A useful risk assessment during uncertain conditions is a living decision process: define the work and its boundaries, gather evidence with workers, identify who could be harmed, judge severity, likelihood and exposure without pretending missing information is certain, install controls in order of effectiveness, assign actions, and review whenever conditions change. The method below works across sectors, but legal duties, record-keeping thresholds and required forms depend on your country and industry.
What a risk assessment must achieve when conditions are changing
A risk assessment is not a score on a completed form. It should show which hazards exist, who may be exposed, what controls are in place, what still needs to be done, who owns each action and when the decision will be revisited. During periods of uncertainty, the assessment also needs to make assumptions and information gaps visible.
Uncertainty can come from a new process, altered staffing, unfamiliar equipment, a changed substance, an unusual workload, incomplete exposure data, a temporary work location or an evolving emergency. Treating uncertainty as a reason to continue without controls is unsafe. Treat it as a reason to use proportionate interim controls while you obtain better information or specialist advice.

The Health and Safety Executive (HSE) states: “You’re not expected to eliminate all risks but you need to do everything ‘reasonably practicable’ to protect people from harm.” That wording is guidance for Great Britain; readers elsewhere must apply their own regulator’s standard.

1. Set a clear assessment boundary
Define the work, place and conditions
Write a short scope statement before looking for hazards. Identify the activity, physical locations, equipment, substances, people, shifts and environmental conditions covered. State whether the assessment addresses one task, a department, a project phase or an entire site. A boundary that is too broad hides task-specific hazards; one that is too narrow can miss interactions between activities.
Record what is changing or unknown. Examples include a reduced crew, an unfamiliar contractor, a substituted chemical, a temporary layout, remote operation, a new software-controlled machine or a power interruption. Note the date and the person approving the scope so later reviewers can see which conditions were considered.
Include routine, nonroutine and emergency work
Do not assess only the normal production step. Include foreseeable maintenance, cleaning, inspection, setup, startup, shutdown, troubleshooting and waste handling. Consider emergencies such as fire, medical events, loss of utilities, severe weather, security incidents or evacuation while normal staffing and communications may be disrupted. OSHA guidance specifically calls for attention to emergency and nonroutine situations, and HSE guidance highlights nonroutine operations.
Map interfaces and handoffs
List where the work meets another team, contractor, delivery route, public area or control system. A change that appears safe within one task can create a hazard at a handoff—for example, a maintenance team may isolate equipment differently from operators, or a temporary worker may not receive a changed instruction. Include those interfaces in the boundary or document them as linked assessments.
2. Gather evidence and involve the people doing the work
Review existing information
Use more than a blank checklist. Assemble relevant:
- Incident, near-miss and first-aid records.
- Inspection, audit and maintenance findings.
- Safety data sheets and chemical inventories.
- Equipment manuals, specifications, alarms and interlock information.
- Exposure measurements or occupational-health information that already exists.
- Current safe-work procedures, permits, training records and emergency plans.
Check whether each document describes the present arrangement. A procedure written for a full crew may not be adequate when staffing or supervision has changed.
Inspect the workplace, not just the paperwork
Walk through the task at the time and under the conditions being assessed. Observe access, lighting, noise, ventilation, housekeeping, guarding, storage, traffic, manual handling and communication. If work occurs on several shifts or remotely, inspect representative conditions rather than assuming one observation applies everywhere.

Ask workers what differs in practice
Consult employees who perform the work and, where relevant, contractors, temporary staff, supervisors and people who receive or share the work area. Ask open questions: What has changed? Where do instructions not match reality? Which alarms, tools or controls are unreliable? What happens during a rush, a handover or an equipment fault? Workers often identify practical control problems that a desk review misses.
Give workers a way to report concerns after the assessment is issued. A new observation may be evidence that an assumption was wrong, not merely a request for retraining.
3. Identify who could be harmed and how
List every affected population
Consider employees on every shift, contractors, visitors, delivery personnel, customers, neighbors and members of the public who could enter or be affected by the work. Include people who may be exposed indirectly, such as a cleaner entering after a process change or a security worker responding to an alarm.
Check whether particular people need additional consideration under local law and policy. HSE guidance highlights young workers, migrant workers, new or expectant mothers and people with disabilities. Also consider new starters, agency workers, people who do not speak the primary workplace language, lone workers and anyone whose medical or functional needs affect exposure or emergency response.

Use the task and evidence to identify hazards rather than forcing every job into a preset list. Potential categories include:
- Physical: moving machinery, vehicles, falls, noise, vibration, heat, cold, electricity and energy releases.
- Chemical: toxic, corrosive, flammable, reactive or sensitizing substances, including unexpected by-products.
- Biological: infectious agents, contaminated materials, animals or biological waste.
- Ergonomic: force, repetition, awkward posture, manual handling, workstation design and fatigue.
- Psychosocial and work-organisation: excessive demand, unclear roles, inadequate staffing, isolation, violence, harassment, poor shift handover or pressure that encourages bypassing controls.
- Emergency and process hazards: loss of containment, fire, explosion, uncontrolled movement, utility failure or an emergency response that exposes people to a secondary hazard.
Describe the mechanism of harm, not just a label. “Chemical risk” is less useful than “operator may inhale vapour while transferring solvent from a drum to a smaller container.”
4. Evaluate risk without creating false certainty
Ask four connected questions
- What event could occur? Describe the unwanted event and the initiating conditions.
- How likely is it under the conditions being assessed? Use observations, incident history, exposure information, worker knowledge and relevant technical data.
- What could the consequences be? Consider the credible range of injury, illness, environmental harm, equipment damage and disruption.
- How many people, how often and for how long could exposure occur? Include simultaneous work, repeated tasks, peak periods and people passing through the area.
OSHA’s framework emphasizes severity, likelihood and the number of people exposed. NIOSH’s occupational-health framework similarly considers the event, its likelihood and its consequences, particularly when exposure data are incomplete. Neither source establishes one universal numerical matrix.

State assumptions and information gaps
Write down facts you know, estimates you are using and questions still open. For example, you might know the substance identity but not the airborne concentration, or know the normal staffing level but not who will respond during an overnight alarm. Identify who will obtain the missing information, by what date and what interim restriction applies until then.
Do not let a low-confidence estimate produce a reassuring score. If uncertainty could conceal a serious consequence, choose a conservative interim control, limit the task, increase supervision, obtain measurement or seek competent occupational-safety advice.
Use a matrix only as a prioritisation aid
A qualitative or numerical matrix can help order actions, but it does not prove that work is safe. If you use one, document its definitions and assumptions and keep the underlying evidence with the assessment.
| Dimension | Questions to document | What a weak assessment misses |
|---|---|---|
| Severity | What is the credible worst outcome and what range of harm is plausible? | Rare but catastrophic outcomes or long-latency illness. |
| Likelihood | What evidence supports the estimate under current conditions? | Changes in staffing, equipment condition or work practice. |
| Exposure | How many people, how often and for how long? | Concurrent tasks, visitors and intermittent exposure. |
| Uncertainty | Which assumptions or data gaps could change the decision? | False precision from an unsupported number. |
5. Prioritize controls in the order of effectiveness
Apply the hierarchy of controls
Select controls that remove or isolate the hazard before relying on individual behavior. Compare each option for effectiveness, durability, feasibility and how its performance will be checked.
| Order | Control approach | Examples of the decision |
|---|---|---|
| 1 | Elimination | Stop the hazardous step, redesign the task or remove the need for entry. |
| 2 | Substitution | Use a less hazardous substance, process or energy source where suitable. |
| 3 | Engineering control | Guard, enclose, isolate, ventilate, interlock or automate so people are separated from the hazard. |
| 4 | Administrative or work-practice control | Change sequencing, access, permits, supervision, training, staffing, inspection or emergency instructions. |
| 5 | Personal protective equipment | Use correctly selected, fitted, maintained PPE as a final layer or while stronger controls are being implemented. |
Administrative rules and PPE can fail when people are rushed, tired, unfamiliar with the task or working in an emergency. They may still be necessary, but do not use them to compensate for a hazard that could reasonably be eliminated or engineered out.

When a permanent control needs design, procurement or installation, specify the temporary arrangement, its limits, the person responsible and its expiry or review date. Examples include stopping a task, restricting access, reducing throughput, adding a spotter, isolating equipment, changing a schedule or requiring a specialist assessment. Tell affected workers what the interim rule is and what event will trigger removal or extension.
Define verification before closing the action
“Install guard” is an action; “verify the guard is installed, prevents access to the danger zone and remains effective during the task” is a completion test. Decide whether verification will involve observation, inspection, functional testing, exposure measurement, worker feedback, maintenance records or an emergency drill. Record the evidence and the date.

6. Assign, record and communicate the decisions
Keep a usable risk register
The record should be detailed enough for another competent person to understand the decision without guessing. A practical structure is:
| Field | What to enter |
|---|---|
| Scope and version | Task, location, date, conditions, assessor and revision identifier. |
| Hazard and event | What can cause harm and how the event could occur. |
| People exposed | Roles, contractors, visitors, public and groups needing particular consideration. |
| Existing controls | Controls observed, their condition and any evidence of effectiveness. |
| Assessment basis | Severity, likelihood, exposure, assumptions and information gaps. |
| Additional actions | Elimination, substitution, engineering, administrative and PPE measures in priority order. |
| Owner and due date | A named role or person with authority and a realistic completion date. |
| Interim controls | Restrictions used before the final action, with an expiry or review date. |
| Verification and review | How completion will be checked, evidence recorded and next review triggered. |
Make ownership explicit
Assign actions to people who control the relevant resources. A safety coordinator can track an engineering change, but the equipment owner may need to approve it and maintenance may need to install it. Escalate overdue actions rather than silently extending the due date.
Communicate changes at the point of work
Explain changed procedures, access restrictions, alarms, PPE, emergency arrangements and temporary controls to everyone affected. Use methods suitable for the workforce, including translated or accessible information where needed. Confirm understanding through discussion, demonstration or supervised practice when a written notice is insufficient.
Know your local recording duty
HSE guidance for Great Britain says employers with five or more people must record significant findings. That “5 or more people” threshold is jurisdiction-specific; it is not a universal rule. Check the current requirements for your country, sector, workforce size and regulator before deciding what must be recorded or retained.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.7. Review when conditions change and verify performance
Trigger a review, rather than waiting for an annual date
Revisit the assessment when staffing, processes, substances, equipment, work practices or exposure conditions change. Also review it after a worker raises a concern, an incident or near miss occurs, a control fails, inspection finds deterioration, or an emergency reveals a weakness. A review should ask whether the original boundary, people, hazards, assumptions and controls still describe reality.
Check controls in operation
After implementation, inspect and evaluate the controls where the work occurs. Look for bypassed guards, damaged equipment, blocked ventilation, unsuitable PPE, workarounds, unexpected alarms and signs that the procedure is impractical. Ask workers whether the control works during peak demand and nonroutine tasks, not only during a planned demonstration.

Preserve version history
Keep superseded assessments identifiable rather than overwriting them. A simple revision history showing what changed, why, who approved it and when it became effective helps workers use the current instruction and helps investigators understand which conditions existed at a particular time.
A worked example: uncertain staffing during equipment maintenance
Suppose a facility must maintain a machine while two experienced operators are absent and a contractor is covering one shift. The assessment boundary includes isolation, access, maintenance, testing, restart, cleaning and an emergency stop. It identifies operators, the contractor, cleaners and anyone using the adjacent walkway.
Evidence includes the maintenance procedure, isolation instructions, prior near misses, equipment information and a walk-through during the affected shift. Worker interviews reveal that the written handoff assumes two qualified people, while the temporary crew has one person unfamiliar with the machine’s alarm sequence.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
The assessment records a possible unexpected startup, the people who could be exposed, the uncertainty about the contractor’s familiarity and the consequences of an isolation error. Before permanent staffing is restored, the site stops solo maintenance, requires a competent person to verify isolation, restricts access and briefs the contractor. The action owner arranges supervised familiarisation and updates the handoff procedure. Verification consists of observing an isolation and restart test, checking the access control and asking the workers to explain the emergency response. The assessment is reviewed when staffing returns or if a near miss occurs.
This example illustrates why a changed condition can invalidate a routine-task assessment: the machine did not change, but the people, handoff and failure opportunities did.
How to choose a template or assessment method
No single template or scoring system is established as universal by HSE, OSHA or NIOSH. Compare a method against the work you actually need to control.
| Selection question | What a suitable method provides |
|---|---|
| Does it cover the real task? | Routine, nonroutine, emergency, maintenance, startup, shutdown and changed conditions. |
| Does it handle exposure? | Severity, likelihood, number of people, frequency and duration, with assumptions visible. |
| Can workers contribute? | A practical way to capture observations from employees, contractors and temporary staff. |
| Does it drive action? | Controls in priority order, interim measures, named owners, due dates and completion evidence. |
| Does it support review? | Revision history, change triggers and a way to verify controls in practice. |
| Does it fit your duties? | Records, approvals, competence and retention that match local legal and sector requirements. |
A short form that prompts these decisions is more useful than a lengthy form that produces an unexplained score. If hazards involve complex exposures, hazardous substances, process safety or a changing emergency, involve a competent occupational-safety professional and consult the applicable regulator’s current requirements.
Quick Recap
Common failure modes and corrective actions
- Assessing the normal day only: add nonroutine, emergency and transition steps to the boundary.
- Copying an old assessment: compare its assumptions with current staffing, equipment, substances and layout; issue a new revision where they differ.
- Relying on a desk review: inspect the workplace and ask workers where practice diverges from the procedure.
- Using a score as proof of safety: show the evidence, uncertainty and exposure behind the prioritisation.
- Listing PPE as the main solution: work up the hierarchy toward elimination, substitution and engineering controls.
- Leaving actions ownerless: name a person or role with authority, a due date and a verification test.
- Letting temporary controls become permanent: give them an expiry or review date and track restoration of the intended condition.
- Failing to communicate the revision: brief affected people at the point of work and confirm they understand the changed instruction.
- Applying another country’s rule automatically: verify the jurisdiction, sector and current regulator guidance before relying on a legal threshold or deadline.
