Compare biological age tests by what they measure, which sample they use, what the score is designed to predict, and how well the method has been validated and repeated—not by the “years younger” or “years older” label alone. There is no single standardized biological-age measure, so tests using different biomarkers or algorithms can give different results without either being a direct diagnosis or a definitive measure of lifespan.

What a biological-age test is actually estimating

Chronological age is the time since birth. “Biological age” is an umbrella term for estimates built from biological or functional measurements associated with age, health, or risk. The term does not identify one universally accepted quantity: the tissue sampled, biomarkers selected, and mathematical model all affect the result.

A test might estimate how closely a sample resembles samples from people of a particular chronological age, predict risk of future illness or mortality, estimate the pace of aging, or combine health measures into an age-like score. Those are different targets. A score expressed in years does not make them interchangeable.

A 2023 review in npj Aging noted that there is no standardized, widely accepted biomarker set and mathematical model for quantifying biological age. Methods can include linear regression, principal-components analysis, the Klemera-Doubal method, machine learning, and epigenetic calculators. The model and its intended use matter alongside the biomarkers.

Compare the method, not just the score

Start by identifying the specimen, assay, and biological signal. The categories below can overlap in purpose, but they measure different things and have different interpretive limits.

Method What it measures What to check Main limitation
DNA methylation (epigenetic clocks) Age-associated chemical marks on DNA, often measured in cells from blood or another tissue. Clock name and version; tissue or sample; assay; intended target, such as chronological age, risk, or pace; and validation evidence. Clocks target different outcomes. A blood result reflects cells represented in that sample, not a direct measurement of every organ. Cell composition and measurement noise can affect the estimate.
Proteomic clocks Patterns in the levels of proteins in blood or other bodily fluids; some approaches associate profiles with particular organs. Which proteins and platform are used; whether the result is a whole-body estimate or organ-associated; and which population and outcomes were used for validation. Protein patterns can reflect several processes. An organ-associated estimate is not a diagnosis of that organ.
Clinical or metabolic composites Combinations of routine measures, which may include glucose, cholesterol, inflammatory markers, or immune markers. The included biomarkers; the algorithm, such as KDM or another composite; the reference population; and whether the score adds predictive value beyond chronological age. The score may describe current health and risk factors more than an aging trajectory.
Telomere length The length of protective chromosome ends in sampled cells. Assay method, sample and cell type, repeatability, and evidence for the specific claim being made. Telomere length varies substantially between people and, on its own, gives limited information about biological age.
RNA or transcriptomic clocks RNA transcripts that reflect active gene expression. Collection and processing conditions, assay reproducibility, and external validation. Gene transcription can change quickly and may be noisy.
Physiological, cognitive, or wearable measures Function or signals such as blood pressure, fitness, strength, mobility, cognition, or wearable-derived vital signs. The exact function or signal, collection period, age reference, and whether the output is a functional measure or an age claim. These measures are not equivalent to molecular clocks; they may chiefly describe current function or health status.

A cohort study by Belsky and colleagues, published in 2017, compared seven measures of biological aging in 800 members of the Dunedin birth cohort and examined their relationships with physical, cognitive, and subjective health-span characteristics. The comparison illustrates why a single label should not be assumed to capture every dimension of aging.

Questions to ask before choosing or interpreting a test

  1. What sample and assay are used?

Look for a precise description: for example, DNA methylation measured in blood, proteins measured in plasma, telomere length in a specified cell sample, or a composite of routine clinical biomarkers. “Biological age test” by itself does not tell you what was measured.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  2. What is the score designed to predict?

Distinguish chronological-age estimation from disease or mortality risk, pace of aging, and a composite score expressed in years. Stanford Medicine’s explainer describes early Hannum and Horvath clocks as designed to estimate chronological age, while GrimAge was designed around mortality or future disease risk. A method validated for one target should not automatically be treated as valid for another.

    • What You Get: Anker USB-C Hub (7-in-1), welcome guide, 18-month warranty, and our friendly customer service.
  3. What evidence supports the stated use?

Look for peer-reviewed external validation in a population relevant to you, the outcomes the test was evaluated against, and evidence that the measure predicts something beyond chronological age. A correlation with chronological age alone does not establish clinical usefulness. If a service claims that a score tracks change over time, longitudinal evidence is especially relevant.

  4. How repeatable is the result?

Check whether repeat testing under comparable conditions produces sufficiently consistent results to distinguish a real change from measurement variation. Ask how uncertainty is reported and whether sample handling or cell composition can affect the result. Stanford Medicine reports that many epigenetic clocks have roughly three years of error and that blood-cell proportions can vary over a day; this is a qualified observation about many clocks, not a universal error margin for every test.

  5. Does the sample support the claim?

A blood-based test measures signals in the cells or fluid represented in blood. It does not directly sample the brain, heart, or kidneys. Organ-specific aging signatures remain an area of research rather than an established individual diagnostic tool.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  6. What decision would the result change?

Consider whether the score can lead to a meaningful, evidence-supported action. A consumer score that does not establish a medical diagnosis should not replace clinical evaluation or be the sole basis for health decisions.

Why two tests can disagree

Different results may reflect different biological signals, samples, reference populations, algorithms, or targets. One clock may be designed to estimate chronological age while another is oriented toward risk; a clinical composite may reflect current health measures while a molecular test reflects a different signal. Even tests in the same broad category can use different versions, assays, and validation methods.

Variation can also arise from measurement noise and, for some methods, changes in the cells represented in a sample. Stanford Medicine notes that blood-cell proportions can vary over a day. A difference between two scores therefore does not, by itself, show that one test is wrong or that a person’s aging changed between tests. First compare what each test measures and whether its uncertainty and repeatability are reported.

How to interpret an age-in-years result

  • Read the method and target before the number. Find the clock or algorithm, sample type, and stated purpose in the test documentation.
  • Check the reference population. Validation in a particular cohort does not automatically establish that the score performs equally well in every population.
  • Separate association from diagnosis. A relationship between a biomarker and age or risk does not by itself make the result a diagnosis or establish that an intervention has reversed aging.
  • Do not treat one tissue as the whole body. A blood-derived estimate does not establish that all organs share one biological age.
  • Look for uncertainty and repeatability. Without them, a small difference between scores or over time may be difficult to interpret.

Stanford Medicine’s September 23, 2026 explainer says that direct-to-consumer biological-age clocks are not FDA-approved as medical diagnostic tools in the United States. That statement is specific to U.S. regulation and the medical-diagnostic use described there; it is not a global regulatory conclusion. Singapore’s Ministry of Health stated on September 9, 2026 that tests intended solely for general wellness, with no claim to assess, diagnose, treat, or prevent a medical condition, are generally non-clinical and not regulated as medical devices under its Health Products Act. Regulatory treatment depends on jurisdiction and the claims made for a test.

A practical comparison checklist

  • Identify the specimen, assay, and biomarkers.
  • Find the algorithm or clock name and version.
  • Write down the score’s intended target, not just its display label.
  • Check external validation, the population studied, and relevant outcomes.
  • Look for uncertainty, repeatability, and any conditions that can affect sampling.
  • Decide whether the result offers information that changes a real health decision.
