The best community is the one that can safely meet the resident’s care needs now and as they change, while fitting everyday preferences, location, budget, and contract terms. Compare several communities with the same checklist, verify licensing and certification, visit at different times, and confirm every price and service in writing before signing.

1. Define the care fit before comparing buildings

“Choosing a place to live for long-term care is a big decision,” the National Institute on Aging (NIA) says in How To Choose a Nursing Home or Other Long-Term Care Facility, reviewed October 12, 2023. Start with the person’s needs rather than a facility’s appearance or marketing.

List needs today and likely later

Write down the help the resident needs now and the care that may become necessary. Include assistance with bathing, dressing, toileting, eating, transferring, medication management, mobility, or other daily activities. Note whether the person needs registered-nurse oversight, therapy, wound care, hospice, rehabilitation after a hospital stay, or a secure setting for dementia-related safety risks.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ask the resident’s physician, nurse, or other qualified health professional when you cannot tell which service level is appropriate. A community that works for light assistance may not be licensed, staffed, or equipped for complex nursing or advanced dementia care. Ask each facility what happens if needs increase: whether care can be added, whether the resident must move to another unit, and what charges or eligibility rules would apply.

Record preferences that affect daily life

Care is only one part of fit. Record preferences about meals, religious or cultural practice, language, social activities, pets, smoking policies, room type, privacy, visitors, and access to outdoor areas. Include how close the community should be to family, preferred doctors, pharmacies, hospitals, and familiar neighborhoods. Separate essential requirements from preferences that would be welcome but negotiable.

2. Compare communities on the same eight axes

Use one worksheet for every candidate. The following framework combines the questions that federal guidance asks families to investigate; it is a way to organize evidence, not a universal rating formula.

Comparison axis What to document for each community Decision question
Current and future care Daily-living assistance, nursing, therapy, hospice, medication support, mobility help, and how higher needs are handled Can this setting safely serve the resident now and if the care plan changes?
Staffing and clinical oversight Number of staff on each shift, training, leadership, turnover, routine clinician visits, and who responds to urgent needs Is qualified help available when it is needed, including nights and weekends?
Inspections and quality information Licensing status; Medicare or Medicaid certification when applicable; inspection findings, staffing information, and published quality measures within the setting’s scope What do official records show, and are they relevant to this type of facility?
Specialized services Dementia or memory-care unit, behavioral support, rehabilitation, hospice coordination, and additional requirements or fees Are the services specific to the resident’s condition, and what would they cost?
Resident experience Interactions between staff and residents, cleanliness, maintenance, meals, activities, accessibility, outdoor space, quiet, and family access Would the resident feel respected, comfortable, and able to participate?
Location and availability Distance for family, transportation, appointment arrangements, vacancies, wait-list rules, and expected move-in timing Can family visit reliably, and is a suitable room actually available?
Price and payment fit Base fee, included services, care-level charges, medication or transportation fees, deposits, one-time charges, and accepted payment sources What is the realistic monthly and one-time cost for this resident?
Contract and transition terms Agreement, resident and family responsibilities, discharge or transfer rules, rate changes, grievance process, emergency procedures, and policies What happens if finances, health, availability, or behavior changes?

3. Identify the setting and verify its oversight

“Senior living” can describe assisted living, memory care, continuing care retirement communities, and nursing homes, but these settings do not provide the same level of medical care or operate under identical rules. Ask the facility exactly what license it holds, which services that license permits, and which government programs or insurers it accepts.

Know what Medicare Care Compare can and cannot tell you

Medicare recommends comparing nursing homes through health-inspection results, staffing information, and quality measures. Medicare Care Compare applies to nursing homes that are Medicare- or Medicaid-certified. Confirm a candidate’s certification before using those results.

Nursing-home ratings should not be treated as a general score for assisted-living communities or other residential settings outside the tool’s scope. For those settings, use the relevant state licensing agency’s inspection and complaint information, then ask the facility to explain any findings. Federal and state records can lag behind current conditions, so combine them with a current visit and direct questions.

Check local licensing and current status

  • Confirm the facility’s license type, status, and expiration information with the state agency responsible for long-term-care licensing.
  • Ask whether the specific unit being considered has additional certification or staffing requirements, especially for dementia care.
  • Verify current vacancies, wait-list position, admission criteria, and the date a room could actually be occupied.
  • Ask who handles complaints and how a resident or family member can escalate an unresolved concern.

4. Visit several communities and observe, rather than relying on a tour

Visit more than one candidate and use the same written checklist for each. A polished lobby cannot show how a community operates during a busy meal, overnight shift, or staffing shortage.

Schedule a structured first visit

  1. Arrange a meeting with management and care staff. Ask to speak with the administrator or director of nursing as well as the person who would supervise day-to-day care.
  2. Tour the resident’s likely route. See the room, bathroom, dining area, activity spaces, medication area when access is permitted, exits, elevators, and outdoor space. Check whether flooring, lighting, handrails, door hardware, and bathrooms support the resident’s mobility.
  3. Observe resident-staff interactions. Notice whether staff knock, explain care, speak patiently, protect privacy, and respond when residents request help. Look for residents who appear clean, comfortable, and appropriately dressed.
  4. Check cleanliness and maintenance. Look for odors, overflowing bins, soiled linens, damaged equipment, blocked exits, or repairs that appear overdue. Ask how often common areas and rooms are cleaned.
  5. Attend or observe a meal if possible. Ask how menus accommodate allergies, medical diets, cultural preferences, swallowing problems, and changes in appetite. Find out whether residents can choose alternatives and whether help with eating is available.
  6. Review activities and outdoor access. Ask to see the current activity calendar, how residents who stay in their rooms are included, and whether outdoor areas are safe and accessible.
  7. Ask operational questions in plain language. Use the question list below and record the name and role of each person who answers.
  8. Return without calling ahead. If practical, visit on another day and at a different time, such as an evening or weekend. Conditions may differ by shift and meal period.

Talk with families and residents appropriately

Ask residents or visiting family members, without pressuring anyone to disclose private health information, what communication with staff is like, how quickly concerns are addressed, and whether charges or policies were clear before move-in. Treat individual comments as signals to investigate rather than as a statistical verdict.

5. Ask consistent questions about staffing and care

Staffing, training, and turnover

  • How many direct-care staff are scheduled on days, evenings, nights, weekends, and holidays?
  • Which staff members are licensed nurses, aides, therapists, or other clinicians, and what training is required before they work independently?
  • How are absences covered, and does the community rely on temporary agency staff?
  • Who manages the care plan, how often is it reviewed, and how are changes communicated to family?
  • What are the recent staff-turnover patterns, and how long have the administrator and nursing leaders been in their roles?

Medical care and emergencies

  • Who provides routine medical care, how often do clinicians visit, and can residents keep their own physician?
  • How are medications ordered, administered, reconciled, and reviewed for side effects or interactions?
  • How are laboratory tests, physical therapy, specialist visits, and hospital follow-up arranged?
  • Does the community provide transportation, arrange it through a third party, or require families to arrange it? What fees apply?
  • What is the procedure for falls, sudden illness, missing-person events, severe weather, power loss, fire, and infection outbreaks?
  • How and when are families notified about an incident, a change in condition, or a transfer to a hospital?

Services for dementia and other specialized needs

  • Is there a dedicated dementia or memory-care unit, and is it secured?
  • What dementia-specific training do staff receive, and how are wandering, agitation, sleep changes, and behavioral symptoms handled?
  • What activities are adapted for different cognitive abilities?
  • Can the community safely manage the resident’s medical conditions, mobility limits, communication needs, or behavioral risks?
  • What happens if the resident’s needs exceed the unit’s capabilities, and how much notice is provided before a transfer?

6. Compare the complete cost, not only the advertised base rate

Request a current written fee schedule and a sample resident statement for the expected care level. Costs vary by community and service package, so a base monthly rate is not enough to compare candidates.

Cost area Questions to ask
Base room or residence fee What does the monthly rate include? Does it change by room size, location, or occupancy?
Care-level charges How are assistance needs assessed, and what triggers a higher tier or a rate change?
Specialized care Are memory care, behavioral support, skilled nursing, hospice coordination, or rehabilitation billed separately?
Daily services Are meals, laundry, housekeeping, medication management, incontinence supplies, and personal-care hours included?
Transportation and appointments What rides are provided, how far will the service travel, and what are the per-trip or mileage charges?
One-time charges Are there application fees, entrance fees, deposits, community fees, move-in charges, or refundable portions?
Changes and termination How much notice is required for a rate increase, move-out, transfer, or refund, and are there penalties?

Understand who pays

NIA says professional care in assisted-living facilities and continuing-care retirement communities is almost always paid out of pocket. Do not assume that Medicare, Medicaid, veterans’ benefits, or another public program covers a particular facility or service. Confirm eligibility and covered services directly with the program and the community.

Long-term-care insurance policies differ in the services, settings, benefit amounts, elimination periods, and duration they cover. Read the actual policy and ask the insurer whether the candidate meets the policy’s definition of a covered facility. Keep written confirmation of coverage and any required authorization.

7. Read the admission agreement and policies before signing

Ask for the complete agreement, fee schedule, resident handbook, grievance procedure, medication and transportation policies, emergency plan, and discharge or transfer rules before paying a deposit. Compare the documents with what staff said during the tour.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Clauses that deserve careful review

  • What services are guaranteed, optional, or subject to availability.
  • How the community assesses care needs and changes the monthly rate.
  • When the community can require a transfer, discharge, or move to a higher-care setting.
  • Notice periods, refund rules, room-change provisions, and responsibility for belongings.
  • How complaints, injuries, medication errors, and suspected abuse are reported and investigated.
  • How emergencies, infectious-disease precautions, visitation limits, and temporary relocations are handled.
  • Which person is authorized to make care decisions and how the resident’s preferences are recorded.

Ask the facility to explain every unclear term in writing. For nursing-home admissions, the Consumer Financial Protection Bureau says a home generally cannot require a family member, caregiver, or friend to use personal funds to pay the resident’s bill as a condition of admission or continued stay. That protection concerns nursing-home admissions and should not be assumed to govern every assisted-living contract or financial arrangement; obtain advice about the specific agreement when liability language is unclear.

8. Use quality and safety evidence in context

Official inspection and quality information can reveal patterns that a single tour cannot. For a Medicare- or Medicaid-certified nursing home, review health-inspection results, staffing information, and quality measures through Medicare Care Compare. Check the dates and scope of each measure, and ask the facility to explain corrective actions for deficiencies.

For assisted living, memory care, and other settings outside Medicare Care Compare’s nursing-home scope, use state licensing records, inspection reports, substantiated complaints, and enforcement actions where available. A clean record does not prove that every shift is excellent, and a cited deficiency does not by itself predict the resident’s experience; use the record to form precise follow-up questions.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

9. Evaluate location, communication, and family access

Distance affects how often family can visit and how quickly they can respond when a decision is needed. Test the actual route at likely visiting times, including weather or traffic conditions. Ask about parking, public transportation, accessible entrances, visiting hours, overnight stays, and whether family can join meals or activities.

Clarify how updates are sent: phone, secure portal, email, care conferences, or another method. 10. Turn visits into a defensible decision

Separate non-negotiables from trade-offs

Mark each candidate against the resident’s essential safety and care requirements first. Eliminate any community that cannot provide a required service, lacks the appropriate license, cannot meet a documented medical need, or cannot explain how emergencies are handled. Only then compare preferences such as décor, activity variety, room size, or distance.

Document evidence, not impressions alone

For every candidate, keep the same records:

  • Names, roles, and dates of people who answered questions.
  • Written service descriptions, fee schedules, and contract versions.
  • Inspection, licensing, and certification information checked, including dates.
  • Observed staffing and resident experience during each visit.
  • Answers about future care, specialized services, transportation, and emergencies.
  • Open questions, promised follow-up documents, and the person responsible for sending them.

After each visit, write what you observed separately from what you were told. If an answer changes between visits or differs from the contract, ask for clarification before treating it as a commitment.

Watch for warning signs

  • Staff will not provide a written fee schedule, service list, or agreement before a deposit.
  • Managers cannot explain who is on duty overnight, how emergencies are handled, or how care plans are reviewed.
  • Residents appear unattended, uncomfortable, or spoken to disrespectfully during the visit.
  • The community discourages an unannounced second visit or refuses reasonable questions about licensing, inspections, vacancies, or complaints.
  • Promises about care, refunds, or future availability do not appear in the written agreement.
  • The facility pressures the family to sign immediately or asks a relative to accept personal financial responsibility without explaining the legal terms.

11. Get independent and local help

Discuss the care level with the resident’s health professionals, social workers, and experienced caregivers. People who have direct experience can identify practical differences in communication and staffing that a brochure will not show, although their experiences are personal rather than a substitute for current records.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

NIA identifies the Eldercare Locator, LongTermCare.gov, and local aging agencies as sources for finding services and local assistance. Use the state agency that licenses the facility to verify current status, inspection information, complaint procedures, and rules that apply in that state. Availability, prices, policies, and insurance coverage can change, so confirm them immediately before signing.

12. A final pre-move checklist

  • The resident’s current and possible future care needs are written down and accepted by the community.
  • The facility’s license type and, when relevant, Medicare or Medicaid certification are verified.
  • At least two visits were completed, including a different time or day when practical.
  • Staffing, training, turnover, clinical oversight, emergency response, and specialized care were discussed.
  • The resident’s room, meals, activities, accessibility, outdoor space, cleanliness, and family access were observed.
  • Vacancy, wait-list rules, move-in timing, transportation, and appointment arrangements are confirmed.
  • A written fee schedule identifies included services, extra charges, deposits, and possible rate changes.
  • Payment sources and long-term-care insurance coverage are verified for this facility and service.
  • The full agreement and policies were read, unclear terms were explained in writing, and transition rules are understood.
  • The resident and decision-makers know whom to contact with concerns before and after move-in.

Choose the community that demonstrates a safe care match, transparent terms, and a daily environment the resident can realistically enjoy—not simply the one with the most attractive tour or the highest online rating.
