Choose the kind of help that matches what you need now. A therapist offers private, individualized assessment and treatment; a support group offers shared experience, regular contact, and peer connection. Loneliness does not automatically mean you have a mental-health disorder, and you do not have to choose only one option. You can try a group first, begin therapy, or use both while checking whether the arrangement feels safe, practical, and useful.

The best choice depends on your goals, the intensity and duration of your distress, access, privacy preferences, and how comfortable you feel with the people involved. Use the steps below to compare options rather than assuming that therapy is always required or that any group will be a good fit.

Start with the kind of support you want

Loneliness can mean wanting more everyday contact, feeling disconnected despite being around people, grieving a relationship, struggling after a move, or experiencing distress that is affecting sleep, work, study, or self-care. Those experiences call for different levels and types of help. Begin by naming the outcome you want, in plain language: “I want one regular social connection,” “I need help understanding why I withdraw,” or “I am finding it hard to function and want a professional assessment.”

When a therapist is the better starting point

Consider an individual therapist when loneliness is persistent, is interfering with daily life, or raises questions that require a professional assessment. Therapy can explore thoughts, emotions, relationships, habits, grief, anxiety, depression, trauma, or other concerns that may be contributing to isolation. A clinician can tailor treatment to your needs and medical situation rather than applying a generic loneliness program.

Psychotherapy may be delivered one-to-one or in a group. The National Institute of Mental Health (NIMH) advises that treatment decisions should reflect a person’s individual needs and medical situation under the guidance of a mental-health professional. That does not mean loneliness itself is a diagnosis; it means a clinician can help clarify what is happening and what kind of care, if any, is appropriate.

When a support group may be the better first step

A peer group can be useful when your main need is regular contact with people who understand a similar experience. Groups may be organized around a life transition, bereavement, a health condition, identity, caregiving, recovery, a shared interest, or a general wish for connection. The value is usually the repeated opportunity to listen, participate at your own pace, and build relationships outside a clinical setting.

Some groups are facilitated by peers; others have a trained professional, volunteer, or organization running the meetings. A group is not a substitute for urgent care or individualized diagnosis, and the quality and safety of groups vary. Treat the first meeting as a chance to observe whether the purpose, boundaries, and atmosphere suit you.

Why using both can make sense

Therapy and peer support address different needs and can be combined. For example, therapy might help you understand avoidance or practice communication while a group gives you a predictable place to use those skills. Tell the therapist about any group you attend, and tell the organizer only what you are comfortable sharing. There is no evidence in the sources for this guide that one option is generally more effective than the other, so judge the combination by your own goals, safety, and progress.

Compare the options before you commit

Decision factor Therapist Support group
Primary purpose Individualized professional assessment and treatment. Shared experience, mutual support, and connection.
What to check for fit License or other applicable credential, relevant experience, treatment approach, and rationale. Topic or life experience, facilitator, group norms, and whether members’ circumstances are sufficiently similar.
Relationship and comfort Trust, rapport, and ability to speak openly in private sessions. Psychological comfort, respectful participation, and a sense that you can attend without pressure.
Access Fee, insurance, lower-cost options, new-patient availability, appointment times, location, and online or in-person format. Cost or donation policy, meeting schedule, travel, accessibility, online or in-person format, and help available for joining.
Privacy and boundaries Ask how confidentiality works and what its legal or safety-related limits are. Ask how privacy, participation, contact between members, and disruptive behavior are handled; do not assume group confidentiality is guaranteed.
Progress Agree on goals and a way to review whether treatment is helping. Notice whether attendance leaves you feeling supported, respected, and more connected over time.

When professional therapy deserves priority

Make a therapy appointment a priority if loneliness is accompanied by severe or persistent sadness, panic, hopelessness, inability to manage ordinary responsibilities, substance-use concerns, thoughts of self-harm, or uncertainty about whether a medical or mental-health condition is involved. You do not need to wait for a crisis to ask for an assessment. A primary-care clinician can also help with a referral or with checking physical-health factors that may affect mood and energy.

If you are in the United States and are having suicidal thoughts or are in emotional distress, NIMH lists the free, confidential, 24/7 988 Suicide & Crisis Lifeline, available by call or text. This service information is specific to the United States; elsewhere, use your country’s emergency or crisis service.

How to choose a therapist: a practical process

1. Define your concern and preferred outcome

Write two or three sentences describing what is happening, how long it has been happening, and what you hope will change. You might want to feel less isolated, rebuild a social routine, process a loss, manage social anxiety, or understand a recurring relationship pattern. This short description helps a prospective therapist say whether the concern is within their competence.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

2. Find several plausible candidates

In the United States, NIMH points readers to professional directories and notes that insurers may provide lists of participating providers. Universities, medical schools, and state or local health departments may also have resources. Directory listings are not comprehensive and do not constitute NIMH endorsement, so verify details directly with each clinician. Outside the United States, use the licensing or registration directory maintained by the relevant professional regulator, health service, or jurisdiction.

Search by the concern you want help with, the age group served, language, accessibility needs, and whether you prefer online or in-person sessions. Keep a short list rather than contacting only one person: availability, fees, and fit can differ substantially.

3. Verify credentials and relevant experience

Ask what license, registration, or professional credential the therapist holds, which authority issued it, and whether it is current in your location. Ask how long they have practiced and how often they work with concerns like loneliness, grief, social anxiety, relationship difficulties, or the life transition you described. A credential does not guarantee a good personal fit, but confirming it is a basic safety check.

The American Psychological Association (APA) recommends asking about licensure, years of practice, experience with your particular concern, treatment methods, fees, sliding-scale availability, and insurance. That APA guidance was created in 2019 and is US-focused, so confirm current licensing rules, payment arrangements, and telehealth permissions with local authorities and the provider.

4. Ask what approach the therapist uses and why

Request the name of the treatment approach in ordinary language and ask how it could address your goals. A useful answer should explain what sessions might involve, what evidence or clinical reasoning supports the approach, and how it would be adapted to you. You are not required to know which therapy “should” work before speaking with a professional; you are checking whether the explanation is clear and collaborative.

Ask whether the therapist coordinates with a physician or can discuss medication evaluation if that becomes relevant. A therapist who does not prescribe should be able to explain how they would coordinate with another qualified professional, with your consent.

5. Have a preliminary conversation

NIMH says, “Once you have identified one or more possible therapists, a preliminary conversation can help you understand how treatment will proceed and if you feel comfortable with the therapist.” Use a brief phone or video call, if offered, to ask questions before scheduling ongoing sessions. Notice whether the therapist listens, answers directly, explains limits, and respects your pace.

  • What experience do you have with my concern and with people in my age group or situation?
  • What treatment approach would you consider, and what would sessions typically look like?
  • How would we set goals, and when would we review progress?
  • How long might it take before we reassess the plan?
  • How do you handle confidentiality, and what are its legal or safety-related limits?
  • What happens if I need help between appointments or need a higher level of care?

6. Confirm cost, insurance, and scheduling in writing

Ask for the full fee, whether an initial assessment costs the same as later sessions, cancellation rules, accepted insurance, expected out-of-pocket cost, and whether a sliding scale or other lower-cost arrangement is available. Confirm whether the therapist is accepting new patients and whether appointment times work with your schedule. SAMHSA’s finding-support guidance recommends checking insurance or free and low-cost care, new-patient status, suitable appointment times, and qualities important to you; it also notes that finding a good match may take time.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For online care, ask where the therapist is licensed to practice, what platform is used, how privacy is protected, and what happens if the connection fails. Confirm any technology, language, disability-access, or interpretation needs before the first session.

7. Judge rapport without expecting instant certainty

Feeling nervous in a first session is common, so discomfort alone does not prove the therapist is wrong for you. Look for a pattern: can you correct a misunderstanding, ask questions, and discuss topics that feel difficult? A good therapeutic relationship allows professional boundaries and honest disagreement. You should not feel pressured to disclose more than you are ready to share or to continue a method you do not understand.

How to choose a support group

Match the group’s purpose to your experience

Read the description carefully. “Loneliness” can be the main topic, or it may be addressed indirectly through grief, adjustment, chronic illness, parenting, recovery, disability, identity, or a shared hobby. A relevant commonality can make conversation easier, but perfect similarity is not necessary. Ask what the group is designed to do: emotional support, practical problem-solving, social activity, recovery maintenance, education, or a mixture.

SAMHSA describes national and local groups, many with online and in-person meetings, and gives examples such as NAMI, Alcoholics Anonymous, Narcotics Anonymous, and SMART Recovery. These examples come from a broad mental-health, drug, and alcohol resource page; they are not endorsements and do not mean every listed group addresses loneliness. State, county, or local agencies may identify additional programs through support-group and local-program resources.

Ask how meetings work

Before attending, ask whether meetings are drop-in or require registration, how many people usually attend, whether a facilitator is present, and whether speaking is optional. Find out whether the format is discussion, structured exercises, education, or an activity. Some people find an activity-based meeting less exposing than a circle focused entirely on personal disclosure.

Ask about online participation, cameras, chat, recording, and whether members can contact one another privately. For an in-person group, check the venue, transport, parking, seating, noise, and accessibility. A group that is theoretically suitable but impossible to reach or attend regularly is not a practical fit.

Check norms, privacy, and safety

Request the group’s written guidelines if available. They should address respectful language, interruptions, harassment, discrimination, confidentiality expectations, crisis behavior, and what happens when someone dominates or becomes unsafe. Group confidentiality is usually a shared expectation rather than the same legal protection that may apply to a clinical relationship; ask exactly what organizers can and cannot enforce.

Ask whether members are expected to exchange phone numbers, donate, promote products or services, or follow a particular belief system. You can decline contact details and leave a meeting if pressure, hostility, or unwanted advice makes you uncomfortable. A facilitator should be able to explain how concerns are reported.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Consider cost and participation barriers

Confirm any membership fee, suggested donation, materials charge, transport cost, or requirement to purchase something. Ask whether free places, online attendance, interpreters, captioning, childcare, or other accommodations exist. Time, affordability, anxiety about attending, and fear of judgment or stigma can all make participation difficult.

A UK government report on mental health and loneliness across life stages records participant and stakeholder accounts of these barriers. It also describes people finding group discussion helpful for feeling less alone and more hopeful when there was a good match. These are qualitative experiences, not a guarantee that a particular group will help you.

Use a low-pressure first visit

If attending feels intimidating, start with an online meeting, an activity-based group, or a format where listening is allowed. The same UK report describes suggestions for online or in-person groups built around shared interests or relevant life experiences; some participants valued being able to decide whether and when to meet in person. Treat this as one possible entry route, not a universal solution. If anxiety is blocking access, ask a trusted person to help you register, travel with you, or stay available by phone.

At the first meeting, observe whether people are welcomed, whether the facilitator protects boundaries, and whether the conversation leaves room for different experiences. You do not have to tell your whole story. Afterward, note how you felt during and after the meeting, what you would change, and whether you could realistically attend again.

How to assess fit and progress after you begin

For therapy

Agree on a review point rather than treating the first session as a final verdict. Progress may mean clearer understanding, improved daily functioning, more social contact, better coping, or a reduction in distress; define it in terms that matter to you. Keep brief notes about goals, concerns, and questions for the next session.

If you are not improving after what feels like a reasonable period, raise it directly. NIMH recommends discussing lack of improvement with the therapist and considering another professional or approach. A useful conversation might change the goals, pace, method, or referral plan. You can seek a second opinion or another therapist; doing so is not a failure.

For a support group

Review whether meetings are helping you feel respected and more connected, even if the benefit is gradual. A group may be a poor fit if you routinely leave more distressed, feel pressured to disclose, encounter judgment or discrimination, or cannot participate because of the schedule or cost. Ask the facilitator whether a different meeting, subgroup, or format is available. Leaving is reasonable when the group does not meet your needs.

Ways to make the decision easier

  • Write your non-negotiables: privacy, language, accessibility, a specific life experience, online access, a maximum fee, or a time you can reliably attend.
  • Separate preferences from requirements: an ideal location may be flexible, while a current license or respectful conduct is not.
  • Contact several options at once: use a short message describing your concern, availability, insurance or budget, and preferred format.
  • Plan a first step that is small enough to complete: make one call, attend one online meeting, or ask a primary-care service for a referral.
  • Protect your privacy: use only the information needed for an inquiry, and ask how your contact details are stored.
  • Reassess deliberately: put a date on your calendar to review therapy goals or group attendance instead of continuing automatically.

  • Who leads it, and what training, license, or supervision do they have?
  • How are privacy, consent, and boundaries handled?
  • Can I attend once before committing?
  • What will I pay, and are lower-cost options available?
  • What times, formats, and accessibility supports are offered?
  • What should I do if the service is not helping or I feel unsafe?

Evidence and limits to keep in mind

The World Health Organization’s peer-support guidance describes peer support as sharing experience, hope, and empowerment within person-centered, rights-based services. It is guidance for services, not a consumer rating and not proof that every peer group is safe or suitable. Likewise, the UK report presents qualitative accounts and stakeholder views, while NIMH, SAMHSA, and APA provide practical guidance. These sources do not establish a numerical success rate or show that therapy is superior to peer support, so your decision should rest on fit, safety, access, and the progress you can observe.
