To connect an ElevenLabs conversational agent to Twilio, start with ElevenLabs’ native Twilio integration if you want the simplest documented setup for inbound and outbound calls. Use the register-call API when you need your application to control Twilio webhooks and routing, or SIP trunking when you want to connect an existing PBX or SIP-enabled phone system.
Choose the integration path that fits your call setup
The main decision is who should control call handling: ElevenLabs’ managed integration, your application’s Twilio workflow, or your existing SIP telephony system.
| Approach | Best fit | Trade-off | Documented constraint |
|---|---|---|---|
| Native Twilio integration | Standard inbound and outbound calling with a Twilio number | Managed setup with less custom Twilio logic | Verified caller IDs are outbound only; they cannot receive calls or be assigned to an agent. |
| Register-call | Existing Twilio workflows or custom call routing | Your application retains more control but must handle server-side routing and TwiML | Call transfers are unavailable through this pattern. |
| SIP trunking | An existing PBX or SIP-enabled phone system | Connects current telephony infrastructure but requires SIP, codec, network, and security configuration | Confirm provider-specific setup and eligibility. |
For a typical new application using a Twilio number, the native integration is the natural starting point. ElevenLabs describes it as handling configuration automatically compared with the more advanced register-call flow. If you need Twilio-specific logic before a call reaches the agent, register-call provides that control. If the number is part of a broader phone system, consider SIP trunking.
Connect a Twilio number with the native integration
The native integration imports an existing Twilio phone number and links it to an ElevenLabs agent. It supports inbound and outbound calling, but the type of number matters:
#1 Best Overall
- Digital Stereo Sound: Fine-tuned drivers provide enhanced digital audio for music, calls, meetings and more
- Rotating Noise Canceling Mic: Minimizes unwanted background noise for clear conversations; the rotating boom arm can be tucked out of the way when you’re not using it
- Handy In-line Controls: Simple in-line controls on the headset cable let you adjust the volume or mute calls without disruption
- Plug-and-Play USB Computer Headset: Simply plug the USB-A connector into your computer and you’re ready to talk or listen without the need to install software
- Padded Comfort: Comfortable headphones with adjustable headband features swivel-mounted, leatherette ear cushions for hours of comfort and is easy to clean
- Purchased Twilio number: Supports inbound and outbound calls through the integration.
- Verified caller ID: Can be used for outbound calls only. It cannot receive inbound calls or be assigned to an agent.
Before setting up the agent, confirm the number’s configuration and capabilities in Twilio. A verified caller ID is not a substitute for a phone number that must receive calls. Follow the current setup instructions in ElevenLabs’ native Twilio integration guide.
Use register-call when your application must control Twilio
Register-call is designed for applications that need to keep existing Twilio infrastructure or run custom routing and logic before connecting a call to an agent. The documented flow is:

- How it Fits: On-ear compact design may feel snug initially—adjust properly and wear 30-60 minutes daily for the first week. Optimal comfort achieved after 1-2 weeks as ear cups conform to your ears. Take 10-minute breaks during extended use.
- Wired computer headset with foldable design; ideal for calls, meetings, online learning, and more. Compact headset measures 6.1" W x 7.2" H with 2.8" ear cups and 4.4" boom mic. Ideal fit for small to medium head sizes
- Flexible, adjustable boom mic can be positioned at any angle; unidirectional mic reduces the background noise to ensure crisp, bright conversations (Provided that your conversation is under the correct direction of the microphone)
- 32mm speaker drivers offer an immersive listening experience with clear sound quality
- One-touch mute/unmute with intuitive in-line control box; Using microphone, slide the button upward to unmute and enabled audio settings in your device. For USB connection, ensure the 3.5mm jack (4-pin) is fully inserted into the USB adapter. For direct 3.5mm connection, first remove the USB adapter from your device
- Twilio sends an inbound webhook to your application, or your application initiates an outbound call through Twilio.
- Your application sends the ElevenLabs agent ID and the call’s phone numbers to the register-call endpoint.
- ElevenLabs returns TwiML.
- Your application returns that TwiML to Twilio so Twilio can connect the call.
The API request requires agent_id, from_number, and to_number. The optional direction field accepts inbound or outbound and defaults to inbound. The endpoint returns TwiML for Twilio. See the register-call API documentation and the register Twilio calls guide for the current request details.

Optional conversation initiation data can pass dynamic variables and configuration overrides. That lets the application supply call-specific context without changing the agent’s base configuration.

- Digital Stereo Sound: Fine-tuned drivers provide enhanced digital audio for calls, meetings, music, and more
- Rotating Noise-Canceling Mic: Minimizes unwanted background noise for clear conversations; the rotating boom arm can be tucked out of the way when not in use
- Handy Inline Controls: Simple inline controls on the headset cable let you adjust the volume or mute calls without disruption
- USB-C Plug-and-Play: Simply plug the USB-C cable into your computer, including MacBook Neo laptops, and you're ready to talk or listen without installing software.
- Padded Comfort: Comfortable USB C headphones with adjustable headband feature swivel-mounted, leatherette ear cushions for hours of comfort
What you take on with register-call
- Your application needs a publicly reachable webhook endpoint and must handle the TwiML response.
- You manage the Twilio routing and audio configuration that the native integration otherwise simplifies.
- Call transfers are not available through this pattern because ElevenLabs does not have direct access to the Twilio account credentials.
Keep account credentials on the server and out of client-side code. The examples in the documentation use environment variables for credentials; follow the current guide rather than copying secrets into a deployed application.
Connect an existing PBX with SIP trunking
SIP trunking connects an existing PBX or SIP-enabled phone system to ElevenLabs agents and supports inbound and outbound calls. ElevenLabs lists Twilio as a compatible SIP trunk provider, alongside Vonage, RingCentral, Sinch, Infobip, Telnyx, Exotel, Plivo, and Bandwidth. A provider being listed as compatible does not mean every provider feature or account configuration works identically; verify the specific trunk setup you plan to use.

- An authentication plan. The guide supports digest authentication or source-IP ACL authentication and recommends digest authentication over allowlisting when dynamic IP addresses complicate operations.
- For production, TLS signaling and required media encryption.
The SIP guide provides URI forms for TCP and TLS. It marks UDP as experimental for testing, not production. Enterprise static-IP SIP endpoints are described as an Enterprise feature, including trials; confirm your eligibility and regional configuration with ElevenLabs before depending on them. Treat the URI examples as starting points, not as a complete provider-specific configuration. Read the current ElevenLabs SIP trunking guide and SIP integration overview.

Plan the implementation and verify costs
- Decide who owns call handling. Choose the native integration for managed setup, register-call for application-controlled Twilio workflows, or SIP trunking for existing SIP telephony.
- Check number eligibility. Confirm whether you have a purchased Twilio number or only a verified caller ID, especially if inbound calls are required.
- Prepare the server flow if using register-call. Implement the webhook, send the required agent and phone-number fields, and return ElevenLabs’ TwiML response to Twilio.
- Validate SIP settings if using a trunk. Check codecs or resampling, firewall rules, authentication, transport, media encryption, and the provider’s actual requirements.
- Estimate costs for your use case. Check current Twilio and ElevenLabs pricing for the destinations, call volume, plans, and features you expect to use. The integration documentation does not establish a complete combined cost.
If you are following an older outbound-call tutorial, note that ElevenLabs’ legacy outbound guide says outbound calls are now natively supported and recommends the native integration instead. Check the legacy outbound-calling guide alongside the current integration documentation before adopting an older flow.
