A critical-infrastructure incident response plan must do more than describe malware cleanup. It must help people make safe, authorized decisions while essential services continue, even when corporate networks, process-control systems, or normal communications are unavailable. Build the plan around essential functions, assign authority and alternates, provide an offline first-hour workflow, define OT-safe containment and recovery gates, map reporting duties to your jurisdiction, and exercise the complete cycle through restoration.

The current NIST baseline is SP 800-61 Revision 3, finalized in April 2025. It supersedes Revision 2 and places incident response inside the broader CSF 2.0 risk-management cycle.

Use the current risk-management model

NIST finalized SP 800-61 Revision 3 in April 2025. Revision 3 supersedes Revision 2 and no longer treats incident response as a self-contained collection of technical procedures. Instead, it connects response to CSF 2.0:

  • Govern, Identify, and Protect provide the preparation and risk-management foundation.
  • Detect, Respond, and Recover describe the operational incident-response functions.
  • Improvement analyzes results and feeds lessons back into every function.

For an operator, this means the plan is one controlled part of an ongoing management system. Asset inventories, dependency maps, authority assignments, protective controls, exercises, and corrective actions all affect whether the response will work. A document that is accurate only on the day it is approved is not an operational plan.

Use the plan to answer five questions during pressure:

  1. Which essential service or safety function is at risk?
  2. Who has authority to make the next service-impacting decision?
  3. What can responders safely isolate, preserve, or restore?
  4. How will the organization communicate if ordinary systems are compromised?
  5. Which internal and external notifications are required, and by when?

1. Define essential functions before listing technologies

Start with the services the organization must deliver, not with a list of servers or security tools. For each essential function, document the consequence of degradation, interruption, manipulation, or an unsafe operating state. Then map the systems, facilities, people, data, suppliers, and supporting infrastructure required to deliver it.

Build an essential-function dependency map

Include both information technology and operational technology. Depending on the sector, the map may include supervisory control and data acquisition components, engineering workstations, safety systems, production networks, identity services, telecommunications, data historians, laboratories, dispatch systems, and remote-access paths. Record dependencies that are easy to overlook, such as electrical power, cooling, fuel, physical access, environmental controls, timing services, and specialist contractors.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For every dependency, record an owner, location or logical zone, normal operating mode, safe fallback, and the consequence of losing it. Link important datasets to the process that uses them and identify where authoritative copies, backups, and retention requirements are held.

Prioritize by consequence

Use a risk assessment to rank functions by safety, public-health, environmental, legal, financial, and service-delivery consequences. The ranking should drive triage and escalation. An event affecting a low-criticality office application may need investigation, while a smaller anomaly on a system that controls a vital process may require immediate coordination with operations and safety personnel.

Define impact levels in language your operators can use. For example, a level can distinguish loss of visibility from loss of control, a degraded but stable process from an unsafe process, and a short service interruption from an interruption that threatens life, environmental protection, or statutory obligations. Do not copy a generic severity scale without testing it against actual process consequences.

Record assumptions and boundaries

State which facilities, subsidiaries, cloud services, joint ventures, suppliers, and outsourced functions are covered. Explain how the plan interacts with physical-security, safety-management, business-continuity, disaster-recovery, crisis-communications, and emergency-management plans. Identify who owns each boundary so that responders do not discover during an attack that a crucial system or supplier is outside the plan.

2. Assign authority, roles, and resilient contacts

Put people-and-process information near the front of the document. The UK National Cyber Security Centre (NCSC) describes incident response planning as “a critical step towards a robust and effective incident management and technical response capability” in its incident response processes guidance.

Name a central response lead

Appoint one incident coordinator for each incident and define the authority of that role. The coordinator maintains the operating picture, convenes the right specialists, assigns actions, records decisions, and escalates when thresholds are met. The coordinator should not unilaterally make a process-safety decision merely because the incident began in IT.

Document named roles and alternates for:

  • Cybersecurity monitoring, investigation, and containment.
  • OT operations, control-room leadership, process engineering, and instrumentation.
  • Safety, environmental protection, and physical security.
  • Executive decision-making and business-service ownership.
  • Legal counsel, privacy, regulatory affairs, communications, and public relations.
  • Human resources, insurance, procurement, key suppliers, and specialist responders.
  • Law-enforcement, emergency-management, sector-regulator, and national cyber-agency liaison.

Define decision rights and alternates

For each role, state which decisions it may approve, which require a second authority, and which must be escalated. Include triggers for declaring an incident, moving to crisis management, changing process modes, disconnecting remote access, shutting down equipment, restoring from backup, or returning to normal service.

Assume that people will be unavailable. NCSC recommends at least two contact methods and details for two or more people or groups. Maintain primary and backup contacts, time-zone coverage, supplier escalation routes, and a method for confirming that a caller is authorized. Review those details after personnel, vendor, facility, or telecommunications changes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Provide communications that survive a network outage

List an incident bridge or conference method that remains available for urgent calls, plus an alternate channel if corporate email, identity services, mobile networks, or voice systems are disrupted. Describe how participants authenticate one another and how sensitive information is handled. Keep printed contact cards, an offline copy, or another independently accessible version in locations that responders can reach during a facility or network disruption.

3. Make the first hour executable

The first-hour section should be a short flow, not a narrative policy. It should work for a shift supervisor who has never met the incident coordinator. NCSC guidance recommends basic instructions that cover at least the first few hours; adapt the sequence below to your operating environment and safety procedures.

  1. Receive and record the alert. Capture the reporter, time, affected location, systems, observed symptoms, and immediate safety concerns. Do not delay a safety response while completing a form.
  2. Stabilize the situation. Confirm whether people, the environment, or the physical process is in danger. Follow existing emergency and safe-state procedures before attempting cyber containment.
  3. Open the incident record. Assign a unique identifier, incident coordinator, severity, current status, and next review time. Start separate logs for actions, decisions, findings, communications, and evidence.
  4. Perform an initial triage. Establish what is known, what is suspected, what essential functions are affected, and whether the activity is spreading. Record uncertainty rather than presenting assumptions as facts.
  5. Declare and escalate when thresholds are met. Notify the required operational, safety, executive, legal, and communications roles. Activate the crisis structure if essential services, safety, regulated data, or public confidence are at material risk.
  6. Preserve evidence. Record volatile information and relevant logs according to approved procedures. Avoid reimaging, rebooting, or changing configurations when doing so could destroy evidence or create process risk.
  7. Select an approved response option. The OT, engineering, safety, and security leads jointly assess isolation, segmentation, continued degraded operation, or controlled shutdown. Document the expected benefit, hazards, authority, and rollback.
  8. Communicate a single operating picture. Issue a concise situation report with facts, decisions, owners, open questions, and the next update time. Restrict distribution to people who need the information.

Attach short checklists and forms to the plan: incident declaration, contact activation, evidence collection, decision approval, notification, shift handover, and recovery validation. A form that cannot be completed during a noisy control-room event is too complicated.

4. Establish decision gates for containment, eradication, and recovery

There is no universally safe isolation command for every industrial or essential-service system. An action that limits an attack in an office network can interrupt a physical process, remove operator visibility, or defeat a safety function. Use decision gates that force the right specialists to consider both cyber risk and operational consequence.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use six comparison axes

For each response option, record:

  • Effect on safety and essential-service continuity.
  • Expected containment effectiveness and risk of further spread.
  • Evidence preservation and investigative value.
  • Recovery time and ability to operate in a degraded mode.
  • Reporting, contractual, and legal consequences.
  • Authority, skills, equipment, and staffing available to execute the action.

Compare common response choices

Option When it may be appropriate Risks and required controls
Isolate a host, account, or network segment Evidence or threat indicators show that separation can limit spread without removing a required control or safety function. Loss of visibility, remote support, or process coordination; obtain OT and safety approval, define a rollback, and verify the physical effect.
Continue in a controlled degraded mode The process is stable, immediate isolation could create greater harm, and operators have an approved manual or fallback procedure. Longer exposure and fatigue; set monitoring, staffing, decision-review, and time limits.
Controlled shutdown or transfer Safety, environmental, or integrity risks exceed the consequences of stopping or moving the service. Requires process-specific shutdown authority, communications with affected parties, and a restart-validation plan.
Restore a known-good component Compromise is understood well enough to prevent reinfection and the backup can be validated. Restoring contaminated data or credentials can reintroduce the attacker; verify provenance, dependencies, and monitoring before reconnecting.

Separate eradication from recovery

Eradication removes the cause or foothold, such as malicious persistence, unauthorized accounts, or vulnerable access paths. Recovery returns the function to an approved operating state and proves that it is safe and reliable. Define entry and exit criteria for each phase, including required scans or inspections, credential changes, configuration checks, operator acceptance, and heightened monitoring.

Preserve a decision trail even when the decision is to do nothing immediately. The record should identify the authority, evidence considered, expected consequences, review time, and conditions that would trigger a different action.

5. Create scenario playbooks for the highest risks

The main plan explains governance and common flow; playbooks explain how to handle particular incidents. NCSC examples include malware infection, phishing, and data breach. A critical-infrastructure operator should add scenarios derived from its own risk assessment and sector, such as loss of remote access, manipulation of control logic, ransomware affecting engineering workstations, compromise of a supplier connection, or destruction of configuration data.

Give every playbook the same usable fields

  • Recognition clues and the information needed to confirm or reject the scenario.
  • Contacts, alternates, severity criteria, and the incident-declaration threshold.
  • Immediate safety and process checks.
  • Evidence to preserve, collection authority, and chain-of-custody requirements.
  • Containment and continuity options, with OT-specific approval gates.
  • Internal, supplier, regulator, law-enforcement, and public-communications actions.
  • Recovery prerequisites, validation tests, and heightened monitoring.
  • Post-incident questions and corrective-action owners.

Adapt playbooks to the facility

Do not assume that a playbook written for corporate endpoints applies to a plant, utility, transport network, or healthcare process. List the actual control zones, maintenance windows, manual procedures, safety interlocks, vendor dependencies, and physical access constraints. Where a step depends on a local drawing, configuration, or contact list, reference the controlled location of that material and provide an offline access method.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

6. Build a jurisdiction-aware reporting matrix

Reporting obligations vary by country, sector, incident type, organization, and contract. Do not place a universal deadline in a generic plan. Instead, create a matrix that your legal and regulatory teams populate for each operating jurisdiction.

Matrix field What to specify
Trigger The impact, data type, service condition, safety consequence, or contractual event that requires notification.
Information package Facts to collect, confidence level, affected services, timeline, indicators, actions taken, and information that may follow later.
Receiving authority Sector regulator, national cyber agency or CERT, law enforcement, emergency management, insurer, customer, supplier, or other contractually named recipient.
Responsible role The person who approves, sends, updates, and records the notification, with an alternate.
Timing and update rule The applicable legal, regulatory, or contractual timing and the conditions for supplemental reports.
Recordkeeping Submission method, confirmation, reference number, recipients, and copies retained in the incident record.

Use the operator’s own regulator, counsel, national cyber authority, and sector guidance to complete the matrix. The NCSC framework discusses thresholds, report detail, and authorities and notes that voluntary reporting may be useful in the UK context; that does not create the same duty for every organization. NIST’s incident-response preparation resources index includes specialized material for water and wastewater, including CISA and FBI resources, an EPA incident action checklist, and FEMA/CISA emergency-manager guidance. These are routes to investigate, not evidence that every operator has identical requirements.

Prepare a communications plan for employees, customers, emergency services, regulators, suppliers, and the public. Legal, safety, and operational leads should approve messages before release where feasible. Never allow conflicting technical and public statements to obscure the current safety status.

7. Connect cyber response to continuity and restoration

Cyber response cannot restore an essential function by itself. Link the plan to business-continuity, disaster-recovery, emergency-management, and internal and external communications plans. Identify who can authorize degraded service, a manual workaround, transfer to another site, or a controlled shutdown.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Define recovery objectives that operators can use

For each essential function, record a recovery time objective, acceptable data-loss or data-reconstruction point, minimum safe operating level, and validation evidence. These targets should come from service and safety analysis, not from a generic technology standard. State which objective has priority when they conflict.

Document practical alternatives

List tested backup mechanisms and the credentials, media, tools, and people needed to use them. Consider alternatives such as mobile equipment, temporary facilities, failover sites, manual processing, alternate suppliers, and local control when those options are feasible for the facility. Include dependencies that can defeat an otherwise good backup, such as unavailable power, cooling, network routes, identity services, replacement parts, or vendor support.

Validate before returning to normal

Recovery is complete only after the process owner and safety authority accept the restored state. Validation can include configuration comparison, integrity checks, functional tests, communications checks, monitoring for reinfection, and confirmation that protective controls are active. Keep enhanced monitoring and a clear rollback path until the incident coordinator and service owner agree that normal operations are safe.

8. Prepare an OT-aware forensic and response capability

NISTIR 8428, Digital Forensics and Incident Response (DFIR) Framework for Operational Technology (OT), was published on June 22, 2022. NIST lists Eran Salfati and Michael Pease as its authors. The framework extends traditional response with event escalation, OT-team preparation, and OT digital-forensics techniques; it is supporting guidance, not a procedure that can be copied without adapting to the facility’s safety and process constraints. See the NISTIR 8428 publication page.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Prepare the OT team before an incident by documenting process context, normal states, engineering-change procedures, vendor contacts, time sources, maintenance constraints, and safe evidence-collection methods. Decide in advance which actions require a control-room supervisor, process engineer, safety professional, or equipment vendor. Traditional endpoint collection may be unsafe or technically impossible on a controller, protection relay, medical device, or embedded system.

Identify OT-aware forensic resources, preservation media, write-protection methods, portable logging, and specialists who can work without disrupting the process. Define how evidence moves from the facility to an analysis environment and how chain of custody is recorded. If internal expertise is limited, arrange external specialist help before an incident and verify its availability, OT experience, sector familiarity, confidentiality terms, and authority to work at the site.

9. Choose an achievable response-capability model

Capability can be internal, externally augmented, or predominantly external. The right choice depends on the site’s staffing, process knowledge, risk, budget, and need for immediate availability. Compare models against the following practical dimensions rather than assuming one is universally superior.

Model Strengths Limitations to plan for Controls needed
Predominantly internal Strong knowledge of the site, process, dependencies, and authority structure. Limited surge capacity, specialist forensics, or round-the-clock coverage. Cross-training, alternates, exercises, retained specialist contacts, and fatigue management.
Internal lead with external augmentation Local decision ownership combined with rapid access to specialist investigation or recovery skills. Handoffs, access approvals, confidentiality, and supplier availability can delay action. Pre-agreed scopes, access methods, evidence rules, rates, escalation contacts, and exercises with the external team.
Predominantly external response Broader specialist capacity and potential 24-hour coverage. Less familiarity with the process, slower safety context, and dependency on communications and contracts. Named internal process and safety authorities, clear command relationships, site orientation, and acceptance criteria.

Whichever model you use, retain an internal person who can authorize service-impacting actions and explain the process consequences to responders.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

10. Exercise, review, and improve the plan

Exercises should be risk-informed and should run through restoration of essential functions, not stop after the technical team identifies malware. Use scenarios from the organization’s risk assessment, incidents affecting the organization or peers, and current threat information. Include shift changes, unavailable contacts, compromised identity systems, supplier delays, incomplete evidence, and conflicting service and safety priorities.

Test the complete cycle

  1. Start with an alert or operational anomaly.
  2. Activate contacts and establish the incident record.
  3. Make a triage and escalation decision.
  4. Test an OT-safe containment or continuity choice.
  5. Produce a regulator or stakeholder notification package.
  6. Restore the essential function and perform acceptance checks.
  7. Close the incident, preserve records, and brief leadership.

Record observations, assign corrective actions with owners and due dates, and verify closure. Feed lessons into both the response plan and protective security measures. NCSC expects routine exercising and use of findings, but its reviewed guidance does not establish one universally correct interval. Set the interval according to risk, regulatory expectations, major system or supplier changes, staff turnover, and results from previous exercises.

Review after material change

Recheck contacts, authority, system and supplier dependencies, reporting criteria, backups, communication routes, and recovery procedures after an incident, exercise, acquisition, facility modification, control-system change, network redesign, major contract change, or regulatory update. Keep version history and approval records so responders know which copy is authoritative.

What the finished plan should contain

Before approval, the controlled plan should provide these usable components in the order responders need them:

What’s actually slowing this PC down?
