Compare radiopharmaceutical trials against the actual protocol, your available standard treatments, and the practical demands of participation—not by phase number or radionuclide name alone. Before signing consent, have your oncologist, nuclear-medicine physician, and the trial team review eligibility, administered activity and schedule, imaging and dosimetry, likely benefits and harms, costs, radiation-safety requirements, and what happens if you stop.
Start with the decision you are actually making
A radiopharmaceutical trial is designed to answer a research question. That question may concern safety, an administered activity (the amount of radioactive material given), dosing schedule, preliminary antitumor activity, imaging performance, treatment selection, or comparison with standard care. A trial can be scientifically important without being the right choice for a particular patient.
Write down what you hope participation will accomplish. You may be seeking a treatment option after standard therapies, access to a new imaging method, fewer hospital visits, a chance to help future patients, or information about whether a target is present in your cancer. Then compare each candidate study with the treatments you could receive outside a trial. The National Cancer Institute (NCI) cautions that an investigational treatment may be no better than standard treatment and may cause more serious side effects.

Your treating oncologist remains important even when another center runs the study. Ask the oncologist, a nuclear-medicine physician, and the investigator to review the protocol and consent form together. Public registry summaries cannot establish your individual eligibility, radiation exposure, chance of benefit, or best option.

Build one comparison sheet for every option
Use the same headings for every trial, for standard care, and for any other trial you are considering. Record the protocol version and the site contact so that later answers can be tied to the document you were given.
| Comparison axis | Questions to answer for each option |
|---|---|
| Goal and evidence | What is the study trying to establish? What is already known for this cancer, stage, and treatment setting? |
| Treatment design | What agent, radionuclide, carrier or targeting molecule, administered activity, schedule, route, combination, and comparator are used? |
| Eligibility | Which exact criteria fit or do not fit? What tests, prior treatments, laboratory values, and performance-status requirements are needed? |
| Imaging and radiation | Is imaging diagnostic, therapeutic, used to select participants, or used to assess response? Which scans, dosimetry measurements, and home precautions apply? |
| Benefit and harm | What outcomes matter, what is known about acute and delayed effects, what remains uncertain, and what alternatives exist? |
| Burden | How many screening, treatment, scan, and follow-up visits are required? Are hospital stays, caregiver help, or travel involved? |
| Cost and support | Which expenses are paid by the sponsor, insurer, or patient? Is preauthorization or travel assistance available? |
| Care continuity | Who coordinates with your regular clinicians? What happens if you withdraw, the disease changes, or the study closes? |
Fill in “not stated” when a registry or conversation does not provide an answer. Treat that as a question for the study team, not as evidence that the item is absent.
Interpret phase and purpose together
Phase 1
The International Atomic Energy Agency (IAEA) describes Phase 1 radiopharmaceutical studies as initial human testing that examines pharmacology, effects as the administered activity increases, and early evidence of effectiveness where possible. Radiopharmaceutical Phase 1 studies normally include dosimetry. The IAEA gives a general Phase 1 sample-size range of 20–80 subjects; the publication year is not established in the available material, and that range is a development description—not a prediction of an individual study’s safety, strength, or chance of benefit.
Phase 2
Phase 2 studies seek preliminary evidence of effectiveness and information about common short-term side effects. A Phase 2 trial can still involve substantial uncertainty, especially when the agent, target, cancer type, or combination is new.
Phase 3
Phase 3 gathers further effectiveness and safety evidence to assess overall benefit–risk, often with a standard-care or other comparator. A later phase does not automatically make a study more suitable for you, and an earlier phase is not automatically inappropriate. Read the stated primary objectives, the comparison arm, and the treatment setting.
Ask the investigator: “Why do researchers think this treatment may be better—or may not be better—than the alternatives?” Also ask how response, progression, quality of life, and safety will be measured, and which outcome is the trial’s primary endpoint.

Check eligibility line by line
Compare your medical record with the complete inclusion and exclusion criteria, not just a short public listing. Relevant items can include cancer type and stage, measurable or target-positive disease, prior therapies and washout periods, laboratory and organ-function results, infection status, pregnancy or breastfeeding restrictions, other medical conditions, and performance status (how well you can carry out daily activities).
The U.S. Food and Drug Administration (FDA) says, “The purposes of eligibility criteria for cancer clinical trials are to select the intended patient population and reduce potential risks to trial subjects.” Its July 2026 final guidance on performance status also notes that some criteria can be more restrictive than necessary and that broadening them may improve applicability to U.S. populations. This sponsor- and investigator-facing guidance does not let a patient override screening.
- Ask whether each criterion is a firm safety requirement, a scientific requirement, or a site policy.
- Ask whether a recent scan, laboratory result, or medication could change your status and how long the result remains valid.
- Ask whether another trial has different criteria if one requirement excludes you.
- Ask how long screening takes and whether you must stop a current medicine during screening.
Meeting public-facing criteria does not guarantee enrollment. The study team makes the final decision after reviewing the full protocol, records, examination, laboratory data, and imaging.
Identify exactly what will be administered
Ask for the investigational product’s name, radionuclide, carrier or targeting molecule (if specified), administered activity, number and timing of administrations, route, premedications, hydration or kidney-protection measures, and any combination drugs. Clarify whether treatment is randomized, whether a placebo is possible, and whether the comparator is a specific standard regimen, physician’s choice, or observation.
Find out what happens if the disease progresses, a scan is inconclusive, a dose is delayed, or a laboratory value changes. Some protocols allow dose interruption or reduction; others require withdrawal. Do not infer the dose, safety, or tissue distribution from the radionuclide name alone. Those details depend on the agent, carrier, administered activity, schedule, patient factors, and protocol.

If randomization is used, ask when assignment occurs and whether you can decline the assigned arm. If there is a standard-care comparator, ask whether crossover is allowed after progression and whether treatment outside the protocol remains available.

Understand imaging, dosimetry, and radiation precautions
What the scans are for
A scan may confirm that a molecular target is present, select people for therapeutic treatment, calculate biodistribution, measure response, or perform several of these jobs. Ask which scan is required before treatment, the timing of scans after administration, how long each session lasts, and what happens if the image does not meet the qualifying threshold.
Do not equate a diagnostic scan with proof that treatment will work. The scan may show target expression while the therapeutic agent has a different activity, schedule, tissue penetration, or evidence base.
What dosimetry means
Dosimetry estimates radiation absorbed by organs, tumors, or the whole body. Ask whether the trial performs patient-specific measurements, which organs are prioritized, what samples or images are required, and when results will be available. The European Association of Nuclear Medicine (EANM) notes that first-in-human studies involve varied emitters, carriers, activities, and administration routes; measurement methods and timing therefore require careful selection rather than a single universal procedure.
Recommended Free Tools
The FDA’s document Oncology Therapeutic Radiopharmaceuticals: Dosage Optimization During Clinical Development is an August 2025 draft marked “Not for implementation.” It says its purpose is to help sponsors identify optimized administered activity and schedule during development, but it excludes initial first-in-human activity selection, dosimetry software, population-fixed versus personalized dosimetry, and theranostic co-development. It is not binding guidance and is not a patient-specific dosing rule. Ask the trial team which validated procedures and local regulations govern your study.
Home and clinic radiation safety
Request written instructions covering distance from others, sleeping arrangements, contact with children or pregnant people, toilet and hand hygiene, handling of bodily fluids, travel on public transport, work, sexual activity, breastfeeding, and disposal of contaminated materials. Ask how long each precaution lasts and whom to call if you cannot follow it. Requirements vary by agent, administered activity, route, kidney function, local rules, and whether you receive inpatient care.
When a diagnostic scan is a treatment gate
Some therapeutic radiopharmaceutical trials enroll only people whose diagnostic scan meets a target-uptake or distribution criterion. Ask whether the diagnostic agent is the same agent used in the therapeutic trial, a related compound, or a separate product. Ask how closely the scan identifies the population in which treatment evidence exists and what happens to people with borderline or discordant results.

Japan’s Pharmaceuticals and Medical Devices Agency (PMDA) states: “To evaluate the clinical utility of diagnostic radiopharmaceuticals, it is essential to demonstrate both i. the accuracy of the imaging-derived information and ii. its clinical significance.” That is Japanese regulatory guidance; approval standards and required evidence can differ in the country where your trial is located.

Request separate lists of known, possible, and unknown effects. Ask about immediate reactions, blood-count changes, kidney or liver effects, salivary or tear-gland effects, hormonal effects, fertility, secondary malignancy concerns, and delayed complications when those issues are relevant to the agent. The trial team should explain what monitoring detects each problem, how often tests occur, and which symptoms require an urgent call.
NCI’s patient guidance states, “Every clinical trial is different, which means risks can also differ.” Do not transfer a risk estimate from another radiopharmaceutical, cancer, radionuclide, or phase. No patient-specific likelihood of benefit, serious toxicity, or radiation exposure can be stated without the named cancer, agent, protocol, comparator, and jurisdiction.
Compare the trial with all realistic alternatives: standard treatment now, another approved regimen, supportive care, another trial, or observation when clinically appropriate. Ask what could be lost by delaying standard treatment for screening or by meeting a trial’s washout period. Ask whether the treating team can provide the standard option later if you decline or stop the study.
Calculate the time and daily-life burden
Make a calendar from screening through the last protocol-required follow-up. Record the date and length of every visit, scan, blood draw, infusion, admission, and telephone check. Include travel time, parking, overnight stays, caregiver availability, work or school disruption, and restrictions after administration.
- Ask whether screening requires a fresh biopsy or scans from a specific machine.
- Ask whether treatment is outpatient, inpatient, or conditional on radiation readings.
- Ask how many hours you may spend at the site on administration days.
- Ask whether remote visits, local laboratory testing, or shared care with your oncologist are permitted.
- Ask how follow-up continues if you move, become too ill to travel, or finish treatment early.
NCI notes that locations, procedures, visits, and available support differ among studies. A trial that appears convenient on a registry may require substantially more visits after the first administration.

Separate research costs from routine care
Request a written cost worksheet before enrollment. Identify which items are research-only—such as the investigational product, extra scans, specialized dosimetry, or additional blood tests—and which are routine care that your insurer may process normally. Ask who pays if a test is repeated because of a technical problem or an abnormal result.
Confirm whether insurance preauthorization is required and whether your policy excludes trial-related care. Ask about travel, lodging, parking, meals, childcare, companion costs, lost wages, and emergency treatment away from the study site. Sponsors, insurers, and patients may pay different portions, and a trial does not necessarily cover every expense.
Read consent as an ongoing process
Informed consent should give you time to review the protocol’s purpose, procedures, alternatives, foreseeable risks, unknowns, privacy arrangements, compensation for injury where applicable, and contact information. You can ask for a copy and discuss it with family or your clinicians before deciding.

Questions to take to the trial team
These NCI prompts are useful starting points. Ask them for each candidate study and write down the answer and the name of the person who provided it:
- “What is the purpose of the trial?”
- “How long will I be in the trial?”
- “How many visits will I need to make to the hospital or clinic?”
- “What are the possible side effects or risks of the new treatment?”
- “How do the possible risks and benefits of this trial compare to those of the standard treatment?”
- “What are my other treatment choices, including standard treatments?”
- “Which costs do I have to pay if I take part in the trial?”
Add protocol-specific questions: What finding on the diagnostic scan qualifies me? What administered activity and schedule will I receive? How is dosimetry measured? What radiation precautions apply at home? What happens if my disease progresses or I cannot complete a scan? Who pays for an unexpected admission?
A practical enrollment process
- Collect the documents. Obtain the full protocol summary, consent form, eligibility list, visit calendar, radiation-safety sheet, and cost information for every option.
- Confirm the clinical context. Ask your oncologist whether the trial’s cancer type, stage, prior-treatment requirements, and timing fit your care plan.
- Complete the comparison sheet. Record agent, radionuclide, activity, schedule, imaging, dosimetry, comparator, visits, costs, and withdrawal rules without relying on memory.
- Verify screening. Have the study team review records and identify tests, washout periods, and a realistic screening timeline.
- Model the practical burden. Put every visit, travel leg, caregiver need, work absence, and radiation precaution on a calendar.
- Resolve payment questions. Obtain insurer and sponsor decisions in writing, including preauthorization and travel assistance.
- Discuss uncertainty. Ask what is known, what is not known, how benefit will be measured, and what alternatives remain if the study does not help.
- Review consent without pressure. Take time, involve a trusted person, and ask for clarification of any term you cannot explain back in your own words.
- Make a conditional plan. Decide what you will do if screening fails, randomization assigns an unacceptable arm, side effects require interruption, or the disease changes.
- Keep contacts and copies. Store the coordinator’s numbers, after-hours instructions, consent form, medication list, and radiation precautions where your caregiver can find them.
Common comparison errors
- Choosing by phase alone: Phase describes the research question, not your personal suitability or superiority over standard care.
- Inferring dose from a radionuclide: The same radionuclide can be used with different carriers, activities, routes, schedules, and monitoring.
- Treating a qualifying scan as proof of response: Target detection supports eligibility; it does not establish therapeutic benefit.
- Assuming broad eligibility: Criteria are protocol-specific, and satisfying a public listing does not guarantee enrollment.
- Assuming the sponsor pays everything: Routine care, research-only procedures, travel, and emergency services may have different payers.
- Ignoring the exit plan: Ask in advance about withdrawal, progression, post-trial treatment, and follow-up.
Bottom line
The soundest comparison is a documented, protocol-level review with your clinicians. Judge each radiopharmaceutical trial by its purpose and evidence, exact eligibility, treatment and comparator, imaging and dosimetry plan, realistic risks and alternatives, time and radiation-safety demands, costs, and continuity of care. Only the study team and your treating specialists can translate those details into an individual enrollment decision.
