The right comparison starts with the parent, not the country. Map where your parent lives most of the year, citizenship or immigration status, public-program eligibility, existing insurance, doctors, medicines, chronic conditions, mobility, and the amount of daily help required. Then compare U.S. and Indian options on identical questions: who can provide care, what is covered, where treatment is available, what approvals are needed, what the family pays, and how care will be coordinated when relatives live elsewhere.

There is no defensible universal claim that the United States or India is cheaper or better. Current, matched prices and like-for-like outcome data are not established here. The practical answer is a documented decision for one parent, one location, and one travel pattern, with plan terms verified before enrollment or a trip.

Start with a one-page profile of your parent

Write down the facts that determine eligibility and risk before comparing policies. Recheck the sheet whenever the parent moves, changes immigration status, or develops a new care need.

Residence and legal status

  • Primary residence and the state or Indian city where routine care will occur.
  • U.S. citizenship, lawful presence, visa status, or Indian residency and public-scheme eligibility.
  • How many months each year the parent expects to spend in the other country.
  • Whether a return trip is for routine treatment, a planned procedure, or an emergency.

Health and support needs

  • Diagnoses, recent admissions, planned procedures, allergies, and current medicines.
  • Ability to bathe, dress, use the toilet, eat, transfer, shop, cook, manage money, and travel alone.
  • Preferred clinicians, languages, hospital access, rehabilitation needs, and reliable transport.
  • The family member who can attend appointments, receive records, and make decisions if the parent cannot.

Financial and administrative facts

  • Current premiums, deductibles, copayments, coinsurance, drug costs, and employer or pension benefits.
  • Assets and income relevant to Medicaid or Indian public-program rules.
  • Existing long-term-care insurance, supplemental coverage, or travel policies.
  • Permission to speak with clinicians and insurers, plus copies of identity, insurance, and medical documents.

Do not infer eligibility from age alone. Medicare enrollment, Medicaid, Indian public schemes, and private policies each apply their own residence, income, medical, and enrollment rules.

Compare the same questions in both countries

Use this framework for each realistic arrangement: a parent living in the United States, a parent living in India, or a parent who regularly crosses the border.

Comparison area Questions to answer
Eligibility and enrollment Which public or private program accepts this parent, during which enrollment window, and with what documentation?
Geography and network Can the parent see any qualified clinician, or only contracted providers in a service area? What happens outside that area?
Routine and specialist care Are primary care, specialists, hospitals, rehabilitation, diagnostic tests, and emergency services covered separately?
Referrals and authorization Is a primary-care referral required? Which imaging, admissions, therapies, or drugs need prior authorization?
Prescriptions Are medicines covered, which pharmacies dispense them, and are there formulary or quantity restrictions?
Cost exposure What are the recurring premium, deductible, copayment, coinsurance, and annual out-of-pocket maximum? Which services sit outside those limits?
Pre-existing conditions Are conditions covered immediately, subject to a waiting period, excluded, or handled under a policy-specific definition?
Home and facility care Does the benefit cover skilled treatment only, or also personal help at home, adult day care, assisted living, or nursing facilities?
International care Is emergency treatment abroad covered? Is planned care excluded? Is evacuation or repatriation included?
Continuity and coordination Can clinicians exchange records, communicate with the family, and provide language support? Who tracks claims and follow-up?

Put every answer in writing. A sales summary is not a substitute for the policy certificate, evidence of coverage, government handbook, or hospital estimate.

If your parent lives in the United States, compare Medicare choices first

Confirm eligibility and enrollment

Check the parent’s age, disability or end-stage renal disease pathway, lawful-presence history, work record, and current enrollment before discussing plans. A late or missed enrollment period can create penalties or gaps, so use Medicare’s official eligibility and enrollment channels or a State Health Insurance Assistance Program counselor for the parent’s state.

Original Medicare versus Medicare Advantage

Original Medicare generally lets a beneficiary use any U.S. doctor or hospital that accepts Medicare. Medicare Advantage plans must cover Medicare services but commonly use a provider network and service area for non-emergency care. The distinction matters when the parent lives in one state, visits family in another, or wants a particular specialist.

Feature Original Medicare Medicare Advantage
Provider access Any U.S. doctor or hospital that accepts Medicare; verify participation before treatment. Usually a network and service area for non-emergency care; out-of-network rules depend on plan type.
Referrals Generally no specialist referral requirement from Medicare itself. Some plans require referrals; the plan’s evidence of coverage controls.
Prior authorization May apply to particular services under Medicare rules. Plans can require authorization for specified services, facilities, or drugs.
Part D prescription coverage Usually purchased through a separate Part D plan. Most plans include drug coverage, but formularies and pharmacies vary.
Part B cost sharing After the Part B deductible, the beneficiary usually pays 20% coinsurance for covered services. Copayments or coinsurance are set by the plan and service category.
Annual limit Original Medicare has no annual out-of-pocket limit unless supplemental or other coverage applies. Plans have an annual limit for covered Medicare services; the amount varies by plan and year.
Extra benefits Routine dental, vision, hearing, and similar extras are generally not included as standard Medicare benefits. Some plans offer extras, subject to plan rules and limits.

These are structural differences, not a verdict. Compare the parent’s actual doctors, hospitals, prescriptions, travel pattern, and maximum possible spending against the plan’s current documents. A low premium can be a poor fit if the parent repeatedly needs out-of-network care or authorization-sensitive treatment.

Check a plan at the service level

  1. List every clinician, hospital, laboratory, imaging center, pharmacy, and rehabilitation provider the parent uses.
  2. Search each provider in the plan’s current directory, then call the provider’s billing office to confirm participation; directories can lag.
  3. Enter every prescription, dose, and pharmacy into the plan’s formulary and cost tool.
  4. Read authorization rules for planned admissions, scans, therapy, home health, and durable medical equipment.
  5. Record the deductible, visit copayments, coinsurance, drug phases, and annual out-of-pocket limit for a high-use year.
  6. Ask how urgent and emergency care works while the parent is outside the plan’s service area.

If your parent lives in India, verify the local system rather than relying on a product name

Establish the actual coverage stack

Identify any national or state public scheme for which the parent qualifies, employer or retiree coverage, private insurance, hospital-specific packages, and self-pay funds. Confirm the parent’s residence, age limits, enrollment dates, network hospitals, room-rent rules, cashless process, reimbursement deadlines, and exclusions directly with the administrator or insurer.

The NITI Aayog report Senior Care Reforms in India (2024) describes senior-focused insurance examples and broader access problems. It is useful context, not a current ranking of plans. Product names, premiums, waiting periods, exclusions, claim service, and benefits must be checked against the insurer’s current documents.

Use the report’s statistics with their date attached

  • The report says India had about 104 million people over age 60 and projects that people over 60 will represent 19.5% of the population by 2050.
  • It summarizes LASI findings that 18% of older people reported having any health insurance.
  • It reports that 75% had one or more chronic diseases.
  • It reports an Activities of Daily Living limitation for 24% and at least one Instrumental Activities of Daily Living limitation for 48%.

Those figures describe the report’s evidence base; do not present them as a current 2026 estimate or as proof that a particular policy covers a parent’s condition.

Questions for an Indian insurer or hospital

  • Is a diagnosed condition covered immediately, after a waiting period, or excluded?
  • Are there disease-specific sub-limits, room-category limits, co-payments, deductible requirements, or non-payable consumables?
  • Which hospitals offer cashless treatment in the parent’s city, and what happens during an emergency elsewhere?
  • What is covered for rehabilitation, home nursing, ambulance transport, cancer care, dialysis, and mental-health treatment?
  • How must a claim be notified, documented, and appealed, and who can act for the parent?

Do not assume domestic insurance follows the parent across borders

Medicare and treatment outside the United States

Medicare generally does not cover medical care outside the United States. Medicare.gov describes narrow exceptions involving certain foreign inpatient hospitals, physicians, or ambulances. Foreign hospitals do not have to submit Medicare claims. Some Medigap policies cover limited emergency care abroad, subject to the policy’s deductible, lifetime limit, and other terms. Read the exact policy before departure.

Buy travel medical and evacuation coverage for the trip itself

Travel coverage is a separate decision from ordinary health insurance. The U.S. Department of State advises checking whether the destination is valid, the maximum trip length, emergency medical treatment, transportation back to the United States, treatment of current conditions, planned activities, exclusions, and a 24-hour assistance line. Its guidance warns that policies vary significantly. Medicare.gov also cautions that travel insurance does not necessarily include health insurance.

The Department of State gives a general air-ambulance estimate of $20,000 to $200,000, depending on location and condition (undated guidance accessed in 2026). That is not a quote for a particular route or parent. When reviewing travel medical insurance for a visit to the U.S. or medical evacuation coverage, verify age eligibility, maximum benefit, deductible, pre-existing-condition treatment, trip duration, exclusions, assistance logistics, and the claims process.

Prepare prescriptions for every itinerary

  1. Check medicine rules with the embassy or other official authority of every destination and transit country.
  2. Carry enough medicine for the entire trip plus a few extra days, subject to each country’s quantity rules.
  3. Keep medicines in their original packaging.
  4. Carry copies of prescriptions and a doctor’s letter listing generic names and the medical reason for use.
  5. Keep medicines and documents in hand luggage and retain a list of doses and timing for the caregiver.

Budget daily assistance separately from medical treatment

Understand what Medicare does not usually pay for

Long-term care is often help with daily activities rather than a short episode of skilled medical treatment. Examples include bathing, dressing, toileting, meals, adult day health care, transportation, supervision, and residential support. Medicare and most health insurance, including Medigap, do not pay for most non-medical custodial care. Medicare may cover eligible skilled nursing or home-health services under specific conditions, but that is not the same as ongoing personal assistance.

Match the setting to the need

  • At home: paid aides, family coverage, nursing visits, meal and transport services, and safety modifications.
  • Community-based: adult day health, respite, therapy, and supervised activities.
  • Residential: assisted living or a nursing facility when supervision, transfers, medication management, or 24-hour care exceed what home support can provide.

For a U.S. parent, check state Medicaid eligibility and covered home- and community-based services; rules and benefits vary by state and can change. Consider private long-term-care insurance if it exists and is affordable, but read benefit triggers, elimination periods, inflation protection, and exclusions. For an India-based parent, ask insurers and local providers specifically about home nursing and personal-care benefits rather than assuming hospitalization coverage pays for them. Obtain written rates and staffing details from any care provider; the available evidence does not establish a reliable matched U.S.-India price comparison.

Make family coordination an explicit part of the plan

Get permission before accessing records

The U.S. Department of Health and Human Services’ long-distance caregiving guidance recommends learning the parent’s condition, obtaining permission and required privacy releases before speaking with clinicians, discussing advance directives and a health-care proxy, and identifying applicable benefits. Eligibility and benefits can vary by state and change over time.

Create a practical coordination file

  • Name one primary coordinator and one backup who can answer clinicians’ calls.
  • Store consent forms, proxy or power-of-attorney documents, insurance cards, immigration or identity documents, medication lists, allergies, and emergency contacts.
  • Keep a dated treatment timeline and a log of authorizations, claims, denials, appeals, and promised callbacks.
  • Agree in advance who may authorize treatment, arrange payment, and decide whether to transfer the parent between countries.
  • Use secure sharing and give the parent a paper copy for emergencies.

Choose the arrangement with a decision matrix

Score each realistic option against the parent’s priorities rather than assigning a country-wide label.

Scenario Questions that decide fit Evidence to collect
Parent primarily in the United States Which Medicare path preserves needed doctors, drugs, hospitals, and affordable maximum exposure? Medicare eligibility record, plan evidence of coverage, provider confirmations, formulary results, and state Medicaid information.
Parent primarily in India Which public, employer, private, and self-pay resources work in the parent’s city for chronic and emergency care? Current policy wording, network list, waiting periods, exclusions, claim procedure, hospital estimates, and local care-staffing plan.
Parent travels between countries Who pays for routine care abroad, emergency treatment, evacuation, medicines, and follow-up after return? Both domestic policies, travel policy certificate, assistance number, destination rules, medical letter, and a return-care appointment.
Parent needs daily help Who provides bathing, dressing, meals, transport, supervision, and respite, and how is it funded? Functional assessment, care schedule, Medicaid or public-program rules, long-term-care policy, and written provider terms.

Mark an option “unconfirmed” until a current insurer, government agency, hospital, or clinician answers in writing. This prevents a familiar brand name or an old brochure from becoming the family’s operating plan.

Common mistakes and how to prevent them

  • Assuming Medicare pays in India: verify the narrow foreign-care exceptions and separate emergency travel coverage.
  • Choosing by premium alone: model a normal year and a high-use year with deductibles, cost sharing, drugs, and out-of-network exposure.
  • Treating hospitalization insurance as home-care insurance: price personal assistance and residential care as a separate budget.
  • Trusting a provider directory without calling: confirm participation with the clinician and billing office.
  • Ignoring authorization: ask who submits it, how long it takes, and what happens after a denial.
  • Carrying medicines without destination checks: consult every destination and transit authority and keep original packaging and documentation.
  • Speaking for a parent without consent: arrange releases, a proxy, and a named coordinator while the parent can participate.
  • Using old Indian policy information: obtain the current certificate, schedule of benefits, exclusions, and claim instructions directly from the insurer.

A 30-day implementation checklist

  1. Days 1–5: complete the parent profile, medication list, functional assessment, residence calendar, and emergency contacts.
  2. Days 6–10: confirm U.S. Medicare status or Indian public-program eligibility and gather every current policy document.
  3. Days 11–15: call doctors, hospitals, pharmacies, insurers, and government programs; record network, authorization, waiting-period, and claim answers.
  4. Days 16–20: price daily assistance, transport, rehabilitation, and facility alternatives separately from medical premiums and deductibles.
  5. Days 21–25: if travel is planned, obtain policy certificates, check destination medicine rules, and confirm evacuation and 24-hour assistance details.
  6. Days 26–30: sign privacy releases and advance-care documents, appoint the coordinator and backup, share the emergency file, and set a quarterly review date.

Review the arrangement after a diagnosis, hospitalization, move, insurer change, or international trip. Public benefits, provider networks, formularies, and policy terms can change, so a decision that was sound last year may not remain suitable.
