Build an AI incident process as a documented route from observation to intake, triage, investigation, response, recovery, communication, and learning. NIST’s AI Risk Management Framework (AI RMF) offers voluntary guidance for that work; it does not set a universal incident definition, reporting deadline, or legal notification duty. Your process must fit your systems, the people affected, and the rules that apply to your organization.
What an AI incident reporting process does
An AI incident reporting process tells people how to raise a concern about an AI system and tells the organization what happens next. It should work for more than confirmed failures: a suspected problem, a near miss, an unexpected output, or a report of potential harm may warrant review even before the facts are clear. The organization defines what counts as an incident for its own operations and explains how uncertain cases are handled.
The process is not just an inbox. It connects reports to accountable people, timely decisions, investigation, mitigation, communication, recovery, and changes to controls or systems. Without those links, an organization may collect reports without recognizing patterns, containing continuing harm, or following up with the people who raised concerns.

Govern establishes policies, accountability, and organizational context. Map helps teams understand the system and the setting in which it is used. Measure supports evaluation of risks, while Manage helps prioritize and address them. These functions can guide the design of a reporting process, but they are not a substitute for an organization’s legal analysis. NIST describes the framework as voluntary, and its Playbook presents suggested actions rather than a mandatory checklist. See NIST AI RMF Core, the NIST AI RMF overview, and the NIST AI RMF Playbook.

Use the framework as a way to structure decisions, not as a universal incident-response recipe. The right intake channels, decision authority, urgency criteria, recordkeeping, and communications depend on what the system does, who relies on it, what could go wrong, and which contractual or regulatory obligations apply.
Set ownership and define which systems are covered
Before opening a reporting channel, identify who receives reports and who has authority to act on them. A workable process needs named roles or teams, backup contacts, and clear handoffs. If one person is responsible for every step, specify what happens when that person is unavailable and who can make urgent decisions.
Assign the operational roles
Map the work to the people who can perform it. One person may hold multiple roles in a small organization, but each responsibility still needs an owner. A responsibility map can include:
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitches- Intake owner: monitors the reporting channels, confirms receipt where appropriate, creates a case record, and routes it for review.
- Triage lead: assesses urgency and potential impact, chooses an initial response path, and escalates cases that exceed their authority.
- Investigation lead: coordinates fact-finding, preserves relevant evidence, and records what is known, uncertain, or still being checked.
- Technical or operational responder: can inspect the system and its surrounding workflow, apply containment measures, or request a change, pause, or shutdown.
- Legal, privacy, security, or compliance reviewers: assess matters in their remit, including possible external reporting or notice obligations.
- Communications owner: coordinates appropriate updates to reporters, affected people, internal decision-makers, and other relevant actors.
- Change approver: authorizes material changes to the AI system or its use, and records who approved them and why.
Document how those roles contact one another, how an urgent concern reaches a decision-maker outside normal business hours if necessary, and who can authorize temporary safeguards. NIST calls for documented responsibilities and organizational practices that support incident identification and information sharing. The AI RMF Core also connects governance with inventories of AI systems and management of third-party risks.
Maintain a useful AI system inventory
Scope the process against an inventory, not just a list of models built in-house. Include AI-enabled services that employees use, systems embedded in products or vendor platforms, and components that affect a consequential workflow. For each system, record enough context to route a report and identify the right responders:
- System name, owner, purpose, and current deployment status.
- Where and how it is used, including the decisions or tasks it influences.
- Operators, providers, relevant vendors, and technical or data dependencies.
- Groups that may be affected and the channels available to those groups.
- Known limits, human review or override points, and relevant monitoring contacts.
- Any internal risk classification or escalation rules that apply.
Keep the inventory current as systems, providers, use cases, and dependencies change. A report about a third-party service still needs an internal owner who can assess the organizational impact, contact the provider, and coordinate any mitigation within the organization’s control.

Make reporting accessible to the people who may notice a problem
Provide a clear route for employees and other relevant AI actors to report suspected issues. Depending on the context, options might include an internal form, help desk, safety contact, customer-support route, or escalation channel. These are implementation choices, not channels prescribed by NIST. Select routes that the intended reporters can actually reach, and explain what happens after submission.
Recommended Free Tools
Internal-only reporting can miss problems observed by customers, operators, contractors, or affected communities. Decide which external actors should be able to raise concerns, how they can do so, and how a report will be routed if it arrives through the wrong channel. If a person reports through customer support, for example, staff should know when and how to send it to the incident process rather than treating it as an ordinary product complaint.
Explain what to report and how
Use plain language. Ask people to report a suspected issue, unusual behavior, possible harmful output, or near miss; do not require them to prove that the AI caused harm before they can submit a concern. Tell them what information is useful, whether they can report anonymously where feasible, how their information will be handled, and how to contact the organization again. Avoid promising a particular outcome or response time unless the organization can consistently meet it.
NIST calls for mechanisms to capture and evaluate input from users and other relevant AI actors, including feedback and appeal processes. That means a report route should be more than a technical support mailbox: the organization needs a way to consider the input in evaluation and ongoing monitoring. Where the AI system supports consequential decisions, describe any available appeal or human-review route separately and connect it to incident intake when an appeal reveals a broader system concern.
Design the report form around investigation needs
NIST does not prescribe one universal AI incident form or a mandatory field list. The form below is an operational design based on the need to capture and evaluate input, track and document incidents and errors, and support response and recovery. Keep it short enough to complete, permit “unknown” for facts a reporter may not know, and avoid requiring sensitive personal details unless they are necessary for follow-up.

Recommended information to collect
- System and context: the AI system or service, the task it was performing, and where or how it was being used. Offer a “not sure” option if reporters may not know the system name.
- What happened: a description of the observed behavior, output, decision, or near miss, and why it appears concerning.
- Time and detection: when it happened, when it was noticed, and how the reporter encountered it.
- Potential impact: who or what may have been affected, what harm is observed or possible, and whether the issue may still be happening.
- Supporting material: relevant outputs, screenshots, identifiers, logs, or other evidence, with instructions not to upload information the reporter is not authorized to share.
- Immediate action: any containment or safety step already taken, and whether the system or workflow is still in use.
- Reporter and follow-up: contact details if the person wants follow-up, their preferred contact route, and an anonymity option if the organization can support it.
- Handling status: a case identifier, owner, current status, next review point, and closure rationale maintained by the organization.
Separate the reporter’s observations from the organization’s conclusions. A person should be able to describe what they saw without selecting a technical cause or severity level. The intake owner can add classification after receipt. Limit access to case records to people who need them, especially where reports contain personal, confidential, security-sensitive, or otherwise restricted information.

Triage reports using a documented, risk-based method
Set triage criteria before a difficult case arrives. The method should reflect potential impact, likelihood, urgency, the organization’s risk tolerance, and the resources or authority needed to respond. NIST directs organizations to prioritize and respond to assessed risks, but it does not establish one severity scale for every organization. The categories and example levels below are a local design option, not a NIST taxonomy.
Choose categories that help route work
Possible routing categories include safety or rights impacts, privacy or security events, reliability failures, harmful or misleading outputs, misuse, and third-party service failures. A report may fit more than one category. Use categories to identify appropriate reviewers, not to force complex cases into a single label or imply that an unconfirmed report is unimportant.
Define local urgency and escalation triggers
An organization may create a simple internal scale such as the following. Set actual response targets, approvers, and actions based on the organization’s context and obligations; do not present this example as a legal deadline or universal standard.
| Local triage band | Example indicators | Initial handling decision |
|---|---|---|
| Urgent | Credible indication of continuing or imminent serious harm; a high-impact workflow may be producing unsafe or materially wrong outcomes; evidence may be at risk. | Escalate promptly to the designated decision-maker and relevant technical, safety, legal, privacy, or security owners. Consider immediate safeguards while facts are checked. |
| Elevated | Potentially significant impact, repeated failures, a vulnerable affected group, or an issue spanning several users or systems. | Assign an investigation owner, identify relevant evidence and dependencies, and set a review point for deciding whether stronger containment is needed. |
| Routine review | A limited or uncertain issue with no identified immediate threat, but enough concern to assess for a pattern or control gap. | Record and route it to the appropriate owner; decide whether to monitor, investigate further, or combine it with related reports. |
Escalation should not depend on a single numerical score. A low-confidence report about potentially severe harm may merit urgent human review, while repeated low-impact reports may reveal a systemic issue. Define triggers such as continuing exposure, likely impact on multiple people, an inability to use a human override, a potentially reportable privacy or security event, or a vendor dependency that blocks containment. These are examples for local policy, not universal thresholds.
Record the triage decision
For each case, record the facts considered, the initial category and urgency, who made the decision, what is still uncertain, who owns the next action, and when it will be revisited. If a report is downgraded, closed without investigation, or combined with another case, record the rationale. This creates an auditable path and makes later pattern review more useful.
Investigate, contain, and recover
After triage, establish a lead investigator and a response team appropriate to the issue. Preserve relevant evidence before changing a system when doing so is safe and lawful, and avoid altering records in a way that makes the event difficult to reconstruct. The investigation should separate the reported event from hypotheses about its cause: verify what happened, under what conditions, who may have been affected, and whether the issue is ongoing.

Use containment proportionate to the risk
Potential measures include routing cases to human review, restricting a feature or use case, pausing a workflow, disabling an integration, rolling back a change, or disengaging or decommissioning a system. The appropriate option depends on the risk and on whether an alternative process can operate safely. Identify in advance who may authorize each action and who needs to be informed. Human override is useful only when a person is available, has enough information and authority to intervene, and can do so without introducing a separate unmanaged risk.
Keep a case record through recovery
Track the response from intake to recovery: actions taken, decision-makers, system or workflow changes, evidence reviewed, affected groups considered, and remaining risk. Document what was restored, what safeguards remain, and who confirmed that the system is ready to resume or that the affected process has an alternative. If a third party controls a relevant component, record the provider contact and any dependency that limits the organization’s response.
NIST AI RMF Manage 4.1 includes post-deployment monitoring, appeal and override, incident response, recovery, change management, and, when needed, decommissioning or disengagement. Manage 4.3 addresses communication, tracking, response, and recovery for incidents and errors. The AI RMF Core states: “Incidents and errors are communicated to relevant AI actors, including affected communities.” These activities should be connected in the organization’s process rather than treated as unrelated tasks.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Communicate with reporters, affected people, and other relevant actors
Decide who needs information and who is authorized to provide it. Depending on the event, relevant parties may include the reporter, people or communities affected, system operators, internal leaders, providers, and legal or compliance teams. The content and timing of communications depend on the circumstances and applicable requirements; NIST’s guidance does not create a universal notification deadline or recipient list.
Where appropriate, acknowledge receipt, provide a case reference, explain what will happen next, and give a contact route for additional information. For affected people, communications should be understandable and useful: explain what is known, what remains under review, what action they can take, and how the organization will provide updates when it can. Do not disclose another person’s information or make claims about cause, impact, or resolution before they are established.
Include communication review in the response workflow, not as a decision deferred until the technical investigation is complete. A designated owner can coordinate what may be shared while legal, privacy, security, or operational reviewers assess constraints. Keep a record of significant communications and decisions about whether a communication was needed.

Learn from incidents and near misses
Close a case only after the organization has documented its disposition, response, and follow-up. A confirmed incident is not the only useful input: near misses, user appeals, repeated complaints, and reports dismissed as unrelated may reveal monitoring gaps or design assumptions that no longer hold.
Run a focused post-incident review
Assign an owner and a review date. Examine the initiating conditions, contributing factors, detection path, handoffs, response decisions, and any barriers that delayed containment or communication. Consider which groups were affected or could have been affected, including people who may not have been visible in system logs. Avoid reducing the review to blame for an individual error when process, interface, vendor, or monitoring conditions also shaped the outcome.
Turn findings into tracked changes
For each corrective action, record an owner, due date, evidence of completion, and the risk or control it is intended to address. Actions may include revising instructions, monitoring, testing, access controls, training, human review, vendor arrangements, or use restrictions. Update the system inventory and risk assessment when the incident changes what the organization knows about the system or its context. Feed relevant patterns back into ongoing evaluation and monitoring, then check whether the agreed change had the intended effect.
NIST presents risk management as continuous and emphasizes measurable improvement. Treat a closed incident as an input to that continuing work, not as proof that the risk has disappeared. Keep enough records to identify recurring issues across systems or providers while applying appropriate access and retention controls.
Map legal, regulatory, and contractual duties separately
A voluntary framework does not decide whether an organization must report an event to a regulator, customer, partner, or other authority. Requirements and deadlines can vary by jurisdiction, sector, contract, data involved, and the nature of the event. Assign counsel or a compliance owner to map which rules apply to each relevant system and event type, who determines whether a duty has been triggered, and who submits any required notice.
NIST SP 800-53 control IR-6 says incident-reporting authorities, content, and timeliness should reflect applicable laws, directives, regulations, policies, standards, and guidelines. See the NIST SP 800-53 IR-6 incident-reporting control. Use that principle to keep legal reporting analysis connected to incident handling, while ensuring a business triage label does not replace the required legal assessment.
Maintain a jurisdiction- and sector-aware obligations map with the relevant event triggers, recipients, decision owners, submission channels, and applicable time limits. Review it when the organization enters a new market, changes a system’s use, or adopts a new provider. When a report could trigger a duty, preserve the information needed for counsel or compliance to assess it promptly; do not wait for a complete technical root-cause analysis if the applicable rule requires earlier action.

Publish a short policy that explains scope, reporting routes, roles, escalation authority, records, communications, and the relationship to legal and contractual reporting. Pair it with operational instructions for intake staff and responders. Make sure employees and relevant external actors can find the reporting route without having to understand the organization’s internal governance structure.
- Inventory covered systems. Identify owners, contexts of use, affected groups, providers, dependencies, and available human-review or override points.
- Name primary and backup owners. Assign intake, triage, investigation, response, legal or compliance review, communications, and change approval responsibilities.
- Publish accessible channels. Tell intended reporters what to submit, how reports are handled, and how they can follow up.
- Configure a case record. Capture observations, time, system context, possible impact, evidence, actions, status, ownership, and follow-up without making reporters supply facts they cannot know.
- Approve triage and escalation rules. Define local indicators, decision-makers, urgent safeguards, review points, and conditions for reclassification.
- Connect response and obligations. Document evidence preservation, containment options, recovery decisions, change approval, communications, and legal or contractual review.
- Exercise a realistic scenario. Walk through a sample report from an external user or frontline employee. Check that the report reaches the right owner, that urgent authority is clear, and that recordkeeping, escalation, communications, and follow-up work in practice.
- Review and improve the process. Use incident and near-miss reviews to update controls, routing, guidance, training, monitoring, and the system inventory.
A useful process is one people can use and responders can execute. Review it whenever responsibilities, system context, affected groups, providers, or applicable obligations change, and after a real case exposes a gap in the handoff.
