require an explicit opt-in. An operation may be represented by a generic span because the library-specific instrumentation does not support it. In Microsoft Foundry’s guidance, disabled content recording, missing GenAI semantic-convention opt-in, and uninstrumented operations are all listed as reasons that spans can be incomplete or absent.
Do not “fix” incomplete content by enabling every payload globally. Decide which prompts, tool arguments, outputs, and identifiers are safe to record, then apply redaction and retention controls appropriate to your environment.

When spans exist locally but not centrally
Inspect the exporter and collector path after confirming that the application created the span:
Free tools Windows power users keep installed
One-click scans. No signup required.
- Check exporter endpoint, protocol, credentials, and TLS settings.
- Check collector routing rules and whether the service name is being rewritten.
- Check sampling decisions at the SDK, collector, and backend.
- Check that the application uses the intended project, workspace, or Application Insights connection string.
- Check backend ingestion errors, rate limits, and retention windows.
A trace may appear in one destination while its child spans are routed elsewhere. Search all configured projects and collectors for the child span ID before concluding that it was lost.
Debug asynchronous, queued, and remote agent work
Tasks and worker pools
A spawned task may not inherit the caller’s active context automatically. Thread pools, process pools, callback queues, and coroutine schedulers can each change the execution context. At task creation, record the parent span ID; at task start, record the extracted or explicitly attached context. Compare those values with the span created inside the task.
Do not rely on a global variable as a substitute for context propagation. Concurrent agent runs can overwrite shared state and attach one user’s trace to another user’s work. Use the context mechanism provided by the OpenTelemetry SDK or the framework’s documented task-local facilities.
Queues and delayed delivery
For a queue, the producer injects context into message metadata and the consumer extracts it when handling the message. Preserve the metadata through serialization, dead-letter routing, retries, and scheduled delivery. Decide whether a retry should be a child of the original attempt or a new span linked to it; document that choice so the trace viewer matches your operational model.

Remote execution can legitimately look like separate runs when the client and server do not share propagated context. LangChain’s support documentation describes this behavior for a LangGraph Server RemoteGraph client/server pattern: the client-side and server-side runs can appear separately unless distributed tracing is enabled and the server is configured to accept propagated context.
For a remote graph or tool service, verify all of the following:

- Distributed tracing is enabled in the client and server deployments.
- The client injects context into the transport used by the remote call.
- The server accepts and extracts that context before creating its run span.
- The server’s exporter sends spans to the destination you are querying.
- The framework and SDK versions in the deployed environment support the settings you enabled.
If the remote service intentionally starts an independent trace, preserve a reliable correlation field and document the relationship. Do not label two independent traces as one continuous trace merely because they belong to the same user request.
Use a controlled reproduction instead of a full production run
Reduce the failing workflow to one request and the smallest number of agent steps that still reproduces the break. Run a direct local call and then the asynchronous or remote version. The comparison reveals whether the missing relationship is introduced by the transport or by the operation itself.
- Start a request with a known trace ID visible in application logs.
- Record the active span ID immediately before each outbound call.
- Capture the outgoing carrier, including
traceparentfor HTTP. - At the receiver, log the raw carrier arrival and the extracted trace and parent IDs.
- Record the span ID created for the receiving operation.
- Record the exporter result or collector response for that span.
- Search the backend by trace ID, span ID, service name, and the exact time window.
This sequence follows OpenTelemetry’s documented inject-at-send and extract-at-receive model. It is a diagnostic method, not a guarantee that every framework exposes raw carriers in the same way. If a framework hides the carrier, enable its transport or middleware diagnostics temporarily, or add a narrow wrapper around the boundary.
Interpret common symptoms correctly
| Observed symptom | Most useful first check | What the result means |
|---|---|---|
| Downstream spans have a different trace ID | Compare outgoing and incoming traceparent or message metadata |
The carrier was dropped, changed, or not extracted; alternatively the receiver intentionally started a new trace. |
| The parent span ends and no child span appears | Check instrumentation for the expected operation and local span creation logs | The operation may be uninstrumented, disabled, sampled out, or exported elsewhere. |
| A remote agent appears as a second complete run | Check distributed-tracing and propagated-context settings on both client and server | The remote server may not be accepting the client’s context. |
| Trace exists but prompt or tool details are absent | Check content-recording controls and GenAI semantic-convention configuration | Privacy or semantic-convention settings may intentionally suppress attributes and events. |
| Logs contain the child span ID but the UI does not | Check exporter, collector routing, credentials, sampling, and project selection | The application created the span, but the backend did not ingest or display it in the searched destination. |
| Only some retries or tools are visible | Compare each attempt’s carrier, instrumentation path, and sampling decision | Retries may use a different wrapper, tool libraries may lack instrumentation, or sampling may vary by span. |
Check configuration and project metadata
Some failures are not in the trace graph at all. A service can be sending valid spans to a project, workspace, or Application Insights resource different from the one selected in the viewer. Confirm the connection string or exporter endpoint in the running deployment, not only in a local configuration file. Verify that the service name and environment attributes identify the deployment you are searching.
“Missing Project or Metadata” is therefore a useful problem label: a trace may be present but difficult to find because project selection, service naming, resource attributes, or time filters do not match the deployment. Search by an exact trace ID first, then broaden to service and time only after confirming the ID is not available.
Sampling can also create an apparently incomplete trace. Parent-based sampling may keep a parent while dropping later work, or separate processes may apply different sampling policies. Compare sampling decisions in the SDK and collector. For a diagnostic reproduction, use a policy that retains the test request, then restore the production policy after the boundary is verified.

Protect trace context and agent data
Treat incoming headers as untrusted
External callers can forge trace headers. OpenTelemetry warns that forged incoming headers can manipulate telemetry data or target parsing vulnerabilities. Validate header syntax through the SDK and avoid using an externally supplied trace ID as an authorization, tenancy, or billing identifier.

Control baggage and outgoing propagation
Baggage and propagated context can leak internal details to downstream services. Do not place credentials, personal information, prompts containing secrets, or other sensitive values in baggage. Before forwarding context to an untrusted service, apply an explicit allowlist of fields and propagation destinations. Trace IDs are useful correlation values, but they are not confidential authentication tokens.
Record agent content deliberately
Prompts, tool arguments, retrieved documents, and model outputs may contain personal or proprietary information. Use redaction, hashing, truncation, access controls, and retention limits where appropriate. A complete parent-child graph with no payload content is often safer and sufficient for diagnosing propagation.
Framework and observability choices
If your stack already emits OpenTelemetry data, keep propagation and export in that ecosystem so service boundaries use one context model and one collector path. When evaluating a tracing product or viewer, compare the properties that affect this failure mode rather than choosing on the interface alone.
| Decision axis | Questions to answer |
|---|---|
| Process and remote propagation | Does context survive HTTP, messaging, task queues, tool adapters, and remote workers? |
| Instrumentation coverage | Which agent frameworks, model clients, tool libraries, and languages have supported instrumentation? |
| OpenTelemetry integration | Can the system receive OpenTelemetry spans and route them through existing collectors? |
| Span and content controls | Can you disable payload capture, apply redaction, select GenAI attributes, and control sampling? |
| Deployment and retention | Where is telemetry stored, who can access it, and how long is it retained? |
| Debugging views | Can operators inspect parent-child relationships, trace and span IDs, retries, and multi-turn agent threads? |
LangChain describes LangSmith as providing native tracing for common agent frameworks and OpenTelemetry support. Treat that as a product description, and verify current framework compatibility, export behavior, retention, and privacy controls against the versions and deployment model you operate.
A repeatable runbook
- Select one failed request. Record its trace ID, last visible span, expected next operation, services involved, and exact timestamps.
- Search all telemetry destinations. Use trace ID and span ID across logs, collectors, projects, and regional backends.
- Mark the first missing child. Do not begin with the final absent model or tool span; begin where the parent-child chain first diverges.
- Inspect the producer. Confirm the active parent span and capture the injected carrier.
- Inspect the transport. Check proxies, queues, wrappers, retries, and adapters for carrier preservation.
- Inspect the consumer. Confirm extraction occurs before span creation and that the extracted context becomes the parent.
- Check instrumentation. Verify the missing operation is instrumented and that content or GenAI semantic-convention settings are not suppressing the expected data.
- Check export. Follow the span from SDK to collector to backend, including credentials, routing, sampling, and project selection.
- Reproduce minimally. Compare local and remote or asynchronous paths with one retained diagnostic request.
- Apply the smallest fix. Restore propagation, add only the required instrumentation, or correct routing; then rerun the same request and compare IDs.
- Keep security controls. Do not solve visibility by forwarding unrestricted baggage or recording sensitive payloads.
What a successful repair looks like
After the fix, the producer’s final span ID should match the parent span ID recorded on the first downstream span. The trace ID should remain stable across every boundary that is intended to be part of the same run. Logs should contain the corresponding IDs, and the exporter or collector should acknowledge the spans in the destination you query.
Operations that remain absent should have an explicit explanation: they are outside instrumentation coverage, intentionally sampled, intentionally content-redacted, or intentionally modeled as a separate trace. A trace is reliable when those choices are visible and deliberate—not when every possible payload is recorded.
